    Partial accretion in the propeller stage of low mass X-ray binary Aql X--1

    Aql X--1 is one of the most prolific low mass X-ray binary transients (LMXBTs) showing outbursts almost annually. We present the results of our spectral analyses of RXTE/PCA observations of the 2000 and the 2011 outbursts. We investigate the spectral changes related to the changing disk-magnetosphere interaction modes of Aql X--1. The X-ray light curves of the outbursts of LMXBTs typically show phases of fast rise and exponential decay. The decay phase shows a "knee" where the flux goes from the slow decay to the rapid decay stage. We assume that the rapid decay corresponds to a weak propeller stage at which a fraction of the inflowing matter in the disk accretes onto the star. We introduce a novel method for inferring, from the light curve, the fraction of the inflowing matter in the disk that accretes onto the NS depending on the fastness parameter. We determine the fastness parameter range within which the transition from the accretion to the partial propeller stage is realized. This fastness parameter range is a measure of the scale-height of the disk in units of the inner disk radius. We applied the method to a sample of outbursts of Aql X--1 with different maximum flux and duration times.     A case of quadriparesis due to renal tubular acidosis accompanied by vitamin D deficiency in Sjögren’s syndrome

    Renal tubular acidosis (RTA) is metabolic acidosis disorder with a normal anion gap that occurs resulting from bicarbonate reabsorption or disorder in the hydrogen excretion from the kidney. A variety of tests are required to be administered in a stepwise fashion for the diagnosis and characterization of RTA. Correct diagnosis involves careful evaluation, including exclusion of other entities causing acidosis. The patients were treated with potassium and bicarbonate supplementation. A fifty-one years old female patient presented to the emergency department with quadriparesis dependent on hypokalemia and vitamin D deficiency, was diagnosed with distal renal tubular acidosis (dRTA) combined with Sjögren's Syndrome (SS).     Purpose: Antenatal care, one of the strategies aimed at protecting maternal and child health, is to monitor the mother by trained health personnel at regular intervals throughout pregnancy. This study aimed to evaluate the prenatal care service use of pregnant women who attend Kartal Dr. Lütfi Kırdar City Hospital Gynecology and Obstetrics Polyclinics. Materials and Methods: The sample universe of this cross-sectional study was pregnant women attending Kartal Dr. Lütfi Kırdar City Hospital Gynecology and Obstetrics Polyclinic between 01.08.2020-01.09.2020. The questionnaire developed by the researchers was applied to the participants by face-to-face interview technique. Results: All the participants (n=403) received at least one prenatal care service from a health institution. 73.4% (n=296) of the pregnant women received prenatal care, which is required according to their weeks of gestation quantitatively. The highest rate of physical examination in antenatal care follow-ups was fetal heart rate, while the lowest rate was the cardiac auscultation and edema examination rates of the pregnant woman. Conclusion: It was seen that all of the pregnant women received antenatal care at least once, but the number of prenatal care follow-ups required in accordance with their gestational weeks was still not at the desired level. For this reason, there is a need for a study to determine the factors that prevent compliance with the follow-ups and to carry out the ideal four follow-ups of all pregnant women

    Amaç Kliniğimizde preeklampsi ön tanısı ile tetkik edilen gebelerde spot idrarda dipstik, protein/kreatinin (P/K) oranı ile 24 saatlik idrarda toplam protein miktarı değerlerinin karşılaştırılması amaçlanmıştır. Gereç ve yöntem Çalışmaya, Ocak 2016–Aralık 2018 tarihleri arasında bir ünivesite hastanesinde preeklampsi ön tanısı ile spot idrarda dipstik yöntemi, P/K oranı ve 24 saatlik idrarda toplam protein analizi yapılan 317 hasta çalışmaya dahil edildi. Hastaların bu üç değeri birbiri ile mukayese edildi. Bulgular Dipstik yönteminin sensitivitesi %67,5; spesifitesi %61,5; doğruluğu %64,3 olarak hesaplandı. 24 saatlik idrarda toplam proteinüri eşik değeri 300 mg/gün olduğunda P/K oranı cut-off değeri 0,34 mg protein/mg kreatinin, sensitivite %68,9, spesifite %78,7 olarak hesaplandı. Spot idrar P/K oranı ile 24 saatlik idrardaki toplam proteinüri değerleri arasında orta düzeyde, anlamlı bir korelasyon gözlendi (R: 0,629 p<0,001). Sonuç 24 saatlik toplam idrar analizi için yeterli zamanın olmadığı acil durumlarda spot idrarda P/K oranının 24 saatlik idrarda toplam protein testine alternatif test olarak kullanılabilir

    Psychosocial Loads Caused by Covid-19 Pandemic on Obstetricians and Gynecologists. A Scale Study

    Introduction: The novel coronavirus infection (COVID-19) has caused a worldwide pandemic. The pandemic created a heavy psychological burden on healthcare workers. Healthcare providers in obstetrics and gynecology faced similar challenges. Aim: This study studied obstetricians' and gynecologists' psychosocial burdens and concerns due to the COVID-19 pandemic. Methods: Participants of the study consisted of obstetricians and gynecologists. They reached through social media using the snowball sampling technique. This is a survey and scale study. The researchers created the survey. The survey questioned the demographic characteristics of the participants and the problems caused by the COVID-19 pandemic. In addition, three different scales were applied to the participants: Maslach Burnout Inventory, Coronavirus Anxiety Scale, and Depression Anxiety Stress Scale-21. Results: The statistically significant difference was found for Maslach Burnout Inventory Personal Achievement and Maslach Burnout Inventory General. Maslach Burnout Inventory Depersonalization and Coronavirus Anxiety Scale scores of the participants working in pandemic hospitals were statistically significantly higher than those not working in these hospitals. According to the MANOVA results, independent variables explained 35% of the variance in the Maslach Burnout Inventory General scale, 51% in the Coronavirus Anxiety Scale, and 53% in the overall Depression Anxiety Stress Scale. Conclusion: Healthcare personnel who provide obstetric and gynecologic services have faced unique challenges during the COVID-19 pandemic, and to the difficulties experienced by other healthcare workers

    AMELIORATING EFFECT OF HAWTHORN (CRATAEGUS OXYACANTHA) AND PHYSICAL EXERCISE ON ACUTE PENICILLIN INDUCED SEIZURES IN GERBILS

    The aim of the present study was to assess the effect of Hawthorn (Crataegus oxyacantha) and physical activity. We studied its effect on penicilline induced epilepticus (Pie) in gerbils. Pie was induced by administration of penicilline G (500 IU, ip). The gerbils were divided randomly in four groups (6 animals per each group) and studied as described below: 1) Control group 2) Exercise group (30 min/each day for 8 weeks) (Eg) 3) Extract group, 50mg/kg/day/animal in 1 ml saline, 3 h prior to exercise (Exe) 4) Exercise+Extract + (Exe+Ex). The severity of Pie was observed and recorded. The means of latencies (Mean±SE) were 236±45, 369±36, 386±58 and 433±37 ms in groups of control, Exe, Ex, and Exe+Ex respectively.     Review of Matrix Metalloproteinases’ Effect on the Hybrid Dentin Bond Layer Stability and Chlorhexidine Clinical Use to Prevent Bond Failure

    This review describes the relationship between dentin collagen hybrid bond layer degradation and the Matrix Metalloproteinases (MMPs) after their release by acid etch and rinse adhesives and self etching bonding adhesives that can reduce the bond stability over time. MMP-2, MMP-8 and MMP-9 are indicated as the active proteases that breakdown the collagen fibrils in the hybrid bond layer. Phosphoric acid in the acid etch and rinse bonding process and acid primers in the self etch process are implicated in the release of these proteases and their activation by several non-collagen proteins also released from dentin by the etching. MMPs are released in saliva by salivary glands, by cells in the gingival crevices to crevicular fluid and by pulpal odontoblasts cells to the dentinal fluids. These sources may affect the hybrid layer also. Evidence of the bond strength deterioration over time and the ability of Chlorhexidine to prevent bond deterioration by inhibiting MMP action are discussed. Dentin Bonding procedure utilizing Chlorhexidine for different application times and concentrations are being developed. The application of 2% Chlorhexidine to the phosphoric acid etch surface after rinsing off the acid is the only procedure that has been clinically tested for a longer period of time and shown to prevent bond strength degradation so far. The adoption of this procedure is recommended as means of improving bond stability at this time
