The field K((G)) of generalized power series with coefficients in the field K of characteristic 0 and exponents in the ordered additive abelian group G plays an important role in the study of real closed fields. Conway and Gonshor (see [2, 4]) considered the problem of existence of non-standard irreducible (respectively prime) elements in the huge "ring” of omnific integers, which is indeed equivalent to the existence of irreducible (respectively prime) elements in the ring K((G ≤0)) of series with non-positive exponents. Berarducci (see [1]) proved that K((G ≤0)) does have irreducible elements, but it remained open whether the irreducibles are prime i.e., generate a prime ideal. Le tableau croisé, un outil pour la maîtrise de l'accident collectif Dr J.-C. Pitteloud, Sion (Suisse) Lorsqu'on est confronté Á un accident Á plusieurs victimes, il est souvent difficile de garder en tête Á la fois les victimes et les moyens dont vous disposez. Le tableau croisé est un formulaire qui permet de diriger et de documenter efficacement une intervention sur un accident Á plusieurs victimes. Le principe du tableau croisé est que vous allez regrouper les informations sur les victimes, comme leur numéro et leur catégorie de triage sur les lignes horizontales de votre tableau (l'axe des X). Les information sur les vecteurs (heure d'appel, heure d'arrivée) seront notées sur les colonnes (l'axe des y). L'heure de prise en charge de chaque victime par une unité est notée Á l'intersection correspondante. L'heure d'Évacuation vers l'hôpital est notée au bout de chaque colonne et de chaque ligne. Ce formulaire permet de garder une vue d'ensemble des opérations de secours Á victime ainsi que de documenter l'action de chaque Équipe et le devenir de chaque patient. Il est utilisable dû l'arrivée de la première Équipe sur place, puisqu'il suffit d'un stylo et d'une feuille de papier pour le réaliser. Dans notre expérience, une feuille de format A4 européen permet de gérer une intervention pour une dizaine de victime, une feuille de format A5 jusqu'Á une trentain

Il met en lumière le cheminement permettant au MSP d’appliquer « l’approche positive de la personne » selon Denise Fraser et Lucien Labbé. Non seulement cette recherche donne des éléments pour analyser des situations problématiques, mais elle apporte des stratégies d’intervention. L’accent est mis sur l’attitude que le professionnel peut adopter afin de faire évoluer un processus de réciprocité et d’interdépendance avec la personne. La posture professionnelle empruntée par l’intervenant lui permettra de se positionner et de rediriger la personne vers des alternatives positives pour gérer sa vie

Identifying the unmet health needs of patients with congenital hypogonadotropic hypogonadism using a web-based needs assessment: implications for online interventions and peer-to-peer support.

BACKGROUND: Patients with rare diseases such as congenital hypogonadotropic hypogonadism (CHH) are dispersed, often challenged to find specialized care and face other health disparities. The internet has the potential to reach a wide audience of rare disease patients and can help connect patients and specialists. Therefore, this study aimed to: (i) determine if web-based platforms could be effectively used to conduct an online needs assessment of dispersed CHH patients; (ii) identify the unmet health and informational needs of CHH patients and (iii) assess patient acceptability regarding patient-centered, web-based interventions to bridge shortfalls in care. METHODS: A sequential mixed-methods design was used: first, an online survey was conducted to evaluate health promoting behavior and identify unmet health and informational needs of CHH men. Subsequently, patient focus groups were held to explore specific patient-identified targets for care and to examine the acceptability of possible online interventions. Descriptive statistics and thematic qualitative analyses were used. RESULTS: 105 male participants completed the online survey (mean age 37 ± 11, range 19-66 years) representing a spectrum of patients across a broad socioeconomic range and all but one subject had adequate healthcare literacy. The survey revealed periods of non-adherence to treatment (34/93, 37%) and gaps in healthcare (36/87, 41%) exceeding one year. Patient focus groups identified lasting psychological effects related to feelings of isolation, shame and body-image concerns. Survey respondents were active internet users, nearly all had sought CHH information online (101/105, 96%), and they rated the internet, healthcare providers, and online community as equally important CHH information sources. Focus group participants were overwhelmingly positive regarding online interventions/support with links to reach expert healthcare providers and for peer-to-peer support. CONCLUSION: The web-based needs assessment was an effective way to reach dispersed CHH patients. These individuals often have long gaps in care and struggle with the psychosocial sequelae of CHH. They are highly motivated internet users seeking information and tapping into online communities and are receptive to novel web-based interventions addressing their unmet needs

Platon et les lois de la nature

Dans cet article, j’examinerai la description du modèle (paradeigma) dans le Timée. Après avoir fait quelques commentaires à propos du contexte dans lequel le modèle est introduit en tant que composante de la sphère intelligible, je poserai la question de l’extension exacte de cette sphère dans le Timée. Puis, je suggèrerai que le modèle peut être défini comme la réalisation optimale d’une fonctionnalité. Cela semble être en effet une alternative crédible afin de comprendre ce que sont les Formes dans la métaphysique de Platon. Néanmoins, cette conception n’est pas exempte de difficultés puisque Platon admet également une conception plus substantielle du modèle dans le Timée (ce dernier élément ne sera pas investigué dans cet article). Cela dit, une vision fonctionnaliste du modèle permet de donner une justification satisfaisante à l’idée selon laquelle le cosmos est la meilleure réalisation démiurgique possible

Contrasted host specificity of gut and endosymbiont bacterial communities in alpine grasshoppers and crickets.

Bacteria colonize the body of macroorganisms to form associations ranging from parasitic to mutualistic. Endosymbiont and gut symbiont communities are distinct microbiomes whose compositions are influenced by host ecology and evolution. Although the composition of horizontally acquired symbiont communities can correlate to host species identity (i.e. harbor host specificity) and host phylogeny (i.e. harbor phylosymbiosis), we hypothesize that the microbiota structure of vertically inherited symbionts (e.g. endosymbionts like Wolbachia) is more strongly associated with the host species identity and phylogeny than horizontally acquired symbionts (e.g. most gut symbionts). Here, using 16S metabarcoding on 336 guts from 24 orthopteran species (grasshoppers and crickets) in the Alps, we observed that microbiota correlated to host species identity, i.e. hosts from the same species had more similar microbiota than hosts from different species. This effect was ~5 times stronger for endosymbionts than for putative gut symbionts. Although elevation correlated with microbiome composition, we did not detect phylosymbiosis for endosymbionts and putative gut symbionts: closely related host species did not harbor more similar microbiota than distantly related species. Our findings indicate that gut microbiota of studied orthopteran species is more correlated to host identity and habitat than to the host phylogeny. The higher host specificity in endosymbionts corroborates the idea that-everything else being equal-vertically transmitted microbes harbor stronger host specificity signal, but the absence of phylosymbiosis suggests that host specificity changes quickly on evolutionary time scales

Psychosexual development in men with congenital hypogonadotropic hypogonadism on long-term treatment: a mixed methods study.

INTRODUCTION: Congenital hypogonadotropic hypogonadism (CHH) is a rare, genetic, reproductive endocrine disorder characterized by absent puberty and infertility. Limited information is available on the psychosocial impact of CHH and psychosexual development in these patients. AIM: The aim of this study was to determine the impact of CHH on psychosexual development in men on long-term treatment. METHODS: A sequential mixed methods explanatory design was used. First, an online survey (quantitative) was used to quantify the frequency of psychosexual problems among CHH men. Second, patient focus groups (qualitative) were conducted to explore survey findings in detail and develop a working model to guide potential nursing and interdisciplinary interventions. MAIN OUTCOME MEASURES: Patient characteristics, frequency of body shame, difficulty with intimate relationships, and never having been sexually active were assessed. Additionally, we collected subjective patient-reported outcomes regarding the impact of CHH on psychological/emotional well-being, intimate relationships, and sexual activity. RESULTS: A total of 101 CHH men on long-term treatment (>1 year) were included for the analysis of the online survey (mean age 37 ± 11 years, range 19-66, median 36). Half (52/101, 51%) of the men had been seen at a specialized academic center and 37/101 (37%) reported having had fertility-inducing treatment. A high percentage of CHH men experience psychosexual problems including difficulty with intimate relationships (70%) and body image concerns/body shame (94/101, 93%), and the percentage of men never having been sexually active is five times the rate in a reference group (26% vs. 5.4%, P < 0.001). Focus groups revealed persisting body shame and low self-esteem despite long-term treatment that has lasting impact on psychosexual functioning. CONCLUSIONS: CHH men frequently experience psychosexual problems that pose barriers to intimate relationships and initiating sexual activity. These lingering effects cause significant distress and are not ameliorated by long-term treatment. Psychosexual assessment in CHH men with appropriate psychological support and treatment should be warranted in these patients. Dwyer AA, Quinton R, Pitteloud N, and Morin D.
