    La maladie de DRD est une histiocytose sinusale avec lymphadénopathie massive, rare, bénigne et d’étiologie encore inconnue. Sa symptomatologie se résume le plus souvent (90%) à de volumineuses adénopathies (ADP) cervicales. En milieu tropical, elle peut prêter à confusion avec une tuberculose ganglionnaire (TB). Cet article rapporte le cas d’une fillette de 9 ans référée pour une ADP latéro-cervicale gauche, inflammatoire. Ce tableau avait fait suspecter une TB ganglionnaire et pris en charge par un médecin de sa localité. La biopsie ganglionnaire faite, confirmait un mois après le diagnostic de maladie de DRD. La forme ganglionnaire peut prêter à confusion avec une TB en milieu Tropical. Les moyens limités retardent le diagnostic et la prise en charge. La biopsie ganglionnaire doit être systématique. Its symptomatology is most often paralleled (90%) to voluminous cervical lymph adenopathies (ADP). In tropical area, it can be confusing with ganglionic tuberculosis (TB). This article can give an example of a 9 year-old girl who was referred to us for a ADP latero-cervical lymph on her left side, inflammation. This situation raised suspect of a ganglionic TB and was treated by a doctor in her area. The lymphatic biopsy confirmed, one month after, the diagnosis of DRD disease. The ganglionic form may be confusing with tropical TB. The limited means delay the diagnosis and the treatment. Lymphatic biopsy should be systematic

    Anemia among Apparently Healthy Senegalese Children Aged 9-15 Months

    In Senegal, despite its high frequency, there is no real program to fight against anemia among infants. This work was carried out in the Dakar suburb from 1st September, 2009 to 27th January, 2010 among apparently healthy children aged 9-15 months at the time of their immunization against yellow fever and measles. They showed no known chronic condition or acute infection at the time of the survey. The objectives were to study the diet, prevalence, type and risk factors of anemia. The questionnaire was about whether the father and the mother were working and about the children’s diet during the first six months of their life. All the children underwent anthropometric measurements (weight and height) and a complete blood count. We considered children as anemic if the hemoglobin rate was below 11g/dl. Of the 245 children, 212 were anemic, which was a prevalence of 86.5%. This anemia, frequently of the microcytic hypochromic type (68. 86%) was significantly (p < 0.0003) observed among the children of housewives compared with those whose mothers were employed. Among anemic children, 60.8% were only taking breast milk with or without cereal porridge as a food supplement. The absence of consumption of protein, vegetables, fruits and dairy products was a risk factor for the occurrence of anemia (p <0.0001). In total, at the time immunization is stopped, almost all Senegalese children, while apparently healthy, still face nutritional anemia. The adverse consequences of anemia on child health require the implementation in developing countries of a specific program of struggle against anemia. The activity of vaccination might be the best opportunity to provide the nutritional education these mothers need

    Evolution to 200G Passive Optical Network

    New generation passive optical network aims at providing more than 100 Gb/s capacity. Thanks to recent progress enabling a variety of optical transceivers up to 40 Gb/s, many evolution possibilities to 200G PONs (passive optical network) could be investigated. This work proposes two directly deployable cases of evolution to 200G PON based on the combination of these improved optical transceivers and WDM (wavelength division multiplexing). The physical layer of the optical network has been simulated with OptiSystem software to show the communication links performances behavior when considering key components parameters in order to achieve good network design for a given area.     Solanum cultivar responses to arbuscular mycorrhizal fungi: growth and mineral status

    A greenhouse experiment was carried out in a sandy soil with a low available phosphorus to evaluate responsiveness of four Solanum aethiopicum cultivars to indigenous arbuscular mycorrhizal fungi. Results showed clear interaction between genetic variability of cultivars and fungal isolates on shoot biomass and on mineral status. Arbuscular mycorrhizal fungi can be ranked as Glomus aggregatum > Glomus mosseae > Glomus versiforme for improving yield as well as nitrogen, phosphorus, and potassium acquisition of Solanum cultivars.     Caractéristiques et stock de carbone de la végétation ligneuse des systèmes d’utilisation des terres de la commune de Coumbacara (Kolda, Sénégal)

    Forest degradation causes carbon loss and indirectly contributes to climate change. Thus, the objective of this study is to assess the contribution of land use systems in the commune of Coumbacara to climate change mitigation through an estimate of woody diversity and their carbon stock. Thus, an inventory of woody vegetation was carried out on 72 plots of 30 m x 30 m in the forests adjacent to the fields and 50 m x 50 m in the hut and bush fields. The measurements related to the total height (m), the diameter at breast height (DBH) ≥ 10 cm and the crown diameter (m) of individuals of woody species. In addition to the diversity of this vegetation, the dendrometric data collected made it possible to develop the structure of woody vegetation, to estimate aboveground and belowground biomass as well as carbon stocks using allometric equations. In the forests adjacent to the fields, Combretaceae (61.9%) and Mimosaceae (52.4%) dominate. The woody population of this land use system has a decreasing exponential type structure with a specific richness of 7.85 ± 3.48 species and a density of 4.92 ± 0.39 feet/ha. The carbon stored there rises respectively by 1.57± 0.19 tC/ha for the aboveground biomass and 1.58±0.57 tC/ha for the belowground biomass. At the level of bush fields and hut fields, Anacardiaceae and Malvaceae are the most frequent. These agroforestry systems have a decreasing exponential type structure for the bush fields and an irregular structure for the hut fields. The bush fields have a species richness of 3.31±1.82 against 1.63±1.09 species for the hut fields. The density there is 3.01±0.57 feet/ha (bush fields) and 1.89±0.65 feet/ha (home fields). In addition, the amounts of carbon stored in the aboveground biomass of these systems are respectively 1.05 ±0.3 tC/ha and 0.99 ±0.51 tC/ha for the bush and hut fields against 0. 51±0.37 tC/ha and 0.54±0.49 tC/ha for underground woody biomass

    Analyse physico-chimique de l’eau de l’unité d’hémodialyse du chr de Saint- Louis (Sénégal)

    L’insuffisance rénale chronique est un des problèmes majeurs de la santé publique dans le monde. Au Sénégal, une étude récente réalisée dans la population générale adulte avait retrouvé une prévalence de l’ordre de 6,1%. «L’eau pour dilution de concentrées d’hémodialyse» est produite en continue et utilisée en grande quantité chez l’hémodialysé chronique, donc il apparaît primordial de veiller à sa bonne qualité. Au Sénégal, aucune étude n’a été faite à ce jour pour évaluer l’efficacité du système de traitement de l’eau pour dilution des concentrés de dialyse dans les différents centres de dialyse du pays. L’objectif de ce travail est de contrôler l’efficacité du système de traitement de l’eau pour dilution de concentrées d’hémodialyse afin de s’assurer de son innocuité. Les prélèvements ont été effectués au niveau de trois points clés du circuit de traitement de l’eau pour hémodialyse. A l’entrée et à la sortie de l’osmoseur, à la sortie de la boucle de distribution. Les analyses ont été effectuées à l’aide d’électrodes spécifiques. Les valeurs de potentiels ont permis de tracer les courbes d’étalonnage. La détermination par régression des pentes et des ordonnées à l’origine de la relation de Nernst donne E(mV ) = B - A Log Cm avec des coefficients de corrélation variant entre 0,988 et 0,999 prouvant que 98% au moins des variations de tensions mesurées (E) sont attribuables à la concentration. L’analyse physico-chimique des teneurs en chlorures, nitrates et fluorures révèle des teneurs plus élevées à l’entrée de l’osmoseur qu’à la sortie. Ces basses valeurs découlent du rôle de l’osmoseur qui filtre une grande partie des électrolytes. Par contre, à la sortie de la boucle de distribution les teneurs en électrolytes des différents échantillons étaient plus importantes que celles à la sortie de l’osmoseur. Cette forte concentration en électrolytes atteste d’une pollution par la boucle de distribution de l’eau déjà filtrée par l’osmoseur. L’intérêt de cette étude réside dans le fait qu’elle met en lumière l’importance du contrôle de la qualité de l’eau pour l’hémodialyse et la nécessité d’un bon planning de maintenance préventive efficace de l’ensemble de la boucle de traitement d’eau au sein de chaque centre.© 2016 International Formulae Group. In Senegal, a recent study in the general adult population found a prevalence of about 6,1%. "Water for diluting concentrated hemodialysis" is produced and used continuously at high levels in hemodialysis chronic. It is therefore essential to ensure its quality. In Senegal, no study has been done to assess the effectiveness of water treatment system for dialysis concentrates dilution in different dialysis centers of the country. The objective of this work is to monitor the effectiveness of the water treatment system for hemodialysis concentrated dilution to ensure its safety. The samples were taken on key issues of hemodialysis water’s treatment circuit. Three levels of sampling were selected. At the entrance and at the outlet of the reverse osmosis unit at the outlet of the distribution loop. Analyses were performed using specific electrodes. Potential values allowed us to map the calibration curves. Determination by regression of slopes and intercepts of the Nernst relation gives with correlation coefficients ranging from 0.988 to 0.999 showing that at least 98% of the measured voltages fluctuations (E) are attributable to the concentration. Physicochemical analysis of the levels of chlorides, nitrates and fluorides reveals higher levels at the entrance of the reverse osmosis unit than at the outlet. These low values resulting of the role of reverse osmosis which filter much of the electrolytes. For against, at the outlet of the distribution loop the electrolyte content of the different samples were greater than those at the outlet of the reverse osmosis unit. This high electrolyte concentration attests to pollution of the water distribution loop already filtered by reverse osmosis. The interest of this study lies in the fact that it highlights the importance of monitoring the quality of water for hemodialysis and the need for good planning for effective preventive maintenance throughout the water treatment loop within each center.© 2016 International Formulae Group.     Major Reduction in Anti-Malarial Drug Consumption in Senegal after Nation-Wide Introduction of Malaria Rapid Diagnostic Tests

    BACKGROUND: While WHO recently recommended universal parasitological confirmation of suspected malaria prior to treatment, debate has continued as to whether wide-scale use of rapid diagnostic tests (RDTs) can achieve this goal. Adherence of health service personnel to RDT results has been poor in some settings, with little impact on anti-malarial drug consumption. The Senegal national malaria control programme introduced universal parasite-based diagnosis using malaria RDTs from late 2007 in all public health facilities. This paper assesses the impact of this programme on anti-malarial drug consumption and disease reporting. METHODS AND FINDINGS: Nationally-collated programme data from 2007 to 2009 including malaria diagnostic outcomes, prescription of artemisinin-based combination therapy (ACT) and consumption of RDTs in public health facilities, were reviewed and compared. Against a marked seasonal variation in all-cause out-patient visits, non-malarial fever and confirmed malaria, parasite-based diagnosis increased nationally from 3.9% of reported malaria-like febrile illness to 86.0% over a 3 year period. The prescription of ACT dropped throughout this period from 72.9% of malaria-like febrile illness to 31.5%, reaching close equivalence to confirmed malaria (29.9% of 584,873 suspect fever cases). An estimated 516,576 courses of inappropriate ACT prescription were averted. CONCLUSIONS: The data indicate high adherence of anti-malarial prescribing practice to RDT results after an initial run-in period. The large reduction in ACT consumption enabled by the move from symptom-based to parasite-based diagnosis demonstrates that effective roll-out and use of malaria RDTs is achievable on a national scale through well planned and structured implementation. While more detailed information on management of parasite-negative cases is required at point of care level to assess overall cost-benefits to the health sector, considerable cost-savings were achieved in ACT procurement. Programmes need to be allowed flexibility in management of these funds to address increases in other programmatic costs that may accrue from improved diagnosis of febrile disease

    Long-term cellular immunity of vaccines for Zaire Ebola Virus Diseases

    Recent Ebola outbreaks underscore the importance of continuous prevention and disease control efforts. Authorized vaccines include Merck’s Ervebo (rVSV-ZEBOV) and Johnson & Johnson’s two-dose combination (Ad26.ZEBOV/MVA-BN-Filo). Here, in a five-year follow-up of the PREVAC randomized trial (NCT02876328), we report the results of the immunology ancillary study of the trial. The primary endpoint is to evaluate long-term memory T-cell responses induced by three vaccine regimens: Ad26–MVA, rVSV, and rVSV–booster. Polyfunctional EBOV-specific CD4+ T-cell responses increase after Ad26 priming and are further boosted by MVA, whereas minimal responses are observed in the rVSV groups, declining after one year. In-vitro expansion for eight days show sustained EBOV-specific T-cell responses for up to 60 months post-prime vaccination with both Ad26-MVA and rVSV, with no decline. Cytokine production analysis identify shared biomarkers between the Ad26-MVA and rVSV groups. In secondary endpoint, we observed an elevation of pro-inflammatory cytokines at Day 7 in the rVSV group. Finally, we establish a correlation between EBOV-specific T-cell responses and anti-EBOV IgG responses. Our findings can guide booster vaccination recommendations and help identify populations likely to benefit from revaccination
