    Gestational weight gain in overweight and obese women enrolled in a healthy lifestyle and eating habits program

    OBJECTIVES: To determine whether changes in lifestyle in women with BMI > 25 could decrease gestational weight gain and unfavorable pregnancy outcomes. METHODS: Women with BMI > 25 were randomized at 1st trimester to no intervention or a Therapeutic Lifestyle Changes (TLC) Program including diet (overweight: 1700 kcal/day, obese: 1800 kcal/day) and mild physical activity (30 min/day, 3 times/week). At baseline and at the 36th week women filled-in a Food Frequency Questionnaire. OUTCOMES: gestational weight gain, gestational diabetes mellitus, gestational hypertension, preterm delivery. Data stratified by BMI categories. RESULTS: Socio-demographic features were similar between groups (TLC: 33 cases, CONTROLS: 28 cases). At term, gestational weight gain in obese women randomized to TLC group was lower (6.7 ± 4.3 kg) versus controls (10.1 ± 5.6 kg, p = 0.047). Gestational diabetes mellitus, gestational hypertension and preterm delivery were also significantly lower. TLC was an independent factor in preventing gestational weight gain, gestational diabetes mellitus, gestational hypertension. Significant changes in eating habits occurred in the TLC group, which increased the number of snacks, the intake of fruits-vegetables and decreased the consumption of sugar. CONCLUSIONS: A caloric restriction associated to changes in eating behavior and constant physical activity, is able to reduce gestational weight gain and related pregnancy complications in obese women.Objectives: To determine whether changes in lifestyle in women with BMI > 25 could decrease gestational weight gain and unfavorable pregnancy outcomes.     Detecting the Cold Spot as a Void with the Non-Diagonal Two-Point Function

    The anomaly in the Cosmic Microwave Background known as the "Cold Spot" could be due to the existence of an anomalously large spherical (few hundreds Mpc/h radius) underdense region, called a "Void" for short. Such a structure would have an impact on the CMB also at high multipoles l through Lensing. This would then represent a unique signature of a Void. Modeling such an underdensity with an LTB metric, we show that the Lensing effect leads to a large signal in the non-diagonal two-point function, centered in the direction of the Cold Spot, such that the Planck satellite will be able to confirm or rule out the Void explanation for the Cold Spot, for any Void radius with a Signal-to-Noise ratio of at least O(10).Comment: v1: 6 pages, 2 figures; v2: 6 pages, 2 figures, text improved, to appear on JCA

    Participant retention practices in longitudinal clinical research studies with high retention rates

    Abstract Background There is a need for improving cohort retention in longitudinal studies. Our objective was to identify cohort retention strategies and implementation approaches used in studies with high retention rates. Methods Longitudinal studies with ≥200 participants, ≥80% retention rates over ≥1 year of follow-up were queried from an Institutional Review Board database at a large research-intensive U.S. university; additional studies were identified through networking. Nineteen (86%) of 22 eligible studies agreed to participate. Through in-depth semi-structured interviews, participants provided retention strategies based on themes identified from previous literature reviews. Synthesis of data was completed by a multidisciplinary team. Results The most commonly used retention strategies were: study reminders, study visit characteristics, emphasizing study benefits, and contact/scheduling strategies. The research teams were well-functioning, organized, and persistent. Additionally, teams tailored their strategies to their participants, often adapting and innovating their approaches. Conclusions These studies included specialized and persistent teams and utilized tailored strategies specific to their cohort and individual participants. Studies’ written protocols and published manuscripts often did not reflect the varied strategies employed and adapted through the duration of study. Appropriate retention strategy use requires cultural sensitivity and more research is needed to identify how strategy use varies globally

    Improving deep learning based semantic segmentation with multi view outliner correction

    The goal of this paper is to use transfer learning for semi supervised semantic segmentation in 2D images: given a pretrained deep convolutional network (DCNN), our aim is to adapt it to a new camera-sensor system by enforcing predictions to be consistent for the same object in space. This is enabled by projecting 3D object points into multi-view 2D images. Since every 3D object point is usually mapped to a number of 2D images, each of which undergoes a pixelwise classification using the pretrained DCNN, we obtain a number of predictions (labels) for the same object point. This makes it possible to detect and correct outlier predictions. Ultimately, we retrain the DCNN on the corrected dataset in order to adapt the network to the new input data. We demonstrate the effectiveness of our approach on a mobile mapping dataset containing over 10'000 images and more than 1 billion 3D points. Moreover, we manually annotated a subset of the mobile mapping images and show that we were able to rise the mean intersection over union (mIoU) by approximately 10% with Deeplabv3+, using our approach. © 2020 International Archives of the Photogrammetry, Remote Sensing and Spatial Information Sciences - ISPRS Archives

    Mechanisms by which statins protect endothelial cells from radiation-induced injury in the carotid artery

    BackgroundThe incidental use of statins during radiation therapy has been associated with a reduced long-term risk of developing atherosclerotic cardiovascular disease. However, the mechanisms by which statins protect the vasculature from irradiation injury remain poorly understood.ObjectivesIdentify the mechanisms by which the hydrophilic and lipophilic statins pravastatin and atorvastatin preserve endothelial function after irradiation.MethodsCultured human coronary and umbilical vein endothelial cells irradiated with 4 Gy and mice subjected to 12 Gy head-and-neck irradiation were pretreated with statins and tested for endothelial dysfunction, nitric oxide production, oxidative stress, and various mitochondrial phenotypes at 24 and 240 h after irradiation.ResultsBoth pravastatin (hydrophilic) and atorvastatin (lipophilic) were sufficient to prevent the loss of endothelium-dependent relaxation of arteries after head-and-neck irradiation, preserve the production of nitric oxide by endothelial cells, and suppress the cytosolic reactive oxidative stress associated with irradiation. However, only pravastatin inhibited irradiation-induced production of mitochondrial superoxide; damage to the mitochondrial DNA; loss of electron transport chain activity; and expression of inflammatory markers.ConclusionsOur findings reveal some mechanistic underpinnings of the vasoprotective effects of statins after irradiation. Whereas both pravastatin and atorvastatin can shield from endothelial dysfunction after irradiation, pravastatin additionally suppresses mitochondrial injury and inflammatory responses involving mitochondria. Clinical follow-up studies will be necessary to determine whether hydrophilic statins are more effective than their lipophilic counterparts in reducing the risk of cardiovascular disease in patients undergoing radiation therapy

    Antibacterial and antibiofilm activities of quercetin against clinical isolates of Staphyloccocus aureus and Staphylococcus saprophyticus with resistance profile

    The aim of this study was to determine the antibacterial and antibiofilm properties of quercetin against clinical isolates of Staphyloccocus aureus and Staphylococcus saprophyticus with resistance profile. The antibacterial activity of quercetin was performed by the determination of the minimum inhibitory concentration (MIC) through the microdilution method according to the Clinical and Laboratory Standards Institute (CLSI). The percentage of inhibition of Staphylococcus spp. biofilm, after treatment with sub-inhibitory concentrations of quercetin (MIC/2 and MIC/4), was evaluated by the violet crystal assay. Quercetin showed an antimicrobial activity against clinical isolates of methicillin-susceptible S. aureus (MSSA) (MIC = 250 µg/ml), methicillin-resistant S. aureus (MRSA) (MIC = 500 µg/ml), vancomycin-intermediate S. aureus (VISA) (MIC = 125 and 150 µg/ml), S. saprophyticus resistant to oxacillin (MIC = 62.5 to 125 µg/ml), vancomycin-resistant S. aureus (VRSA) and S. saprophyticus resistant to oxacillin and vancomycin (MIC = 500 to 1000 µg/ml). At MIC/2 and MIC/4 the quercetin inhibit 46.5 ± 2.7% and 39.4 ± 4.3% of the S. aureus biofilm, respectively, and 51.7 ± 5.5% and 46.9 ± 5.5% of the S. saprophyticus biofilm, respectively. According to the results of this study, it was noticed that the quercetin presented an antibacterial activity against strains of Staphylococcus spp. with resistance profile and also inhibited the bacterial biofilm production even in sub-inhibitory concentrations

    Nociceptor-expressed ephrin-B2 regulates inflammatory and neuropathic pain

    Background: EphB receptors and their ephrin-B ligands play an important role in nervous system development, as well as synapse formation and plasticity in the adult brain. Recent studies show that intrathecal treatment with EphB-receptor activator ephrinB2-Fc induced thermal hyperalgesia and mechanical allodynia in rat, indicating that ephrin-B2 in small dorsal root ganglia (DRG) neurons and EphB receptors in the spinal cord modulate pain processing. To examine the role of ephrin-B2 in peripheral pain pathways, we deleted ephrin-B2 in Nav1.8+ nociceptive sensory neurons with the Cre-loxP system. Sensory neuron numbers and terminals were examined using neuronal makers. Pain behavior in acute, inflammatory and neuropathic pain models was assessed in the ephrin-B2 conditional knockout (CKO) mice. We also investigated the c-Fos expression and NMDA receptor NR2B phosphorylation in ephrin-B2 CKO mice and littermate controls.Results: The ephrin-B2 CKO mice were healthy with no sensory neuron loss. However, pain-related behavior was substantially altered. Although acute pain behavior and motor co-ordination were normal, inflammatory pain was attenuated in ephrin-B2 mutant mice. Complete Freund's adjuvant (CFA)-induced mechanical hyperalgesia was halved. Formalin-induced pain behavior was attenuated in the second phase, and this correlated with diminished tyrosine phosphorylation of N-methyl-D-aspartic acid (NMDA) receptor subunit NR2B in the dorsal horn. Thermal hyperalgesia and mechanical allodynia were significantly reduced in the Seltzer model of neuropathic pain.Conclusions: Presynaptic ephrin-B2 expression thus plays an important role in regulating inflammatory pain through the regulation of synaptic plasticity in the dorsal horn and is also involved in the pathogenesis of some types of neuropathic pain

    Dealing with missing standard deviation and mean values in meta-analysis of continuous outcomes: a systematic review

    Background: Rigorous, informative meta-analyses rely on availability of appropriate summary statistics or individual participant data. For continuous outcomes, especially those with naturally skewed distributions, summary information on the mean or variability often goes unreported. While full reporting of original trial data is the ideal, we sought to identify methods for handling unreported mean or variability summary statistics in meta-analysis. Methods: We undertook two systematic literature reviews to identify methodological approaches used to deal with missing mean or variability summary statistics. Five electronic databases were searched, in addition to the Cochrane Colloquium abstract books and the Cochrane Statistics Methods Group mailing list archive. We also conducted cited reference searching and emailed topic experts to identify recent methodological developments. Details recorded included the description of the method, the information required to implement the method, any underlying assumptions and whether the method could be readily applied in standard statistical software. We provided a summary description of the methods identified, illustrating selected methods in example meta-analysis scenarios. Results: For missing standard deviations (SDs), following screening of 503 articles, fifteen methods were identified in addition to those reported in a previous review. These included Bayesian hierarchical modelling at the meta-analysis level; summary statistic level imputation based on observed SD values from other trials in the meta-analysis; a practical approximation based on the range; and algebraic estimation of the SD based on other summary statistics. Following screening of 1124 articles for methods estimating the mean, one approximate Bayesian computation approach and three papers based on alternative summary statistics were identified. Illustrative meta-analyses showed that when replacing a missing SD the approximation using the range minimised loss of precision and generally performed better than omitting trials. When estimating missing means, a formula using the median, lower quartile and upper quartile performed best in preserving the precision of the meta-analysis findings, although in some scenarios, omitting trials gave superior results. Conclusions: Methods based on summary statistics (minimum, maximum, lower quartile, upper quartile, median) reported in the literature facilitate more comprehensive inclusion of randomised controlled trials with missing mean or variability summary statistics within meta-analyses
