Programa de capacitación sobre ITS/VIH/SIDA para estudiantes de quinto año de Tecnología de la Salud
An Educative intervention study was carried out through the implementation of a training program on STD/HIV/AIDS, for 5th year students of health technology at “Haydée Santamaría Cuadrado” Medical Sciences Subsidiary in Manzanillo, since September 2011 to may 2012. From a universe of 603 students that fulfilled the inclusion criteria, there were selected at random 100 of them and they participated in the program. The program was developed through the application of an initial survey in a first stage that allowed to evaluate the level of knowledge of the students before the educative intervention. Later on it was implemented the program designed by the author from previous results with the objective to increase the level of knowledge of the students on this subject, and after the intervention the initial diagnosis was applied once more. The obtained result was valued through the application of theoretical methods (observation, interview, survey, analysis and synthesis) and empirical and statistical methods, showing highly significant differences before and after the application of the training program, and it was evaluated as efficient.Se realizó un estudio de intervención educativa a través de la implementación de un programa de capacitación referente a las ITS/VIH/SIDA destinado a estudiantes de quinto año de Tecnología de la Salud de la Filial de Ciencias Médicas “Haydée Santamaría Cuadrado” del Municipio de Manzanillo, en el período comprendido entre septiembre 2013 a mayo de 2014. De un universo de 603 estudiantes que cumplían con los criterios de inclusión se seleccionaron al azar 100 que participaron en el programa de capacitación. El programa propuesto se desarrolló a partir de la aplicación de una encuesta inicial en una primera etapa que permitió evaluar el nivel de conocimiento que tenían estos alumnos antes de la intervención educativa. Posteriormente se implementó el programa diseñado por la autora a partir de los resultados anteriores, con el objetivo de elevar el nivel del conocimiento que poseían estos estudiantes al respecto, después de la intervención se aplicó nuevamente la encuesta inicial. El resultado obtenido se valoró mediante la aplicación de los métodos teóricos (observación, entrevista, encuesta, análisis y síntesis), empíricos y estadísticos, arrojando diferencias altamente significativas antes y después de aplicado el programa de capacitación, por lo que el mismo fue evaluado de eficaz
Social need of a correct language preparation in English of the students from the clinical cycle of medicine career
El presente trabajo, "Necesidad social de un correcta preparación idiomática en lengua inglesa de los estudiantes del ciclo clínico de la carrera de medicina.", aborda elementos significativos en relación al proceso de comunicación social de una correcta preparación idiomática en lengua inglesa con fines médicos en los estudiantes del quinto año de carrera en lengua Inglesa con fines médicos. En el mismo se realiza un análisis sobre la base de las contradicciones del programa actual de la disciplina Inglés y su relación con la formación idiomática en lengua inglesa de los alumnos que cursan el quinto año de la carrera de medicina y además se identifican las principales contradicciones que aparecen en dicho proceso. A raíz de lo cual emerge un problema social expresado en las limitaciones en el desarrollo del proceso comunicativo en lengua inglesa con fines médicos que presentan los estudiantes del quinto año de la carrera de medicina y que van en detrimento de un elevado y buen servicio de salud con el que se aspira contar en su futuro desempeño profesional. Se caracteriza además el proceso de formación idiomática en el quinto año de la carrera de medicina y se ponen de manifiesto los desaciertos presentes en el mismo. Se hace una revisión al plan de estudio de la carrera para conocer cuál es la aportación de la disciplina antes mencionada en el proceso de formación del médico en sus problemas profesionales, objeto de trabajo, modos y esferas de actuación, lo que justifica la necesidad social de su estudio. Este problema tiene contradicciones epistemológicas, culturales, éticas y científicas que confirman la necesidad de su investigación. Investigaciones relacionadas con el tema no declaran la importancia de la dimensión social de la comunicación en lengua inglesa con fines médicos en la formación profesional lo que constituye el aporte científico fundamental.The present paper, "Social need of a correct language preparation in English of the students from the clinical cycle of the medicine career", deals with significant elements in relation to the process of social communication of a correct language preparation in English for medical purposes in the fifth year students of English language medical career. In this work it is carried out an analysis on the basis of the contradictions of the current program of English discipline and its relation to the language formation in English language of the students who attend the fifth year of medicine career and there are also identified the main contradictions that appear in this process. From which a social problem emerges and it is expressed through limitations in the development of the communicative process of English for medical purposes that fifth year medical students present and go to the detriment of a high and full service of health that we expect to obtained in their future professional performance. It is also characterized the process of language formation in the fifth year of the medical career and there are evidenced the mistakes that it presents. It is performed a revision to the plan of study of the career to know what is the contribution of the aforementioned discipline in the process of formation of the doctors in their professional problems, work, modes and spheres of action, what justifies the social need of its study. This problem has epistemological, cultural, ethical and scientific contradictions which confirm the need for their research. Investigations related to the topic do not declare the importance of the social dimension of communication in English language formation for medical purposes what constitutes the fundamental scientific contribution
Effect of viral storm in patients admitted to intensive care units with severe COVID-19 in Spain: a multicentre, prospective, cohort study
Background: The contribution of the virus to the pathogenesis of severe COVID-19 is still unclear. We aimed to evaluate associations between viral RNA load in plasma and host response, complications, and deaths in critically ill patients with COVID-19. Methods: We did a prospective cohort study across 23 hospitals in Spain. We included patients aged 18 years or older with laboratory-confirmed SARS-CoV-2 infection who were admitted to an intensive care unit between March 16, 2020, and Feb 27, 2021. RNA of the SARS-CoV-2 nucleocapsid region 1 (N1) was quantified in plasma samples collected from patients in the first 48 h following admission, using digital PCR. Patients were grouped on the basis of N1 quantity: VIR-N1-Zero (2747 N1 copies per mL). The primary outcome was all-cause death within 90 days after admission. We evaluated odds ratios (ORs) for the primary outcome between groups using a logistic regression analysis. Findings: 1068 patients met the inclusion criteria, of whom 117 had insufficient plasma samples and 115 had key information missing. 836 patients were included in the analysis, of whom 403 (48%) were in the VIR-N1-Low group, 283 (34%) were in the VIR-N1-Storm group, and 150 (18%) were in the VIR-N1-Zero group. Overall, patients in the VIR-N1-Storm group had the most severe disease: 266 (94%) of 283 patients received invasive mechanical ventilation (IMV), 116 (41%) developed acute kidney injury, 180 (65%) had secondary infections, and 148 (52%) died within 90 days. Patients in the VIR-N1-Zero group had the least severe disease: 81 (54%) of 150 received IMV, 34 (23%) developed acute kidney injury, 47 (32%) had secondary infections, and 26 (17%) died within 90 days (OR for death 0·30, 95% CI 0·16-0·55; p<0·0001, compared with the VIR-N1-Storm group). 106 (26%) of 403 patients in the VIR-N1-Low group died within 90 days (OR for death 0·39, 95% CI 0·26-0·57; p<0·0001, compared with the VIR-N1-Storm group). Interpretation: The presence of a so-called viral storm is associated with increased all-cause death in patients admitted to the intensive care unit with severe COVID-19. Preventing this viral storm could help to reduce poor outcomes. Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. CARB-ES-19 Multicenter Study of Carbapenemase-Producing Klebsiella pneumoniae and Escherichia coli From All Spanish Provinces Reveals Interregional Spread of High-Risk Clones Such as ST307/OXA-48 and ST512/KPC-3
ObjectivesCARB-ES-19 is a comprehensive, multicenter, nationwide study integrating whole-genome sequencing (WGS) in the surveillance of carbapenemase-producing K. pneumoniae (CP-Kpn) and E. coli (CP-Eco) to determine their incidence, geographical distribution, phylogeny, and resistance mechanisms in Spain.MethodsIn total, 71 hospitals, representing all 50 Spanish provinces, collected the first 10 isolates per hospital (February to May 2019); CPE isolates were first identified according to EUCAST (meropenem MIC > 0.12 mg/L with immunochromatography, colorimetric tests, carbapenem inactivation, or carbapenem hydrolysis with MALDI-TOF). Prevalence and incidence were calculated according to population denominators. Antibiotic susceptibility testing was performed using the microdilution method (EUCAST). All 403 isolates collected were sequenced for high-resolution single-nucleotide polymorphism (SNP) typing, core genome multilocus sequence typing (cgMLST), and resistome analysis.ResultsIn total, 377 (93.5%) CP-Kpn and 26 (6.5%) CP-Eco isolates were collected from 62 (87.3%) hospitals in 46 (92%) provinces. CP-Kpn was more prevalent in the blood (5.8%, 50/853) than in the urine (1.4%, 201/14,464). The cumulative incidence for both CP-Kpn and CP-Eco was 0.05 per 100 admitted patients. The main carbapenemase genes identified in CP-Kpn were blaOXA–48 (263/377), blaKPC–3 (62/377), blaVIM–1 (28/377), and blaNDM–1 (12/377). All isolates were susceptible to at least two antibiotics. Interregional dissemination of eight high-risk CP-Kpn clones was detected, mainly ST307/OXA-48 (16.4%), ST11/OXA-48 (16.4%), and ST512-ST258/KPC (13.8%). ST512/KPC and ST15/OXA-48 were the most frequent bacteremia-causative clones. The average number of acquired resistance genes was higher in CP-Kpn (7.9) than in CP-Eco (5.5).ConclusionThis study serves as a first step toward WGS integration in the surveillance of carbapenemase-producing Enterobacterales in Spain. We detected important epidemiological changes, including increased CP-Kpn and CP-Eco prevalence and incidence compared to previous studies, wide interregional dissemination, and increased dissemination of high-risk clones, such as ST307/OXA-48 and ST512/KPC-3
Assessment of plasma chitotriosidase activity, CCL18/PARC concentration and NP-C suspicion index in the diagnosis of Niemann-Pick disease type C : A prospective observational study
Niemann-Pick disease type C (NP-C) is a rare, autosomal recessive neurodegenerative disease caused by mutations in either the NPC1 or NPC2 genes. The diagnosis of NP-C remains challenging due to the non-specific, heterogeneous nature of signs/symptoms. This study assessed the utility of plasma chitotriosidase (ChT) and Chemokine (C-C motif) ligand 18 (CCL18)/pulmonary and activation-regulated chemokine (PARC) in conjunction with the NP-C suspicion index (NP-C SI) for guiding confirmatory laboratory testing in patients with suspected NP-C. In a prospective observational cohort study, incorporating a retrospective determination of NP-C SI scores, two different diagnostic approaches were applied in two separate groups of unrelated patients from 51 Spanish medical centers (n = 118 in both groups). From Jan 2010 to Apr 2012 (Period 1), patients with ≥2 clinical signs/symptoms of NP-C were considered 'suspected NP-C' cases, and NPC1/NPC2 sequencing, plasma chitotriosidase (ChT), CCL18/PARC and sphingomyelinase levels were assessed. Based on findings in Period 1, plasma ChT and CCL18/PARC, and NP-C SI prediction scores were determined in a second group of patients between May 2012 and Apr 2014 (Period 2), and NPC1 and NPC2 were sequenced only in those with elevated ChT and/or elevated CCL18/PARC and/or NP-C SI ≥70. Filipin staining and 7-ketocholesterol (7-KC) measurements were performed in all patients with NP-C gene mutations, where possible. In total across Periods 1 and 2, 10/236 (4%) patients had a confirmed diagnosis o NP-C based on gene sequencing (5/118 [4.2%] in each Period): all of these patients had two causal NPC1 mutations. Single mutant NPC1 alleles were detected in 8/236 (3%) patients, overall. Positive filipin staining results comprised three classical and five variant biochemical phenotypes. No NPC2 mutations were detected. All patients with NPC1 mutations had high ChT activity, high CCL18/PARC concentrations and/or NP-C SI scores ≥70. Plasma 7-KC was higher than control cut-off values in all patients with two NPC1 mutations, and in the majority of patients with single mutations. Family studies identified three further NP-C patients. This approach may be very useful for laboratories that do not have mass spectrometry facilities and therefore, they cannot use other NP-C biomarkers for diagnosis

Enhanced deuteron coalescence probability in jets
The transverse-momentum (pT) spectra and coalescence parameters B2 of (anti)deuterons are measured in pp collisions at s√=13 TeV for the first time in and out of jets. In this measurement, the direction of the leading particle with the highest pT in the event (pleadT>5 GeV/c) is used as an approximation for the jet axis. The event is consequently divided into three azimuthal regions and the jet signal is obtained as the difference between the Toward region, that contains jet fragmentation products in addition to the underlying event (UE), and the Transverse region, which is dominated by the UE. The coalescence parameter in the jet is found to be approximately a factor of 10 larger than that in the underlying event. This experimental observation is consistent with the coalescence picture and can be attributed to the smaller average phase-space distance between nucleons inside the jet cone as compared to the underlying event. The results presented in this Letter are compared to predictions from a simple nucleon coalescence model, where the phase space distributions of nucleons are generated using PYTHIA 8 with the Monash 2013 tuning, and to predictions from a deuteron production model based on ordinary nuclear reactions with parametrized energy-dependent cross sections tuned on data. The latter model is implemented in PYTHIA 8.3. Both models reproduce the observed large difference between in-jet and out-of-jet coalescence parameters, although the almost flat trend of the BJet2 is not reproduced by the models, which instead give a decreasing trend
Investigation of K+K− interactions via femtoscopy in Pb-Pb collisions at √sNN = 2.76 TeV at the LHC
Femtoscopic correlations of non-identical charged kaons (K+K−) are studied in Pb−Pb collisions at a center-of-mass energy per nucleon−nucleon collision sNN−−−√=2.76 TeV by ALICE at the LHC. One-dimensional K+K− correlation functions are analyzed in three centrality classes and eight intervals of particle-pair transverse momentum. The Lednický and Luboshitz interaction model used in the K+K− analysis includes the final-state Coulomb interactions between kaons and the final-state interaction through a0(980) and f0(980) resonances. The mass of f0(980) and coupling were extracted from the fit to K+K− correlation functions using the femtoscopic technique. The measured mass and width of the f0(980) resonance are consistent with other published measurements. The height of the ϕ(1020) meson peak present in the K+K− correlation function rapidly decreases with increasing source radius, qualitatively in agreement with an inverse volume dependence. A phenomenological fit to this trend suggests that the ϕ(1020) meson yield is dominated by particles produced directly from the hadronization of the system. The small fraction subsequently produced by final-state interactions could not be precisely quantified with data presented in this paper and will be assessed in future work
