    Evaluation rechnergestĂŒtzter Pflegedokumentation auf vier Pilotstationen - Abschlussbericht, Teil QualitĂ€tsmessungen. (PIK Studie 2000/2001)

    Die Umstellung der konventionellen Pflegeprozessdokumentation auf ein rechnerbasiertes Pflegedokumentationssystem ist ein wichtiges Thema am UniversitĂ€tsklinikum Heidelberg. Daher werden seit 1998 Erfahrungen mit dem Pflegedokumentationssystem PIKÂź auf inzwischen vier Pilotstationen gesammelt und systematisch evaluiert. Neben Fragen zur Akzeptanz von Computern in der Pflege und des Pflegeprozesses wurden auch Fragen der QualitĂ€t von Pflegedokumentationen untersucht. Die Pflegedokumentation, als wichtiger Teil der klinischen Dokumentation, stellt eine TeilqualitĂ€t einer gesamten PflegequalitĂ€t dar. Der Pflegeprozess und seine Dokumentation werden in der Regel jedoch nur unzureichend auf den Stationen angewandt. Die GrĂŒnde, die hierfĂŒr in der Literatur angegeben werden, sind vielfĂ€ltig. U.a. werden Formulierungsschwierigkeiten und die mangelnde Akzeptanz des Pflegeprozesses angegeben. Der Einzug von rechnergestĂŒtzten Dokumentationssystemen in der Krankenpflege fĂŒhrt unweigerlich zu dem Vergleich von rechnergestĂŒtzten und konventionellen Pflegedokumentationssystemen. Die wissenschaftliche Begleitung der EinfĂŒhrung rechnergestĂŒtzter Pflegedokumentation ermöglichte es quantitative und qualitative Zahlen zur QualitĂ€t von Pflegedokumentationen zu erheben. Dies geschah anhand einer QualitĂ€tscheckliste, die aufgrund einer intensiven Literatursichtung erstellt wurde. Um die VerĂ€nderung in der QualitĂ€t der Pflegedokumentationen zu ermitteln, wurde von den vier Pilotstationen, auf denen PIKÂź in Routine lĂ€uft, je 20 Dokumentationen zu drei verschiedenen Zeitpunkten, von zwei externen Pflegeexperten, begutachtet - 3 Monate vor der EinfĂŒhrung von PIKÂź sowie 3 und 9 Monate nachher (insgesamt 240 Dokumentationen). Die PflegekrĂ€fte sehen in der Benutzung der rechnergestĂŒtzten Pflegedokumentation eine QualitĂ€tsverbesserung sowie eine Verbesserung der Lesbarkeit und der VollstĂ€ndigkeit der Pflegedokumentation. Diese subjektiven Aussagen der PflegekrĂ€fte wurden den objektiven Ergebnissen der QualitĂ€tsmessungen gegenĂŒbergestellt. Dabei wurde nachgeprĂŒft ob neben formalen Aspekten (wie Umfang und VollstĂ€ndigkeit) sich auch die inhaltliche QualitĂ€t verbessert hat. Die Gutachter konnten eine signifikante Verbesserung in der QualitĂ€t der Dokumentationen feststellen, vor allem bedingt durch die Erhöhung der formalen VollstĂ€ndigkeit. Die inhaltliche Ausgestaltung der Pflegedokumentationen und die Individualisierung der Pflegeplanung sind noch verbesserungsfĂ€hig. Die EinfĂŒhrung rechnergestĂŒtzter Pflegedokumentation stellt eine Möglichkeit dar nicht nur den Pflegeprozess erfolgreich einzufĂŒhren sondern auch eine qualitative Verbesserung der Pflegedokumentationen zu erreichen. Wichtiger Erfolgsfaktor fĂŒr eine qualitative Verbesserung der Dokumentation ist die Reflexion der Mitarbeiter ĂŒber ihre Dokumentationspraxis und die Betreuung bei der EinfĂŒhrung eines neuen Dokumentationsystems durch eine qualifizierte Pflegekraft

    Seit Jahren besteht der Wunsch, die Pflegedokumentation durch EDV-Einsatz zu unterstĂŒtzen, um die QualitĂ€t der Dokumentation zu erhöhen, AufwĂ€nde fĂŒr die PflegekrĂ€fte zu reduzieren, pflegerische AblĂ€ufe zu unterstĂŒtzen, und Auswertungsmöglichkeiten fĂŒr Pflegemanagement und Pflegeforschung zu verbessern. Die EinfĂŒhrung des rechnergestĂŒtzten Pflegedokumentationssystems PIK auf vier Pilotstationen am UniversitĂ€tsklinikum Heidelberg wird seit 1998 durch eine umfassende Evaluationsstudie begleitet. Ziel der Studie ist es, Erfolgsfaktoren und Auswirkungen rechnergestĂŒtzter Pflegedokumentationssysteme zu ermitteln, und Empfehlungen fĂŒr eine optimale EinfĂŒhrungsstrategie aufzustellen. Daher werden unter anderen die Auswirkungen der PIK-EinfĂŒhrung auf QualitĂ€t und VollstĂ€ndigkeit der Pflegedokumentation, auf die Akzeptanz des Pflegeprozesses, und auf die Akzeptanz von Computern allgemein und in der Pflege untersucht. Ausserdem werden die Akzeptanz von PIK bei pflegerischen und nicht-pflegerischen Mitarbeitern sowie die Auswirkungen auf die multiprofessionelle Kooperation berĂŒcksichtigt. Der vorliegende Forschungsbericht berichtet umfassend ĂŒber die Planung, DurchfĂŒhrung und Auswertung der Evaluation von PIK. Als Ergebnis der Studie wird unter AbwĂ€gung aller bisherigen Erkenntnisse empfohlen, ein rechnergestĂŒtztes Pflegedokumentationssystem mittelfristig am Klinikum Heidelberg einzufĂŒhren

    Pediatric in-hospital emergencies: real life experiences, previous training and the need for training among physicians and nurses

    Objective: Pediatric emergencies challenge professional teams by demanding substantial cognitive effort, skills and effective teamwork. Educational designs for team trainings must be aligned to the needs of participants in order to increase effectiveness. To assess these needs, a survey among physicians and nurses of a tertiary pediatric center in Germany was conducted, focusing on previous experience, previous training in emergency care, and individual training needs. Results: Fifty-three physicians and 75 nurses participated. Most frequently experienced emergencies were respiratory failure, resuscitation, seizure, shock/sepsis and arrhythmia. Resuscitations were perceived as being particularly precarious. Team collaboration and communication were major issues arising from previous emergency situations, but perceptions differed between physicians and nurses. Regarding previous training, physicians were accustomed to self-directed learning, whereas nurses usually attended practical courses. Both physicians and nurses rated themselves as having moderate levels of knowledge and skills for pediatric emergencies, though residents reported the significantly lowest preparedness. Both professions reported a high need for training of basic procedures and emergency algorithms, physicians even more than nurses

    EZH2 Depletion Blocks the Proliferation of Colon Cancer Cells

    The Enhancer of Zeste 2 (EZH2) protein has been reported to stimulate cell growth in some cancers and is therefore considered to represent an interesting new target for therapeutic intervention. Here, we investigated a possible role of EZH2 for the growth control of colon cancer cells. RNA interference (RNAi)-mediated intracellular EZH2 depletion led to cell cycle arrest of colon carcinoma cells at the G1/S transition. This was associated with a reduction of cell numbers upon transient transfection of synthetic EZH2-targeting siRNAs and with inhibition of their colony formation capacity upon stable expression of vector-borne siRNAs. We furthermore tested whether EZH2 may repress the growth-inhibitory p27 gene, as reported for pancreatic cancer. However, expression analyses of colon cancer cell lines and colon cancer biopsies did not reveal a consistent correlation between EZH2 and p27 levels. Moreover, EZH2 depletion did not re-induce p27 expression in colon cancer cells, indicating that p27 repression by EZH2 may be cell- or tissue-specific. Whole genome transcriptome analyses identified cellular genes affected by EZH2 depletion in colon cancer cell lines. They included several cancer-associated genes linked to cellular proliferation or invasion, such as Dag1, MageD1, SDC1, Timp2, and Tob1. In conclusion, our results demonstrate that EZH2 depletion blocks the growth of colon cancer cells. These findings might provide benefits for the treatment of colon cancer

    Common Promoter Elements in Odorant and Vomeronasal Receptor Genes

    In mammals, odorants and pheromones are detected by hundreds of odorant receptors (ORs) and vomeronasal receptors (V1Rs and V2Rs) expressed by sensory neurons that are respectively located in the main olfactory epithelium and in the vomeronasal organ. Even though these two olfactory systems are functionally and anatomically separate, their sensory neurons show a common mechanism of receptor gene regulation: each neuron expresses a single receptor gene from a single allele. The mechanisms underlying OR and VR gene expression remain unclear. Here we investigated if OR and V1R genes share common sequences in their promoter regions

    Ku counteracts mobilization of PARP1 and MRN in chromatin damaged with DNA double-strand breaks

    In mammalian cells, the main pathway for DNA double-strand breaks (DSBs) repair is classical non-homologous end joining (C-NHEJ). An alternative or back-up NHEJ (B-NHEJ) pathway has emerged which operates preferentially under C-NHEJ defective conditions. Although B-NHEJ appears particularly relevant to genomic instability associated with cancer, its components and regulation are still largely unknown. To get insights into this pathway, we have knocked-down Ku, the main contributor to C-NHEJ. Thus, models of human cell lines have been engineered in which the expression of Ku70/80 heterodimer can be significantly lowered by the conditional induction of a shRNA against Ku70. On Ku reduction in cells, resulting NHEJ competent protein extracts showed a shift from C- to B-NHEJ that could be reversed by addition of purified Ku protein. Using a cellular fractionation protocol after treatment with a strong DSBs inducer followed by western blotting or immunostaining, we established that, among C-NHEJ factors, Ku is the main counteracting factor against mobilization of PARP1 and the MRN complex to damaged chromatin. In addition, Ku limits PAR synthesis and single-stranded DNA production in response to DSBs. These data support the involvement of PARP1 and the MRN proteins in the B-NHEJ route for the repair of DNA DSBs

    Cardiorespiratory capacity in children and adolescents on maintenance haemodialysis

    Background. Paediatric patients on maintenance haemodialysis (HD) often report their own physical fitness to be hampered. Their physical endurance capacity is known to be significantly lower than that of healthy controls. However, physical endurance was up to now only examined on non-HD days. We were interested in the effect of HD on the cardiorespiratory capacity of children and adolescents on maintenance HD. Methods. We therefore examined the endurance capacity by cardiopulmonary exercise testing on a cycle ergospirometer, before and after HD in 14 patients (9 male, 5 female; mean age: 15.1 +/- 3.0 years; mean period on HD: 16.3 +/- 11.5 months). Results. The feasible time spent on the cycle ergospirometer was significantly reduced after dialysis (8.6 +/- 3.2 versus 6.4 +/- 2.5 min, P < 0.001), also the maximal workload was decreased (95 +/- 36 W to 74 +/- 29 W, P < 0.003). The starting and recovery heart rates were higher after than before dialysis. The peak oxygen uptake (VO2 peak, VO2 peak/kg) was reduced from 1.644 +/- 600 to 1.422 +/- 450 mL/min (P < 0.02) and from 35 +/- 7 to 31 +/- 5 mL/min/kg (P < 0.03), respectively. The respiratory exchange ratio as well as the lactate values showed significant differences in varied workload levels (P < 0.05). Conclusions. Paediatric patients on maintenance HD in general have a significantly lower exercise capacity compared to a healthy age-matched population, which is expressed much more after dialysis
