    Agro-economic and socio-environmental assessments of food and virtual water trades of Iran

    Ending hunger and ensuring food security are among targets of 2030 s SDGs. While food trade and the embedded (virtual) water (VW) may improve food availability and accessibility for more people all year round, the sustainability and efficiency of food and VW trade needs to be revisited. In this research, we assess the sustainability and efficiency of food and VW trades under two food security scenarios for Iran, a country suffering from an escalating water crisis. These scenarios are (1) Individual Crop Food Security (ICFS), which restricts calorie fulfillment from individual crops and (2) Crop Category Food Security (CCFS), which promotes eating local by suggesting food substitution within the crop category. To this end, we simulate the water footprint and VW trades of 27 major crops, within 8 crop categories, in 30 provinces of Iran (2005 2015). We investigate the impacts of these two scenarios on (a) provincial food security (FSp) and exports; (b) sustainable and efficient blue water consumption, and (c) blue VW export. We then test the correlation between agro-economic and socio-environmental indicators and provincial food security. Our results show that most provinces were threatened by unsustainable and inefficient blue water consumption for crop production, particularly in the summertime. This water mismanagement results in 14.41 and 8.45 billion m3 y-1 unsustainable and inefficient blue VW exports under ICFS. Eating local improves the FSp value by up to 210% which lessens the unsustainable and inefficient blue VW export from hotspots. As illustrated in the graphical abstract, the FSp value strongly correlates with different agro-economic and socio-environmental indicators, but in different ways. Our findings promote eating local besides improving agro-economic and socio-environmental conditions to take transformative steps toward eradicating food insecurity not only in Iran but also in other countries facing water limitations. © 2021, The Author(s).We would like to thank the reviewers of the manuscript for their constructive comments.     Educational difference in the prevalence of osteoporosis in postmenopausal women: a study in northern Iran

    <p>Abstract</p> <p>Background</p> <p>Osteoporosis is the most common metabolic bone disease in the world and it is rapidly increasing in Iran. In this study the relationship between educational levels and osteoporosis was investigated among Iranian postmenopausal women.</p> <p>Method and subjects</p> <p>Seven hundred and six women aged 50-75 years old were randomly recruited from urban (<it>n </it>= 440) and rural (<it>n </it>= 266) areas in Guilan. Osteoporosis was diagnosed by quantitative ultrasound technique and dual X-ray absorptiometry. Serum 25(OH) D3, body weight and height were measured in all subjects. Other data including age, educational level, menopause age, medications and history of illness were also collected.</p> <p>Results</p> <p>We found that the prevalence of osteoporosis was significantly greater among women with low educational level than women with high educational status (18.0% vs 3.8% <it>P </it>< 0.0001). However, women with low educational level had higher mean serum level of vitamin D than women with high educational level. Osteoporosis was significantly more prevalent among women living in rural areas than women living in urban areas (19.1% v.s 13.3%, <it>P </it>< 0.0001).</p> <p>Conclusion</p> <p>This study showed that educational level is associated with bone health in this population of postmenopausal women with significantly higher osteoporosis found in lower social groups. Therefore, we suggest that women with low social level should be carefully evaluated for signs of osteoporosis during routine physical examinations.</p

    Obese and female adolescents skip breakfast more than their non-obese and male peers

    We examined the association between overweight/gender and skipping breakfast among adolescent students in Tehran city using a cross-sectional study and a multistage random sampling method. All educational zones in Tehran city were covered during the educational year of 2000-01. In total, 2321 students aged 11-16 years (1068 male; 1263 female) participated in the study. Weight and height were measured and body mass index (BMI; kg/m2) was calculated. Overweight, pre-obesity and obesity were defined as BMI � 85 th , 85 th to 95 th , and � 95 th percentile of age-sex-specific BMI reference values, respectively. Self-reported frequency of breakfast consumption was categorized as usual/always, often, and rarely/never (5-7, 2-4 and 0-1 times/wk, respectively). Student's t and Chi-square tests were employed to analyze the data. Statistical inferences were made at α = 0.05. In boys and girls, the mean ± standard deviation of BMI was 19.8 ± 4.0 and 20.6 ± 4.1 kg/m2, the 18.8 and 23.1 were overweight, and 7.3 and 8.3 were obese, respectively. There was a significant difference in the frequency of breakfast consumption between obese and normal male students (P &lt; 0.001). Differences between pre-obese and normal, and obese and normal female students were also significant (P &lt; 0.002 and P &lt; 0.001, respectively). A significant difference was found in the frequency of breakfast consumption between male and female adolescents in all three categories (P &lt; 0.001). These results suggest that obese and female adolescents are more likely to skip breakfast than their normal and male peers and are therefore at higher risk for growth deficits and low educational performance. Preventive/educational programs are urgently needed in this age group. © 2007 Versita Warsaw and Springer-Verlag

    Reproductive life disorders in Italian celiac women. A case-control study

    BACKGROUND: The aim of this study is to explore the association between celiac disease and menstrual cycle, gestation and puerperal disorders. METHODS: The association between celiac disease and menstrual cycle, gestation and puerperal disorders in a sample of 62 childbearing age women (15-49 age) was assessed within an age and town of residence matched case-control study conducted in 2008. Main outcome measures were the presence of one or more disorders in menstrual cycle and the presence of one or more complication during pregnancy. RESULTS: 62 celiac women (median age: 31.5, range: 17-49) and 186 healthy control (median age: 32.5, range: 15-49) were interviewed. A higher percentage of menstrual cycle disorders has been observed in celiac women. 19.4% frequency of amenorrhea was reported among celiac women versus 2.2% among healthy controls (OR = 33, 95% CI = 7.17-151.8;, p = 0.000). An association has been observed between celiac disease and oligomenorrhea, hypomenorrhea, dysmenorrhea and metrorrhagia (p < 0.05). The likelihood of having at least one complication during pregnancy has been estimated to be at least four times higher in celiac women than in healthy women (OR = 4.1, 95% CI = 2-8.6, p = 0.000). A significant correlation has emerged for celiac disease and threatened abortion, gestational hypertension, placenta abruption, severe anaemia, uterine hyperkinesia, intrauterine growth restriction (p < 0.001). A shorter gestation has on average been observed in celiac women together with a lower birth weight of celiac women babies (p < 0.001). CONCLUSIONS: The occurrence of a significant correlation between celiac disease and reproductive disorders could suggest to consider celiac disease diagnostic procedures (serological screening) in women affected by these disorders

    A Subset of Latency-Reversing Agents Expose HIV-Infected Resting CD4⁺ T-Cells to Recognition by Cytotoxic T-Lymphocytes

    Resting CD4⁺ T-cells harboring inducible HIV proviruses are a critical reservoir in antiretroviral therapy (ART)-treated subjects. These cells express little to no viral protein, and thus neither die by viral cytopathic effects, nor are efficiently cleared by immune effectors. Elimination of this reservoir is theoretically possible by combining latency-reversing agents (LRAs) with immune effectors, such as CD8⁺ T-cells. However, the relative efficacy of different LRAs in sensitizing latently-infected cells for recognition by HIV-specific CD8⁺ T-cells has not been determined. To address this, we developed an assay that utilizes HIV-specific CD8⁺ T-cell clones as biosensors for HIV antigen expression. By testing multiple CD8⁺ T-cell clones against a primary cell model of HIV latency, we identified several single agents that primed latently-infected cells for CD8⁺ T-cell recognition, including IL-2, IL-15, two IL-15 superagonists (IL-15SA and ALT-803), prostratin, and the TLR-2 ligand Pam₃CSK₄. In contrast, we did not observe CD8⁺ T-cell recognition of target cells following treatment with histone deacetylase inhibitors or with hexamethylene bisacetamide (HMBA). In further experiments we demonstrate that a clinically achievable concentration of the IL-15 superagonist ‘ALT-803’, an agent presently in clinical trials for solid and hematological tumors, primes the natural ex vivo reservoir for CD8⁺ T-cell recognition. Thus, our results establish a novel experimental approach for comparative evaluation of LRAs, and highlight ALT-803 as an LRA with the potential to synergize with CD8⁺ T-cells in HIV eradication strategies.United States.     Risk Factors of Growth Faltering among 3-36 Months Old Children in Ahwaz, Iran

    Abstract: Background & Aims: Malnutrition is defined as nutritional disorders or unfavourable health status that can be the result of overconsumption or underconsumption of one or more nutrients. The causes of malnutrition are complex, multidimensional, and population specific. United Nations Children’s Fund (UNICEF) has adapted a conceptual framework for facilitating researchers in determining the causes of malnutrition in each area. The aim of this study was to determine some risk factors of growth faltering among 3-36 month old children in Ahwaz, Iran. Methods: This case-control study was conducted on 180 children 3-36 months old in Ahwaz. The subjects were children whose growth curves were horizontal or downward for at least two recent consecutive months. The controls were children with upward growth curves. Data collection was done by face to face interview, family file at the health centers, and direct measurements (mainly for anthropometric measurements). Data analyses were performed by SPSS software. Moreover, chi-square, Mann-Whitney, Kolmogorov-Smirnov, Student’s t-test, and logistic regression were statistical tests which were used according to the type and distribution of the variables. Results: From household variables, the number of family members (P=0.013), number of siblings (P=0.012), and mother’s educational level (P=0.019) showed significant association with growth faltering. Among child variables, the child’s age (P<0.001), child’s birth order (P=0.031), and birth interval with the next child (P=0.004) had significant relationships with growth faltering. Of the child health care variables, the mother’s child care status (P=0.017), and the mother’s nutritional knowledge (P=0.006) had significant relationships with growth faltering. Finally, the pattern of child nutrition (P<0.001), and the age of starting weaning (P < 0.001) showed significant differences between the two groups. Conclusion: This study revealed that the child nutritional pattern, number of family members, number of siblings, child’s age, child’s birth order, birth interval with the next child, mother’s educational level, mother’s child care status, mother’s nutritional knowledge, and the age of starting weaning were related to child malnutrition in Ahwaz. Therefore, appropriate intervention strategies, according to these findings, should be considered by health and other relevant sectors in order to control the problem of childhood malnutrition.     Hydro-Environmental Sustainability of Crop Production under Socioeconomic Drought

    A comprehensive framework for revealing the jeopardization between SDGs 2 and 6 is provided in this study. Along with a water footprint (WF) assessment, the 30-years pattern of agricultural WFs and its hydro-environmental, social, and ecopolitical (SEP) consequences were quantified for the major food producer regions of Iran, as it is a water-bankrupted country under socioeconomic drought. In addition, the enforced impacts of major water/food-related policies on environmental sustainability were analyzed through an institutional assessment. During 1986–2016, BWS and GWD raised with annual average rates of 5% and 44%, respectively. Consequently, SEP status prospered along with an 18% increase in irrigated area, 198% in added-value by crop production and 5% by staple-crop exports, and 51% in the number of agricultural workers. The Pearson correlation analysis revealed a significant tradeoff between self-supplied food availability and SEP. A 54% increase in food production occurred at the cost of 80% overexploitation in blue water resources and quality degradation. An annual average increase of 1.1% in P/ETo indicates the dominant role of anthropogenic interventions in such deteriorations. The institutional assessment demonstrated that environmental sustainability policies have never been applied as promoting policies to boost self-sufficiency in food production. According to the results, hydrological sustainability requires a transformative vision in national policies to exploit limited water and soil resources while preserving the environment

    The effect of pomegranate juice on clinical signs, matrix metalloproteinases and antioxidant status in patients with knee osteoarthritis

    BACKGROUND: Osteoarthritis (OA) is one of the commonest forms of musculoskeletal disorders that leads to joint degeneration and has a major impact on patients' quality of life. Experimental and in vitro studies have suggested the protective roles of pomegranate juice (PJ) as a rich antioxidant source for mitigating cartilage inflammation. In this interventional study, 38 patients with knee OA were randomly divided into two groups: PJ or control for 6 weeks to evaluate the effect of this intervention on clinical signs, inflammation and antioxidant status. RESULTS: Significant decreases in Western Ontario and McMaster Universities Osteoarthritis index (WOMAC) total score (P = 0.01), stiffness score (P = 0.00) and physical function score (P = 0.01) were observed in PJ group after the intervention. The means of serum levels of matrix metalloproteinase (MMP)-13 was significantly decreased (P = 0.02) and glutathione peroxidase was increased in the intervention group compared with the control group after the study period (P = 0.02). CONCLUSIONS: According to the findings of this clinical trial, PJ consumption can improve physical function and stiffness, decrease breakdown cartilage enzymes and increase antioxidant status in patients with knee OA. © 2016 Society of Chemical Industry. © 2016 Society of Chemical Industr
