    Land Planning and Extreme Snow Events: The Case of Buthier Avalanche in Aosta Valley - IT

    It is well known that climate change affects all sectors of human life also increasing the natural hazard. More sensitive to its effects, the Alpine regions are coped with unpredictable scenarios caused by unexpected weather conditions. The risk management is carried out by different defence methods - from the infrastructures to the management – and nowadays, it must take into account necessarily the variability of weather conditions caused by climate change. The article reports the example of the indispensable update of hazard mapping used to reduce the snow avalanche danger with urban constraints in Cogne – Aosta Valley, in north-eastern of Italian Alps. On 15th December 2008, an event induced by extreme weather and snow conditions occurred exceeding the known limits of the avalanche basin: the paper shows how the Avalanche Warning Service of Aosta Valley had to investigate in order to estimate the return period of the event and to update hazard maps. Due to the lack of information, the study was carried out coupling the historical and dendrochronological investigations of the woods embodied in avalanche basin and destroyed by the event.     LEARNING FROM THE BUILDING: DIRECT SOURCES FOR THE PRESERVATION PROJECT. THE EXPERIENCE OF BESOZZO'S TOWN HALL (VARESE, ITALY)

    The Town Hall of Besozzo (Varese, Italy) is located in the city centre of the village and its first construction phase is dated back to the XIV-XV century. It shows a complex palimpsest which is the result of the numerous transformations occurred during its life: enlargements, super elevations, demolitions, inner spaces subdivisions and use changes. Currently a project has been issued for the reuse of the building which assigns new spaces for the town offices to the northern wing recently acquired. The aim of the research was to provide a diagnostic insight, useful for the development of the conservation project which will necessarily take into account the multitude of values registered on the building. Owing to a lack of meaningful archival documentation, the elevation's stratigraphic reading and the methods for dating historical buildings proved to be an invaluable resource for the comprehension of the building's transformations. Cross-referencing readings of indirect sources carried on the building with the results of the in-depth analysis made it possible to rebuild the growth of the structure from its origin to the present days. Such analysis includes: geometric survey, photographic rectifications of facade and inner sections, non-destructive diagnostic investigations, bricks, mortar and plaster chemical-physical analysis, mensiochronology, study of the building techniques and chronotypology which is a stylistic analysis performed both on the constructive (apertures) and decorative (shelves, graffiti, colourings traces) architectural elements. Blending the results of these dating techniques produced the complexity of the stratigraphic reading which has been conveyed with adequate hatching on the rectified images (U.S. – Stratigraphic Unity) while schematic 3D reconstructions exemplify the chronological sequence of the building activities. Individuation and comprehension of the building constructive phases made also possible to understand which were the different uses of each room inside this domestic architecture thus providing the client and the bodies in charge of protection with valuable data for the preservation project

    Yambo: an \textit{ab initio} tool for excited state calculations

    {\tt yambo} is an {\it ab initio} code for calculating quasiparticle energies and optical properties of electronic systems within the framework of many-body perturbation theory and time-dependent density functional theory. Quasiparticle energies are calculated within the GWGW approximation for the self-energy. Optical properties are evaluated either by solving the Bethe--Salpeter equation or by using the adiabatic local density approximation. {\tt yambo} is a plane-wave code that, although particularly suited for calculations of periodic bulk systems, has been applied to a large variety of physical systems. {\tt yambo} relies on efficient numerical techniques devised to treat systems with reduced dimensionality, or with a large number of degrees of freedom. The code has a user-friendly command-line based interface, flexible I/O procedures and is interfaced to several publicly available density functional ground-state codes.Comment: This paper describes the features of the Yambo code, whose source is available under the GPL license at www.yambo-code.or

    Protocols of Anesthesia Management in Parturients with SARS-CoV-2 Infection

    Background: Our hospital became a referral center for COVID-19-positive obstetric patients from 1 May 2020. The aim of our study is to illustrate our management protocols for COVID-19-positive obstetric patients, to maintain safety standards for patients and healthcare workers. Methods: Women who underwent vaginal or operative delivery and induced or spontaneous abortion with a SARS-CoV-2-positive nasopharyngeal swab using real-time PCR (RT-PCR) were included in the study. Severity and onset of new symptoms were carefully monitored in the postoperative period. All the healthcare workers received a nasopharyngeal swab for SARS-CoV-2 using RT-PCR serially every five days. Results: We included 152 parturients with COVID-19 infection. None of the included women had general anesthesia, an increase of severe symptoms or onset of new symptoms. The RT-PCR test was “negative” for the healthcare workers. Conclusions: In our study, neuraxial anesthesia for parturients’ management with SARS-CoV-2 infection has been proven to be safe for patients and healthcare workers. Neuraxial anesthesia decreases aerosolization during preoxygenation, face-mask ventilation, endotracheal intubation, oral or tracheal suctioning and extubation.     Clinical efficacy of Enzyme Replacement Therapy in paediatric Hunter patients, an independent study of 3.5 years

    BACKGROUND: Hunter Syndrome is an X-linked lysosomal storage disorder due to the deficit of iduronate 2-sulfatase, an enzyme catalysing the degradation of the glycosaminoglycans (GAG) dermatan- and heparan-sulfate. Treatment of the disease is mainly performed by Enzyme Replacement Therapy (ERT) with idursulfase, in use since 2006. Clinical efficacy of ERT has been monitored mainly by the Hunter Outcome Survey (HOS) while very few independent studies have been so far conducted. The present study is a 3.5-years independent follow-up of 27 Hunter patients, starting ERT between 1.6 and 27 years of age, with the primary aim to evaluate efficacy of the therapy started at an early age (<12 years). METHODS: In this study, we evaluated: urinary GAG content, hepato/splenomegaly, heart valvulopathies, otorinolaryngological symptoms, joint range of motion, growth, distance covered in the 6-minute walk test, neurological involvement. For data analysis, the 27 patients were divided into three groups according to the age at start of ERT: ≤5 years, >5 and ≤ 12 years and > 12 years. Patients were analysed both as 3 separate groups and also as one group; in addition, the 20 patients who started ERT up to 12 years of age were analysed as one group. Finally, patients presenting a “severe” phenotype were compared with “attenuated” ones. RESULTS: Data analysis revealed a statistically significant reduction of the urinary GAG in patients ≤5 years and ≤ 12 years and of the hepatomegaly in the group aged >5 and ≤ 12 years. Although other clinical signs improved in some of the patients monitored, statistical analysis of their variation did not reveal any significant changes following enzyme administration. The evaluation of ERT efficacy in relation to the severity of the disease evidenced slightly higher improvements as for hepatomegaly, splenomegaly, otological disorders and adenotonsillar hypertrophy in severe vs attenuated patients. CONCLUSIONS: Although the present protocol of idursulfase administration may result efficacious in delaying the MPS II somatic disease progression at some extent, in this study we observed that several signs and symptoms did not improve during the therapy. Therefore, a strict monitoring of the efficacy obtained in the patients under ERT is becoming mandatory for clinical, ethical and economic reasons.
