Glomerular epithelial cell products stimulate mesangial cell proliferation in culture
Glomerular epithelial cell products stimulate mesangial cell proliferation in culture. Glomerular epithelial cells (GEC) and mesangial cells (MC) are both involved in glomerular diseases. To elucidate potential interactions between these glomerular cell types, we examined whether products of GEC affect the proliferative activity of MC. We found that cultured rat GEC secrete soluble factors into the supernate (GEC-CM) that induce proliferation of quiescent rat MC. The mitogenic activity was trypsin sensitive and partially heat-labile. Biochemical analysis of GEC-CM by gel filtration HPLC, reverse phase HPLC, and isoelectric focusing revealed at least three mitogenic fractions as well as inhibitory activity present in GEC-CM. Competitive binding assays with 125I-labeled PDGF did not show significant amounts of PDGF in GEC-CM. The biochemical features of the GEC-derived MC growth factors are distinct from IL-6, PDGF, bFGF, and endothelin, previously described GEC-derived MC growth factors. Additionally, significant contributions of known growth factors such as IL-1, IL-2, IL-3, IL-4, IL-5, TNFα, TGFβ, and GM-CSF are unlikely. The results indicate that GEC produce several biochemically-distinct MC growth regulators. While these epithelial cell-derived mitogens for MC require further characterization, they may play an important role in the regulation of MC replication, such as during embryogenesis and glomerular disease
Impact of daclizumab, low-dose cyclosporine, mycophenolate mofetil and steroids on renal function after kidney transplantation
Background. Early and long-term use of cyclosporine A (CsA) leads to increased risks of renal toxicity. We hypothesized that administration of daclizumab in combination with mycophenolate mofetil (MMF) allows a relevant reduction in the dose of CsA. Methods. We carried out a 3-year, prospective, randomized, controlled clinical multi-centre trial in 156 patients. The patients were randomized to standard treatment (CsA, MMF, steroids) or to high-dose daclizumab (first dose: 2 mg/kg), in combination with low-dose CsA, MMF and steroids. We maintained the mean CsA levels of daclizumab patients at 57% of standard patients (132 versus 216 ng/ml) on Day 7 post-transplant, and 84% by 6 months. Results. Primary outcome, creatinine clearance (with imputation of informative dropouts) at 12 months, was significantly better in daclizumab-treated (34 ± 17) than standard patients (29 ± 17; P = 0.028, two sided). Only 5 cases of BPAR were recorded in the daclizumab compared to 22 in the standard group (P = 0.0016). Daclizumab patients had 91% event-free survival after 1 year compared to 66% in standard patients (P = 0.00017). Conclusion. We demonstrate here that high-dose daclizumab in combination with lower CsA levels in adult renal transplant recipients is as or more effective than standard regimen (CsA, MMF, steroids) and may result in better outcomes at 12 months post-transplant with no increase in adverse reaction

IL-17 Expression in the Time Course of Acute Anti-Thy1 Glomerulonephritis
Background Interleukin-17 (IL-17) is a new pro-inflammatory cytokine involved
in immune response and inflammatory disease. The main source of IL-17 is a
subset of CD4+ T-helper cells, but is also secreted by non-immune cells. The
present study analyzes expression of IL-17 in the time course of acute anti-
thy1 glomerulonephritis and the role of IL-17 as a potential link between
inflammation and fibrosis. Methods Anti-thy1 glomerulonephritis was induced
into male Wistar rats by OX-7 antibody injection. After that, samples were
taken on days 1, 5, 10 (matrix expansion phase), 15 and 20 (resolution phase).
PBS-injected animals served as controls. Proteinuria and histological matrixes
score served as the main markers for disease severity. In in vitro
experiments, NRK-52E cells were used. For cytokine expressions, mRNA and
protein levels were analyzed by utilizing RT-PCR, in situ hybridization and
immunofluorescence. Results Highest IL-17 mRNA-expression (6.50-fold vs. con;
p<0.05) was found on day 5 after induction of anti-thy1 glomerulonephritis
along the maximum levels of proteinuria (113 ± 13 mg/d; p<0.001), histological
glomerular-matrix accumulation (82%; p<0.001) and TGF-β1 (2.2-fold; p<0.05),
IL-6 mRNA expression (36-fold; p<0.05). IL-17 protein expression co-localized
with the endothelial cell marker PECAM in immunofluorescence. In NRK-52E
cells, co-administration of TGF-β1 and IL-6 synergistically up-regulated IL-17
mRNA 4986-fold (p<0.001). Conclusions The pro-inflammatory cytokine IL-17 is
up-regulated in endothelial cells during the time course of acute anti-thy1
glomerulonephritis. In vitro, NRK-52E cells secrete IL-17 under pro-fibrotic
and pro-inflammatory conditions
