    Hydronéphrose géante sur urétérocÚle compliquée de lithiase urétérale

    L'hydronéphrose géante est définie comme une importante dilatation des cavités pyélo-calicielles occupant une grande partie de la cavité abdominale, ou une dilatation contenant plus d'un litre d'urine débordant la ligne médiane. Elle est dûe le plus souvent à un syndrome de la jonction pyélo-urétérale. L'obstruction par urétérocÚle compliquée de lithiase est une cause trÚs rare. Nous rapportons le cas d'un patient de 45 ans, ayant comme antécédent des douleurs lombaires droites non explorées, qui a consulté pour une masse abdominale évoluant depuis quelques années associée à une constipation intermittente. L'examen clinique a objectivé une distension abdominale asymétrique avec une matité débordant la ligne médiane. L'échographie a montré une formation liquidienne multi-cloisonnée occupant toute la région abdomino-pelvienne droite et refoulant les structures digestives. L'uroscanner a dévoilé une énorme dilatation urétéro-pyélo-calicielle droite (pyelon mesurant 15,2cm) laminant complÚtement le parenchyme rénal avec absence d'excrétion du produit de contraste, en amont d'une urétérocÚle compliquée d'un calcul de 2cm. Une néphro-urétérectomie a été réalisée par laparotomie et les suites post opératoires ont été simples

    Surgical management of acute quadriceps tendon rupture (a case report with literature review)

    Quadriceps tendon rupture is uncommon and often overlooked in emergency. Tearing affects weakening tendon by systemic diseases or some medications. The mechanism is generally indirect. Inability to actively extend the knee associated to a supra-patellar defect evoke easily the diagnosis without other investigations. Surgical repair is realized in emergency to completely restore the extension. We report a case of a patient who has sustained of complete quadriceps tendon tear after a long period of tendon weakening by statin therapy,  hypertension and diabetes. The repair has consisted on end-to-end Krackow sutures associated with bone suture to the proximal pole of the patella. Surgeons and emergency physicians must think to this form of extensor apparatus rupture, because early diagnosis leads to early treatment and to best outcomes.Key words: Quadriceps tendon, tear, Krackow suture

    Based on a combined geometrical and mineralogical analysis, a three-stage model of formation of the mineralized veins of the giant Imiter silver deposit (Anti-Atlas, Morocco) is herein proposed. A first episode is characterized by the development of quartz, pink dolomite and Ag-rich minerals veins formed during a dextral transpressive event. The second episode is associated with a normal left-lateral motion that re-opens previous structures, filled by pink dolomite gangue. Alteration stages contribute to a local Ag enrichment.     Structure refinement, EPR, Specific magnetic heat and magnetic properties of ZnMnP2O7 diphosphate

    The structure of the diphosphate ZnMnP2O7 is determined on powder by the Rietveld method. It is of monoclinic symmetry C2/m. The cell parameters are: a = 6.6141(3) Å, b = 8.4366 (3) Å, c = 4.5305(2) Å and b= 103.948(2) °. The structural framework is built by MO6 octahedron planes where M is a mixed site statistically occupied by Zn2+ and Mn2+ ions. These planes are parallel to the crystallographic plane (001) and are linked together by sheets of P2O74- groups.The value of the paramagnetic temperature Qp is -4 K, which proves that the interactions are antiferromagnetic . The magnetic specific heat of this compound has a symmetric maximum at T = 5.3 K less acute compared to that of Mn2P2O7 of isotypic structure which is of ltype. This behavior change  can be explained by the interruption of three-dimensional interactions between Mn2+ ions which become limited to  2D order when 50% of these ions are replaced by Zn2+ ions

    Zinc-rich clays in supergene non-sulfide zinc deposits

    International audienceThe nature and the origin of zinc clays are poorly understood. With the example of the Bou Arhous Zn-Pb ore deposit in the Moroccan High Atlas, this study presents new data for the mineralogical and chemical characterization of barren and zinc clays associated with non-sulfide zinc ores. In the field, white to ocher granular clays are associated with willemite (Zn2SiO4), while red clays fill karst-related cavities cutting across the non-sulfide ore bodies. Red clays (kaolinite, chlorite, illite, and smectite) present evidence of stratification that reflects internal sedimentation processes during the karst evolution. White clays contain 7-Å clay mineral/smectite irregular interstratified minerals with less than 20 % of smectite layers. Willemite is partially dissolved and is surrounded by authigenic zinc clay minerals. Together with XRD results, WDS analyses on newly formed clay aggregates suggest that this interstratified mineral is composed of fraipontite and sauconite. CEC measurements support that zinc is only located within the octahedral sheets. These new results support the following process: (i) dissolution of willemite, leading to release of Si and Zn, (ii) interaction between Zn-Si-rich solutions and residual-detrital clays, and (iii) dissolution of kaolinite and formation of interstratified zinc clay minerals that grew over detrital micas

    Rare earth elements as proxies of supergene alteration processes from the giant Imiter silver deposit (Morocco)

    International audienceThe giant Imiter silver mine located at the northern edge of the West African craton in Morocco is assumed to be a late Neoproterozoic epithermal deposit mainly characterized by a hypogene paragenesis of Agrich sulphides and sulfosalts, and Ag-Hg alloys occuring preferentially in quartz-rich veins. The secondary enrichment zone at Imiter reaches a thickness of 50 to 150 m below ground surface. The upper levels, famous because of giant native silver crystals, grade up to 300 kg/t. Metallographic observations, SEM-EDS and XRD analyses reveal the presence of a quite complex secondary paragenesis made of acanthite, cinnabar, imiterite, perroudite, cerussite, mimetite, iron oxyhydroxides, synchisite and coronadite. Supergene alteration processes of the giant Imiter silver mine deposit consist of the remobilisation of the primary hypogene paragenesis by (i) deep and old basinal brines and (ii) downward infiltrations of surficial waters becoming progressively more reduced and F-enriched in response to fluid-rock interactions. Development of such a supergene mineralization strongly suggests prevalence of arid to semiarid conditions

    Surgical management of acute distal biceps tendon rupture associated with contralateral radial palsy

    Acute distal biceps tendon rupture constitute a rare lesion of biceps injuries, typically, easy to diagnosis after lifting a heavy object. Treatment is controversial, nonoperative for sedentary and elderly patients; surgical for young and active individuals. Many operative techniques are described, they all aim to restore an excellent strength of flexion and supination. We opted for one-incision method and fixation using trans-osseous anchoring for our patient, because we are convinced that is a simpler and safer technique. Postoperative rehabilitation, after a period of elbow immobilization, must be operated for returning to full activity. Biceps tendon repair has permitted to our patient who suffer from right upper limb handicap due to radial nerve palsy, recuperating the lost strength and force in his dominant limb and maintaining some quality of life.Pan African Medical Journal 2015; 2

    Interleukin (IL)-4 Induces Leukocyte Infiltration In Vivo by an Indirect Mechanism

    Interleukin (IL)-4 is a cytokine known mainly for its anti-inflammatory activity. Using the in vivo murine air pouch model, we found that IL-4 significantly increased the number of leukocytes after 9 hours of treatment, consisting mainly of neutrophil (60%) and monocytic (40%) cell populations. Using an antibody array, we found that the expression of several analytes (predominantly CCL2) was increased by IL-4 before the arrival of leukocytes. The IL-4-induced expression of CCL-2 was confirmed by ELISA. Air pouch resident lining cells were harvested and were found to express IL-4Rα. CCL2 mRNA expression was monitored in lining cells, cells isolated from the air pouch skin, in RAW264.7 macrophage and in epithelial Mode-K cells and its expression was increased in response to IL-4 in all conditions. We conclude that IL-4 can attract leukocytes in vivo by an indirect mechanism involving the production of several analytes by, at least, resident cells
