    Post cardiac surgery vasoplegia is associated with high preoperative copeptin plasma concentration

    International audienceABSTRACT: INTRODUCTION: Post cardiac surgery vasodilatation is possibly related to a vasopressin deficiency that could be related to a chronic stimulation of the adeno-hypophysis. To assess vasopressin system activation, perioperative course of copeptin and vasopressin plasma concentrations have been studied in consecutive patients operated on cardiac surgery. METHODS: 64 consecutive patients scheduled for elective cardiac surgery with cardiopulmonary bypass were studied. Haemodynamic, laboratory and clinical data were recorded before and during cardiopulmonary bypass, and at the 8th post-operative hour (H8). At the same time, point's blood was withdrawn to determine plasma concentrations of arginine-vasopressin (AVP, radioimmunoassay) and copeptin (immunoluminometric assay). Post cardiac surgery vasodilation (PCSV) was defined as a mean arterial blood pressure less than 60 mmHg with a cardiac index [equal to or greater than] 2.2 L * min^-1 * m^-2, and was treated with norepinephrine (NE) in order to restore a mean blood pressure > 60 mmHg. Patients with PCSV were compared to the other patients (controls). Student's t, Fisher's exact test, or non parametric tests (Mann Whitney, Wilkoxon) were used when appropriate. A correlation between AVP and copeptin has been evaluated and a receiver-operator characteristic (ROC) analysis was calculated to assess the utility of preoperative copeptin to distinguish between controls and PCSV patients. RESULTS: Patients who experienced a PCSV have significantly higher copeptin plasma concentration before cardiopulmonary bypass (P <0.001) but lower AVP concentrations at H8 (P <0.01) than controls. PCSV patients had preoperative hyponatremia and decreased left ventricle ejection fraction, and experienced more complex surgery (redo). The area under the ROC curve of preoperative copeptin concentration was 0.86[plus/minus]0.04 [95%CI: 0.78-0.94] (P <0.001). The best predictive value for preoperative copeptin plasma concentration was 9.43 pmol/L with a sensitivity of 90% and a specificity of 77%. CONCLUSIONS: High preoperative copeptin plasma concentration is predictive of PSCV and suggests an activation of the AVP system before surgery that may facilitate depletion of endogenous AVP stores and a relative AVP deficit after surgery

    Communication, Localization and Synchronization of Spacecraft for Swarm Missions

    Swarm missions are based on the use of several spacecraft working together to pursue a specific task for a specific mission. To allow these elements to work together, it is necessary for them to be able to communicate with each other and to synchronize themselves within the swarm. Moreover, the mission may likely require knowing the relative or absolute positions of the spacecraft in the swarm. In order to collect simultaneous measurements allowing computing localization and synchronization in the swarm, a full duplex CDMA communication method is studied by CNES. An Inter Satellite Link (ISL) transmitter prototype is currently under development and first performance evaluation is conducted. CNES is also working on measurement signal processing. Based on signal exchange between satellites, one can estimate jointly the distance and clock offset between a pair of satellites. In parallel, CNES is developing a swarm simulator implying both dynamics and functional behavior of each spacecraft in the swarm. First, this simulator will be software only but its architecture will allow integration of hardware equipment in a future version. This simulator will be used for the validation of the services provided by the link at a system level

    Effects of blood pressure lowering on cerebral white matter hyperintensities in patients with stroke: the PROGRESS (Perindopril Protection Against Recurrent Stroke Study) Magnetic Resonance Imaging Substudy.: The PROGRESS MRI Substudy.

    International audienceBACKGROUND: The prevalence of white matter hyperintensities (WMHs) detected on cerebral MRI is associated with hypertension, but it is not known whether blood pressure lowering can arrest their progression. We report here the results of an MRI substudy of PROGRESS (Perindopril Protection Against Recurrent Stroke Study), a randomized trial of blood pressure lowering in subjects with cerebrovascular disease. METHODS AND RESULTS: The substudy comprised 192 participants who had a cerebral MRI both at baseline and after a mean follow-up time of 36 months (SD=6.0 months). At the first MRI, WMHs were graded with a visual rating scale from A (no WMH) to D (severe WMH). Participants were assigned to a combination of perindopril plus indapamide (or their placebos; 58%) or to single therapy with perindopril (or placebo). At the time of the second MRI, the blood pressure reduction in the active arm compared with the placebo arm was 11.2 mm Hg for systolic blood pressure and 4.3 mm Hg for diastolic blood pressure. Twenty-four subjects (12.5%) developed new WMHs at follow-up. The risk of new WMH was reduced by 43% (95% CI -7% to 89%) in the active treatment group compared with the placebo group (P=0.17). The mean total volume of new WMHs was significantly reduced in the active treatment group (0.4 mm3 [SE=0.8]) compared with the placebo group (2.0 mm3 [SE=0.7]; P=0.012). This difference was greatest for patients with severe WMH at entry, 0.0 mm3 (SE=0) in the active treatment group versus 7.6 mm3 (SE=1.0) in the placebo group (P<0.0001). CONCLUSIONS: These results indicate that an active blood pressure-lowering regimen stopped or delayed the progression of WMHs in patients with cerebrovascular disease

    Functional Simulation of the Uses of the Onboard Inter-Satellite Network in a Swarm

    There are several network simulators (some on the shelves) that are very useful and helpful for network engineering. Nevertheless, none is efficient enough for simulating an onboard network spread among spacecrafts that are moving in space according to Kepler laws. Therefore, we tried to fill this gap by building our own tool based on the CNES expertise in space simulator benches. Our bench simulates the environment and the dynamics of each spacecraft, provides a model of the physical layer of the network layer (i.e. Radio Frequency transmission between moving vehicles) and offers an interface to run actual Flight Software. Embedding flight software allows us to perform functional demonstration of the network use at system level. As a first step, in order to characterize the network, we have implemented a routing policy based on the flooding principle

    <p>Abstract</p> <p>Background</p> <p>The adult central nervous system (CNS) contains different populations of immature cells that could possibly be used to repair brain and spinal cord lesions. The diversity and the properties of these cells in the human adult CNS remain to be fully explored. We previously isolated Nestin<sup>+ </sup>Sox2<sup>+ </sup>neural multipotential cells from the adult human spinal cord using the neurosphere method (i.e. non adherent conditions and defined medium).</p> <p>Results</p> <p>Here we report the isolation and long term propagation of another population of Nestin<sup>+ </sup>cells from this tissue using adherent culture conditions and serum. QPCR and immunofluorescence indicated that these cells had mesenchymal features as evidenced by the expression of Snai2 and Twist1 and lack of expression of neural markers such as Sox2, Olig2 or GFAP. Indeed, these cells expressed markers typical of smooth muscle vascular cells such as Calponin, Caldesmone and Acta2 (Smooth muscle actin). These cells could not differentiate into chondrocytes, adipocytes, neuronal and glial cells, however they readily mineralized when placed in osteogenic conditions. Further characterization allowed us to identify the Nkx6.1 transcription factor as a marker for these cells. Nkx6.1 was expressed in vivo by CNS vascular muscular cells located in the parenchyma and the meninges.</p> <p>Conclusion</p> <p>Smooth muscle cells expressing Nestin and Nkx6.1 is the main cell population derived from culturing human spinal cord cells in adherent conditions with serum. Mineralization of these cells in vitro could represent a valuable model for studying calcifications of CNS vessels which are observed in pathological situations or as part of the normal aging. In addition, long term propagation of these cells will allow the study of their interaction with other CNS cells and their implication in scar formation during spinal cord injury.</p

    Genome-Wide Control of the Distribution of Meiotic Recombination

    Meiotic recombination events are not randomly distributed in the genome but occur in specific regions called recombination hotspots. Hotspots are predicted to be preferred sites for the initiation of meiotic recombination and their positions and activities are regulated by yet-unknown controls. The activity of the Psmb9 hotspot on mouse Chromosome 17 (Chr 17) varies according to genetic background. It is active in strains carrying a recombinant Chr 17 where the proximal third is derived from Mus musculus molossinus. We have identified the genetic locus required for Psmb9 activity, named Dsbc1 for Double-strand break control 1, and mapped this locus within a 6.7-Mb region on Chr 17. Based on cytological analysis of meiotic DNA double-strand breaks (DSB) and crossovers (COs), we show that Dsbc1 influences DSB and CO, not only at Psmb9, but in several other regions of Chr 17. We further show that CO distribution is also influenced by Dsbc1 on Chrs 15 and 18. Finally, we provide direct molecular evidence for the regulation in trans mediated by Dsbc1, by showing that it controls the CO activity at the Hlx1 hotspot on Chr 1. We thus propose that Dsbc1 encodes for a trans-acting factor involved in the specification of initiation sites of meiotic recombination genome wide in mice

    Study and suppression of the microstructural anisotropy generated during the consolidation of a carbonyl iron powder by field-assisted hot pressing

    Published OnlineA spherical carbonyl iron powder was consolidated by the field-assisted hot pressing technique using graphite tools at two different temperatures, both above the austenitizing temperature. The microstructures obtained exhibited a compositional gradient in carbon along the consolidated material. Thus, the outer rim of the cylindrical samples was composed of cementite and pearlite that gradually turned to pearlite, leading to a fully ferritic microstructure at the core of the sample. The increase in the temperature has led to a higher introduction of carbon within the sample. The interposition of a thin tungsten foil between the graphite die/punches and the powders has significantly reduced the diffusion of the carbon through the iron matrix and has suppressed the microstructural anisotropy.Publicad
