Smoking cessation after 12 months with multi-component therapy
El tabaquismo es una de las causas de morbimortalidad más importantes en los paÃses desarrollados. Uno de los objetivos prioritarios de los programas de salud pública es la disminución de su prevalencia lo que implica que millones de personas dejen de fumar, sin embargo los programas de cesación a menudo tienen resultados discretos, especialmente con algunos grupos de población. El objetivo de este estudio fue analizar la eficacia de un tratamiento de cesación tabáquica multicomponente realizado en una unidad de tabaquismo hospitalaria. Fue realizado en la Unidad de Tabaquismo del Hospital de Manresa, e incluyó 314 pacientes (91,4% presentaban un nivel de dependencia medio o alto). Se observó que el nivel de estudios, no convivir con fumadores, seguir la terapia multicomponente y utilizar tratamiento farmacológico son factores relevantes en el éxito al dejar de fumar. La tasa de abstinencia no se asocia con otras caracterÃsticas como el sexo, la edad, las caracterÃsticas del hábito tabáquico o el presentar antecedentes psiquiátricos. La combinación del tratamiento farmacológico y psicológico aumentó las tasas de éxito en la terapia multicomponente. La terapia psicológica única también obtuvo resultados positivos aunque más modestos
Multimodal treatment for smoking cessation with varenicline in alcoholic, methadone-maintained, and psychotic patients: A one-year follow-up
Introduction: Numerous studies have evaluated the efficacy and safety of varenicline for smoking cessation in smokers in the general population and, to a lesser extent, among the psychiatric population. However, few studies have evaluated varenicline in patients with other addictions. The present study was conducted to assess outcomes of a multimodal treatment for smoking cessation intervention with varenicline in a sample of alcohol and substance use disorders and patients with psychotic disorders. Methods: This was a prospective, multicenter study. The patient sample comprised alcoholics in remission, methadone-maintained patients, and patients with psychotic disorders, all of whom wanted to stop smoking. All participants received multimodal treatment for smoking cessation therapy (psychological therapy plus varenicline). Smoking abstinence and changes in the psychopathological state of patients were assessed at predefined time points during a 12-month follow-up. The probability of tobacco abstinence after one year of treatment was computed using Kaplan-Meier life tables. Results: The probability of abstinence at one year was 0.225 (95% CI: 0.143- 0.319). By group, the probabilities were as follows: patients with psychotic disorders 0.254 (95% CI: 0.118-0.415); alcoholics 0.237 (95% CI: 0.098- 0.409); and methadone-maintained patients 0.177 (95% CI: 0.065-0.335). Patients with previous quit attempts had a higher probability of achieving abstinence at one year (p<0.01). Conclusions: The results of this study support the use of multimodal treatment with varenicline in patients with alcohol addiction in remission, patients on methadone maintenance, and patients with stable psychotic disorders. Previous smoking cessation attempts were predictive of smoking cessation success in these patients
Dependencia a la nicotina y preparación para dejar de fumar en la población española
Objetivo: Describir la dependencia a la nicotina y la preparación para dejar de fumar en la población fumadora. Métodos: Estudio transversal de una muestra representativa de la población española e18 años. La información se obtuvo mediante entrevistas telefónicas realizadas en junio y julio de 2006. Se estudió la dependencia a la nicotina mediante el test de Fagerström (FTND) y la preparación para dejar de fumar de acuerdo a los estadios del cambio del modelo transteórico en una muestra de fumadores de cigarrillos. Resultados: El 22,5% de los participantes (IC 95%: 20,9-24,2%) fumaba cigarrillos. Su consumo medio fue de 14,4 cigarrillos diarios (desviación estándar 9,15) y su puntuación media en el FTND fue de 2,8, sin diferencias según los estadios del cambio. El 64,3% (IC 95%: 60,3-68,2%) de los fumadores estaba en el estadio de precontemplación, el 25,4% (IC 95%: 21,8-28,9%) en el de contemplación y el 10,4% (IC 95%: 7,9-12,9%) en el estadio de preparación, sin diferencias por sexo. Los fumadores más dependientes a la nicotina (FTNDe6) tenÃan mayoritariamente estudios primarios, comenzaron a fumar a edad más temprana y fumaban más cigarrillos al dÃa. Conclusiones: Los fumadores presentan mayoritariamente un bajo nivel de dependencia a la nicotina y se encuentran en fase de precontemplación. Los programas de deshabituación deberÃan ir dirigidos a reducir el nivel de dependencia, ayudar a los fumadores a avanzar a través de los estadios del cambio y, consecuentemente, disminuir la prevalencia de fumadores en la población
Measuring cigarette dependence: A comparison of two scales in two different groups of smokers
INTRODUCTION The Fagerstrom Test for Cigarette Dependence (FTCD) and the Cigarette Dependence Scale (CDS) are usually used to assess cigarette dependence in clinical-and population-based studies. Our objective was to compare these two scales within groups of smokers from both contexts. METHODS The study was observational with smokers from a representative sample of the adult general population (n=188) and smokers attending a smoking cessation clinic in Barcelona, Spain (n=759). The FTCD and the CDS-5 (short version of 5 items) were used to assess cigarette dependence. We compared the standardized median scores obtained with both scales within each group of smokers by selected variables. To this aim, we re-scaled the scores of both scales to allow their comparison and assess their correlation within both groups. RESULTS The scores obtained with both scales were highly correlated within both groups of smokers (p60 min; 1.7 vs 1.1; p<0.001). CONCLUSIONS While the CDS-5 scored higher more frequently in the population group, the FTCD scored higher more frequently in the clinical group. These differences should be considered when designing either clinical-or population -based studies
Motivació i efectes del tractament de deshabituació tabà quica en pacients psiquià trics

La prevalença de fumadors en població general ha disminuït en els darrers anys, no ha estat aixà entre les persones amb malalties psiquià triques, amb una prevalença molt superior a la població general per la qual cosa pateixen major mortalitat i morbiditat. L’APA recomana intervencions per que aquests pacients deixin de fumar, hi ha evidència de que els fumadors tenen major risc de consum de substà ncies, menor adherència al tractament psiquià tric, pitjor puntuació a l’escala GAF, més simptomatologia psiquià trica i més risc de conducta i ideació autolÃtica. Les intervencions per deshabituació tabà quica en aquests pacients no estan generalitzades són poc estructurades. Les hipòtesis que ens plantejà vem: 1) Els pacients amb malalties psiquià triques estan motivats per deixar de fumar 2) Les taxes d’èxit en la deshabituació tabà quica no són diferents dels pacients no psiquià trics amb les mateixes caracterÃstiques de dependència del tabac 3) Els pacients psiquià trics es poden beneficiar dels mateixos tractaments farmacològics que la població general. Els objectius foren analitzar: 1) els resultats d’un tractament multicomponent per deixar de fumar i com afecten els antecedents de trastorn psiquià tric en l’abstinència. 2) els efectes adversos del tractament amb vareniclina en població psiquià trica. 3) els resultats d’un programa de deshabituació tabà quica combinat amb vareniclina i terà pia psicològica.
METODOLOGIA:
Es realitzen 2 estudis observacionals, en ambdós s’analitzen els resultats de la terà pia multicomponent per a la deshabituació tabà quica a un any de seguiment. L’estudi 1 analitza els resultats de la unitat de tabaquisme de l’Hospital de Manresa, s’estudia una mostra de 312 pacients. S’utilitzen models de regressió logÃstica bivariant i multivariant. L’ estudi 2 és un multicèntric desenvolupat a diferents unitats de tabaquisme. S’estudia una mostra de 90 pacients, 31 diagnosticats de trastorn psicòtic, 29 de trastorn per us d’alcohol i 30 en programa de manteniment amb metadona (PMM), tots ells segueixen una terà pia multicomponent per deixar de fumar i com a tractament farmacològic la vareniclina. S’analitzen a) efectes adversos que presenten els 3 grups al deixar de fumar utilitzant vareniclina, s’aplica estadÃstica paramètrica i no paramètrica per a dades aparellades b) mitjançant la prova Kaplan-Meier es calcula taxes d’abstinència acumulada i diferencies entre subgrups, amb regressió de Cox la probabilitat d’estar abstinent en funció de diferents variables.
RESULTATS:
En l’estudi 1 el 94% de la mostra té un nivell de dependència mig o alt, el 85% venen derivats d’altres serveis i ja havien fets intents previs i el 58,5% tenen antecedents psiquià trics. A l’any el 23,9% del total es manté abstinent, el 16% dels que només van fer tractament psicològic i el 37,2% dels que van seguir la terà pia multicomponent amb tractament farmacològic més psicològic.
En l’estudi 2 els resultats mostren com els efectes adversos de vareniclina són similars a la població general, la reducció de dosis quan apareixen facilita l’adherència i no es detecten recidives en simptomatologia psiquià trica ni efectes adversos greus. Pel que fa als resultats el 22,5% dels pacients està abstinent a l’any; el 25,5% dels pacients amb T. psicòtics, el 23,7% dels pacients amb T. d’alcohol i el 17,7% dels de PMM.
CONCLUSIONS:
Els pacients psiquià trics estan motivats per deixar de fumar; els resultats obtinguts són similars als fumadors greus sense antecedents psiquià trics i lleument inferiors a la població general quan reben terà pia multicomponent; els intents previs per deixar de fumar i una bona adherència a la terà pia multicomponent augmenten les probabilitats d’èxit; els tractaments farmacològics amb vareniclina són ben tolerats en pacients amb trastorns mentals i presenten efectes adversos similars a la població general.[eng] Smokers in general population have decreased, but not in psychiatric disorders patients. Study 2 is developed in different units. A sample of 90 patients (31 with psychotic disorder, 29 with alcohol disorder and 30 in methadone maintenance program MMP) follow a multicomponent therapy for quitting smoking with varenicline. The adverse effects using this treatment and the cumulative abstinence rates and differences between subgroups are analyzed.
RESULTS:
In Study 1, 58.5% have psychiatric history, 23.9% remain abstinent, 16% underwent psychological treatment and 37.2% followed multicomponent therapy with psychological and pharmacological treatment.
In Study 2, results show that the adverse effects of varenicline are similar to general population, the reduction of doses facilitates adherence and no recurrences or serious adverse effects are detected. Results show that 22.5% of patients are abstinent. The results by groups are: 25.5% for psychotic disorder, 23.7% for alcohol disorder and 17.7% for MMP.
CONCLUSIONS:
Psychiatric patients are motivated to quit smoking; results obtained are similar to heavy smokers and slightly inferior to general population when they receive multicomponent therapy. Previous attempts to quit smoking and a good adherence at treatment increase likelihood of success. Introduction: Numerous studies have evaluated the efficacy and safety of varenicline for smoking cessation in smokers in the general population and, to a lesser extent, among the psychiatric population. However, few studies have evaluated varenicline in patients with other addictions. The present study was conducted to assess outcomes of a multimodal treatment for smoking cessation intervention with varenicline in a sample of alcohol and substance use disorders and patients with psychotic disorders. Methods: This was a prospective, multicenter study. The patient sample comprised alcoholics in remission, methadone-maintained patients, and patients with psychotic disorders, all of whom wanted to stop smoking. All participants received multimodal treatment for smoking cessation therapy (psychological therapy plus varenicline). Smoking abstinence and changes in the psychopathological state of patients were assessed at predefined time points during a 12-month follow-up. The probability of tobacco abstinence after one year of treatment was computed using Kaplan-Meier life tables. Results: The probability of abstinence at one year was 0.225 (95% CI: 0.143- 0.319). By group, the probabilities were as follows: patients with psychotic disorders 0.254 (95% CI: 0.118-0.415); alcoholics 0.237 (95% CI: 0.098- 0.409); and methadone-maintained patients 0.177 (95% CI: 0.065-0.335). Patients with previous quit attempts had a higher probability of achieving abstinence at one year (p<0.01). Conclusions: The results of this study support the use of multimodal treatment with varenicline in patients with alcohol addiction in remission, patients on methadone maintenance, and patients with stable psychotic disorders. Previous smoking cessation attempts were predictive of smoking cessation success in these patients

Effectiveness of a telephone-based intervention for smoking cessation in patients with severe mental disorders: study protocol for a randomized controlled trial.
BackgroundUp to 75% of inpatients with mental disorders smoke, and their life expectancy is decreased by up to 25 years compared to the general population. Hospitalized patients without monitoring after discharge quickly return to prehospitalization levels of tobacco use. The aim of the 061 QuitMental study is to assess the effectiveness of a multicomponent and motivational telephone-based intervention to stop smoking through a quitline addressed to smokers discharged from mental health hospital wards.MethodsA pragmatic randomized controlled trial, single blinded, will include 2:1 allocation to the intervention group (IG) and the control group (CG). The IG will receive telephone assistance to quit smoking (including psychological and psychoeducational support, and pharmacological treatment advice if required) proactively for 12 months, and the CG will receive only brief advice after discharge. The sample size, calculated with an expected difference of 15 points on smoking abstinence between groups (IG, 20% and CG, 5%), α = 0.05, β = 0.10, and 20% loss, will be 334 participants (IG) and 176 participants (CG). Participants are adult smokers discharged from psychiatric units of five acute hospitals. Measurements include dependent variables (self-reported 7-day point prevalence smoking abstinence (carbon monoxide verified), duration of abstinence, number of quit attempts, motivation, and self-efficacy to quit) and independent variables (age, sex, and psychiatric diagnoses). In data analysis, IG and CG data will be compared at 48 h and 1, 6, and 12 months post discharge. Multivariate logistic regression (odds ratio; 95% confidence interval) of dependent variables adjusted for potential confounding variables will be performed. The number needed to treat to achieve one abstinence outcome will be calculated. We will compare the abstinence rate of enrolled patients between groups.DiscussionThis trial evaluates an innovative format of a quitline for smokers with severe mental disorders regardless of their motivation to quit. If effective, the pragmatic nature of the study will permit transfer to routine clinical practice in the National Health System.Trial registrationClinicalTrials.gov, NCT03230955 .
