Caries preventive efficiency of therapeutic complex accomponying orthodontic treatment of children with initial dental caries.

The use of orthodontic non-removable appliance in orthodontic treatment inter­feres with the process of  teeth mineralization, worsens level of oral cavity hygiene, stimulates development of caries process. The situation is complicated when a patient has an initial tooth decay. The aim of this study was to determine genetic characteristics of children with initial caries and clinical evaluation of effectiveness of the developed caries preventive therapeutic complex accompanying treatment of jaw facial anomalies (JFA). 47 children aged 12-14 with initial tooth decay participated in the examination. Complex diagnostics, including molecular genetic studies was carried out. Therapeutic complex for children, of the main group included remineralizing, adaptogenic, biogenic agents, which increase non-specific resistance, as well as infiltration ICON therapy before fixing braces. Caries preventive complex accompanying JFA treatment in children with primary tooth decay developed with regard to revealed genetic disorders of amelogenesis, 2-nd of phase detoxification, collagen formation, functional responses in the oral cavity, state of hard tissues of teeth and periodontal tissues enabled to preserve existing carious process, normalize periodontal and hygienic indices at all stages of treatment

Biophysical parameters of the oral fluid and cells of the buccal epithelium in children with diseases of the thyroid gland in the course of orthodontic treatment of maxillodental anomalies.

The thyroid status disturbance may be reflected on the biological processes which underlie orthodontic tooth movement. Aim of research – to study state of the oral fluid and buccal epithelium cells (BEC) in the course of a complex orthodontic treatment of children with thyroid gland diseases. In 79 children with a diffuse nontoxic goiter (DNG) of the I-III degrees, stability of oral fluid pH (ΔpH), rate of mobile nuclei of BEC, ratio of electrophoretic displacement range of nuclei and plasmolemms was estimated. Estimation of corresponding parameters in the initial state, in 1, 3, 6 and 8 months after fixation of the fixed orthodontic apparatus was carried out. Children of the basic group according to pathology degree in addition received a prophylactic complex including preparations: “Vitrum Perfomens", "Teraflex", "Kalcicor", «Potassium iodide», «Echinacea compositum C», "Lizodent", "Osteobios". Ap­plication of the prophylactic complex in orthodontic treatment of children with DNG prevented growth of ΔрН after fixation of brackets. Throughout all time of supervision it promoted reduction of this rate which slightly exceeded average rate for the given age of children in 8 months. Application of this prophylactic complex already in 1 month after fixation of brackets has raised quantity of mobile BEC nuclei by 44% and by 1,64 times has increased the relation of ranges of plasmolemms and nuclei displacement. In 8 months of supervision percent of mobile nuclei BEC and the relation of Apl/Aya in the basic group was by 2 times more, than in a comparison group, approaching to average rate. Сhildren with thyroid gland diseases need active treatment-and-prophylactic measures at stages of active apparatus treatment. The developed therapeutic complexes of orthodontic treatment of maxillo-dental anomalies in children with DNG allows to effectively support and improve functional reactions in the organism and in the oral cavity, compensating stressful action of fixed orthodontic apparatuses

COUNSELLING FOR MITIGATING THE PSYCHO-SOCIAL TRAUMA OF FLOOD DISASTER VICTIMS

Psycho-social trauma comes about when a traumatic event overloads an individual’s capacity to cope with his or her usual emotional state.  Psycho-social trauma cannot be predicted based on the events that precede them.  Flood is an environmental hazard that occurs naturally and relatively sudden as a result of climate change.  It happens in a devastating way and are brought down in good numbers. The resultant effects are particularly damaging in terms of properties and other economic/income generation structures.  The focus of this work is on the psychosocial wellbeing of the victims.  There is an assumption that women are more likely to suffer disproportionately in most disasters more especially flood, thee is hence they are more vulnerable due to minimal access to source of emergency.  The core concept here is to prevent stress disorder in the victims of flood and provide coping interventions for traumatized people from the aftermath of major flood disaster leading to displacement from homes and work place.  Most flood victims make more emotional investment and are more likely to experience more post traumatic stress disorder associated with flood and consequently become more devastated emotionally and psycho-socially as it happens.  The authors hold a strong view that if psycho-social therapy is applied to victims irrespective of standing, it will remove ugly memories of the disturbance; extinguish the fears associated with the stressful events especially that which relates to re-location and loss of victims’ property.  The conclusion implies an integrative approach involving experts from diverse field of human being endeavours particularly counselors who are expected to mitigate victims’ state of depression, anxiety and other flood inducing.&nbsp

Показники жирової маси тіла при хронічному генералізованому пародонтиті на фоні метаболічного синдрому

Summary. Pathological changes in the periodontium are directly dependent on the violation of fat metabolism, which is one of the main components of the metabolic syndrome.The aim of the study – to determine the main indicators of fatty body mass of patients with chronic generalized periodontitis and metabolic syndrome.Materials and Methods. The study involved patients with diagnoses of chronic generalized periodontitis and metabolic syndrome. Patients of the main group received a treatment and prophylactic complex 2 times a year, normalizing fat metabolism, protein assimilation, reducing cholesterol and normalizing metabolism. The main indicators of fat metabolism were evaluated in patients with bioimpedance analyzers of the body composition components ABC-01 Medass (Russia) and OMRON BF511 (Japan).Results and Discussion. The results of the research of the main indicators of fat metabolism in the patient's body are presented as averaged percentage deviation from the norm, which was calculated for each particular patient according to the average statistical indicators, taking into account age, weight, height and specific basic metabolismConclusions. In patients with chronic generalized periodontitis on a background of metabolic syndrome, there was a significant excess of the norm of the main indicators of fat metabolism in the body. Use of the developed therapeutic and prophylactic complex led to a certain normalization of the fat mass that correlated with improvement of dental status.Резюме. Паталогические изменения в пародонте находятся в прямой зависимости от нарушения жирового обмена, являющегося одним из основных составляющих метаболического синдрома.Цель исследования – определить основные показатели жировой массы тела пациентов с хроническим генерализованным пародонтитом и метаболическим синдромом.Материалы и методы. В исследовании принимали участие пациенты с диагнозами хронического генерализованого пародонтита и метаболического синдрома. Лица основной группы получали 2 раза в году лечебно-профилактический комплекс, нормализующий жировой обмен, усвоение белков, снижающий холестерин и нормализующий обмен веществ. Основные показатели жирового обмена оценивали с помощью биоимпедансных анализаторов компонент-состава тела АВС-01 «Медасс» (Россия) и OMRON BF511 (Япония).Результаты исследований и их обсуждение. Результаты проведенных исследований основных показателей жирового обмена в организме пациентов приведены в виде усредненного процентного отклонения показателя от нормы, рассчитаной для каждого конкретного пациента по среднестатистическим показателям с учетом возраста, веса, роста и удельного основного обмена.Выводы. У пациентов с хроническим генерализованным пародонтитом на фоне метаболического синдрома наблюдалось значительное превышение нормы основных показателей жирового обмена в организме. Применение разработанного лечебно-профилактического комплекса привело к определенной нормализации показателей жировой массы, что коррелировало с улучшением стоматологического статуса.Резюме. Патологічні зміни в пародонті перебувають у прямій залежності від порушення жирового обміну, що є однією з основних складових метаболічного синдрому.Мета дослідження – визначити основні показники жирової маси тіла пацієнтів із хронічним генералізованим пародонтитом і метаболічним синдромом.Матеріали і методи. У дослідженні брали участь пацієнти з діагнозами хронічного генералізованого пародонтиту і метаболічного синдрому. Особи основної групи отримували 2 рази на рік лікувально-профілактичний комплекс, що нормалізує жировий обмін, засвоєння білків, знижує холестерин і нормалізує обмін речовин. Основні показники жирового обміну оцінювали за допомогою біоімпедансних аналізаторів компонент-складу тіла АВС-01 «Медасс» (Росія) і OMRON BF511 (Японія).Результати досліджень та їх обговорення. У пацієнтів із хронічним генералізованим пародонтитом на тлі метаболічного синдрому спостерігалося значне перевищення норми основних показників жирового обміну в організмі. Застосування розробленого лікувально-профілактичного комплексу привело до певної нормалізації показників жирової маси, що корелювало з поліпшенням стоматологічного статусу.Висновки. Результати проведених досліджень основних показників жирового обміну в організмі пацієнтів подано у вигляді усередненого відсоткового відхилення показника від норми, яку розрахували для кожного конкретного пацієнта за середньо-статистичними показниками з урахуванням віку, маси, зросту і питомого основного обміну

Оцінка ефективності застосування препаратів «Ехінацея композитум С» і «Терафлекс» при ортодонтичному переміщенні зубів на тлі експериментального зобу

Disturbances of the thyroid status may be negatively reflected on the biological processes underlying orthodontic tooth movement. Research object – in experiment to estimate efficacy of application of drug complex with osteotropic and immunomodulating action in orthodontic moving of teeth (ОМT) on the background of euthyroid goiter. In  75 rats of Vistar line of gregarious breeding (females, 5 months, 210±28 g) experimental goiter was modeled by injection of 1% perchlorate potassium solution with drinking water within 20 days. Intragastric solution of potassium Iodidum (PI) in the dose of 20 mkg/kg was introduced to rats of the 3rd, 4th and 5th groups from the 22nd day. Intragastrically  drug «Echinacea compositum C» was introduced to animals of the 3rd and 5th groups from the 22nd day during 5 days. OMT was reproduced on the 29th day from the beginning of experiment. On the background of PI injection, teraflex in a dose on glucosaminglican of 750 mg/kg was introduced to the rats of the 4th and 5th groups. A mandible for calculation of carious cavities and definition of atrophy degree of an alveolar process, and the maxilla – for definition of alkaline activity, acidic phosphatase, elastase and general proteolytic activity (GPA), pulp of incisors – for phosphatases definition of activity were isolated. Echinacea compositum, potassium Iodidum and teraflex in the idea of monotherapy caused  some positive influence on phosphatases of an alveolar bone. To completely prevent disturbances of activity of bone phosphatases and proteinases was possibly only at application of a full complex: PІ – throughout all treatment, echinacea compositum C – before fixation of closing springs, teraflex – after fixation. Thus, stage-by-stage use of drugs – potassium Iodidum, «Echinacea compositum C» and Teraflex promoted to normalization of activity of alkaline and acidic phosphatase in pulp and bone tissue of alveolar process, and rendered protective action on proteinase of bone tissue of jaws of experimental animals as well.Нарушения тиреоидного статуса может негативно отразиться на биологических процессах, лежащих в основе ортодонтического перемещения зубов. Цель исследования – оценить в экспе­ри­менте эффективность применения комплекса препаратов, обладающих остеотропным и иммуно­модули­рующим действием при ортодонтическом перемещении зубов  (ОМПЗ) на фоне эутиреоидного зоба. У 75 крыс линии Вистар стадного разведения (самки, 5 месяцев, 210±28 г) моделировали экспериментальный зоб путем введения 1% раствора перхлората калия с питьевой водой  в течение 20 дней. С 22-го дня, крысам 3-й, 4-й и 5‑й групп вводили  внутрижелудочно раствор йодида калия (KI) в дозе 20 мкг/кг. Животным 3-й и 5-й групп с 22-го дня на протяжение 5 дней внутрижелудочно вводили препарат «Эхинацея композитум С». На 29-й день от начала эксперимента воспроизводили ОМПЗ. Крысам 4-й и 5-й групп на фоне введения KI внутри­желудочно вводили терафлекс в дозе по глюкозамингликану 750 мг/кг. У крыс выделяли нижнюю челюсть для подсчёта кариозных полостей и определения степени атрофии альвеолярного отростка,  верхнюю челюсть – для определения активности щелочной, кислой фосфатазы, эластазы и общей протеолитической активности (ОПА), пульпу резцов – для определения активности фосфатаз. Эхинацея композитум С, йодид калия и терафлекс в идее монотерапии оказывали некоторое позитивное влияние на фосфатазы альвеолярной кости. Полностью предупредить нарушения активности костных фосфатаз и протеиназ удалось только при применении полного комплекса: КІ – на протяжении всего лечения, эхинацея композитум С – перед фиксацией закрывающих пружин, терафлекс – после фиксации. Таким образом, поэтапное использование препаратов - Йодид калия, «Эхинацея композитум С» и Терафлекс способствовало нормализации активности щелочной и кислой фосфатазы в пульпе и костной ткани альвеолярного отростка, а также оказывало протекторное действие на протеиназы костной ткани челюстей экспериментальных животных.Disturbances of the thyroid status may be negatively reflected on the biological processes underlying orthodontic tooth movement. This paper is part of a series describing systems for the provision of oral health care in countries that surround or are near to the Black Sea. It starts by outlining the country and its population structure. It then provides a description of the general and oral public and private health care systems in Ukraine. It describes the dental workforce (dentists, oral hygienists, dental specialists and dental technicians) and its education, setting out the numbers of dentists and other dental workers. A section on dental education at undergraduate, specialist, and continuing levels follows. Finally, the paper gives some information on the oral epidemiology and costs of oral care in Ukraine

Исследовано состояние функциональных реакций и неспецифической резистентности у детей 12–14-летнего возраста с зубочелюстными аномалиями до начала ортодонтического лечения. Определены: состояние гигиены полости рта и пародонта; интенсивность кариеса зубов; рН и доверительный интервал рН ротовой жидкости (∆рН); состояние буккального эпителия, проведены: спектроколориметрия твердых тканей зубов и тканей пародонта, ультразвуковая денситометрия, эхоостеометрия верхней и нижней челюстей; биохимические исследования ротовой жидкости.The state of functional reactions and nonspecific resistance in 12–14-year old children which have maxillodental anomalies before the beginning of orthodontic treatment was determined in the study. Показано, что разработанный терапевтический комплекс сопровождения ортодонтического лечения ЗЧА у детей с начальным кариесом зубов, включающий на подготовительном этапе инфильтрационную терапию ICON, а также адаптогенные и биостимулирующие препараты, позволяет законсервировать кариозный процесс, нивелировать негативные последствия ортодонтического лечения, связанные с нарушением минерализующей функции ротовой жидкости, развитием воспалительных реакций, размножением условно-патогенной микрофлоры, интенсификацией пероксидации липидов, угнетением антиоксидантной и антибактериальной защиты полости рта.Показано, що розроблений терапевтичний комплекс супроводу ортодонтического лікування ЗЩА у дітей з початковим карієсом зубів, що включає на підготовчому етапі інфільтраційну терапію ICON, а також адаптогенні та біостимулюючі препарати, дозволяє законсервувати каріозний процес, нівелювати негативні наслідки ортодонтичного лікування, пов'язані з порушенням мінералізуючої функції ротової рідини, розвитком запальних реакцій, розмноженням умовно-патогенної мікрофлори, інтенсифікацією пероксидації ліпідів, пригнобленням антиоксидантного і антибактеріального захисту порожнини рота.Using a long time non-removable orthodontic appliances for the treatment of dento-alveolar abnormalities (DAA) in children interferes with the mineralization of teeth affects the level of hygiene and stimulates the development of caries. Marginal adaptation of filling and their edges pigmentation in the process of complex prevention of secondary caries lesion

Электрометрический способ оценки качества краевого прилегания и спектроколориметрический способ оценки краевого окрашивания пломб являются более объективными и точными по сравнению с качественными критериями G. Ruge. Комплексное лечение кариозных полостей с использованием гиомера «Beautifil II», высоких концентраций озона и кариеспрофилактического комплекса позволяет увеличить сроки сохранности пломб, предотвратить появления вторичного кариеса.Спосіб електрометричної оцінки якості краєвого прилягання пломб та спектроколориметричний спосіб оцінки краєвого їх фарбування є об'єктивнішими і точнішими в порівнянні з якісними критеріями G. Ruge. Комплексне лікування карієсу зубів з використанням гіомера «Beautifil II», високих концентрацій озону і карієспрофілактичного комплексу дозволяє збільшити терміни збереження пломб, запобігти виникненню вторинного карієсу.Influence estimation of filling materials and methods of their use in dental status and development of new technologies filling the cavities is an important task of scientific and practical dentistry.
