    Correlation between HIV and HCV in Brazilian prisoners: evidence for parenteral transmission inside prison

    OBJETIVO: É um fato correntemente aceito que as condições de confinamento aumentam o risco de algumas infecções relacionadas às práticas sexuais e/ou ao uso de drogas injetáveis. Realizou-se estudo para estimar a densidade de incidência da infecção pelo HIV na população prisional com aplicação de técnicas matemáticas. MÉTODOS: Foram entrevistados em São Paulo, SP, 631 prisioneiros da maior prisão da América do Sul, que abrigava aproximadamente 4.900 presos na ocasião do estudo. Foi colhido sangue da população entrevistada, analisado o risco para a infecção pelo HIV e realizados testes sorológicos para HIV, HCV e sífilis. Técnicas matemáticas foram usadas para se estimar a densidade de incidência do HIV relacionada ao tempo de encarceramento. RESULTADOS: As prevalências gerais encontradas foram: HIV -- 16%; HCV -- 34%; sífilis -- 18%. Os principais fatores associados à infecção pelo HIV foram a soropositividade ao HCV (OR=10,49) e a confissão do uso de drogas injetáveis (OR=3,36). A análise matemática mostrou que o risco de adquirir a infecção pelo HIV aumenta com o tempo de detenção, atingindo o máximo por volta de 3 anos de aprisionamento. CONCLUSÕES: A correlação entre a soroprevalência do HIV e do HCV e os resultados da análise matemática sugerem que a transmissão do HIV nestsa população se dá preferencialmente pela via parenteral e que seu risco aumenta com o tempo de encarceramento.OBJECTIVE: It is an accepted fact that confinement conditions increase the risk of some infections related to sexual and/or injecting drugs practices. Mathematical techniques were used to estimate the incidence density ratio, as related to the time of imprisonment. RESULTS: Prevalence were: HIV -- 16%; HCV -- 34%; and syphilis -- 18%. The main risk behaviors related to HIV infection were HCV prevalence (OR=10.49) and the acknowledged use of injecting drugs (OR=3.36). Incidence density ratio derivation showed that the risk of acquiring HIV infection increases with the time of imprisonment, peaking around three years after incarceration. CONCLUSIONS: The correlation between HIV and HCV seroprevalence and the results of the mathematical analysis suggest that HIV transmission in this population is predominantly due to parenteral exposure by injecting drug, and that it increases with time of imprisonment

    Varicella Admissions in Children and Adolescents in Portugal: 2000-2015

    OBJECTIVES Varicella is a common, usually benign, and autolimited disease in children but can lead to severe complications and hospitalization. With this study, we aim to analyze all varicella hospitalizations to provide epidemiological information to help outline preventive policies. METHODS We assessed all varicella hospitalizations in children aged 0 to 17 years, from 2000 to 2015, in mainland, public Portuguese hospitals using a Portuguese administrative database. Seasonality, geographic distribution, severity, complications, risk factors, use of diagnostic and treatment procedures and hospitalization costs were analyzed. RESULTS A total of 5120 hospitalizations were registered, with an annual rate of 17.3 hospitalizations per 100 000 inhabitants. A higher number of hospitalizations occurred during the summer period and in Southern regions. The median length of stay was of 4 days (interquartile range: 3.0–7.0). We found a high rate of severe complications, mostly dermatologic (19.6%), neurologic (6.0%), and respiratory (5.1%). Of the total number of patients, 0.8% were immunocompromised and 0.1% were pregnant. Total direct hospitalization costs during the 16-year period were estimated to be 7 110 719€ (8 603 970 USD), with a mean annual cost of 444 419.92€ (537 748.10 USD). CONCULSIONS This is the first national study in which useful epidemiological data to evaluate the burden and impact of varicella in Portugal is provided.No external funding

    Evidence Levels for Neuroradiology Articles: Low Agreement Among Raters

    BACKGROUND AND PURPOSE: Because evidence-based articles are difficult to recognize among the large volume of publications available, some journals have adopted evidence-based medicine criteria to classify their articles. Our purpose was to determine whether an evidence-based medicine classification used by a subspecialty-imaging journal allowed consistent categorization of levels of evidence among different raters. MATERIALS AND METHODS: One hundred consecutive articles in the American Journal of Neuroradiology were classified as to their level of evidence by the 2 original manuscript reviewers, and their interobserver agreement was calculated. After publication, abstracts and titles were reprinted and independently ranked by 3 different radiologists at 2 different time points. Interobserver and intraobserver agreement was calculated for these radiologists. RESULTS: The interobserver agreement between the original manuscript reviewers was -0.2283 (standard error = 0.0000; 95% CI, -0.2283 to -0.2283); among the 3 postpublication reviewers for the first evaluation, it was 0.1899 (standard error = 0.0383; 95% CI, 0.1149-0.2649); and for the second evaluation, performed 3 months later, it was 0.1145 (standard error = 0.0350; 95% CI, 0.0460-0.1831). The intraobserver agreement was 0.2344 (standard error = 0.0660; 95% CI, 0.1050-0.3639), 0.3826 (standard error = 0.0738; 95% CI, 0.2379-0.5272), and 0.6611 (standard error = 0.0656; 95% CI, 0.5325-0.7898) for the 3 postpublication evaluators, respectively. These results show no-to-fair interreviewer agreement and a tendency to slight intrareviewer agreement. CONCLUSIONS: Inconsistent use of evidence-based criteria by different raters limits their utility when attempting to classify neuroradiology-related articles.info:eu-repo/semantics/publishedVersio

    Globally increased ultraconserved noncoding RNA expression in pancreatic adenocarcinoma

Available from the publisher via the DOI in this record.Transcribed ultraconserved regions (T-UCRs) are a class of non-coding RNAs with 100% sequence conservation among human, rat and mouse genomes. T-UCRs are differentially expressed in several cancers, however their expression in pancreatic adenocarcinoma (PDAC) has not been studied. We used a qPCR array to profile all 481 T-UCRs in pancreatic cancer specimens, pancreatic cancer cell lines, during experimental pancreatic desmoplasia and in the pancreases of P48Cre/wt; KrasLSL-G12D/wt mice. Fourteen, 57 and 29% of the detectable T-UCRs were differentially expressed in the cell lines, human tumors and transgenic mouse pancreases, respectively. The vast majority of the differentially expressed T-UCRs had increased expression in the cancer. T-UCRs were monitored using an in vitro model of the desmoplastic reaction. Twenty-five % of the expressed T-UCRs were increased in the HPDE cells cultured on PANC-1 cellular matrix. UC.190, UC.233 and UC.270 were increased in all three human data sets. siRNA knockdown of each of these three T-UCRs reduced the proliferation of MIA PaCa-2 cells up to 60%. The expression pattern among many T-UCRs in the human and mouse pancreases closely correlated with one another, suggesting that groups of T-UCRs are co-activated in PDAC. Successful knockout of the transcription factor EGR1 in PANC-1 cells caused a reduction in the expression of a subset of T-UCRs suggesting that EGR1 may control T-UCR expression in PDAC. We report a global increase in expression of T-UCRs in both human and mouse PDAC. Commonalties in their expression pattern suggest a similar mechanism of transcriptional upregulation for T-UCRs in PDAC.Supported by grants R21/R33CA114304 and U01CA111294.     Supercritical CO(2) recovery of caffeine from green coffee oil: New experimental solubility data and modeling

    The caffeine solubility in supercritical CO(2) Was studied by assessing the effects of pressure and temperature on the extraction of green coffee oil (GCO). The Peng-Robinson(1) equation of state was used to correlate the solubility of caffeine with a thermodynamic model and two mixing rules were evaluated: the classical mixing rule of van der Waals with two adjustable parameters (PR-VDW) and a density dependent one, proposed by Mohamed and Holder(2) with two (PR-MH, two parameters adjusted to the attractive term) and three (PR-MH3 two parameters adjusted to the attractive and one to the repulsive term) adjustable parameters.
