    Evaluation of Metabolic Parameters of Gestational Diabetic Patients in Postpartum Period

    Aim: Gestational diabetes mellitus (GDM) is associated with an increased risk for type 2 DM and metabolic dysfunction after pregnancy. Therefore, we aimed to evaluate the metabolic parameters in women with a history of GDM. Methods: In this study, we included a total of 40 patients with high fasting and postprandial glucose levels and abnormal oral glucose tolerance test at 22-28 weeks of pregnancy who attended the endocrinology outpatient clinic between 2011 and 2014. We evaluated the metabolic parameters, weight, height and body mass index (BMI) at six months postpartum. In addition, fasting and postprandial glucose, insulin, hemoglobin A1c (HbA1c), urea, and creatinine levels, spot urine protein-to-creatinine ratio, and lipid profile were analyzed. Results: Postpartum DM was detected in seven patients (17.5%) in the follow-up period. Mean fasting glucose level and HbA1c values were statistically significantly higher in subjects with postpartum diabetes (p=0.001, p=0.005, respectively). Mean BMI, waist circumference, and triglyceride levels were similar between those with and without postpartum diabetes (p=0.521, p=0.607, p=0.240, respectively). Conclusion: In our study, there was no finding predicting postpartum DM in patients with GDM, however, GDM increases the risk of postpartum DM

    Prediabetes and Type 2 Diabetes are Independent Risk Factors for Computed Tomography-Estimated Nonalcoholic Fatty Pancreas Disease

    OBJECTIVES: Nonalcoholic fatty pancreas disease (NAFPD) is characterized by excessive fat deposition in the pancreas in the absence of alcohol consumption. In this study, we aimed to detect a possible relationship between adipose tissue accumulation, prediabetes and diabetes. METHODS: This cross-sectional and retrospective study included 110 patients. Three groups were classified as controls, patients with prediabetes and patients with type 2 diabetes. The abdominal computed tomography (CT) attenuation measurement results of the pancreas were evaluated independently by two experienced radiologists. CT measurements and biochemical parameters were compared between study groups. The relationship between continuous variables was assessed by using one-way ANOVA. To determine the changes in the dependent variable for the effects on study groups, the independent variable was adjusted using ANCOVA. A p-value less than 0.05 was considered statistically significant. RESULTS: The presence of prediabetes and type 2 diabetes was correlated with a decrease in the mean Hounsfield Unit (HU) value of the pancreas (p=0.002). Age was determined to be an independent risk factor and was correlated with NAFPD (p=0.0001). When compared to the controls (p=0.041), 71% of patients with prediabetes and 67% of patients with type 2 diabetes were observed to have an increased incidence of NAFPD. Decreased serum amylase was found to be correlated with the mean HU value of the pancreas (p=0.043). CONCLUSION: NAFPD was independently correlated with both prediabetes and type 2 diabetes adjusted for age (p=0.0001) in this study.     OBJECTIVE: This study was performed to evaluate the effects of metabolic parameters and thyroid dysfunction on the development of non-alcoholic fatty liver disease (NAFLD). METHODS: The current study evaluated a total of 115 patients, 75 female and 40 male. Physical examination and anthropometric measurements were applied to all participants. Hypothyroidism was considered at a thyroid stimulating hormone level ≥ 4.1 mIU/L. Patients with euthyroidism and patients with hypothyroidism were compared. Abdominal ultrasonography was used to diagnose non-alcoholic fatty liver disease. The participants were further compared with regard to the presence of non-alcoholic fatty liver disease. Logistic regression modeling was performed to identify the relationship between non-alcoholic fatty liver disease and independent variables, such as metabolic parameters and insulin resistance.     Frequency of Paroxysmal Nocturnal Hemoglobinuria Clone in Turkish Myelodysplastic Syndrome Group

    Aim:Retrospective, cross-sectional, observational study to examine the frequency and features of paroxysmal nocturnal hemoglobinuria (PNH) clones in patients with myelodysplastic syndrome (MDS).Methods:Data were analyzed from the medical files of 41 MDS patients diagnosed and followed up in the hematology department at a referral center between 2006 and 2017. Descriptive data, cytogenetic and hematologic characteristics, prognostic features and PNH clone sizes were assessed. PNH clone sizes were evaluated using the fluorescently labeled inactive toxin aerolysin method.Results:The study population comprised 22 (53.7%) female and 19 (46.3%) male patients with confirmed MDS; the mean age of the patients was 68.20±9.84 years (range, 45-85). PNH clones were detected in eight (19.5%) patients. The number of patients with a PNH clone size of >10%, >1%, >0.1% and >0.01% was one, one, one and eigth, respectively (p<0.001 for all subgroups).Conclusion:These data indicate that PNH clones exist in approximately one-fifth of MDS patients. Further studies on a more extensive cohort are required to better understand the pathophysiological and clinical relationships between MDS and PNH

    Work Related Musculoskeletal Disorders Among the Physiotherapists: Sample of a Region in Turkey

    Objective: The aim of this study was to evaluate the work related musculoskeletal disorders (WRMD) of physiotherapists; to determine the prevalence, characteristics and causes. Material and Methods: The research was conducted as a descriptive study. The study was conducted among 100 physiotherapists via online survey with 17 questions that includes personal and occupational history. Results: 84% of the physiotherapists have had musculoskeletal injury at least once in their lives. 61.9% of the subjects with musculoskeletal problems were female and 38.1% were male. Occupational injuries have been mostly low back 46.4%, cervical 42.9% and wrist-hand 35.7%. These injuries occurred mostly during bending or twisting, lifting and working in the same position for a long time. 33.3% of the physiotherapists with occupational injury visited a physician; they mostly preferred the techniques applied by their professional knowledge and skills as the treatment options. 73.8% of the physiotherapists who experienced occupational injuries changed their work habits and 46.4% of them limited contact time with patients. Conclusion: Physiotherapists tend to occupational injuries as one of the health-related occupational groups. According to the study, the lifelong prevalence of occupational injuries has been found to be high among physiotherapists. Many risk factors that cause WRMDs should be considered to prevent injuries.Amaç: Bu çalışmanın amacı çalışan fizyoterapistlerin mesleğe bağlı kas iskelet problemlerini değerlendirerek; prevelans, özellik ve nedenlerini ortaya koymaktır. Gereç ve Yöntemler: Tanımlayıcı türde bir çalışmadır. Çalışma 100 fizyoterapistin katılımıyla, kişisel ve mes?lek ile ilgili 17 soru içeren anket yoluyla yapılmıştır. Bulgular: Fizyo?terapistlerin %84'ü hayatlarında en az bir kez kas-iskelet sistemi hasarı geçirmiştir.Mesleki kas iskelet sistemi yaralanmaları olan fizyotera?pistlerin %61,9'u kadın, %38,1'i erkekti. Mesleki yaralanmalar çoğun?lukla %46,4 oranında bel, %42,9 boyun ve %35,7 el-el bileğinde görüldü. Yaralanmalar daha çok eğilme aktivitesi, hasta transfer ve kal?dırma hareketleri sırasında meydana gelmiştir. Meslek yaralanması olan fizyoterapistlerin %33,3'ü doktora gitmeyi tercih etmiştir. Tedavi seçe?neği olarak ise fizyoterapistler en sık kendi mesleki bilgi ve becerilerini kullanmıştır. İş kazası geçiren fizyoterapistlerin %73,8'i çalışma alış?kanlıklarını değiştirmiş ve %46,4'ü hastalarla sınırlı temas süresine sahip olduğu bulunmuştur. Sonuç: Fizyoterapistler sağlıkla ilgili mes?lek gruplarından biri olarak mesleki yaralanmalara eğilimlidir. Bu ça?lışmanın sonuçlarına göre fizyoterapistlerde mesleğe bağlı kas iskelet sistemi problemlerinin yaşam boyu prevelansı yüksek bulunmuştur. İşle ilgili kas iskelet sistemi problemlerine neden olan birçok risk faktörü yaralanmaları önlemek için göz önünde bulundurulmalıdır

    Vitamin D Deficiency Is a Potential Risk for Blood Pressure Elevation and the Development of Hypertension

    Background and objectives: Hypertension is a global health problem and a major risk factor for cardiovascular diseases. Vitamin D deficiency is closely related to high blood pressure and the development of hypertension. This study investigated the relationship between the vitamin D and blood pressure status in healthy adults, and their 8-year follow-up was added. Materials and Methods: A total of 491 healthy middle-aged participants without any chronic illness, ages 21 to 67 at baseline, were divided into two groups as non-optimal blood pressure (NOBP) and optimal blood pressure (OBP). NOBP group was divided into two subgroups: normal (NBP) and high normal blood pressure (HNBP). Serum 25-hydroxy vitamin D levels were measured with the immunoassay method. 8-year follow-up of the participants was added. Results: The average vitamin D level was detected 32.53 &plusmn; 31.50 nmol/L in the OBP group and 24.41 &plusmn; 14.40 nmol/L in the NOBP group, and a statistically significant difference was found (p &lt; 0.001). In the subgroup analysis, the mean vitamin D level was detected as 24.69 &plusmn; 13.74 and 24.28 &plusmn; 14.74 nmol/L in NBP and HNBP, respectively. Together with parathyroid hormone, other metabolic parameters were found to be significantly higher in the NOBP. During a median follow-up of 8 years, higher hypertension development rates were seen in NOBP group (p &lt; 0.001). Conclusions: The low levels of vitamin D were significantly associated with NBP and HNBP. The low levels of vitamin D were also associated with the development of hypertension in an 8-year follow-up
