    A prospective cohort study in patients with type 2 diabetes mellitus for validation of biomarkers (PROVALID) –study design and baseline characteristics

    Background/Aims: The prevalence of diabetes mellitus type 2 and kidney disease in these patients varies widely between European countries. Methods: In addition to store biosamples the β€œProspective cohort study in patients with type 2 diabetes mellitus for validation of biomarkers” collects information on history, physical status, laboratory measurements and medication in 4000 patients with diabetes mellitus type 2, being taken care of at the primary level of healthcare in 5 European countries (Austria, Hungary, Netherlands, Poland and Scotland). Next to comparing the rate of loss of eGFR between the countries, a further objective of the PROVALID study is to determine the 5-year cumulative incidence of renal and cardiovascular outcomes. Results: The mean age of the population recruited is 62.9Β±10 years, 54.6% are male and the mean BMI is 30.9Β±5.4 kg/m2 . Metabolic control (median HBA1c 6.8 % (6.2;7.5)) is achieved via administration of metformin in 67.4% of the patients and insulin in 30.3%. Median systolic and diastolic blood pressure at recruitment is 135 (125;146) and 80 (72;85) mmHg, 65.4% of subjects received RAAS blocking agents. Mean eGFR is 80.7Β±29.2 ml/min/1.73m2 and median baseline albumin/creatinine ratio 8.3 mg (IQR: 3.8 and 25.1). Conclusion: PROVALID will provide information on incidence and progression of renal and cardiovascular disease and therapy in patients with type 2 diabetes mellitus in different European countries. Thus, in contrast to many other cohort studies we will be able to associate national clinical practise pattern with outcome in this highly vulnerable patient population

    Imaging gene and environmental effects on cerebellum in Attention-Deficit/Hyperactivity Disorder and typical development

    AbstractThis study investigates the effects of XKR4, a recently identified candidate gene for Attention-Deficit/Hyperactivity Disorder (ADHD), birth weight, and their interaction on brain volume in ADHD. XKR4 is expressed in cerebellum and low birth weight has been associated both with changes in cerebellum and with ADHD, probably due to its relation with prenatal adversity. Anatomical MRI scans were acquired in 58 children with ADHD and 64 typically developing controls and processed to obtain volumes of cerebrum, cerebellum and gray and white matter in each structure. DNA was collected from saliva. Analyses including data on birth weight were conducted in a subset of 37 children with ADHD and 51 controls where these data were retrospectively collected using questionnaires. There was an interaction between genotype and birth weight for cerebellum gray matter volume (p=.020). The combination of homozygosity for the G-allele (the allele previously found to be overtransmitted in ADHD) and higher birth weight was associated with smaller volume. Furthermore, birth weight was positively associated with cerebellar white matter volume in controls, but not ADHD (interaction: p=.021). The interaction of genotype with birth weight affecting cerebellum gray matter is consistent with models that emphasize increased influence of genetic risk-factors in an otherwise favorable prenatal environment. The absence of an association between birth weight and cerebellum white matter volume in ADHD suggests that other genetic or environmental effects may be at play, unrelated to XKR4. These results underscore the importance of considering environmental effects in imaging genetics studies

    Suppression of Star Formation in NGC 1266

    NGC1266 is a nearby lenticular galaxy that harbors a massive outflow of molecular gas powered by the mechanical energy of an active galactic nucleus (AGN). It has been speculated that such outflows hinder star formation (SF) in their host galaxies, providing a form of feedback to the process of galaxy formation. Previous studies, however, indicated that only jets from extremely rare, high power quasars or radio galaxies could impart significant feedback on their hosts. Here we present detailed observations of the gas and dust continuum of NGC1266 at millimeter wavelengths. Our observations show that molecular gas is being driven out of the nuclear region at αΉ€_(out) β‰ˆ 110M_βŠ™ yr^(βˆ’1), of which the vast majority cannot escape the nucleus. Only 2M_βŠ™ yr^(βˆ’1) is actually capable of escaping the galaxy. Most of the molecular gas that remains is very inefficient at forming stars. The far-infrared emission is dominated by an ultra-compact (β‰Ύ50 pc) source that could either be powered by an AGN or by an ultra-compact starburst. The ratio of the SF surface density (Ξ£_(SFR)) to the gas surface density (Ξ£_(H2)) indicates that SF is suppressed by a factor of β‰ˆ 50 compared to normal star-forming galaxies if all gas is forming stars, and β‰ˆ150 for the outskirt (98%) dense molecular gas if the central region is is powered by an ultra-compact starburst. The AGN-driven bulk outflow could account for this extreme suppression by hindering the fragmentation and gravitational collapse necessary to form stars through a process of turbulent injection. This result suggests that even relatively common, low-power AGNs are able to alter the evolution of their host galaxies as their black holes grow onto the M-Οƒ relation

    The challenge: * Technological progress and changes in energy supply are not sufficient for a transition to a low-carbon energy system; demand also needs to be considered. Energy efficiency and reducing total consumption - the topics of this bibliography - are typical elements of a demand side approach. * The uptake of energy efficient technologies, and understanding how we might use less energy, represent big challenges for researchers, policymakers, practitioners and end-users themselves. The aim: * European energy policy has so far mainly relied on research from Science, Technology, Engineering and Mathematics (STEM) disciplines. Energy-related Social Sciences and Humanities (SSH) have been significantly underrepresented. This bibliography aims to discuss different disciplinary perspectives on energy efficiency and using less and to demonstrate their relevance for energy policy. Coverage: * A major focus of this bibliography is on behaviour and behavioural change. The bibliography highlights the diversity of end-users and their needs, the impacts they experience, abilities, as well as the range of sites where energy is consumed. * It also looks at how SSH research addresses more structural elements of demand - such as markets, institutions, and policy - and how these interact. Key findings: * There is no such thing as a one size fits all approach; different disciplines frame the problems of energy efficiency and using less differently, and do not always agree. Economics is very highly represented in research about energy efficiency, closely followed by Sociology. Other disciplines such as Urban Studies and Industrial Design are slowly becoming part of the work. * Most disciplines focus mainly on mainstream types of users and use. Fewer studies focus on the exceptions - deviants, others, non-users or energy poor, excessive users - or low-energy practices such as sleep, music making or sports. * Electricity is the main focus of most social science research on energy use and efficiency, possibly due to a focus on monitoring savings which is more difficult for gas and energy for hot water use. * There is an overrepresentation of work on feedback devices and smart meters, in contrast to more everyday technologies such as water heaters or washing machines. Several studies urge for more study of this everyday material culture because it strongly shapes how users can engage in using less or using more efficiently; some technologies are simply built to have high energy use. * Less research is done on the responsibility of stakeholders (other than the end-user) for the energy transition, especially the market. It is argued that markets are not neutral or depoliticised, but bear responsibility for the energy transition too. * Dominant areas of research include: a focus on the gap between awareness and actual energy behaviour action; and rebound effects, which may arise when increased energy efficiency leads to lower costs for energy which in turn may lead to increased energy consumption. * New areas of research include new demand side initiatives, services/business models and markets such as peer-to-peer, DIY, and community approaches to engagement. * Most demand side approaches in the policy domain focus on cost reduction, education and communication. Insights from Social Sciences such as Sociology, Anthropology, Urban studies, Ethics, and Science and Technology Studies see less uptake in the policy domain

    Contribution of the community health volunteers in the control of buruli ulcer in BΓ©nin

    Buruli ulcer (BU) is a neglected tropical disease caused by Mycobacterium ulcerans. Usually BU begins as a painless nodule, plaque or edema, ultimately developing into an ulcer. The high number of patients presenting with ulcers in an advanced stage is striking. Such late presentation will complicate treatment and have long-term disabilities as a consequence. The disease is mainly endemic in West Africa. The primary strategy for control of this disease is early detection using community village volunteers.In this retrospective, observational study, information regarding Buruli ulcer patients that reported to one of the four BU centers in BΓ©nin between January 2008 and December 2010 was collected using the WHO/BU01 forms. Information used from these forms included general characteristics of the patient, the results of diagnostic tests, the presence of functional limitations at start of treatment, lesion size, patient delay and the referral system. The role of the different referral systems on the stage of disease at presentation in the hospital was analyzed by a logistic regression analysis. About a quarter of the patients (26.5%) were referred to the hospital by the community health volunteers. In our data set, patients referred to the hospital by community health volunteers appeared to be in an earlier stage of disease than patients referred by other methods, but after adjustment by the regression analysis for the health center, this effect could no longer be seen. The Polymerase Chain Reaction (PCR) for IS2404 positivity rate among patients referred by the community health volunteers was not systematically lower than in patients referred by other systems.This study clarifies the role played by community health volunteers in BΓ©nin, and shows that they play an important role in the control of BU

    The UV-Optical Color Magnitude Diagram II: Physical Properties and Morphological Evolution On and Off of a Star-Forming Sequence

    We use the UV-optical color magnitude diagram in combination with spectroscopic and photometric measurements derived from the SDSS spectroscopic sample to measure the distribution of galaxies in the local universe (z<0.25) and their physical properties as a function of specific star formation rate (SSFR) and stellar mass. Throughout this study our emphasis is on the properties of galaxies on and off of a local "star-forming sequence." We discuss how the physical characteristics of galaxies along this sequence are related to scaling relations typically derived for galaxies of different morphological types. We find, among other trends that our measure of the star formation rate surface density is nearly constant along this sequence. We discuss this result and implications for galaxies at higher redshift. For the first time, we report on measurements of the local UV luminosity function versus galaxy structural parameters as well as inclination. We also split our sample into disk-dominated and bulge-dominated subsamples using the i-band Sersic index and find that disk-dominated galaxies occupy a very tight locus in SSFR vs. stellar mass space while bulge-dominated galaxies display a much larger spread of SSFR at fixed stellar mass. A significant fraction of galaxies with SSFR and SF surface density above those on the "star-forming sequence" are bulge-dominated. We can use our derived distribution functions to ask whether a significant fraction of these galaxies may be experiencing a final episode of star formation (possibly induced by a merger or other burst), soon to be quenched, by determining whether this population can explain the growth rate of the non-star-forming galaxies on the "red sequence."     Differential Brain Development with Low and High IQ in Attention-Deficit/Hyperactivity Disorder

    Attention-Deficit/Hyperactivity Disorder (ADHD) and intelligence (IQ) are both heritable phenotypes. Overlapping genetic effects have been suggested to influence both, with neuroimaging work suggesting similar overlap in terms of morphometric properties of the brain. Together, this evidence suggests that the brain changes characteristic of ADHD may vary as a function of IQ. This study investigated this hypothesis in a sample of 108 children with ADHD and 106 typically developing controls, who participated in a cross-sectional anatomical MRI study. A subgroup of 64 children also participated in a diffusion tensor imaging scan. Brain volumes, local cortical thickness and average cerebral white matter microstructure were analyzed in relation to diagnostic group and IQ. Dimensional analyses investigated possible group differences in the relationship between anatomical measures and IQ. Second, the groups were split into above and below median IQ subgroups to investigate possible differences in the trajectories of cortical development. Dimensionally, cerebral gray matter volume and cerebral white matter microstructure were positively associated with IQ for controls, but not for ADHD. In the analyses of the below and above median IQ subgroups, we found no differences from controls in cerebral gray matter volume in ADHD with below-median IQ, but a delay of cortical development in a number of regions, including prefrontal areas. Conversely, in ADHD with above-median IQ, there were significant reductions from controls in cerebral gray matter volume, but no local differences in the trajectories of cortical development
