Time-varying Fiscal Multipliers Identified by Systematic Component: A Bayesian Approach to TVP-SVAR model
Abstract This study estimates time varying fiscal multipliers from the aspect of fiscal policy rules derived from the systematic component along the line of âAgnostic Identification Procedureâ proposed by Caldara and Kamps (2017) for the US economy between 1952:Q1-2018:Q1. To do so, we adopt time-varying parameter structural vector autoregressive (TVP-SVAR) with MCMC procedure by a Bayesian approach, and identify both of government spending and tax cut shocks using the zero and sign restrictions method proposed by Arias, Rubio-Ramirez and Waggoner (2018). And we compare those values with time varying version identified by standard sign restriction along the line of Mountford and Uhlig (2009). Our estimation reports that time-varying fiscal multipliers of output by government spending rule could be nearly double for one year but decline to unity after eight years, and seem to have been very stable for long terms such as sixty years. Impact of the day of the week on the discontinuation of broad-spectrum antibiotic prescriptions; a multi-centered observational study
To encourage and guide antimicrobial stewardship team (AST) activity and promote appropriate antibiotic use, we studied the impact of day of the week on the initiation and discontinuation of antibiotic administration. This was a multicenter observational study conducted at 8 Japanese hospitals from April 1 to September 30, 2019, targeting patients who underwent treatment with broad-spectrum antibiotics, such as anti-methicillin-resistant Staphylococcus aureus agents and anti-pseudomonal agents. We compared the weekly numbers of initiations and discontinuations of antibiotic prescription on each day of the week or on the days after a holiday. There was no statistical difference in the number of antibiotic initiations on both weekdays and the day after a holiday. However, antibiotic discontinuation was significantly higher from Tuesday onward than Monday and from the second day than the first day after a holiday. Similar trends were observed regardless of the categories of antibiotics, hospital and admission ward, and AST activity. This study suggests that broad-spectrum antibiotics tend to be continued during weekends and holidays and are most likely to be discontinued on Tuesday or the second day after a holiday. This was probably due to behavioral factors beyond medical indications, requiring further antimicrobial stewardship efforts in the future
Comparison of the slowâpull and aspiration methods of endobronchial ultrasoundâguided transbronchial needle aspiration for nextâgeneration sequencingâcompatible tissue collection in nonâsmall cell lung cancer
Abstract Background Personalized treatment for nonâsmall cell lung cancer (NSCLC) has advanced rapidly, and elucidating the genetic changes that trigger this disease is crucial for appropriate treatment selection. Both slowâpull and aspiration methods of endobronchial ultrasoundâguided transbronchial needle aspiration (EBUSâTBNA) are accepted methods for collecting samples suitable for nextâgeneration sequencing (NGS) to examine driver gene mutations and translocations in NSCLC. Here, we aimed to determine which of these two methods is superior for obtaining higherâquality samples from patients with NSCLC. Methods Seventyâone patients diagnosed with NSCLC via EBUSâTBNA using the slowâpull or aspiration (20âmL negative pressure) methods between July 2019 and September 2022 were included. A total of 203 tissue samples from the 71 patients were fixed in formalin, embedded in paraffin, and mounted on slides. The presence of tissue cores, degree of blood contamination, and number of tumor cells were compared between the groups. The success rate of NGS, using Oncomine Dx Target Test MultiâCDx, was also compared between the groups. Results The slowâpull method was associated with a higher yield of tissue cores, lower degree of blood contamination, and higher number of tumor cells than the aspiration method. The success rate of the NGS was also significantly higher for the slowâpull group (95%) than for the aspiration group (68%). Conclusion Overall, these findings suggest that the slowâpull method is a superior technique for EBUSâTBNA to obtain highâquality tissue samples for NGS. The slowâpull method may contribute to the identification of driver gene mutations and translocations and facilitate personalized treatment of NSCLC
Combined Pre-Supernova Alert System with Kamland and Super-Kamiokande
International audiencePreceding a core-collapse supernova, various processes produce an increasing amount of neutrinos of all flavors characterized by mounting energies from the interior of massive stars. Among them, the electron antineutrinos are potentially detectable by terrestrial neutrino experiments such as KamLAND and Super-Kamiokande via inverse beta decay interactions. Once these pre-supernova neutrinos are observed, an early warning of the upcoming core-collapse supernova can be provided. In light of this, KamLAND and Super-Kamiokande have been monitoring pre-supernova neutrinos since 2015 and 2021, respectively. Recently, we performed a joint study between KamLAND and Super-Kamiokande on pre-supernova neutrino detection. A pre-supernova alert system combining the KamLAND detector and the Super-Kamiokande detector is developed and put into operation, which can provide a supernova alert to the astrophysics community. Fully leveraging the complementary properties of these two detectors, the combined alert is expected to resolve a pre-supernova neutrino signal from a 15 M star within 510 pc of the Earth, at a significance level corresponding to a false alarm rate of no more than 1 per century. For a Betelgeuse-like model with optimistic parameters, it can provide early warnings up to 12 hours in advance

A global metagenomic map of urban microbiomes and antimicrobial resistance
We present a global atlas of 4,728 metagenomic samples from mass-transit systems in 60 cities over 3 years, representing the first systematic, worldwide catalog of the urban microbial ecosystem. This atlas provides an annotated, geospatial profile of microbial strains, functional characteristics, antimicrobial resistance (AMR) markers, and genetic elements, including 10,928 viruses, 1,302 bacteria, 2 archaea, and 838,532 CRISPR arrays not found in reference databases. We identified 4,246 known species of urban microorganisms and a consistent set of 31 species found in 97% of samples that were distinct from human commensal organisms. Profiles of AMR genes varied widely in type and density across cities. Cities showed distinct microbial taxonomic signatures that were driven by climate and geographic differences.
