    Visual impairment amongst adult diabetics attending a tertiary outpatient clinic

    Background: To determine the prevalence of diabetic retinopathy maculopathy and cataract amongst diabetics and the prevalence of visual impairment amongst diabetics attending the Komfo  Anokye Teaching Hospital eye unit. There are no current data in the study area on the prevalence of visual impairment and blindness amongst diabetics. This data is required in planning for screening and prevention of blindness due to diabetics.Methods: This was a hospital based cross sectional study. Subjects were diabetic patients attending the diabetic clinic. Simple random sampling was used. Demographic was taken. Visual acuity and refraction was done. Slit lamp examination was used to identify cataract. Maculopathy and retinopathy was determined using a 90D Volk lens and an indirect ophthalmoscope.Results: Non -insulin dependent diabetics constituted 97.1% whilst 2.9% were insulin dependent diabetics. The prevalence of the outcomes measures was: Cataract (23.7%) mild and moderate retinopathy (13.7%) severe proliferative retinopathy (1.8%) maculopathy (6.8%). Prevalence of low vision and blindness was 18.4%. Amongst diabetics 59.1% had no previous eye evaluation. Impaired vision due to cataract was 24.0 % representing a 40% decline in a decade.Conclusion: The prevalence of visual impairment was high at 18.4%. The reduction in impaired vision due to cataract over a decade is suggestive of either an improved cataract surgical rate or improved diabetic care or both. Majority of the diabetic patients 59.1% had not received prior ocular evaluation.     Potential of the Pentacam in Screening for Narrow Angles in Patients with Chronic Angle-Closure Glaucoma

    Background: Chronic angle-closure glaucoma (CACG) is a visually destructive disease. Effective management of CACG requires identifying eyes with narrow angle. Objective: To compare pentacam with gonioscopy in detecting narrow angles in eyes with CACG. Method: We enrolled 101 eyes with glaucoma. Gonioscopy was performed on all eyes. Using Shaffer’s grading, subjects were classified into angle closure and open angle. Anterior chamber volume (ACV) and anterior chamber depth (ACD) were measured with the pentacam. Receiver operating curve was constructed for each parameter and the area under the curve (AUC) was calculated. Results: Ten eyes (9.9%) were classified as angle closure on gonioscopy. To detect narrow angles, ACV (AUC 0.956; 95% confidence interval (CI) 0.894–0.987) performed similar to ACD (AUC=0.930, p=0.33). Using a cutoff of 102 mm3 , ACV had 100% sensitivity and 88.5% specificity for detecting narrow angles in CACG patients. With an ACV cutoff of 102 mm3 , the PPV for detecting angle closure was 48.9% (95% CI, 34.8–68.2), while the NPV was 100% (94.1– 100%), using 9.9% prevalence of angle closure from this study. Conclusion: ACV and ACD measured by the pentacam have the potential to determine narrow angles in eyes with CACG.     Some neglected issues in the economic analysis of agricultural project management in Ghana

    The Agricultural Development Bank (ADB) is an important financial organisation and is the main source of credit for agricultural projects in Ghana. The purpose of this study is to examine the procedures and method of analysis in project planning and management adopted by the ADB with the main object of identifying some of the neglected issues and to suggest some methods for improvement. Agricultural project planning and management by the ADB have been found to be plagued by a number of problems and inefficiencies in the areas of identification and selection, formulation, approval, implementation, supervision, monitoring and control. These areas are systematically examined and the main problems and bottlenecks are identified. An examination of the current state of project appraisal methodology is undertaken. The main project appraisal procedures are also reviewed, bringing out the difficulties of economic costbenefit analysis and offering a critique of the appraisal procedures. Suggestions for improving project identification and selection, formulation, implementation and supervision in the ADB are made based on the writer's insights into and experiences of the various problems identified with the production process. Cues are also taken from recommendations made for developing countries by such organisations as the FAO, IBRD and USAID. The major contribution of the study is the use of probability appraisal to analyse risk in a maize and pineapple project financed by the ADB. The Monte Carlo Simulation technique is adapted for use in this analysis. Results from this technique are compared with original results of the same projects obtained by the use of conventional cost-benefit analysis where single value estimations of costs and prices are made. Results from the probability analysis indicate that the methodology enables us to provide management with more appropriate information for making the right decisions by recognising the variability of outcomes of projects in the face of risk and uncertainty. Mean values of three indicators of worthiness obtained under this technique arc also thought to be more reliable and acceptable, especially since we do not have to fear any large variations around these values. Limitations of the analysis and directions for future research are indicated

    Indications and Outcomes of Corneal Transplant Surgery in Ghana

    Background: Corneal blindness contributes to 25% of all blindness. We review corneal transplant, a common surgical remedy, in Ghana to determine indications and visual outcomes in resource-poor environments. Methods: A retrospective cross-sectional study of keratoplasty evaluating indications, pre- and postoperative outcomes complications and  their associations, between January 2014 and December 2018 at a teaching  hospital in Ghana. Descriptive statistics and McNemar’s test were used for the analyses. Results: Seventy-five eyes were studied. The mean ± standard deviation (SD) age of patients was 45.08 ± 17.85 years, the majority being 20–39 years (58.7% were male). Pseudophakia bullous keratopathy (PBK) was the commonest indication for keratoplasty (26.7%) followed by keratoconus (21.3%). Preoperatively 96% of eyes were blind with vision <3/60 with 64% out of the total eyes with vision <1/60. Postoperatively, 60% of all grafts had uncorrected vision of 3/60 or better after the last follow-up. McNemar’s test revealed a statistically  significant difference between postsurgical and pre-surgical visual acuity (VA) (p < 0. 001). The median follow-up period was 12 months. The commonest postsurgical complication was raised intraocular pressure (IOP) (22.7%) with a total of 14.7% of grafts failures. Conclusion: In this setting, PBK is the leading indication for corneal transplant. Visual outcomes for corneal transplant in this  resource poor area are not worse than in other settings. We need to pay attention to corneal transplant services to cater for the expected increase  in PBK from the increasing cataract surgical rate

    Predictors and pathways of language and motor development in four prospective cohorts of young children in Ghana, Malawi, and Burkina Faso

    BackgroundPrevious reviews have identified 44 risk factors for poor early child development (ECD) in low- and middle-income countries. Further understanding of their relative influence and pathways is needed to inform the design of interventions targeting ECD.MethodsWe conducted path analyses of factors associated with 18-month language and motor development in four prospective cohorts of children who participated in trials conducted as part of the International Lipid-Based Nutrient Supplements (iLiNS) Project in Ghana (n = 1,023), Malawi (n = 675 and 1,385), and Burkina Faso (n = 1,122). In two cohorts, women were enrolled during pregnancy. In two cohorts, infants were enrolled at 6 or 9 months. In multiple linear regression and structural equation models (SEM), we examined 22 out of 44 factors identified in previous reviews, plus 12 additional factors expected to be associated with ECD.ResultsOut of 42 indicators of the 34 factors examined, 6 were associated with 18-month language and/or motor development in 3 or 4 cohorts: child linear and ponderal growth, variety of play materials, activities with caregivers, dietary diversity, and child hemoglobin/iron status. Factors that were not associated with child development were indicators of maternal Hb/iron status, maternal illness and inflammation during pregnancy, maternal perceived stress and depression, exclusive breastfeeding during 6 months postpartum, and child diarrhea, fever, malaria, and acute respiratory infections. Associations between socioeconomic status and language development were consistently mediated to a greater extent by caregiving practices than by maternal or child biomedical conditions, while this pattern for motor development was not consistent across cohorts.ConclusionsKey elements of interventions to ensure quality ECD are likely to be promotion of caregiver activities with children, a variety of play materials, and a diverse diet, and prevention of faltering in linear and ponderal growth and improvement in child hemoglobin/iron status

    Impact of small quantity lipid-based nutrient supplements on infant and young child feeding practices at 18 months of age: results from four randomized controlled trials in Africa.

    Optimal infant and young child feeding (IYCF) practices can help ensure nutrient adequacy and support healthy growth and development. Small-quantity lipid-based nutrient supplements (SQ-LNS) have been proposed to help fill nutrient gaps, but little is known about the impact of provision of SQ-LNS on breastfeeding or complementary feeding practices. In the context of four coordinated randomized controlled nutrient supplementation trials in diverse sites in Africa, we compared IYCF practices at infant age 18 months (after 9-12 months of supplementation) between those receiving and not receiving SQ-LNS. Practices were assessed by caregiver recall. Continued breastfeeding ranged from 74% (Ghana site) to 97% (Burkina Faso site) and did not differ between groups in any site; prevalence of frequent breastfeeding also did not differ. In two sites (Burkina Faso and Malawi), infants receiving SQ-LNS were more likely to meet the World Health Organization recommendations for frequency of feeding (percentage point differences of 12-14%, P < 0.0001 and P = 0.005, respectively; the remaining two sites did not have data for this indicator). Most indicators of infant dietary diversity did not differ between groups in any site, but in the same two sites where frequency of feeding differed, infants receiving SQ-LNS were less likely to have low frequency of consumption of animal-source foods in the previous week (percentage point differences of 9-19% for lowest tertile, P = .02 and P = 0.04, respectively). We conclude that provision of SQ-LNS did not negatively impact self-reported IYCF practices and may have positively impacted frequency of feeding

    Increased risk of preterm delivery with high cortisol during pregnancy is modified by fetal sex : a cohort study

    Background: Previous studies show an association between maternal plasma and salivary cortisol and preterm birth but have been primarily conducted in high-income countries. It is unknown whether salivary cortisol is a risk factor for preterm birth in Ghana. Our objective was to determine whether maternal salivary cortisol during pregnancy was associated with pregnancy duration and preterm delivery in Ghana. Methods: We conducted a cohort study of 783 pregnant women in Ghana. We measured salivary cortisol at baseline (mean 16 wk), 28 wk., and 36 wk. gestation. Pregnancy duration was determined primarily by ultrasound. We used adjusted linear regression models to examine the association between cortisol and pregnancy duration and Poisson regression models to determine the risk of preterm delivery among women with high cortisol at baseline or 28 wk. gestation. Results: Mean pregnancy duration was 39.4 ± 1.8 wk. and 6.6% had a preterm delivery. Mean maternal cortisol increased throughout pregnancy, from 4.9 ± 2.7 nmol/L at baseline (16 wk) to 6.4 ± 3.2 nmol/L at 28 wk. and 7.9 ± 3.0 nmol/L at 36 wk. gestation. In adjusted analyses, higher cortisol concentrations at baseline (β = − 0.39, p =.002) and 28 wk. (β = − 0.49, p =.001), but not 36 wk. (β = − 0.23, p =.084) were associated with a shorter pregnancy duration. Women with high cortisol at baseline (> 6.3 nmol/L) had an increased relative risk of preterm delivery (RR (95% CI): 1.96 (1.13, 3.40)), but the association between high cortisol at 28 wk. and preterm delivery was not significant. There was a significant interaction with fetal sex (p-for-interaction = 0.037): among women carrying male fetuses, high cortisol at baseline increased the risk of preterm delivery threefold (3.18 (1.51, 6.71)) while there was no association (1.17 (0.50, 2.74)) among women carrying female fetuses. Conclusion: Higher maternal cortisol is associated with a shorter pregnancy duration and an increased risk of preterm delivery. Subgroup analysis by fetal sex revealed that this association is evident primarily among women carrying male fetuses. Future studies of cortisol and preterm delivery should include consideration of fetal sex as a potential effect modifier.publishedVersionPeer reviewe

    Maternal and Infant Supplementation with Small-Quantity Lipid-Based Nutrient Supplements Increases Infants' Iron Status at 18 Months of Age in a Semiurban Setting in Ghana: A Secondary Outcome Analysis of the iLiNS-DYAD Randomized Controlled Trial

    Background Interventions are needed to address iron deficiency in low-income settings. Objective This secondary outcome analysis aimed to compare the hemoglobin (Hb) and iron status [zinc protoporphyrin (ZPP)] of children born to women enrolled in the iLiNS-DYAD trial in Ghana. Methods Women ≤20 wk pregnant (n = 1320) were assigned to receive 60 mg Fe/d and 400 µg folic acid/d until delivery and placebo thereafter, and no supplementation for infants (IFA group); or multiple micronutrients containing 20 mg Fe/d until 6 mo postpartum and no supplementation for infants (MMN); or small-quantity lipid-based nutrient supplements (SQ-LNSs) containing 20 mg Fe/d until 6 mo postpartum, and SQ-LNSs for infants from 6 to 18 mo of age (LNS). We compared infants’ Hb (g/L) and ZPP (µmol/mol heme) at 6 and 18 mo of age. Results At 6 mo of age, groups did not differ in mean ± SD Hb (overall: 113 ± 9.9 g/L) or geometric mean (95% CI) ZPP [overall: 62.6 (60.6, 64.7)]. At 18 mo of age, mean ± SD Hb (overall: 112 ± 10.4 g/L) did not differ significantly between groups, whereas geometric mean (95% CI) ZPP was lower (P = 0.031) in the LNS group [53.9 (50.7, 57.3)] than the IFA [60.4 (56.7, 64.3)] but not the MMN [58.8 (55.6, 62.2)] group. Further, the LNS group, compared with the IFA and MMN groups combined, had a lower prevalence of elevated (\u3e70) ZPP (27.5% compared with 35%; P = 0.02) and a marginally lower prevalence of anemia (38.7% compared with 44.9%; P = 0.06). These results generally remained unchanged when controlling for prespecified covariates or correcting for inflammation. Conclusions In this setting, providing SQ-LNSs or multiple micronutrients with 20 mg Fe/d, compared with iron (60 mg/d) and folic acid, to pregnant women does not affect their infants’ Hb or iron status at 6 mo of age, but maternal and infant supplementation with SQ-LNSs increases infants’ iron status at 18 mo of age

    A method to develop vocabulary checklists in new languages and their validity to assess early language development

    Background: Since the adoption of United Nations\u2019 Sustainable Goal 4.2 to ensure that all children have access to quality early child development (ECD) so that they are ready for primary education, the demand for valid ECD assessments has increased in contexts where they do not yet exist. The development of early language ability is important for school readiness. Our objective was to evaluate the validity of a method to develop vocabulary checklists in new languages to assess early language development, based on the MacArthur-Bates Communicative Development Inventories. Methods: Through asking mothers of young children what words their children say and through pilot testing, we developed 100-word vocabulary checklists in multilingual contexts in Malawi and Ghana. In Malawi, we evaluated the validity of the vocabulary checklist among 29 children age 17\u201325 months compared to three language measures assessed concurrently: Developmental Milestones Checklist-II (DMC-II) language scale, Malawi Developmental Assessment Tool (MDAT) language scale, and the number of different words (NDW) in 30-min recordings of spontaneous speech. In Ghana, we assessed the predictive validity of the vocabulary checklist at age 18 months to forecast language, pre-academic, and other skills at age 4\u20136 years among 869 children. We also compared the predictive validity of the vocabulary checklist scores to that of other developmental assessments administered at age 18 months. Results: In Malawi, the Spearman\u2019s correlation of the vocabulary checklist score with DMC-II language was 0.46 (p = 0.049), with MDAT language was 0.66 (p = 0.016) and with NDW was 0.50 (p = 0.033). In Ghana, the 18-month vocabulary checklist score showed the strongest (rho = 0.12\u20130.26) and most consistent (8/12) associations with preschool scores, compared to the other 18-month assessments. The largest coefficients were the correlations of the 18-month vocabulary score with the preschool cognitive factor score (rho = 0.26), language score (0.25), and pre-academic score (0.24). Conclusions: We have demonstrated the validity of a method to develop vocabulary checklists in new languages, which can be used in multilingual contexts, using a feasible adaptation process requiring about 2 weeks. This is a promising method to assess early language development, which is associated with later preschool language, cognitive, and pre-academic skills
