    HOPE (SOLTI-1903) breast cancer study: real-world, patient-centric, clinical practice study to assess the impact of genomic data on next treatment decision-choice in patients with locally advanced or metastatic breast cancer

    Background Metastatic breast cancer (mBC) causes nearly all BC-related deaths. Next-generation sequencing (NGS) technologies allow for the application of personalized medicine using targeted therapies that could improve patients' outcomes. However, NGS is not routinely used in the clinical practice and its cost induces access-inequity among patients. We hypothesized that promoting active patient participation in the management of their disease offering access to NGS testing and to the subsequent medical interpretation and recommendations provided by a multidisciplinary molecular advisory board (MAB) could contribute to progressively overcome this challenge. We designed HOPE (SOLTI-1903) breast cancer trial, a study where patients voluntarily lead their inclusion through a digital tool (DT). The main objectives of HOPE study are to empower mBC patients, gather real-world data on the use of molecular information in the management of mBC and to generate evidence to assess the clinical utility for healthcare systems.Trial design After self-registration through the DT, the study team validates eligibility criteria and assists patients with mBC in the subsequent steps. Patients get access to the information sheet and sign the informed consent form through an advanced digital signature. Afterwards, they provide the most recent (preferably) metastatic archival tumor sample for DNA-sequencing and a blood sample obtained at the time of disease progression for ctDNA analysis. Paired results are reviewed by the MAB, considering patient's medical history. The MAB provides a further interpretation of molecular results and potential treatment recommendations, including ongoing clinical trials and further (germline) genetic testing. Participants self-document their treatment and disease evolution for the next 2 years. Patients are encouraged to involve their physicians in the study. HOPE also includes a patient empowerment program with educational workshops and videos about mBC and precision medicine in oncology. The primary endpoint of the study was to describe the feasibility of a patient-centric precision oncology program in mBC patients when a comprehensive genomic profile is available to decide on a subsequent line of treatment

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Introduction: Physical exercise enhances the psychophysical well-being of older adults. However, sociodemographic varia- bles that may either facilitate or impede the engagement in physical activities, such as education, income, gender, and marital status, have been insufficiently explored. Furthermore, limited attention has been given to these variables concerning specific physical activi- ties, including gymnastics, aquatic activities, yoga, pilates, and dance. Objective: The aim of this study is to delineate the sociodemo- graphic profile of physically active older adults based on the specific type of physical activity they participate in. Procedure: A sample of 358 individuals aged over 60 years (M = 69.66, SD = 4.74) participated in the study, utilizing the Quality of Life Scale and a sociodemographic questionnaire, specifically the CUBRECAVI. Results: The findings indicate that older adults engaged in gymnastics are more likely to possess lower academic qualifications (p < .001), lower income (p < .001), and be women (p = .003). Additionally, single and divorced individuals participate in dance activities at a higher rate compared to other activities (p = .004). Conclusion: In summary, the choice of physical activity among older adults is associated with their educational level, income, gender, and marital status.2023-2

    Fungemia in Hospitalized Adult Patients with Hematological Malignancies: Epidemiology and Risk Factors

This is a retrospective cohort of adult patients with HM and fungemia between 2012 and 2019 in institutions of Bogotá, Colombia. The epidemiological, clinical, and microbiological characteristics are described, and risk factors related to mortality are analyzed. One hundred five patients with a mean age of 48 years (SD 19.0) were identified, 45% with acute leukemia and 37% with lymphomas. In 42%, the HM was relapsed/refractory, 82% ECOG > 3, and 35% received antifungal prophylaxis; 57% were in neutropenia, with an average duration of 21.8 days. In 86 (82%) patients, Candida spp. was identified, and other yeasts in 18%. The most frequent of the isolates were non-albicans Candida (61%), C. tropicalis (28%), C. parapsilosis (17%), and C. krusei (12%). The overall 30-day mortality was 50%. The survival probability at day 30 in patients with leukemia vs. lymphoma/multiple myeloma (MM0 group was 59% (95% CI 46–76) and 41% (95% CI 29–58), p = 0.03, respectively. Patients with lymphoma or MM (HR 1.72; 95% CI 0.58–2.03) and ICU admission (HR 3.08; 95% CI 1.12–3.74) were associated with mortality. In conclusion, in patients with HM, non-albicans Candida species are the most frequent, and high mortality was identified; moreover, lymphoma or MM and ICU admission were predictors of mortality.https://orcid.org/0000-0001-6396-3267https://orcid.org/0000-0001-5363-5729Revista Internacional - IndexadaA1N

    El trabajo obtuvo un Premio Tomás García Verdejo a las buenas prácticas educativas en la Comunidad Autónoma de Extremadura para el curso 2018/2019, Modalidad B.Se describe una experiencia llevada a cabo en el IES El Brocense (Cáceres) que consistió en desarrollar distintas acciones utilizando como recurso el cine. Además de aprender contenidos de las asignaturas impartidas a través del cine, se perseguía fomentar la convivencia escolar y promover la igualdad entre hombres y mujeres. Algunas de las actividades realizadas fueron: jornadas sobre el cine, elaboración y grabación de cortos cinematográficos, asistencia a un musical, participación en conferencias y jornadas sobre cine, elaboración de carteles, etc.ExtremaduraES
