    Use of health information systems in small municipalities in Southern Brazil

    OBJECTIVE: To describe the use of health information systems in towns with fewer than 10,000 inhabitants. METHODS: Study conducted in the state of Rio de Grande do Sul, Southern Brazil, between 2003 and 2004. A self-administered questionnaire was sent to municipal managers, containing 11 single-choice questions, three multiple-choice questions and three open questions on the structure available, use of information, indicators valued and satisfaction with the systems. The questionnaire was answered by managers in 127 of the municipalities in this state with fewer than 10,000 inhabitants (37.7%). The responses were tabulated in an electronic spreadsheet and the differences between respondent and non-respondent municipalities were evaluated using the chi-square test, taking the significance level to be p < 0.05. RESULTS: All the municipalities had computers available (mean of three per municipality) and 94% had internet access. The personnel responsible for information system inputs and analysis were public employees (59%) who also performed other tasks. The systems most used related to budget control and transfer of funds. Data analysis and generation of information used in local planning was carried out in 59.1% of the municipalities. The indicators cited as important for local planning were the same ones used in arrangements agreed with the state, but there was difficulty in understanding the terms "indicators" and "statistical data". Only 4.7% were fully satisfied with the information obtained from the health information systems. CONCLUSIONS: Two realities coexisted: municipalities that perceived that inputs to health information systems were a task to be complied with because of orders from central levels, in contrast with municipalities that saw the potential for these systems but had difficulty in using them.OBJETIVO: Describir el uso de los Sistemas de Información en Salud en ciudades con menos de 10 mil habitantes. O questionário foi respondido por gestores de 127 (37,7%) dos municípios gaúchos com menos de 10 mil habitantes. As respostas foram tabuladas em planilha eletrônica e a diferença entre municípios respondentes e não-respondentes foi avaliada pelo teste qui-quadrado, considerando-se significativo p < 0,05. RESULTADOS: Todos os municípios dispunham de computadores (média de três por município) e 94% tinham acesso à Internet. Os responsáveis pela alimentação e análise dos sistemas de informação eram funcionários estatutários (59%) que acumulavam outras tarefas. Os sistemas mais utilizados relacionavam-se a controle orçamentário e repasse de verbas. Em 59,1% dos municípios havia análise dos dados e geração de informações utilizadas no planejamento local. Os indicadores citados como importantes para o planejamento local foram os mesmos utilizados na pactuação com o Estado, mas houve dificuldade de compreensão dos termos "indicadores" e "dados estatísticos". Apenas 4,7% estavam plenamente satisfeitos com as informações obtidas dos sistemas de informação em saúde. CONCLUSÕES: Duas realidades coexistem: municípios que percebem a alimentação dos sistemas de informação em saúde como tarefa a ser cumprida por ordem dos níveis centrais, em contraposição a municípios que visualizam o potencial desses sistemas, mas têm dificuldades em sua utilização

    The objective of this study was to demonstrate face validity with a novel resource allocation framework designed to maximize equity into dental booking systems. The study was carried out in 2014. Eleven experts in primary dental care practice in Southern Brazil participated, using a three-round consensus group technique. First, the experts reached consensus on the items to be included in a 5-level diagnostic scale. They identified 21 clinical conditions and categorized them according to the oral health intervention required. Then, they described workload and activity standards for dental staff to carry out health promotion, oral disease prevention, dental treatment, dental rehabilitation, and urgent dental care. Finally, they agreed upon a set of wait times for primary dental care, establishing maximum waits from 2 to 365 days, according to the diagnostic classification. The framework demonstrated potential ability to promote more equitable access to primary dental services, since equal diagnostic classifications share the same waiting times for the dental care they require.O objetivo do estudo foi demonstrar validade de face com uma nova matriz destinada a maximizar a equidade nos sistemas de agendamento odontológico. O estudo foi realizado em 2014, no qual participaram 11 dentistas com experiência de trabalho na rede básica de saúde da região sul do Brasil, utilizando a técnica de grupo de consenso em três rodadas de discussão. Primeiro, os participantes chegaram ao consenso quanto aos itens que deveriam estar presentes em uma escala de classificação diagnóstica de 5 níveis. Identificaram 21 condições clínicas de saúde bucal e as categorizaram conforme a intervenção necessária. A seguir, os participantes descreveram as cargas de trabalho e os padrões de atividade recomendados para a equipe odontológica realizar promoção da saúde, prevenção de doenças bucais, tratamento odontológico, reabilitação dentária, e atendimento odontológico de urgência. Por último, os dentistas chegaram ao consenso sobre tempos máximos de espera para atendimento odontológico na rede básica, estabelecendo prazos de 2 até 365 dias conforme a classificação diagnóstica atribuída. Este estudo demonstrou o potencial da matriz de alocação de recursos para promover acesso mais equitativo aos serviços odontológicos da rede básica, uma vez que classificações diagnósticas iguais compartilham os mesmos prazos de espera para o atendimento odontológico requerido

    Occurrence of dental emergency events in primary health care services

    The aim of this study was to describe the occurrence of dental emergency and its association with individual factors and primary health care services. A follow-up study was conducted with data extracted from an exploratory study about the classification of dental care needs over time according to a care framework. There were included 1831 patients of five services. The outcome was the occurrence of dental emergency analyzed according to sex, age, skin color, service and maximum waiting time for dental care. A multivariate analysis with Poisson regression was used to estimate weighted prevalence ratio (PR) with 95% Confidence Intervals (CI) and survival analysis was conducted. The prevalence of dental emergency was 12.6%, varying according to age (13–19: PRa =1.79 (95%CI: 1.0–3.21); 20–65: PRa = 2.71 (95%CI: 1.73–4.26); Over 65: PRa = 2.51 (95%CI: 1.41–4.46)) and Primary Health Care service (FHS 2: PRa = 2.20 (95%CI: 1.37–3.53), FHS 3: PRa = 1.43 (95%CI: 0.90–2.27); FHS 4: PRa = 3.25 (95%CI: 2.15–4.92), FHS 5: PRa = 2.49 (95%CI: 1.56–3.97)) For 231 cases classified as emergency, the failure rate was 7.4%. For 214 cases of emergency, the non-continuity after appointment rate was 53.7%. The incidence of dental emergency was 8.3% and recurrence was 7.2%. Considering all 262 emergency cases attended, the resolution rate was 93.5% and most cases (n = 252, 96.1%) received care within one day. The results point to high effectiveness in emergency dental care within Primary Health Care services. There are indications of the need for improvements in retention and continuity of care

    Myoelectric activity during electromagnetic resistance alone and in combination with variable resistance or eccentric overload.

    The purpose of this study was to compare the effects of electromagnetic resistance alone, as well as in combination with variable resistance or accentuated eccentric methods, with traditional dynamic constant external resistance exercise on myoelectric activity during elbow flexion. The study employed a within-participant randomized, cross-over design whereby 16 young, resistance-trained male and female volunteers performed elbow flexion exercise under each of the following conditions: using a dumbbell (DB); using a commercial electromagnetic resistance device (ELECTRO); variable resistance (VR) using a setting on the device that attempts to match the level of resistance to the human strength curve, and; eccentric overload (EO) using a setting on the device that increases the load by 50% on the eccentric portion of each repetition. Surface electromyography (sEMG) was obtained for the biceps brachii, brachioradialis and anterior deltoid on each of the conditions. Participants performed the conditions at their predetermined 10 repetition maximum. The order of performance for the conditions was counterbalanced, with trials separated by a 10-minute recovery period. The sEMG was synced to a motion capture system to assess sEMG amplitude at elbow joint angles of 30°, 50°, 70°, 90°, 110°, with amplitude normalized to maximal voluntary isometric contraction. The anterior deltoid showed the largest differences in amplitude between conditions, where median estimates indicated greater concentric sEMG amplitude (~7 to 10%) with EO, ELECTRO and VR compared with DB. Concentric biceps brachii sEMG amplitude was similar between conditions. In contrast, results indicated a greater eccentric amplitude with DB compared to ELECTRO and VR, but unlikely to exceed a 5% difference. Data indicated a greater concentric and eccentric brachioradialis sEMG amplitude with DB compared to all other conditions, but differences were unlikely to exceed 5%. The electromagnetic device tended to produce greater amplitudes in the anterior deltoid, while DB tended to produce greater amplitudes in the brachioradialis; amplitude for the biceps brachii was relatively similar between conditions. Overall, any observed differences were relatively modest, equating to magnitudes of ~5% and not likely greater than 10%. These differences would seem to be of minimal practical significance

    Current models of landscape response to Holocene climate change in midcontinent North America largely reconcile Earth orbital and atmospheric climate forcing with pollen-based forest histories on the east and eolian chronologies in Great Plains grasslands on the west. However, thousands of sand dunes spread across 12,000 km2 in eastern upper Michigan (EUM), more than 500 km east of the present forest-prairie ecotone, present a challenge to such models. We use 65 optically stimulated luminescence (OSL) ages on quartz sand deposited in silt caps (n = 8) and dunes (n = 57) to document eolian activity in EUM. Dune building was widespread ca. 10–8 ka, indicating a sharp, sustained decline in forest cover during that period. This decline was roughly coincident with hydrologic closure of the upper Great Lakes, but temporally inconsistent with most pollen-based models that imply canopy closure throughout the Holocene. Early Holocene forest openings are rarely recognized in pollen sums from EUM because faint signatures of non-arboreal pollen are largely obscured by abundant and highly mobile pine pollen. Early Holocene spikes in nonarboreal pollen are recorded in cores from small ponds, but suggest only a modest extent of forest openings. OSL dating of dune emplacement provides a direct, spatially explicit archive of greatly diminished forest cover during a very dry climate in eastern midcontinent North America ca. 10–8 ka

    Gaining more from doing less? The effects of a one-week deload period during supervised resistance training on muscular adaptations.

    Based on emerging evidence that brief periods of cessation from resistance training (RT) may re-sensitize muscle to anabolic stimuli, we aimed to investigate how a 1-week deload interval at the midpoint of a 9-week RT program affected muscular adaptations in resistance-trained individuals. Thirty-nine young men (n=29) and women (n=10) were randomly assigned to one of two experimental, parallel groups: An experimental group that abstained from RT for 1 week at the midpoint of a 9-week, high-volume RT program (DELOAD) or a traditional training group that performed the same RT program continuously over the study period (TRAD). The lower body routines were directly supervised by the research staff while upper body training was carried out in an unsupervised fashion. Muscle growth outcomes included assessments of muscle thickness along proximal, mid and distal regions of the middle and lateral quadriceps femoris as well as the mid-region of the triceps surae. Adaptations in lower body isometric and dynamic strength, local muscular endurance of the quadriceps, and lower body muscle power were also assessed. Results indicated no appreciable differences in increases of lower body muscle size, local endurance, and power between groups. Alternatively, TRAD showed greater improvements in both isometric and dynamic lower body strength compared to DELOAD. Additionally, TRAD showed some slight psychological benefits as assessed by the readiness to train questionnaire over DELOAD. In conclusion, our findings suggest that a 1-week deload period at the midpoint of a 9-week RT program appears to negatively influence measures of lower body muscle strength but has no effect on lower body hypertrophy, power or local muscular endurance
