Regional Contagion and the Globalization of Securities Markets

This paper argues that the globalization of securities markets may promote contagion among investors by weakening incentives for gathering costly country-specific information and by strengthening incentives for imitating arbitrary market portfolios. In the presence of short-selling constraints, the utility gain of gathering information at a fixed cost converges to a constant level and may diminish as securities markets grow. Moreover, if a portfolio manager's marginal cost for yielding below-market returns exceeds the marginal gain for above-market returns, there is a range of optimal portfolios in which all investors imitate arbitrary market portfolios and this range widens as the market grows. Numerical simulations suggest that these frictions can have significant quantitative implications and they may induce large capital flows in emerging markets.

Caracterización clínica, microbiológica y ecocardiográfica de pacientes con Endocarditis infecciosa en la Fundación Cardioinfantil 2002-2012

Introducción: La endocarditis infecciosa es una infección microbiana del endocardio poco estudiada en nuestro país, la cual se asocia con múltiples complicaciones clínicas, y que presenta una mortalidad importante, que no ha disminuido a lo largo del tiempo sin importar los cambios epidemiológicos, de tratamiento y el aumento de la cirugía valvular que se presentan en la actualidad. Metodología: Se realizó la búsqueda de los pacientes adultos con diagnóstico de endocarditis infecciosa entre los años 2002 a 2012 en la Fundación Cardioinfantil para la revisión de los registros médicos y la descripción de las características clínicas, microbiológicas y ecocardiográficas. Resultados: se detectaron 144 pacientes hallando un predominio de la lesión de válvula nativa y el compromiso de válvula aórtica, encontrando como germen más común el S. aureus, sin embargo una tasa de 32.6% de hemocultivos negativos. La mortalidad intrahospitalaria fue de 22.2%. Discusión: los datos hallados en general son semejantes a los descritos en la literatura mundial con excepción de algunas variables discordantes en cuanto a la válvula aórtica mayormente comprometida y factores asociados a la endocarditis tricuspidea. Conclusión: la epidemiología de la endocarditis infecciosa de nuestra serie es muy semejante a la de la literatura actual de los países industrializados, aunque con algunas excepciones, requiriéndose la implementación y profundización del estudio de esta patología a nivel nacional.Background: Infective endocarditis is an endocardial microbial infection poorly studied in our country that is related with multiple complications with an important mortality burden regardless the epidemiology changes in time, the treatment and the increasing number in valve surgery nowadays. Methods: we search adult patients with diagnosis of infective endocarditis between 2002-2012 in the cardioinfantil foundation clinic for the review of the clinical records to get a clinical, microbiologic and echocardiographic description of this population. Results: we found 144 patients that show a predominant native valve affection and aortic valve lesions with S. aureus as the most frequent isolated bacteria, nevertheless the blood culture was negative in 32.6% of cases; in-hospital mortality was 22.2%. Discussion: our data are similar to the international publications although some variables are discordant as the aortic lesion predominance and the factors related to the tricuspid valve endocarditis. Conclusions: the epidemiology of infective endocarditis in our series was comparable to the international epidemiology of industrialized countries with some little exceptions. We need an implementation and consolidation in the national study of this pathology

Auditando el capital sexual: Pasos hacia una ecología de las relaciones de género

Despite being a generalist or multidisciplinary sociologist, I do frequent certain specialties more often than others. The fields of study which have been my priority are gender, politics, age, the family, culture and sociological theory. And to describe the inner workings of my research process there is nothing better than to refer to the development of my studies on gender relations, to which I have been dedicated during the last 30 years, since I received my degree in sociology in 1977 with an undergraduate thesis on couples and the cognitive structure of desire (Útero, Deseo y Safo, unpublished [The Uterus, Desire and Sappho]). Subsequently, I have published four books in this field (La mujer cuarteada [Woman fractured] in 1991, El nuevo sexo débil [The new weaker sex] in 1997, Medias Miradas [The Hidden Gaze] in 2000 and Máscaras masculinas [Masculine masks], in 2006), as well as diverse articles. Today I continue, embarked on this same line of research, as I prepare my next book projected to be published with the title El capital sexual [Sexual capital]. To explain such a stubborn insistence on this object of cognitive desire I could say that it is all due to a certain sexual obsession, whose repeated frustration has been sublimated into a social-scientific interest and dressed up with a patina of academic respectability. Therefore, willing to publically reveal my research strategies, I have opted to expose my most unconfessable mental habits to focus, not on the production of a particular work, but rather on the progressive development of a work in progress; one that has been accompanying me throughout my academic life as if I were leading an unsuspected but covetous and stimulating double life. This is my most valuable sexual capital.Pese a ser un sociólogo generalista o multidisciplinar, frecuento algunas especialidades con preferencia sobre las demás, siendo mis campos de estudio preferidos el género, la política, la edad, la familia, la cultura y la teoría sociológica. Y para describir la trastienda de mi proceso investigador, nada mejor que referirme a la gestación de mis estudios sobre relaciones de género, a los que me vengo dedicando a lo largo de mis últimos 30 años, desde que en 1977 me licencié en sociología con una Tesina sobre el emparejamiento y la estructura cognitiva del deseo (Útero, Deseo y Safo, inédita). Posteriormente, he publicado cuatro libros más sobre dicho campo (La mujer cuarteada en 1991, El nuevo sexo débil en 1997, Medias Miradas en 2000 y Máscaras masculinas, en 2006), así como diversos artículos relacionados. Y hoy continúo embarcado en esa misma línea de investigación, pues preparo un próximo libro que proyecto publicar bajo el título de El capital sexual. Para explicar tan porfiada insistencia en este objeto de deseo cognitivo podría decirse que todo se debe a una cierta obsesión sexual, cuya reiterada frustración se habría sublimado en sentido científico-social para revestirla con una pátina de respetabilidad académica. Por ello, puesto a revelar en público mis estrategias investigadoras, he optado por desnudar mis hábitos mentales más inconfesables para centrarme no en la fabricación de una obra singular sino en el progresivo desarrollo de una obra en marcha, que me viene acompañando a lo largo de mi biografía académica como si llevara una doble vida insospechada pero codiciosa y estimulante. Delirium management in critically ill patients

Delirium among critically ill patients is common. Presence of delirium imparts a poorer prognosis to patients, including longer ICU and hospital length of stay, increased risk of institutionalization, higher health related costs, and elevated mortality. Even with such grave consequences, the rates of delirium diagnosis are dire. The importance of early recognition through validated tools and appropriate management of this life-threatening condition cannot be over emphasized.
