    82 p.Pese a que la investigación respecto a la economía circular es amplia, aún existe bajo conocimiento respecto a este concepto y su implementación en las organizaciones, en especial en las organizaciones pertenecientes a la agroindustria. Por lo que en este trabajo se estudió el análisis del grado de implementación de la economía circular en las empresas agroindustriales pertenecientes al programa estratégico regional Maule Alimenta ubicadas en la región del Maule, Chile. Dadas las características de esta investigación, esta investigación fue planteada desde el nivel micro de investigación e implementación de la economía circular. Se utilizó como método para el análisis de la implementación de la economía circular a nivel micro el uso de herramientas digitales de diagnóstico validadas en la literatura, dada las características del sector agroindustrial y debido a las variables de interés en que se enfocan estas herramientas como la sustentabilidad, las etapas del ciclo de vida, el interés en el producto y servicio, entre otras. Se invitó a participar a 8 empresas agroindustriales hortofrutícola y de alimentos ubicadas en la región las cuales pertenecen al PER-Maule Alimenta. Cada empresa designo a un encargado con experiencia en el área para responder el cuestionario de 90 preguntas correspondientes a las preguntas requeridas por las herramientas digitales para realizar la evaluación. Estas herramientas digitales para el diagnóstico de la economía circular proporcionaron los resultados y descubrimientos de la investigación. Los resultados obtenidos pueden ser interpretados como un punto de partida para determinar el grado de implementación de la economía circular a través de los indicadores globales de carácter cualitativo. Las empresas del caso de estudio obtuvieron puntajes sobre el nivel básico y operativo en la mayoría de los indicadores, observando diferencias leves en algunos indicadores dependiendo el área o rubro en el que pertenece la organización. // ABSTRACT: Although the research regarding the circular economy is extensive, there is still low knowledge regarding this concept and its implementation in organizations, especially in organizations belonging to agroindustry. Therefore, in this work, the analysis of the degree of implementation of the circular economy in the agroindustrial companies belonging to the Maule Alimenta regional strategic program located in the Maule region, Chile, was studied. Given the characteristics of this research, this research was raised from the micro level of research and implementation of the circular economy. The use of digital diagnostic tools validated in the literature was used as a method for the analysis of the implementation of the circular economy at the micro level, given the characteristics of the agro-industrial sector and due to the variables of interest on which these tools focus, such as sustainability, life cycle stages, interest in the product and service, among others. 8 agro-industrial fruit and vegetable and food companies located in the region, which belong to PER-Maule Alimenta, were invited to participate. Each company appointed a person in charge with experience in the area to answer the 90-question questionnaire corresponding to the questions required by the digital tools to carry out the evaluation. These digital tools for diagnostics of the circular economy provided the results and findings of the research. The results obtained can be interpreted as a starting point to determine the degree of implementation of the circular economy through global indicators of a qualitative nature. The companies in the case study obtained scores on the basic and operational level in most of the indicators, observing slight differences in some indicators depending on the area or category in which the organization belongs

    Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

    stg fimbrial operon from S. Typhi STH2370 contributes to association and cell disruption of epithelial and macrophage-like cells

    BACKGROUND: Salmonella enterica serovar Typhi (S. Typhi) stg operon, encoding a chaperone/usher fimbria (CU), contributes to an increased adherence to human epithelial cells. However, one report suggests that the presence of the Stg fimbria impairs the monocyte-bacteria association, as deduced by the lower level of invasion to macrophage-like cells observed when the stg fimbrial cluster was overexpressed. Nevertheless, since other CU fimbrial structures increase the entry of S. Typhi into macrophages, and considering that transcriptomic analyses revealed that stg operon is indeed expressed in macrophages, we reassessed the role of the stg operon in the interaction between S. Typhi strain STH2370 and human cells, including macrophage-like cells and mononuclear cells directly taken from human peripheral blood. RESULTS: We compared S. Typhi STH2370 WT, a Chilean clinical strain, and the S. Typhi STH2370 Astg mutant with respect to association and invasion using epithelial and macrophage-like cells. We observed that deletion of stg operon reduced the association and invasion of S. Typhi, in both cellular types. The presence of the cloned stg operon restored the WT phenotype in all the cases. Moreover, we compared Salmonella enterica sv. Typhimurium 14028s (S. Typhimurium, a serovar lacking stg operon) and S. Typhimurium heterologously expressing S. Typhi stg. We found that the latter presents an increased cell disruption of polarized epithelial cells and an increased association in both epithelial and macrophage-like cells. CONCLUSIONS: S. Typhi stg operon encodes a functional adhesin that participates in the interaction bacteria-eukary-otic cells, including epithelial cells and macrophages-like cells. The phenotypes associated to stg operon include increased association and consequent invasion in bacteria-eukaryotic cells, and cell disruption

    Phosphate Favors the Biosynthesis of CdS Quantum Dots in Acidithiobacillus thiooxidans ATCC 19703 by Improving Metal Uptake and Tolerance

    Recently, we reported the production of Cadmium sulfide (CdS) fluorescent semiconductor nanoparticles (quantum dots, QDs) by acidophilic bacteria of the Acidithiobacillus genus. Here, we report that the addition of inorganic phosphate to Acidithiobacillus thiooxidans ATCC 19703 cultures favors the biosynthesis of CdS QDs at acidic conditions (pH 3.5). The effect of pH, phosphate and cadmium concentrations on QDs biosynthesis was studied by using Response Surface Methodology (RSM), a multivariate technique for analytical optimization scarcely used in microbiological studies to date. To address how phosphate affects intracellular biosynthesis of CdS QDs, the effect of inorganic phosphate on bacterial cadmium-uptake was evaluated. By measuring intracellular levels of cadmium we determined that phosphate influences the capacity of cells to incorporate this metal. A relation between cadmium tolerance and phosphate concentrations was also determined, suggesting that phosphate participates in the adaptation of bacteria to toxic levels of this metal. In addition, QDs-biosynthesis was also favored by the degradation of intracellular polyphosphates. Altogether, our results indicate that phosphate contributes to A. thiooxidans CdS QDs biosynthesis by influencing cadmium uptake and cadmium tolerance. These QDs may also be acting as a nucleation point for QDs formation at acidic pH. This is the first study reporting the effect of phosphates on QDs biosynthesis and describes a new cadmium-response pathway present in A. thiooxidans and most probably in other bacterial species

    Evolution over Time of Ventilatory Management and Outcome of Patients with Neurologic Disease∗

    OBJECTIVES: To describe the changes in ventilator management over time in patients with neurologic disease at ICU admission and to estimate factors associated with 28-day hospital mortality. DESIGN: Secondary analysis of three prospective, observational, multicenter studies. SETTING: Cohort studies conducted in 2004, 2010, and 2016. PATIENTS: Adult patients who received mechanical ventilation for more than 12 hours. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Among the 20,929 patients enrolled, we included 4,152 (20%) mechanically ventilated patients due to different neurologic diseases. Hemorrhagic stroke and brain trauma were the most common pathologies associated with the need for mechanical ventilation. Although volume-cycled ventilation remained the preferred ventilation mode, there was a significant (p &lt; 0.001) increment in the use of pressure support ventilation. The proportion of patients receiving a protective lung ventilation strategy was increased over time: 47% in 2004, 63% in 2010, and 65% in 2016 (p &lt; 0.001), as well as the duration of protective ventilation strategies: 406 days per 1,000 mechanical ventilation days in 2004, 523 days per 1,000 mechanical ventilation days in 2010, and 585 days per 1,000 mechanical ventilation days in 2016 (p &lt; 0.001). There were no differences in the length of stay in the ICU, mortality in the ICU, and mortality in hospital from 2004 to 2016. Independent risk factors for 28-day mortality were age greater than 75 years, Simplified Acute Physiology Score II greater than 50, the occurrence of organ dysfunction within first 48 hours after brain injury, and specific neurologic diseases such as hemorrhagic stroke, ischemic stroke, and brain trauma. CONCLUSIONS: More lung-protective ventilatory strategies have been implemented over years in neurologic patients with no effect on pulmonary complications or on survival. We found several prognostic factors on mortality such as advanced age, the severity of the disease, organ dysfunctions, and the etiology of neurologic disease
