    Measurement properties of quality of life measurement instruments for infants, children and adolescents with eczema: protocol for a systematic review

    Background: Eczema is a common chronic or chronically relapsing skin disease that has a substantial impact on quality of life (QoL). By means of a consensus-based process, the Harmonising Outcome Measures in Eczema (HOME) initiative has identified QoL as one of the four core outcome domains to be assessed in all eczema trials. Few measurement instruments exist to measure QoL in infants and children with eczema, but there is a great variability in both content and quality (for example, reliability and validity) of the instruments used, and it is not always clear if the best instrument is being used. Therefore, the aim of the proposed research is a comprehensive systematic assessment of the measurement properties of the existing measurement instruments that were developed and/or validated for the measurement of patient-reported QoL in infants and children with eczema. Methods/Design: This study is a systematic review of the measurement properties of patient-reported measures of QoL developed and/or validated for infants and children with eczema. Medline via PubMed and EMBASE will be searched using a selection of relevant search terms. Eligible studies will be primary empirical studies evaluating, describing, or comparing measurement properties of QoL instruments for infants and children with eczema. Eligibility assessment and data abstraction will be performed independently by two reviewers. Evidence tables will be generated for study characteristics, instrument characteristics, measurement properties, and interpretability. The adequacy of the measurement properties will be assessed using predefined criteria. Methodological quality of studies will be assessed using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist. A best evidence synthesis will be undertaken if more than one study has investigated a particular measurement property. Discussion: The proposed systematic review will produce a comprehensive assessment of measurement properties of existing QoL instruments in infants and children with eczema. We aim to identify one best currently available instrument to measure QoL in infants and/or children with eczema

    Methodological Diversity in Citizen Science Mosquito Surveillance: A Scoping Review

    Global concern regarding mosquito-borne disease emergence and re-emergence has driven the development of citizen science mosquito surveillance initiatives. Although these initiatives have shown great potential to assist local health authorities, ensuring outcomes are translatable to improved public health policy and practice remains challenging. Here we present a summary of citizen science mosquito surveillance programs worldwide, their focus, strategies, and outcomes, with a view to how best to apply this approach in their local areas. A scoping review of studies and reports was conducted through systematic search on electronic databases (Scopus, PubMed, Web of Science, and Google Scholar), grey literature, and other documents listed in the references of selected articles. A total of 33 citizen science studies included in this review described 29 citizen science mosquito surveillance projects operating in 16 countries, besides three programs with wide geographic coverage. The selected programs focused on different strategies and methods according to their local and national contextual needs. The majority of the programs reported being free or low in cost, and amenable to participants. Also, citizen scientists valued the opportunity to actively contribute to a scientific activity in which they saw value. Local and national programs have been successful in involving the broader public and yielding data on mosquito populations. However, to ensure the best public health outcomes, sustainability, and scalability, there is a need to continue engaging with stakeholders, including community members, researchers, public health agents, industry, and policymakers, and to bridge existing collaborations across different sectors

    Protein kinase Cα (PKCα) regulates bone architecture and osteoblast activity.

    Bones' strength is achieved and maintained through adaptation to load bearing. The role of the protein kinase PKCα in this process has not been previously reported. However, we observed a phenotype in the long bones of Prkca(-/-) female but not male mice, in which bone tissue progressively invades the medullary cavity in the mid-diaphysis. This bone deposition progresses with age and is prevented by disuse but unaffected by ovariectomy. Castration of male Prkca(-/-) but not WT mice results in the formation of small amounts of intramedullary bone. Osteoblast differentiation markers and Wnt target gene expression were up-regulated in osteoblast-like cells derived from cortical bone of female Prkca(-/-) mice compared with WT. Additionally, although osteoblastic cells derived from WT proliferate following exposure to estradiol or mechanical strain, those from Prkca(-/-) mice do not. Female Prkca(-/-) mice develop splenomegaly and reduced marrow GBA1 expression reminiscent of Gaucher disease, in which PKC involvement has been suggested previously. From these data, we infer that in female mice, PKCα normally serves to prevent endosteal bone formation stimulated by load bearing. This phenotype appears to be suppressed by testicular hormones in male Prkca(-/-) mice. Within osteoblastic cells, PKCα enhances proliferation and suppresses differentiation, and this regulation involves the Wnt pathway. These findings implicate PKCα as a target gene for therapeutic approaches in low bone mass conditions

    Is productivity of mesic savannas light limited or water limited? Results of a simulation study

    A soil-plant-atmosphere model was used to estimate gross primary productivity (GPP) and evapotranspiration (ET) of a tropical savanna in Australia. This paper describes model modifications required to simulate the substantial C4 grass understory together with C3 trees. The model was further improved to include a seasonal distribution of leaf area and foliar nitrogen through 10 canopy layers. Model outputs were compared with a 5-year eddy covariance dataset. Adding the C4 photosynthesis component improved the model efficiency and root-mean-squared error (RMSE) for total ecosystem GPP by better emulating annual peaks and troughs in GPP across wet and dry seasons. The C4 photosynthesis component had minimal impact on modelled values of ET. Outputs of GPP from the modified model agreed well with measured values, explaining between 79% and 90% of the variance and having a low RMSE (0.003-0.281gCm-2day-1). Approximately, 40% of total annual GPP was contributed by C4 grasses. Total (trees and grasses) wet season GPP was approximately 75-80% of total annual GPP. Light-use efficiency (LUE) was largest for the wet season and smallest in the dry season and C4 LUE was larger than that of the trees. A sensitivity analysis of GPP revealed that daily GPP was most sensitive to changes in leaf area index (LAI) and foliar nitrogen (Nf) and relatively insensitive to changes in maximum carboxylation rate (Vcmax), maximum electron transport rate (Jmax) and minimum leaf water potential (ψmin). The modified model was also able to represent daily and seasonal patterns in ET, (explaining 68-81% of variance) with a low RMSE (0.038-0.19mmday-1). Current values of Nf, LAI and other parameters appear to be colimiting for maximizing GPP. By manipulating LAI and soil moisture content inputs, we show that modelled GPP is limited by light interception rather than water availability at this site. © 2011 Blackwell Publishing Ltd

    Investigating anti-doping roles of athlete support personnel working with international disabled athletes in six European nations using the COM-B Model and Theoretical Domains Framework

    Objectives: Athlete support personnel (ASP) can play a significant role in protecting the integrity of sport and welfare of athletes. Yet, their perspective on how they proactively contribute to clean sport is rarely considered in anti-doping research. Studies with ASP working in elite disabled sport are rarer still. This study amplifies the voice of ASP working with international level disabled athletes to answer the following research questions: 1) What actions do ASP take in anti-doping? 2) What are the factors influencing ASPs’ actions? Methods: Individual semi-structured interviews were conducted with 41 ASP (Men n=17, Women n=23, n=1 did not report) in six European countries working as Psychologists (n=7), Mangers/Performance Directors (n=6), Physiotherapists (n=6), Doctors (n= 5), Nutritionists (n=5), Strength and Conditioning coaches (n=4), Agents (n=2), Technical coaches (n=2) and a dual role as a Coach/Manager (n=3) or Coach/Physiotherapist (n=1). Abductive reflexive thematic analysis was conducted, beginning with inductive coding and development of themes, followed by deductive identification of constructs from the COM-B Model and Theoretical Domains Framework. Results: Most ASP shared a narrow repertoire of behaviors that targeted helping athletes avoid ‘accidental’ doping through medicine and supplement use. However, psychologists’ actions addressed a broader range of doping vulnerability factors, such as self-esteem and emotion regulation. Regardless of the exact nature of ASPs’ anti-doping roles, behavior appeared to be influenced by ASPs’ professional role and identity, as well as ASP operating in performance pressured environments and/or ASP perceiving that doping would never occur among their athletes. Conclusions: ASP are primarily concerned with preventing inadvertent doping. This concern aligns with the content of anti-doping education programmes. To reduce the risk of intentional doping alongside inadvertent doping, a more behaviorally informed approach to anti-doping is required where ASP are deliberative agents in the pursuit of clean sport

    Mathematically gifted and talented learners: Theory and practice

This article reviews policy developments and current research and theory on giftedness in mathematics. It includes a discussion of the nature of mathematical ability as well as the factors that make up giftedness in mathematics. The article is set in the context of current developments in Mathematics Education and Gifted Education in the UK and their implications for Science and Technology. It argues that early identification and appropriate provision for younger mathematically promising pupils capitalizes on an intellectual resource which could provide future mathematicans as well as specialists in Science or Technology. Drawing on a Vygotskian framework, it is suggested that the mathematically gifted require appropriate cognitive challenges as well as attitudinally and motivationally enhancing experiences. In the second half of this article we report on an initiative in which we worked with teachers to identify mathematically gifted pupils and to provide effective enrichment support for them, in a number of London Local Authorities. A number of significant issues are raised relating to the identification of mathematical talent, enrichment provision for students and teachers’ professional development

    A cross-national study on the antecedents of work–life balance from the fit and balance perspective

    Drawing on the perceived work–family fit and balance perspective, this study investigates demands and resources as antecedents of work–life balance (WLB) across four countries (New Zealand, France, Italy and Spain), so as to provide empirical cross-national evidence. Using structural equation modelling analysis on a sample of 870 full time employees, we found that work demands, hours worked and family demands were negatively related to WLB, while job autonomy and supervisor support were positively related to WLB. We also found evidence that resources (job autonomy and supervisor support) moderated the relationships between demands and work–life balance, with high resources consistently buffering any detrimental influence of demands on WLB. Furthermore, our study identified additional predictors of WLB that were unique to some national contexts. For example, in France and Italy, overtime hours worked were negatively associated with WLB, while parental status was positively associated with WLB.     Report from the third international consensus meeting to harmonise core outcome measures for atopic eczema/dermatitis clinical trials (HOME).

    This report provides a summary of the third meeting of the Harmonising Outcome Measures for Eczema (HOME) initiative held in San Diego, CA, U.S.A., 6-7 April 2013 (HOME III). The meeting addressed the four domains that had previously been agreed should be measured in every eczema clinical trial: clinical signs, patient-reported symptoms, long-term control and quality of life. Formal presentations and nominal group techniques were used at this working meeting, attended by 56 voting participants (31 of whom were dermatologists). Significant progress was made on the domain of clinical signs. Without reference to any named scales, it was agreed that the intensity and extent of erythema, excoriation, oedema/papulation and lichenification should be included in the core outcome measure for the scale to have content validity. The group then discussed a systematic review of all scales measuring the clinical signs of eczema and their measurement properties, followed by a consensus vote on which scale to recommend for inclusion in the core outcome set. Research into the remaining three domains was presented, followed by discussions. The symptoms group and quality of life groups need to systematically identify all available tools and rate the quality of the tools. A definition of long-term control is needed before progress can be made towards recommending a core outcome measure
