    Addressing Geographic Confounding through Spatial Propensity Score Analysis for Hierarchical Data

    Motivated by recent work exploring cluster-level confounding in multilevel observational data, we develop methods specifically addressing geographic confounding, which occurs when measured or potentially unmeasured confounding factors vary by geographic location. Accounting for this source of confounding achieves spatially-balanced global estimates of the treatment effect of interest, allowing researchers to compare individuals as if they were residentially similar and leading to policy decisions that benefit patients and areas most in need. This dissertation consists of three aims: 1. To develop a hierarchical spatial doubly robust estimator in propensity score analysis framework; 2. To develop spatial propensity score matching methods for hierarchical data; 3. To apply spatial propensity score matching to more complex analyses of spatially varying, zero-inflated outcomes. Each of these aims strives to explore the issue of geographic confounding and contribute to its resolution. Aim 1 seeks to build upon multilevel propensity score methods through augmentation of modeling with spatial random effects to create a spatially balanced estimator that is demonstrated in simulation to exhibit favorable performance under various sample sizes and levels of spatial heterogeneity. Aim 2 seeks to develop methods in a propensity score matching framework, allowing for a more complete understanding of geographic confounding remediation techniques and extensions to additional applications. Finally, as modeling non-binary, spatially varying outcomes can prove challenging, Aim 3 seeks to incorporate spatial matching to alleviate geographic imbalance to allow for a minimally confounded analysis.     Music, Dance, and Tourism in Los Cabos, Mexico

    For a thriving music and dance scene to flourish in tourist destinations, certain factors for performers must be present including economic stability, a baseline level of patronage, resilience to seasonal tourist fluctuations, and adaptation to demographic changes over time. In the Los Cabos arts community, local aesthetics are transformed and embodied by work of performers whose activities are multicultural and multilingual. Over the past fifty years, Los Cabos was transformed from a sleepy fishing village into a major international destination. The island-inspired arts scene includes live performances featuring cultural representations from other lands besides Mexico. Grounded in the humanities, I use journals, artistic collaborations, oral histories, visual artifacts, scholarship, and participant observation between 2004 and 2018 to describe five case studies in Los Cabos. I argue that artists maintain a beneficial relationship with the tourist industry by incorporating global music and dance into performances in ways that fulfill audience expectations. En la comunidad artística de Los Cabos, los ejecutantes de música y danza encarnan y transforman las estéticas locales a través de actividades multiculturales y multilingües. En los últimos cincuenta años, el pequeño pueblo pesquero de Los Cabos se ha transformado en un renombrado destino turístico internacional. Aunque inspiradas por la cultura isleña, las artes escénicas incluyen actos en vivo que representan culturas de otros lugares además de México. Con fundamento en las humanidades, me baso en diarios, colaboraciones artísticas, historias orales, artefactos visuales, estudios, y observación participante entre los años 2004 y 2018 para describir cinco casos de estudio en Los Cabos. Argumento que los artistas mantienen una relación beneficiosa con la industria turística al incorporar música y danzas globales que satisfacen las expectativas de sus públicos

    Racial/Ethnic Disparities in Diabetes Care and Outcomes: A Mixed Methods Study

    Limited research has examined racial/ethnic differences in diabetes care and outcomes among primary care patients. This study examined racial/ethnic differences in diabetes care and outcomes among an ambulatory patient population and explored patient perceptions of the patient-provider relationship to inform strategies to improve care delivery. Using data from 62,149 adults with diabetes who received care within Atrium Health in 2013, regression models assessed associations between race/ethnicity and the following outcomes: glycated hemoglobin (HbA1c) tests, low density lipoprotein (LDL) and blood pressure (BP) screening, foot and eye exams, and HbA1c, LDL, and BP control. Eleven patients with diabetes and uncontrolled hypertension participated in three focus groups about their perceptions of the patient-provider relationship. Compared to non-Hispanic Whites, non-Hispanic Blacks had 22% to 73% higher odds of receiving screenings (HbA1c, LDL, BP, foot and eye exams;
