    Design of Equipment Rack with TRIZ Method to Reduce Searching Time in Change Over Activity (Case Study : PT. Jans2en Indonesia)

    Janssen is a manufacturing plant that works in furniture assembly. Component shortages often occurs, it will cause the increase of work in process (WIP) in assembly section. In previous studies, we analyze the root causes with FMEA and then it is resulted that router section is the constraint of the system. There are many non value added activities such as searching and transportation caused by a messy condition of work places and the devices that aren’t put in the right place. The impact is that the time allocated for every change over is higher than before. There are many components that are worked by the router section, so improvements are needed to minimize changes in over time. 5S method and the use of a new design of rack by TRIZ method are suggested for fixing the conditions of work environment. It is expected to eliminate non value added activities and changes in over time. Result shows that we can reduce non value activities in change over of regular components up to 41% and the elimination of this time is 41,6%.     Kekurangan komponen yang sering terjadi pada router section mengakibatkan peningkatan WIP pada assembling section.Dari FMEA diperoleh hasil bahwa section router yang menjadi constrain sistem. Hal ini dikarenakan banyak aktivitas searching dan transportasi berlebih dikarenakan kondisi tempat kerja dan peletakan peralatan tidak teratur dan berserakan. Akibatnya waktu change over menjadi lebih tinggi. Dengan jenis komponen yang banyak di section router, perlu tindakan perbaikan untuk meminimasi waktu change over. Penggunaan metode 5S untuk memperbaiki kondisi lingkungan kerja dan rancangan rak peralatan baru dapat mengeliminasi searching time. Hasilnya adalah aktivitas non value added pada change over komponen reguler berkurang 41% dan mengurangi waktu sebesar 41,6%. Pada pembuatan Item baru, aktivitas non value added berkurang 36,6% dan waktunya berkurang 53,3%.Makalah ini memaparkan rancangan 5S di router section PT Janssen Indonesia.     Analysis of factors related to the implementation of pregnant gymnastics in purwokerto western region health center: During pregnancy, pregnant women would usually have complaints. Moderated exercise such as pregnancy exercise would make your feet and heels more comfortable. Percentage of antenatal care in health centers of West Navan especially for K1 and K4 already met the target of 95% . The purpose of this studied is analysis of factors associated with the implementation of the health center pregnancy exercise in Navan western region in 2013 . This researched used analytic studied with cross -sectional correlation . The population in this study were all pregnant women in the region checkups Navan West Health Center in 2012 as many as 871 pregnant women then took sample of 59 samples using accidental sampling methode . Analyzed used univariate and bivariate analysis with the chi square test . These results it could be concluded that most of the samples have a relatively good knowledge (67.8 %) ,good attitude (69.5 %) , not working (55.9 %) , support pregnant women during pregnancy (67.8 %) and doing pregnancy exercise (71.2 %). The chi-square statistical test found no relationship between knowledge , attitude and family support with the implementation of the pregnancy exercise with each value p = 0.005 , p = 0.001 and p = 0.005 then found that there was no relationship between job execution pregnancy exercise with p = 0.36

Understanding words and expressions allows us to convey the meaning of our intentions in the process of communication. The goal of a teacher of a foreign language is to gradually expand the vocabulary of the student and activate the studied vocabulary by organizing a system of exercises that stimulate communicative expressions. «The secondary language personality is viewed by modern linguistics as abilities to communicate in foreign languages. It is difficult to overestimate the importance of that conclusion for the process of teaching a foreign language as it assumes basic distinctions in the choice of teaching methods for various groups of language learners». One of the major problems in learning English is listening comprehension. This problem is faced not only by beginners, but also people with a high level of knowledge.     Pancreatic neoplasms producing exclusively glucagon associated with glucagon cell hyperplasia of the islets and not related to hereditary endocrine syndromes have been recently described. They represent a novel entity within the panel of non-syndromic disorders associated with hyperglucagonemia. This case report describes a 36-year-old female with a 10 years history of non-specific abdominal pain. No underlying cause was evident despite extensive diagnostic work-up. More recently she was diagnosed with gall bladder stones. Abdominal ultrasound, computerised tomography and magnetic resonance imaging revealed no pathologic findings apart from cholelithiasis. Endoscopic ultrasound revealed a 5.5 mm pancreatic lesion. Fine needle aspiration showed cells focally expressing chromogranin, suggestive but not diagnostic of a low grade neuroendocrine tumor. OctreoScan(®) was negative. Serum glucagon was elevated to 66 pmol/L (normal: 0-50 pmol/L). Other gut hormones, chromogranin A and chromogranin B were normal. Cholecystectomy and enucleation of the pancreatic lesion were undertaken. Postoperatively, abdominal symptoms resolved and serum glucagon dropped to 7 pmol/L. Although H and E staining confirmed normal pancreatic tissue, immunohistochemistry was initially thought to be suggestive of alpha cell hyperplasia. A count of glucagon positive cells from 5 islets, compared to 5 islets from 5 normal pancreata indicated that islet size and glucagon cell ratios were increased, however still within the wide range of normal physiological findings. Glucagon receptor gene (GCGR) sequencing revealed a heterozygous deletion, K349_G359del and 4 missense mutations. This case may potentially represent a progenitor stage of glucagon cell adenomatosis with hyperglucagonemia in the absence of glucagonoma syndrome. The identification of novel GCGR mutations suggests that these may represent the underlying cause of this condition

    Bezlotoxumab for prevention of recurrent Clostridium difficile infection in patients at increased risk for recurrence

    Background: Bezlotoxumab is a human monoclonal antibody against Clostridium difficile toxin B indicated to prevent C. difficile infection (CDI) recurrence (rCDI) in adults at high risk for rCDI. This post hoc analysis of pooled monocolonal antibodies for C.difficile therapy (MODIFY) I/II data assessed bezlotoxumab efficacy in participants with characteristics associated with increased risk for rCDI. Methods: The analysis population was the modified intent-to-treat population who received bezlotoxumab or placebo (n = 1554) by risk factors for rCDI that were prespecified in the statistical analysis plan: age ≥65 years, history of CDI, compromised immunity, severe CDI, and ribotype 027/078/244. The proportion of participants with rCDI in 12 weeks, fecal microbiota transplant procedures, 30-day all cause and CDI-associated hospital readmissions, and mortality at 30 and 90 days after randomization were presented. Results: The majority of enrolled participants (75.6%) had ≥1 risk factor; these participants were older and a higher proportion had comorbidities compared with participants with no risk factors. The proportion of placebo participants who experienced rCDI exceeded 30% for each risk factor compared with 20.9% among those without a risk factor, and the rCDI rate increased with the number of risk factors (1 risk factor: 31.3%; ≥3 risk factors: 46.1%). Bezlotoxumab reduced rCDI, fecal microbiota transplants, and CDI-associated 30-day readmissions in participants with risk factors for rCDI. Conclusions: The risk factors prespecified in the MODIFY statistical analysis plan are appropriate to identify patients at high risk for rCDI. While participants with ≥3 risk factors had the greatest reduction of rCDI with bezlotoxumab, those with 1 or 2 risk factors may also benefit.     Understanding Sanctuary Cities

    In the wake of President Trump’s election, a growing number of local jurisdictions around the country have sought to disentangle their criminal justice apparatus from federal immigration enforcement efforts. These localities have embraced a series of reforms that attempt to ensure immigrants are not deported when they come into contact with the criminal justice system. The Trump administration has labeled these jurisdictions “sanctuary cities” and vowed to “end” them by, among other things, attempting to cut off their federal funding. This Article is a collaborative project authored by law professors specializing in the intersection between immigration and criminal law. In it, we set forth the central features of the Trump administration’s mass deportation plans and its campaign to “crack down” on sanctuary cities. We then outline the diverse ways in which localities have sought to protect their residents by refusing to participate in the Trump immigration agenda. Such initiatives include declining to honor immigration detainers, precluding participation in joint operations with the federal government, and preventing immigration agents from accessing local jails. Finally, we analyze the legal and policy justifications that local jurisdictions have advanced. Our examination reveals important insights for how sanctuary cities are understood and preserved in the age of Trump

    An expression signature of the angiogenic response in gastrointestinal neuroendocrine tumours: correlation with tumour phenotype and survival outcomes.

    BACKGROUND: Gastroenteropancreatic neuroendocrine tumours (GEP-NETs) are heterogeneous with respect to biological behaviour and prognosis. As angiogenesis is a renowned pathogenic hallmark as well as a therapeutic target, we aimed to investigate the prognostic and clinico-pathological role of tissue markers of hypoxia and angiogenesis in GEP-NETs. METHODS: Tissue microarray (TMA) blocks were constructed with 86 tumours diagnosed from 1988 to 2010. Tissue microarray sections were immunostained for hypoxia inducible factor 1α (Hif-1α), vascular endothelial growth factor-A (VEGF-A), carbonic anhydrase IX (Ca-IX) and somatostatin receptors (SSTR) 1–5, Ki-67 and CD31. Biomarker expression was correlated with clinico-pathological variables and tested for survival prediction using Kaplan–Meier and Cox regression methods. RESULTS: Eighty-six consecutive cases were included: 51% male, median age 51 (range 16–82), 68% presenting with a pancreatic primary, 95% well differentiated, 51% metastatic. Higher grading (P=0.03), advanced stage (P<0.001), high Hif-1α and low SSTR-2 expression (P=0.03) predicted for shorter overall survival (OS) on univariate analyses. Stage, SSTR-2 and Hif-1α expression were confirmed as multivariate predictors of OS. Median OS for patients with SSTR-2+/Hif-1α-tumours was not reached after median follow up of 8.8 years, whereas SSTR-2-/Hif-1α+ GEP-NETs had a median survival of only 4.2 years (P=0.006). CONCLUSION: We have identified a coherent expression signature by immunohistochemistry that can be used for patient stratification and to optimise treatment decisions in GEP-NETs independently from stage and grading. Tumours with preserved SSTR-2 and low Hif-1α expression have an indolent phenotype and may be offered less aggressive management and less stringent follow up

    Molecular genetics and comparative genomics reveal RNAi is not functional in malaria parasites

    Techniques for targeted genetic disruption in Plasmodium, the causative agent of malaria, are currently intractable for those genes that are essential for blood stage development. The ability to use RNA interference (RNAi) to silence gene expression would provide a powerful means to gain valuable insight into the pathogenic blood stages but its functionality in Plasmodium remains controversial. Here we have used various RNA-based gene silencing approaches to test the utility of RNAi in malaria parasites and have undertaken an extensive comparative genomics search using profile hidden Markov models to clarify whether RNAi machinery exists in malaria. These investigative approaches revealed that Plasmodium lacks the enzymology required for RNAi-based ablation of gene expression and indeed no experimental evidence for RNAi was observed. In its absence, the most likely explanations for previously reported RNAi-mediated knockdown are either the general toxicity of introduced RNA (with global down-regulation of gene expression) or a specific antisense effect mechanistically distinct from RNAi, which will need systematic analysis if it is to be of use as a molecular genetic tool for malaria parasites

    Rapid and simple quantitative analysis isoflavones tempe using densitometric TLC has been done. The mobile phase of the system was chloroform: methanol: ethylacetate (45: 5: 0.75). Thin layer chromatography was performed on aluminium TLC plates.Ascending distance of 1 μL sample was performanced 10 cm. Then the plate was scanned at 261 nm. A linear relationship obtained at 0.08 - 2 μg/spot with r= 0.9986. The LOD and LOQ of isoflavone were 0.014 μg/spot and 0.048 μg/spot. Genistein contained in tempe was 0.151± 0.005 % b/b
