From door to door. healthcare in mutual aid associations: the case of ASMECL
Entre o final do século XVII e o início do século XX as associações mutualistas afirmaram-se como as principais instituições que prestavam socorros de saúde às classes mais desfavorecidas. O objetivo deste texto é analisar o papel do mutualismo nos modelos de cuidados de saúde existentes em Portugal e enfatizar a necessidade de aprofundar a análise dos mecanismos de proteção social desenvolvidos no período que medeia entre o final do Antigo Regime e a fundação das primeiras instituições mutualistas.
A metodologia seguida privilegia um caso de estudo, a evolução da Associação de Socorros Mútuos dos Empregados no Comércio de Lisboa, a primeira associação mutualista portuguesa a desenvolver infraestruturas especificamente dedicadas à prestação de cuidados clínicos.From the end of the 18th century and the early 20th century, mutual associations asserted themselves as the main providers of health care to the most disadvantaged classes. However, only a small number created specific infrastructures to provide this care. Stunting and wasting in a growing economy : biological living standards in Portugal,1924-1994
Portugal's real income per head grew by a factor of eight during the second half of the twentieth century, a period of fast convergence towards Western European standards of living. We use a new sample of about 2,000 children to document trends in the prevalence of stunting and wasting in the city of Lisbon between 1945 and 1994. We find that stunting and wasting fell quickly in the 1950s and 1960s. This happened for males and females, and for infants (0 to 36 months of age) as well as children (2 to 10 years of age). We additionally use a sample of 17,000 young adult males covering the entire country which shows a similar decrease in the incidence of wasting and stunting, with the expected time lag. We discuss these trends in relation to changes in income and public policy which affected the ontogenetic environment of children

We use a new sample of about 3,400 infants and children living in Lisbon to document trends in the prevalence of stunting and wasting between 1906 and 1994. We find that stunting and wasting fell quickly from around 1950, for both males and females. We additionally use a sample of more than 26,000 young adult males covering the entire country, which shows a consistent decrease in wasting and stunting with the expected time lag. We discuss these trends in relation to changes in income and public policy, which affected the ontogenetic environment of children. Entre mutualismo e capitalismo : os caminhos do Montepio Geral (1840-1930)
Esta tese apresenta um estudo de caso do Montepio Geral, enquanto instituição
mutualista, desde a sua fundação em 1840 até 1930, quando o Estado Novo introduz uma
nova legislação. O mutualismo entrou em Portugal no início do século XIX,
desenvolvendo-se a partir de meados desse século com grande vigor, tendo sido criadas
centenas de instituições num curto espaço de tempo, especialmente nos centros urbanos.
Por volta de 1930, a maior parte destas instituições tinha reduzida massa associativa,
poucos capitais e oferecia vantagens limitadas, sendo o Montepio Geral uma das poucas
exceções. O Montepio Geral não só era a maior associação, como também detinha uma
das principais instituições bancárias portuguesas. Este trajeto é bastante diferente do que
aconteceu na maior parte dos países europeus, onde o mutualismo evoluiu no sentido de
mais instituições oferecerem uma proteção social mais alargada, mas em Portugal, poucas
foram as instituições que o conseguiram. Como tal, esta tese realiza um estudo de caso
sobre o Montepio Geral de forma a compreender as razões para a sua resiliência, tanto
enquanto associação mutualista, mas também como organização bancária. A tese começa
por situar o Montepio no contexto do mutualismo português. De seguida, procede à
análise dos vários mecanismos internos de governação e de tomada de decisão,
procurando identificar as razões que o conduziram a afirmar-se no final do século XIX
como a principal associação deste tipo. De seguida, analisa a forma como a sua caixa
económica, concebida inicialmente como uma fonte de rendimento complementar, se
transformou numa instituição-chave do sistema bancário português, um caminho que não
estava projetado em 1840. Por fim, a tese procura analisar estes temas à luz da Teoria
Institucional. Esta análise revela que a singularidade do caso em estudo se deve a um
modelo de governação flexível mas suficientemente robusto para evitar as pressões
redistributivas dos sócios e a uma caixa económica que se afirmou como uma verdadeira
organização bancária e não apenas como uma instituição de suporte desenhada sob o
modelo de caixas económicas europeu dominante no século XIX. Nesse sentido, a tese
confirma a hipótese já enunciada no século XIX por autores como Costa Godolfim, que a
principal ameaça às associações mutualistas portuguesas eram os seus deficientes
mecanismos de governação.This dissertation provides a case study of a Portuguese mutual-aid society in the
modern period - Montepio Geral – from its foundation in 1840 to 1930 (when the
dictatorial Estado Novo imposed a new institutional framework for these associations).
The ideas and practices of mutual aid were introduced in Portugal by 1800 and spread
rapidly across the country, mostly in urban areas. In the last quarter of the nineteenth
century, the country was home to a few hundreds of these associations. However, by
1930, the vast majority of these institutions was small, lacked funds and offered few
benefits to its members. Montepio Geral was one of the few exceptions to this rule, as it
had grown into the largest mutual-aid association and it owned a major savings bank that
was then a key player in the Portuguese banking system. The trajectory of mutual-aid in
Portugal was thus different from most European countries, where a pool of strong,
competing institutions offered social protection. By pinpointing the factors behind the
success of Montepio Geral, we can gain a better understanding of the fate of mutual-aid
in Portugal. Thus, after setting Montepio Geral within the context of Portuguese mutualaid
societies in part I, we observe how its governance system and decision-making
process contributed to the largest mutual aid society in the country. Part III studies how
the savings bank (Caixa Económica Montepio Geral) owned by the society unintendedly
went from a subsidiary source of revenue to a major player in the national banking system,
something the founders did not design nor expect. Lastly part, we analyse the evolution
of this society under the light of Institutional Theory. Our study concludes that the
outstanding trajectory of the Montepio Geral was possible due to two factors: 1) a
governance system that proved both flexible enough to adapt to different demands and
strong and independent enough to resist the redistributive pressures of the members two
conditions that smaller associations hardly met; 2) a savings bank, which was designed
as a mutual-aid institution on its own became competitive and ran like a commercial bank.
As such, to some extent this dissertation confirms an hypothesis that was advanced by
some nineteenth-century authors, notably Costa Godolfim, who claimed that the greatest
problem facing Portuguese mutual-aid societies was their poor governance system
