    Vehicle Suspension Design Based on a Six-Bar Linkage

    A synthesis technique for designing novel vehicle suspension linkages based on the Watt I six-bar is presented. The goal is to maintain near vertical alignment of the wheels to the road during cornering. The complete suspension is analyzed as a symmetric planar 12-bar linkage with ground pivots located at the contact patches. The design procedure specifies the vehicle chassis orientation and the tire camber angles of the vehicle when cornering. As well, two task positions of the wheels with respect to the chassis are specified for suspension movement in straightaways. The result is 18 design equations with 18 unknowns that have a total degree of 2,097,152, though only 336 roots. An example design is presented

    Using computer simulation to improve patient flow at an outpatient internal medicine department

    This paper presents the use of discrete-event simulation to support process improvements at an outpatient internal medicine department. This department is significantly effective upon treating patients; however, patient waiting times tend to be longer and consequently patient satisfaction rates continue to decrease. With the aid of this technique, 3 improvement scenarios proposed by medical and administrative staff from this department were designed and simulated including changes related to installed capacity and an emphasis on physicians keeping to the schedule. Statistical analysis of output data evidenced which scenarios resulted in poor performance (statistically equal or higher waiting times) and which strategies caused lower waiting times. In this case, Scenario 3 was selected as the best improvement choice with 71.28 % and 19.28 % reduction in average waiting time and standard deviation respectively. With this approach, inefficient strategies can be avoided and real improvement alternatives can be identified

    Controlling the Movement of a TRR Spatial Chain With Coupled Six-Bar Function Generators for Biomimetic Motion

    This paper describes a synthesis technique that constrains a spatial serial chain into a single degree-of-freedom mechanism using planar six-bar function generators. The synthesis process begins by specifying the target motion of a serial chain that is parameterized by time. The goal is to create a mechanism with a constant velocity rotary input that will achieve that motion. To do this we solve the inverse kinematics equations to find functions of each serial joint angle with respect to time. Since a constant velocity input is desired, time is proportional to the angle of the input link, and each serial joint angle can be expressed as functions of the input angle. This poses a separate function generator problem to control each joint of the serial chain. Function generators are linkages that coordinate their input and output angles. Each function is synthesized using a technique that finds 11 position Stephenson II linkages, which are then packaged onto the serial chain. Using pulleys and the scaling capabilities of function generating linkages, the final device can be packaged compactly. We describe this synthesis procedure through the design of a biomimetic device for reproducing a flapping wing motion

    Intracardiac anatomical relationships and potential for streaming in double inlet left ventricles.

    The aim of this study was to gain better understanding of the variable anatomical features of double inlet left ventricle hearts without cavopulmonary connection that would potentially facilitate favorable streaming. Thirty-nine post-mortem specimens of double inlet left ventricle without cavopulmonary connection were investigated. The focus was on anatomical characteristics that could influence the flow and separation of deoxygenated and oxygenated blood in the ventricles. Elements of interest were the ventriculoarterial connection, the spatial relationship of the ventricles, the position and size of the great arteries, the ventricular septal defect, the presence of relative outflow tract stenosis and the relationship of the inflow and outflow tracts. The most common anatomy was a discordant ventriculoarterial connection with an anatomically left-sided morphologically right ventricle (n = 12, 31%). When looking at the pulmonary trunk/aorta ratio, 21 (72%) hearts showed no pulmonary stenosis relative to the aorta. The ventricular septal defect created a relative subpulmonary or subaortic stenosis in 13 (41%) cases. Sixteen (41%) hearts had a parallel relationship of the inflow and outflow tracts, facilitating separation of deoxygenated and oxygenated blood streams. On the other end of the spectrum were 10 (25%) hearts with a perpendicular relationship, which might lead to maximum mixing of the blood streams. The relationship of the inflow and outflow tracts as well as the presence of (sub-) pulmonary stenosis might play a crucial role in the distribution of blood in double inlet left ventricle hearts. Additional in vivo studies will be necessary to confirm this postulation

    Oestradiol and prostaglandin F2 alpha regulate sexual displays in females of a sex-role reversed fish

    The mechanisms regulating sexual behaviours in female vertebrates are still poorly understood, mainly because in most species sexual displays in females are more subtle and less frequent than displays in males. In a sex-role reversed population of a teleost fish, the peacock blenny Salaria pavo, an external fertilizer, females are the courting sex and their sexual displays are conspicuous and unambiguous. We took advantage of this to investigate the role of ovarian-synthesized hormones in the induction of sexual displays in females. In particular, the effects of the sex steroids oestradiol (E2) and testosterone (T) and of the prostaglandin F2 alpha (PGF2 alpha) were tested. Females were ovariectomized and their sexual behaviour tested 7 days (sex steroids and PGF2 alpha) and 14 days (sex steroids) after ovariectomy by presenting females to an established nesting male. Ovariectomy reduced the expression of sexual behaviours, although a significant proportion of females still courted the male 14 days after the ovary removal. Administration of PGF2 alpha to ovariectomized females recovered the frequency of approaches to the male's nest and of courtship displays towards the nesting male. However, E2 also had a positive effect on sexual behaviour, particularly on the frequency of approaches to the male's nest. T administration failed to recover sexual behaviours in ovariectomized females. These results suggest that the increase in E2 levels postulated to occur during the breeding season facilitates female mate-searching and assessment behaviours, whereas PGF2 alpha acts as a short-latency endogenous signal informing the brain that oocytes are mature and ready to be spawned.     A current perspective on cancer immune therapy: Step‑by‑step approach to constructing the magic bullet

    Immunotherapy is the new trend in cancer treatment due to the selectivity, long lasting effects, and demonstrated improved overall survival and tolerance, when compared to patients treated with conventional chemotherapy. Despite these positive results, immunotherapy is still far from becoming the perfect magic bullet to fight cancer, largely due to the facts that immunotherapy is not effective in all patients nor in all cancer types. How and when will immunotherapy overcome these hurdles? In this review we take a step back to walk side by side with the pioneers of immunotherapy in order to understand what steps need to be taken today to make immunotherapy effective across all cancers. While early scientists, such as Coley, elicited an unselective but effective response against cancer, the search for selectivity pushed immunotherapy to the side in favor of drugs focused on targeting cancer cells. Fortunately, the modern era would revive the importance of the immune system in battling cancer by releasing the brakes or checkpoints (anti-CTLA-4 and anti-PD-1/PD-L1) that have been holding the immune system at bay. However, there are still many hurdles to overcome before immunotherapy becomes a universal cancer therapy. For example, we discuss how the redundant and complex nature of the immune system can impede tumor elimination by teeter tottering between different polarization states: one eliciting anti-cancer effects while the other promoting cancer growth and invasion. In addition, we highlight the incapacity of the immune system to choose between a fight or repair action with respect to tumor growth.     Expression of Drug Targets in Patients Treated with Sorafenib, Carboplatin and Paclitaxel

    Introduction: Sorafenib, a multitarget kinase inhibitor, targets members of the mitogen-activated protein kinase (MAPK) pathway and VEGFR kinases. Here we assessed the association between expression of sorafenib targets and biomarkers of taxane sensitivity and response to therapy in pre-treatment tumors from patients enrolled in ECOG 2603, a phase III comparing sorafenib, carboplatin and paclitaxel (SCP) to carboplatin, paclitaxel and placebo (CP). Methods: Using a method of automated quantitative analysis (AQUA) of in situ protein expression, we quantified expression of VEGF-R2, VEGF-R1, VEGF-R3, FGF-R1, PDGF-Rβ, c-Kit, B-Raf, C-Raf, MEK1, ERK1/2, STMN1, MAP2, EB1 and Bcl-2 in pretreatment specimens from 263 patients. Results: An association was found between high FGF-R1 and VEGF-R1 and increased progression-free survival (PFS) and overall survival (OS) in our combined cohort (SCP and CP arms). Expression of FGF-R1 and VEGF-R1 was higher in patients who responded to therapy ((CR+PR) vs. (SD+PD+ un-evaluable)). Conclusions: In light of the absence of treatment effect associated with sorafenib, the association found between FGF-R1 and VEGF-R1 expression and OS, PFS and response might reflect a predictive biomarker signature for carboplatin/paclitaxel-based therapy. Seeing that carboplatin and pacitaxel are now widely used for this disease, corroboration in another cohort might enable us to improve the therapeutic ratio of this regimen. © 2013 Jilaveanu et al

    Cross-sectional survey of users of internet depression communities

    Background: Internet-based depression communities provide a forum for individuals to communicate and share information and ideas. There has been little research into the health status and other characteristics of users of these communities. Methods: Online cross-sectional survey of Internet depression communities to identify depressive morbidity among users of Internet depression communities in six European countries; to investigate whether users were in contact with health services and receiving treatment; and to identify user perceived effects of the communities. Results: Major depression was highly prevalent among respondents (varying by country from 40% to 64%). Forty-nine percent of users meeting criteria for major depression were not receiving treatment, and 35% had no consultation with health services in the previous year. Thirty-six percent of repeat community users who had consulted a health professional in the previous year felt that the Internet community had been an important factor in deciding to seek professional help. Conclusions: There are high levels of untreated and undiagnosed depression in users of Internet depression communities. This group represents a target for intervention. Internet communities can provide information and support for stigmatizing conditions that inhibit more traditional modes of information seeking
