Photonic waveguide engineering using pulsed lasers - A novel approach for non-clean room fabrication!
Over the last 25 years has seen an unprecedented increase in the growth of phonic components based on
semiconductor and solid-state lasers, glass and polymer based optical fibres, and organic LEDs. Emerging
technology for component engineering must embed dissimilar materials based devices into an integrated form
which is more efficient. In this article, we demonstrate techniques for overcoming the materials related
limitations by adopting thin-film deposition techniques based on nano- and femto-second pulsed laser
deposition. Three examples of thin-film fabrication for near-IR devices using Er3+-ion doped glass-on-GaAs,
Er3+-ion doped glass-polydimethyl silane (PDMS) polymer, and Tm3+-doped nano-silicon thin films and gain
medium waveguides are discussed.
The modelling tools are used a priori for waveguide engineering for ascertaining the extent to which the
structural incompatibility due to mismatch strain can be minimized. The structure and spectroscopic properties of
Er3+- ion doped thin films on silica, polymer, and semiconductor GaAs substrates were examined in detail and are
reported. We demonstrate the formation of glass-polymer superlattice structures for waveguide fabrication for
overcoming the solubility limits of Er3+-ions in PDMS polymers. For inscribing waveguides in superlattice
structures and nano silicon structures, the ablation machining using fs-pulsed Ti-sapphire laser was used, and the

Gaps in the Child Tuberculosis Care Cascade in 32 Rural Communities in Uganda and Kenya.
Background:Reducing tuberculosis (TB) deaths among children requires a better understanding of the gaps in the care cascade from TB diagnosis to treatment completion. We sought to assess the child TB care cascade in 32 rural communities in Uganda and Kenya using programmatic data. Methods:This is a retrospective cohort study of 160,851 children (ages <15 years) living in 12 rural communities in Kenya and 22 in Uganda. We reviewed national TB registries from health centers in and adjacent to the 32 communities, and we included all child TB cases recorded from January 1, 2013 to June 30, 2016. To calculate the first step of the child TB care cascade, the number of children with active TB, we divided the number of reported child TB diagnoses by the 2015 World Health Organization (WHO) child TB case detection ratio for Africa of 27%. The remaining components of the Child TB Care Cascade were ascertained directly from the TB registries and included: diagnosed with TB, started on TB treatment, and completed TB treatment. Results:In two and a half years, a total of 42 TB cases were reported among children living in 32 rural communities in Uganda and Kenya. 40% of the children were co-infected with HIV. Using the WHO child TB case detection ratio, we calculated that 155 children in this cohort had TB during the study period. Of those 155 children, 42 were diagnosed and linked to TB care, 42 were started on treatment, and 31 completed treatment. Among the 42 children who started TB treatment, reasons for treatment non-completion were loss to follow up (7%), death (5%), and un-recorded reasons (5%). Overall, 20% (31/155) of children completed the child TB care cascade. Conclusion:In 32 rural communities in Uganda and Kenya, we estimate that 80% of children with TB fell off the care cascade. Reducing morbidity and mortality from child TB requires strengthening of the child TB care cascade from diagnosis through treatment completion
Population levels and geographical distribution of HIV RNA in rural Ugandan and Kenyan communities, including serodiscordant couples: a cross-sectional analysis.
BackgroundAs sub-Saharan Africa transitions to a new era of universal antiretroviral therapy (ART), up-to-date assessments of population-level HIV RNA suppression are needed to inform interventions to optimise ART delivery. We sought to measure population viral load metrics to assess viral suppression and characterise demographic groups and geographical locations with high-level detectable viraemia in east Africa.MethodsThe Sustainable East Africa Research in Community Health (SEARCH) study is a cluster-randomised controlled trial of an HIV test-and-treat strategy in 32 rural communities in Uganda and Kenya, selected on the basis of rural setting, having an approximate population of 10â000 people, and being within the catchment area of a President's Emergency Plan for AIDS Relief-supported HIV clinic. During the baseline population assessment in the SEARCH study, we did baseline HIV testing and HIV RNA measurement. We analysed stable adult (aged âĽ15 years) community residents. We defined viral suppression as a viral load of less than 500 copies per mL. To assess geographical sources of transmission risk, we established the proportion of all adults (both HIV positive and HIV negative) with a detectable viral load (local prevalence of viraemia). We defined transmission risk hotspots as geopolitical subunits within communities with an at least 5% local prevalence of viraemia. We also assessed serodiscordant couples, measuring the proportion of HIV-positive partners with detectable viraemia. The SEARCH study is registered with ClinicalTrials.gov, number NCT01864603.FindingsBetween April 2, 2013, and June 8, 2014, of 303â461 stable residents, we enumerated 274â040 (90¡3%), of whom 132â030 (48¡2%) were adults. Of these, 117â711 (89¡2%) had their HIV status established, of whom 11â964 (10¡2%) were HIV positive. Of these, we measured viral load in 8828 (73¡8%) people. Viral suppression occurred in 3427 (81¡6%) of 4202 HIV-positive adults on ART and 4490 (50¡9%) of 8828 HIV-positive adults. Regional viral suppression among HIV-positive adults occurred in 881 (48¡2%) of 1827 people in west Uganda, 516 (45¡0%) of 1147 in east Uganda, and 3093 (52¡8%) of 5854 in Kenya. Transmission risk hotspots occurred in three of 21 parishes in west Uganda and none in east Uganda and in 24 of 26 Kenya geopolitical subunits. In Uganda, 492 (2¡9%) of 16â874 couples were serodiscordant: in 287 (58¡3%) of these couples, the HIV-positive partner was viraemic (and in 69 [14¡0%], viral load was >100â000 copies per mL). In Kenya, 859 (10¡0%) of 8616 couples were serodiscordant: in 445 (53¡0%) of these couples, the HIV-positive partner was viraemic (and in 129 [15%], viral load was >100â000 copies per mL).InterpretationBefore the start of the SEARCH trial, 51% of east African HIV-positive adults had viral suppression, reflecting ART scale-up efforts to date. Accommodation and vergence response gains to different near cues characterize specific esotropias
Aim. To describe preliminary findings of how the profile of the use of blur, disparity and proximal cues varies between non-strabismic groups and those with different types of esotropia.
Design. Case control study
Methodology. A remote haploscopic photorefractor measured simultaneous convergence and accommodation to a range of targets containing all combinations of binocular disparity, blur and proximal (looming) cues. 13 constant esotropes, 16 fully accommodative esotropes, and 8 convergence excess esotropes were compared with age and refractive error matched controls, and 27 young adult emmetropic controls. All wore full refractive correction if not emmetropic. Response AC/A and CA/C ratios were also assessed.
Results. Cue use differed between the groups. Even esotropes with constant suppression and no binocular vision (BV) responded to disparity in cues. The constant esotropes with weak BV showed trends for more stable responses and better vergence and accommodation than those without any BV. The accommodative esotropes made less use of disparity cues to drive accommodation (p=0.04) and more use of blur to drive vergence (p=0.008) than controls. All esotropic groups failed to show the strong bias for better responses to disparity cues found in the controls, with convergence excess esotropes favoring blur cues. AC/A and CA/C ratios existed in an inverse relationship in the different groups. Accommodative lag of >1.0D at 33cm was common (46%) in the pooled esotropia groups compared with 11% in typical children (p=0.05).
Conclusion. Esotropic children use near cues differently from matched non-esotropic children in ways characteristic to their deviations. Relatively higher weighting for blur cues was found in accommodative esotropia compared to matched controls
Long-term annual and monthly changes in mysids and caridean decapods in a macrotidal estuarine environment in relation to climate change and pollution
Š 2018 Elsevier B.V. A 26-year time series of monthly samples from the water intake of a power station has been used to analyse the trends exhibited by number of species, total abundance, and composition of the mysids and caridean decapods in the inner Bristol Channel. During this period, annual water temperatures, salinities and the North Atlantic Oscillation Index (NAOI) in winter did not change significantly, whereas annual NAOI declined. Annual mean monthly values for the number of species and total abundance both increased over the 26 years, but these changes were not correlated with any of the measured physico-chemical/climatic factors. As previous studies demonstrated that, during a similar period, metal concentrations in the Severn Estuary and Bristol Channel (into which that estuary discharges) declined and water quality increased, it is proposed that the above changes are due to an improved environment. The fauna was dominated by the mysids Mesopodopsis slabberi and Schistomysis spiritus, which collectively contributed 94% to total abundance. Both species, which were represented by juveniles, males, non-brooding females and brooding females, underwent statistically-indistinguishable patterns of change in abundance over the 26 years. When analysis was based on the abundances of the various species, the overall species composition differed significantly among years and changed serially with year. When abundances were converted to percentage compositions, this pattern of seriation broke down, demonstrating that changes in abundance and not percentage composition were responsible for the seriation. As with the number and abundance of species, changes in composition over the 26 years were not related to any of the physico-chemical/climatic factors tested. Species composition changed monthly in a pronounced cyclical manner throughout the year, due to statistically different time-staggered changes in the abundance of each species. This cyclicity was related most strongly to salinity
An examination of the self-referent executive processing model of test anxiety: control, emotional regulation, self-handicapping, and examination performance
According to the self-referent executive processing (S-REF) model, test anxiety develops from interactions between three systems: executive self-regulation processes, self-beliefs, and maladaptive situational interactions. Studies have tended to examine one system at a time, often in conjunction with how test anxiety relates to achievement outcomes. The aim of this study was to enable a more thorough test of the S-REF model by examining one key construct from each of these systems simultaneously. These were control (a self-belief construct), emotional regulation through suppression and reappraisal (an executive process), and self-handicapping (a maladaptive situational interaction). Relations were examined from control, emotional regulation, and self-handicapping to cognitive test anxiety (worry), and subsequent examination performance on a high-stakes test. Data were collected from 273 participants in their final year of secondary education. A structural equation model showed that higher control was indirectly related to better examination performance through lower worry, higher reappraisal was indirectly related to worse examination performance through higher worry, and higher self-handicapping was related to worse examination performance through lower control and higher worry. These findings suggest that increasing control and reducing self-handicapping would be key foci for test anxiety interventions to incorporate. Š 2018 The Author(s

Student and school factors associated with school suspension: A multilevel analysis of students in Victoria, Australia and Washington State, United States
One of the common issues schools face is how best to handle challenging student behaviors such as violent behavior, antisocial behavior, bullying, school rule violations, and interrupting other students' learning. School suspension may be used to remove students engaging in challenging behaviors from the school for a period of time. However, the act of suspending students from school may worsen rather than improve their behavior. Research shows that suspensions predict a range of student outcomes, including crime, delinquency, and drug use. It is therefore crucial to understand the factors associated with the use of school suspension, particularly in sites with different policy approaches to problem behaviors. This paper draws on data from state-representative samples of 3129 Grade 7 and 9 students in Washington State, United States and Victoria, Australia sampled in 2002. Multilevel modeling examined student and school level factors associated with student-reported school suspension. Results showed that both student (being male, previous student antisocial and violent behavior, rebelliousness, academic failure) and school (socioeconomic status of the school, aggregate measures of low school commitment) level factors were associated with school suspension and that the factors related to suspension were similar in the two states. The implications of the findings for effective school behavior management policy are that, rather than focusing only on the student, both student and school level factors need to be addressed to reduce the rates of school suspension
