    Les enjeux actuels des études de population : Une analyse des thèses de doctorat soutenues en France

    Résumé Cet article vise à explorer le champ des études de population en France à travers le contenu des thèses de doctorat soutenues dans ce pays au cours de la dernière décennie (2000-2012). Nous utilisons pour cela une base de données regroupant 943 thèses. Les analyses menées sont issues de modèles multivariés et de métho­des de statistique textuelle. Les résultats montrent la présence majoritaire de thè­ses réalisées en référence principalement à la sociologie et dans une moindre mesure à la démographie. La migration constitue la thématique la plus fréquemment traitée. Elle est principalement abordée en lien avec la culture, l’identité, la religion, les minorités, de façon qualitative, ou avec les notions d’environnement, de territoire, de logement. Les études sur la mortalité et la santé ou sur la dynamique de population adoptent généralement une démarche plus quantitative et de modélisation tandis que celles portant sur la fécondité, la famille, la sexualité et la nuptialité occupent une position intermédiaire, proche des études de genre. La scolarisation et l’enseignement sont quant à eux traités en lien avec différentes thématiques, sans prépondérance de l’une d’entre elles. Cette première exploration de la base des thèses apporte ainsi des éléments déterminants pour mieux comprendre le contour des études de population ainsi que la manière dont les disciplines se complètent en leur sein. À plus long terme, elle devrait être étendue à une période plus longue et pourrait servir au développement d’une base de données internatio­nale sur les thèses de doctorat en études de population. Abstract This article aims at exploring the field of population studies in France through the lens of doctoral theses defended in this country during the last decade (2000-2012). We rely on the analysis of a database of 943 doctoral theses. Analyses performed include multivariate modeling and textual analysis. Results show that the theses refer mainly to sociology and to a lower extent to demography. Migration is the most frequently researched topic. It is mainly associated either with culture, identity, religion, minorities using qualitative approaches or oriented towards the notions of environment, housing and territory. In contrast, studies on mortality and health or population dynamics tend to involve more quantitative approach and modeling whereas fertility, family, nuptiality and sexuality are often analyzed in conjunction with gender relations, in an intermediate position. Teaching and schooling are equally linked with many topics. This first exploration of the theses database provides insights into population studies boundaries as well as the way disciplines complement each other in this field. In the long term, it could be extended to a longer period and set the basis for the development of a comparative international database of doctoral theses in population studies

    Development and Validation of a Multi-institutional Nomogram of Outcomes for PSMA-PET-Based Salvage Radiotherapy for Recurrent Prostate Cancer.

    IMPORTANCE Prostate-specific antigen membrane positron-emission tomography (PSMA-PET) is increasingly used to guide salvage radiotherapy (sRT) after radical prostatectomy for patients with recurrent or persistent prostate cancer. OBJECTIVE To develop and validate a nomogram for prediction of freedom from biochemical failure (FFBF) after PSMA-PET-based sRT. DESIGN, SETTING, AND PARTICIPANTS This retrospective cohort study included 1029 patients with prostate cancer treated between July 1, 2013, and June 30, 2020, at 11 centers from 5 countries. The initial database consisted of 1221 patients. All patients had a PSMA-PET scan prior to sRT. Data were analyzed in November 2022. EXPOSURES Patients with a detectable post-radical prostatectomy prostate-specific antigen (PSA) level treated with sRT to the prostatic fossa with or without additional sRT to pelvic lymphatics or concurrent androgen deprivation therapy (ADT) were eligible. MAIN OUTCOMES AND MEASURES The FFBF rate was estimated, and a predictive nomogram was generated and validated. Biochemical relapse was defined as a PSA nadir of 0.2 ng/mL after sRT. RESULTS In the nomogram creation and validation process, 1029 patients (median age at sRT, 70 years [IQR, 64-74 years]) were included and further divided into a training set (n = 708), internal validation set (n = 271), and external outlier validation set (n = 50). The median follow-up was 32 months (IQR, 21-45 months). Based on the PSMA-PET scan prior to sRT, 437 patients (42.5%) had local recurrences and 313 patients (30.4%) had nodal recurrences. Pelvic lymphatics were electively irradiated for 395 patients (38.4%). All patients received sRT to the prostatic fossa: 103 (10.0%) received a dose of less than 66 Gy, 551 (53.5%) received a dose of 66 to 70 Gy, and 375 (36.5%) received a dose of more than 70 Gy. Androgen deprivation therapy was given to 325 (31.6%) patients. On multivariable Cox proportional hazards regression analysis, pre-sRT PSA level (hazard ratio [HR], 1.80 [95% CI, 1.41-2.31]), International Society of Urological Pathology grade in surgery specimen (grade 5 vs 1+2: HR, 2.39 [95% CI, 1.63-3.50], pT stage (pT3b+pT4 vs pT2: HR, 1.91 [95% CI, 1.39-2.67]), surgical margins (R0 vs R1+R2+Rx: HR, 0.60 [95% CI, 0.48-0.78]), ADT use (HR, 0.49 [95% CI, 0.37-0.65]), sRT dose (>70 vs ≤66 Gy: HR, 0.44 [95% CI, 0.29-0.67]), and nodal recurrence detected on PSMA-PET scans (HR, 1.42 [95% CI, 1.09-1.85]) were associated with FFBF. The mean (SD) nomogram concordance index for FFBF was 0.72 (0.06) for the internal validation cohort and 0.67 (0.11) in the external outlier validation cohort. CONCLUSIONS AND RELEVANCE This cohort study of patients with prostate cancer presents an internally and externally validated nomogram that estimated individual patient outcomes after PSMA-PET-guided sRT

    Association Between Exposure to Effervescent Paracetamol and Hospitalization for Acute Heart Failure: A Case-Crossover Study

    International audienceWe investigated whether effervescent paracetamol, as an important source of nondietary sodium and fluid load, is associated with a transient increase in the risk of hospitalization for acute heart failure (AHF). We conducted a unidirectional case-crossover study using data from the 1 in 97th representative sample from the French health care database. Subjects aged ≥18 years, hospitalized for AHF during the 2014-2016 period, were included. Exposure to effervescent paracetamol was compared between a risk period (ie, 15 days immediately before admission for AHF) and 3 earlier 15-day control periods, to test a possible trigger effect of effervescent paracetamol intake on AHF. Adjusted odds ratios (aORs) were estimated with a conditional logistic regression. We identified 4301 patients hospitalized for AHF. We found that 5.7% of AHF subjects were exposed to effervescent paracetamol during the risk period, as compared with 4.1% during the control periods (aOR, 1.56 [95% confidence interval [CI], 1.27-1.90]; P < .001). This association was also found in the subgroup of subjects with hypertension (aOR, 1.45 [95%CI, 1.13-1.87]; P = .004, n = 2648) and in the subgroup of subjects aged ≥83 years (aOR, 1.70 [95%CI: 1.28-2.24], P < .001, n = 2238). A similar analysis, considering exposure to noneffervescent paracetamol, did not support the existence of an indication bias likely to explain the association observed for effervescent paracetamol. This study suggests an association between effervescent paracetamol and admission for AHF and should be confirmed with other complementary study designs

    Comparative performances of machine learning algorithms in radiomics and impacting factors

    International audienceAbstract There are no current recommendations on which machine learning (ML) algorithms should be used in radiomics. The objective was to compare performances of ML algorithms in radiomics when applied to different clinical questions to determine whether some strategies could give the best and most stable performances regardless of datasets. This study compares the performances of nine feature selection algorithms combined with fourteen binary classification algorithms on ten datasets. These datasets included radiomics features and clinical diagnosis for binary clinical classifications including COVID-19 pneumonia or sarcopenia on CT, head and neck, orbital or uterine lesions on MRI. For each dataset, a train-test split was created. Each of the 126 (9 × 14) combinations of feature selection algorithms and classification algorithms was trained and tuned using a ten-fold cross validation, then AUC was computed. This procedure was repeated three times per dataset. Best overall performances were obtained with JMI and JMIM as feature selection algorithms and random forest and linear regression models as classification algorithms. The choice of the classification algorithm was the factor explaining most of the performance variation (10% of total variance). The choice of the feature selection algorithm explained only 2% of variation, while the train-test split explained 9%

    Reference values for IGF-I serum concentrations: Comparison of six immunoassays

    International audienceContext: Measurement of IGF-I is essential for diagnosis and management of patients with disorders affecting the somatotropic axis. However, even when IGF-I kit manufacturers follow recent consensus guidelines, different kits can give very different results for a given sample. Objectives: We sought to establish normative data for six IGF-I assay kits based on a large random sample of the French general adult population. Subjects and Methods: In a cross-sectional multicenter cohort study, we measured IGF-I in 911 healthy adults (18-90 years) with six immunoassays (iSYS, LIAISON XL, IMMULITE, IGFI RIACT, Mediagnost ELISA, and Mediagnost RIA). Pairwise concordance between assays was assessed with Bland-Altman plots for both IGF-1 raw data and standard deviation scores (SDS), as well as with the percentage of observed agreement and the weighted Kappa coefficient for categorized IGF-I SDS. Results: Normative data included the range of values (2.5-97.5 percentiles) given by the six IGF-I assays according to age group and sex. A formula for SDS calculation is provided. Although the lower limits of the reference intervals of the six assays were similar, the upper limits varied markedly. Pairwise concordances were moderate to good (0.38-0.70). Conclusion: Despite being obtained in the same healthy population, the reference intervals of the six commercial IGF-1 assay kits showed noteworthy differences.     Classification of Patients With GH Disorders May Vary According to the IGF-I Assay

    International audienceContext: Insulinlike growth factor I (IGF-I) measurement is essential for the diagnosis and management of growth hormone (GH) disorders. However, patient classification may vary substantially according to the assay technique. Objective: We compared individual patient data and classifications obtained with six different IGF-I assay kits in a group of patients with various GH disorders. Design: In this cross-sectional study, we measured IGF-I with six immunoassays in 102 patients with active or treated acromegaly or GH deficiency. IGF-I normative data previously established for the same six assay kits were used to classify the patients (high, low, or normal IGF-I levels), using both raw data and standard deviation scores (SDSs). Pairwise concordance between assays was assessed with Bland-Altman plots and with the percentage of observed agreement and the weighted κ coefficient for categorized IGF-I SDS. Results: We observed marked variability both across each individual's IGF-I raw data and across IGF-I SDS values obtained with each of the six immunoassays. Pairwise concordance between assay values, as assessed with the weighted κ coefficient, ranged from 0.50 (moderate) to 0.81 (excellent). Conclusion: Even when using normative data obtained in the same large population of healthy subjects and when using calculated IGF-I SDSs, agreement among IGF-I assay methods is only moderate to good. Differences in assay performance must be taken into account when evaluating and monitoring patients with GH disorders. This argues for the use of the same IGF-I assay for a given patient throughout follow-up

    Occurrence of Major Local Lower Limb Events in Type 2 Diabetic Patients with Lower Extremity Arterial Disease: Impact of Metformin

    International audienceBackgroundsPatients with type 2 diabetes mellitus (T2DM) are particularly at risk of developing major adverse cardiovascular events (MACE) and peripheral artery disease (PAD) due to an acceleration of the atherosclerotic process linked to hyperglycemia and inflammation with a greater risk of local complications. We aimed to identify the predictive factors for major adverse limb events (MALE) in T2DM patients with PAD to manage modifiable factors at an early stage.MethodsThis is a prospective study in which T2DM patients with PAD were included from November 2017 to May 2018 and followed over 12 months. The predictive factors for the onset of MALE, MACE, and death from all causes have been identified.ResultsA total of 100 patients were included; 37% of the patients developed a MALE. After multivariate analysis, metformin was associated with a decrease of MALE (odds ratio (OR) = 0.26; 95% confidence interval (CI) [0.10; 0.68]; P = 0.007), and a history of the treatment of intravenous iloprost was associated with an increased risk of MALE (OR = 5.70; 95% CI [1.31; 31.93]; P = 0.029). Regular physical activity was associated with a decreased risk of MACE (OR = 0.07; 95% CI [0; 0.44]; P = 0.021). A history of stroke and a history of venous thromboembolism were associated with an increased all-cause mortality risk with OR = 3.68; 95% CI [1.17; 11.5]; P = 0.025 and OR = 3.78; 95% CI [1.16; 12.3]; P = 0.027.ConclusionsMetformin is protective against local complications in people with diabetes with PAD and should be prescribed to diabetic patients with PAD at an early stage
