    Social determinants of anesthesia choice for cesarean section in mothers attending selected primary health care centers of Tehran

      Background: There are different techniques for anesthesia in Cesarean Section (C/S), which can be affected by different factors including mothers and health care providers' preferences. The objective of the present study was to evaluate social determinants of anesthesia choice for Cesarean Section in mothers attending selected primary health care centers of Tehran.  Methods: The current descriptive cross-sectional study was conducted in mothers who had a history of C/S and settled in catchment area of selected Primary Health Care centers known as Defined Population (DP). The DP is linked to Social Determinants of Health Research center affiliated to Shahid Beheshti University of Medical Sciences, Tehran.    Results: Totally, 1408 mothers participated in the study. The mean±SD age of mothers was 27.6±4.42, more than 50% of the participants had university education, and about 25% of mothers were unemployed. The previous history of C/S was reported by 29.9% of mothers and 91.9% of these mothers had experienced general anesthesia. Anesthesia choice of the study participants was as follows: 562 (39.9%) general anesthesia, 566 (40.2%) local anesthesia, 231 (16.4) had no idea about the anesthesia method, and 46 (3.5%) did not want to have C/S. The most frequent person with whom mothers discussed the method of anesthesia prior to their admission was their gynecologist (44%). The final decisions for choosing anesthesia method were made by the patient in about 40% of the participants.  Conclusion: Informing mothers appropriately about anesthesia procedure and possible complications of each technique in the prenatal period can help mothers for choosing the best method of anesthesia and may increase mothers' tendency to have local anesthesia

    Self-report demands of patients referred to social workers in Taleghani hospital, Tehran

      Background: Social workers have a responsibility to tackle the social determinants of health which cause health inequities. The hospitals’ social work units have the responsibility to improve patients' physical, mental, and social health. In this regard, understanding patients' demands is essential. The objective of the present study was to determine self-report demands of patients who were referred to social work unit in Taleghani hospital, Tehran.  Methods: The current cross-sectional study was conducted on the clients who were referred to social work unit of Talghani hospital, Tehran. Data was collected during autumn 2015 using convenience sampling method. Data collection form included demographic characteristics of the participants, the reason for referral to the social worker, self-report demands of the patients, and the response of the social worker to the patients. Data was analysed using IBM SPSS Statistics for Windows, Version 22.0.   Results: Of all clients who had attended social work unit, 772 accepted to participate in the study (response rate 87.1%). About 43% of the patients were female. Out of 990 requests, about 50% of the clients needed assistance to resolve financial problems. Help for resolving familial and insurance problems were the subsequent demands of the clients. Meanwhile, the most common response given by the social worker to the patients’ demands was financial assistance (n=276) (28.9%), and helping to resolve patient's familial problems (n=107) (11.2%) and offering health insurance services (n=106) (11.1%) were the second and the third most common responses, respectively.  Conclusion: The present study showed that still we have a long way to meet the aims of social work unit for empowering the patients and their families to withstand difficult conditions following the disease

    Arg77His and Trp187Arg are the Most Common Mutations Causing FXIII Deficiency in Iran

    The aim of this study was to review the literature for the genetic mutations causing inherited factoe XIII (FXIII) deficiency in patients from Iran, where the consanguineous marriage is common. Data were collected from 30 patients (18 males and 12 females) with FXIII deficiency, from 26 unrelated families. Data of mutation analysis were obtained from 2 previously published studies. A total of 7 mutations consisting of 5 new mutations and 2 previously reported mutations were identified. Of the 5 novel missense mutations, 2, Arg77His and Trp187Arg, were the most common in Iranian FXIII-deficient patients. In regions like Iran with high rate of consanguineous marriages, the identification of common mutations in disease like severe FXIII deficiency increases the capacity to make a precise screening and diagnosis assays to screen and diagnose families with high risk of FXIII deficiency for prevention of clinical complications in them

    The effects of natural nano-sized clinoptilolite and Nigella sativa supplementation on serum bone markers in diabetic rats

    Introduction: Many studies confirm that diabetes mellitus is associated with higher risks of bone fracture. The beneficial effects of Nigella sativa (NS) and clinoptilolite in preventing/reducing some diabetes-related disorders have been shown. This study was conducted to examine the effects of separate and concurrent supplementation of natural nano-sized clinoptilolite (NCLN) and NS on serum bone markers in rats with type 2 diabetes. Methods: A total of 42 (case=36 and control=6) adult male Wistar rats were divided into 2 groups: diabetic and non-diabetic. An oral glucose tolerance test and a homeostatic model assessment of insulin resistance (HOMA-IR) test were conducted to confirm diabetes. Then, the diabetic group was divided into 4 subgroups: [1] control (n=9), [2] NS 1%/food (n=9), [3] NCLN 2%/food (n=9), [4] NS 1%/food + NCLN 2%/food (n=9). After 7 weeks, serum levels of bone markers were determined using ELISA kits. Results: Analysis showed that serum levels of alkaline phosphatase (ALP) in the NCLN group (1318.6 ± 217.5 U/L) was significantly (P<0.05) higher than other intervented groups. On the other hand, serum levels of calcium in NCLN+NS group (10.8 ± 2.6 mg/dL) were higher (P=0.027) compared to all other study groups. However, rats in the NS group had higher (535.8 ± 49.3 pg/mL) PTH (P<0.0001) compared to other supplementation groups. There were no significant differences in vitamin D and osteoprotegerin. Conclusion: The results of the current study suggest that bone mineralization may be affected by concurrent use of NS and NCLN through influencing calcium circulation. Moreover, dietary NS administration is strongly related to an augmented level of PTH

    Utility of the ACC/AHA Lesion Classification to Predict Outcomes After Contemporary DES Treatment:Individual Patient Data Pooled Analysis From 7 Randomized Trials

    BACKGROUND: Use of the modified American College of Cardiology (ACC)/American Heart Association (AHA) lesion classification as a prognostic tool to predict short‐ and long‐term clinical outcomes after percutaneous coronary intervention in the modern drug‐eluting stent era is uncertain. METHODS AND RESULTS: Patient‐level data from 7 prospective, randomized trials were pooled. Clinical outcomes of patients undergoing single lesion percutaneous coronary intervention with second‐generation drug‐eluting stent were analyzed according to modified ACC/AHA lesion class. The primary end point was target lesion failure (TLF: composite of cardiac death, target vessel myocardial infarction, or ischemia‐driven target lesion revascularization). Clinical outcomes to 5 years were compared between patients treated for noncomplex (class A/B1) versus complex (class B2/C) lesions. Eight thousand five hundred sixteen patients (age 63.1±10.8 years, 70.5% male) were analyzed. Lesions were classified as A, B1, B2, and C in 7.9%, 28.5%, 33.7%, and 30.0% of cases, respectively. Target lesion failure was higher in patients undergoing percutaneous coronary intervention of complex versus noncomplex lesions at 30 days (2.0% versus 1.1%, P=0.004), at 1 year (4.6% versus 3.0%, P=0.0005), and at 5 years (12.4% versus 9.2%, P=0.0001). By multivariable analysis, treatment of ACC/AHA class B2/C lesions was significantly associated with higher rate of 5‐year target lesion failure (adjusted hazard ratio, 1.39 [95% CI, 1.17–1.64], P=0.0001) driven by significantly higher rates of target vessel myocardial infarction and ischemia‐driven target lesion revascularization. CONCLUSIONS: In this pooled large‐scale analysis, treating complex compared with noncomplex lesions according to the modified ACC/AHA classification with second‐generation drug‐eluting stent was associated with worse 5‐year clinical outcomes. This historical classification system may be useful in the contemporary era for predicting early and late outcomes following percutaneous coronary intervention

A combination of antibiotic therapy and cardiac surgery were performed, it seems this approach reduces mortality and complications. Abstract Brucellosis (also known as “undulant fever,” “Mediterranean fever,” or “Malta fever”) is a zoonotic infection transmitted to humans from infected animals (cattle, sheep, goats, camels, pigs, or other animals) by ingestion of food products (such as unpasteurized dairy products) or by contact with tissue or fluids. It is the most frequent zoonosis globally and a major public health issue in many resource‐poor nations. Endocarditis is one of the rarest and most dangerous consequences of brucellosis. Additionally, the combination of endocarditis with cardiomyopathy increases its rarity. This condition is usually treated with a high level of suspicion Serological, clinical, and epidemiological data can all be used to make a diagnosis. The use of echocardiography aids in the early diagnosis. Due to the high risk of recurrence and the extent of tissue destruction brought on by Brucella, the majority of experts advise an early surgical intervention; nevertheless, other writers assert that low‐risk patients also require cautious therapy. In this article, we discuss the situation of a patient who underwent surgery and had Brucella endocarditis and heart failure. In conclusion, a combination of antibiotic therapy and cardiac surgery, reduces mortality and complications associated with Brucella endocarditis and improves patient quality of life

    Epidemic size, trend and spatiotemporal mapping of SARS-CoV-2 using geographical information system in Alborz Province, Iran

    Abstract Background The first confirmed cases of COVID-19 in Iran were reported in Qom city. Subsequently, the neighboring provinces and gradually all 31 provinces of Iran were involved. This study aimed to investigate the case fatility rate, basic reproductive number in different period of epidemic, projection of daily and cumulative incidence cases and also spatiotemporal mapping of SARS-CoV-2 in Alborz province, Iran. Methods A confirmed case of COVID-19 infection was defined as a case with a positive result of viral nucleic acid testing in respiratory specimens. Serial interval (SI) was fitted by gamma distribution and considered the likelihood-based R0 using a branching process with Poisson likelihood. Seven days average of cases, deaths, doubling times and CFRs used to draw smooth charts. kernel density tool in Arc GIS (Esri) software has been employed to compute hot spot area of the study site. Results The maximum-likelihood value of R0 was 2.88 (95%, CI: 2.57–3.23) in the early 14 days of epidemic. The case fatility rate for Alborz province (Iran) on March 10, was 8.33% (95%, CI:6.3–11), and by April 20, it had an increasing trend and reached 12.9% (95%,CI:11.5–14.4). The doubling time has been increasing from about two days and then reached about 97 days on April 20, 2020, which shows the slowdown in the spread rate of the disease. Also, from March 26 to April 2, 2020 the whole Geographical area of Karj city was almost affected by SARS-CoV-2. Conclusions The R0 of COVID-19 in Alborz province was substantially high at the beginning of the epidemic, but with preventive measures and public education and GIS based monitoring of the cases,it has been reduced to 1.19 within two months. This reduction highpoints the attainment of preventive measures in place, however we must be ready for any second epidemic waves during the next months
