    Frequency of Predisposing Factor of Nausea and Vomiting After Chest Surgery Under General Anaesthesia

    Background: Postoperative nausea and vomiting are common and distressing postsurgical symptoms. These symptoms are of particular concern in outpatient surgery because they may require additional direct resources, such as supplies and antiemetic drugs, and may delay discharge. The objective of this study was to measure the frequency of factors which can cause nausea and vomiting under general anaesthesia after chest surgery. Methodology: This descriptive case series evaluated frequency of predisposing factor of nausea and vomiting in patients of anaesthesia department of Gulab Devi Hospital Lahore. Questionnaire is made and patients were asked about their age, fever, previous surgery, NPO status, smoking history and hospital stay. This study included 140 patients with post-operative nausea and vomiting. Results: In this study, 140 patients were taken in which  65 (46.43%) were female and 75 (53.57%) were male. In 140 patient 134 (95.7%) were NPO and 6 (4.3%) were not  NPO, 25 (17.9%) were obese and 115 (82.1%) were not obese, 88 (62.9%) patients were suffering fever and 52 (37.1%) were not suffering fever, 80 (57.1%) were infected and 59 (42.1%) were not infected, 53 (37.9%) patients had previous surgery and 87 (62.1%) had no previous surgery, 94 (67.1%) patients had received nitrous oxide and 46 (32.9%) didn\u27t, 97 (69.3%) received volatile gases and 43 (30.7%) not received, 29 (20.7%) received ketamine and 111 (79.3%) not received, 87 (62.1%) received suxamethonium and 53 (37.9%) not received, 119 (85.0%) received propofol and 21 (15.0%) not received, 110 (78.6%) received naluphine and 28 (20.0%) not received. Out of 140 patients, there were 122 (87.1%) who were suffering from pain and 18 (12.9%) were not. 91 (65.0%) patients had gastric distention and 49 (35.0%) patients didn\u27t. Opioids were given to 34 (24.3%) patients and not given to 106 (75.7%) patient. Conclusion: It is concluded that the nausea and vomiting after surgey under genral anesthesia is due to patient related factors in which most frequent is NPO. Drug related factors include propofol and nalbupin administration. Post operative factors include pain. In whole study of 140 patients, the  most frequent is patient related factor (NPO) other than drug related factors and post-operative factors

    Association of depression with treatment outcomes in Type 2 Diabetes Mellitus: A cross-sectional study from Karachi, Pakistan

    Background: To assess the associations of depression with glycemic control and compliance to self-care activities in adult Patients with Type 2 Diabetes Mellitus. Methods: This cross-sectional study was conducted at a tertiary-care hospital in Karachi (Aga Khan University Hospital). Equal numbers of depressed and non-depressed Patients were consecutively recruited from the diabetic clinic. Information on demographic and clinical characteristics was collected in face-to-face interviews and from medical records. Hospital Anxiety Depression Scale (HADS) was used to measure depression. Associations of depressed status (HADS \u3e= 8) with poor glycemic control (Hemoglobin A1c level \u3e= 7%) and compliance to self-care activities were assessed by logistic regression analyses. Results: A total of 286 Patients were included in this study with a male-female ratio of 1.2:1. Mean age was 52 years and in 64.7% of them, the duration of diabetes was more than 3 years. Depressed Patients were more likely to be female (adjusted odds ratio [OR] = 1.88, 95% confidence interval [95% CI] = 1.07-3.31), had a family history of diabetes (OR = 2.64, 95% CI = 1.26-5.55), and poor glycemic control (OR = 5.57, 95% CI = 2.88-10.76) compared with non-depressed Patients. Depression was also associated with low compliance to self-care activities such as taking dose as advised (OR = 0.32, 95% CI = 0.14-0.73), dietary restrictions (OR = 0.45, 95% CI = 0.26-0.79) and foot care (OR = 0.38, 95% CI = 0.18-0.83). Conclusions: Adult Patients with Type 2 Diabetes who have depression were more likely to have poor glycemic control and lower compliance to self-care activities, and they might need particular attention during follow-up visits

    Ogbu and the debate on educational achievement: an exploration of the links between education, migration, identity and belonging

    This paper looks at some of the issues raised by Ogbu’s work in relation to the education of different minority ethnic groups. Ogbu poses questions such as the value attached to education, its links to the future and its measurable outcomes in terms of ‘success’ as experienced by black participants. The desire for better life chances leads families to consider migration to a new country or resettlement within the same country, thus making migration both a local and a global phenomenon. As an example, attention is drawn to the situation facing South Asian children and their families in the UK. In terms of ethnicity and belonging, the wider question that is significant for many countries in the West after ‘Nine-Eleven’ is the education of Muslim children. A consideration of this current situation throws Ogbu’s identification of ‘autonomous minority’ into question. It is argued that a greater understanding of diverse needs has to be accompanied by a concerted effort to confront racism and intolerance in schools and in society, thus enabling all communities to make a useful contribution and to avoid the ‘risk’ of failure and disenchantment

    EFFECT OF SLOPE STEEPNESS AND WHEAT CROP ON SOIL, RUNOFF AND NUTRIENT LOSSES IN ERODED LAND OF MALAKAND AGENCY, NWFP, PAKISTAN

    ABSTRACT Field experiment was conducted during 2003-2005 on slopping land near Thana Malakand Agency, NWFP on wheat plots in comparison with bare plots. The test plots were maintained at three slope positions, i.e. top-slope (6 %), mid-slope (3 %) and bottom-slope (0 %). The effect of both slope position and wheat crop was statistically significant on runoff, soil and nutrient losses. The respective losses of runoff and soil were 33 and 42% higher from mid-slope as compared to bottom-slope, 24 and 30 % higher from top-slope as compared to mid-slope, and 49 and 59 % higher from top-slope as compared to bottom-slope. Wheat crop reduced runoff losses by 60 % and soil losses by 64% as compared to bare plots. The order of nutrient losses from all plots during the experimental period was: K > OM > N > Fe > P > Cu > Mn > Zn. Nutrient enrichment ratio of the sediments was greater than 1 for all the nutrients which indicated higher losses of nutrients through surface runoff.     Avian Adeno viruses and Chicken Anemia Viruses cause serious economic losses to the poultry industry of Pakistan each year. Timely and efficient diagnosis of the viruses is needed in order to practice prevention and control strategies. In the first part of this study, we investigated broilers, breeder and Layer stocks for morbidity and mortality rates due to AAV and CAV infections and any co-infections by examining signs and symptoms typical of their infestation or post mortem examination. In the second part of the study, we developed a duplex PCR assay for the detection of AAV and CAV which is capable to simultaneously detect both the viral types prevalent in Pakistan with high sensitivity and 100% specificity

    Toward automated irrigation management with integrated crop water stress index and spatial soil water balance

    Decision support systems intended for precision irrigation aim at reducing irrigation applications while optimizing crop yield to achieve maximum crop water productivity (CWP). These systems incorporate on-site sensor data, remote sensing inputs, and advanced algorithms with spatial and temporal characteristics to compute precise crop water needs. The availability of variable rate irrigation (VRI) systems enables irrigation applications at a sub-field scale. The combination of an appropriate VRI system along with a precise decision support system would be ideal for improved CWP. The objective of this study was to compare and evaluate two decision support systems in terms of seasonal applied irrigation, crop yield, and CWP. This study implemented the Spatial EvapoTranspiration Modeling Interface (SETMI) model and the Irrigation Scheduling Supervisory Control and Data Acquisition (ISSCADA) system for management of a center pivot irrigation system in a 58-ha maize-soybean field during the 2020 and 2021 growing seasons. The irrigation scheduling methods included: ISSCADA plant feedback, ISSCADA hybrid, common practice, and SETMI. These methods were applied at irrigation levels of 0, 50, 100, and 150% of the full irrigation prescribed by the respective irrigation scheduling method. Data from infrared thermometers (IRTs), soil water sensors, weather stations, and satellites were used in the irrigation methods. Mean seasonal irrigation prescribed was different among the irrigation levels and methods for the 2 years. The ISSCADA plant feedback prescribed the least irrigation among the methods for majority of the cases. The common practice prescribed the largest seasonal irrigation depth among the methods for three crop-year cases. The maize yield in rainfed was found to be significantly lower than the irrigated levels in 2020 since 2020 was a dry year. No significant differences were observed in crop yield among the different irrigation methods for both years. The CWP among the different irrigation methods ranged between 2.72 and 3.15 kg m−3 for 2020 maize, 1.03 and 1.13 kg m−3 for 2020 soybean, 3.57 and 4.24 kg m−3 for 2021 maize, and 1.19 and 1.48 kg m−3 for 2021 soybean. Deficit level (50%) had the largest irrigation water productivity in all crop-year cases in this study. The ISSCADA and SETMI systems were found to reduce irrigation applications as compared to the common practice while maintaining crop yield. This study was the first to implement the newly developed integrated crop water stress index (iCWSI) thresholds and the ISSCADA system for site-specific irrigation of maize and soybean in Nebraska

    Comparison of stationary and mobile canopy sensing systems for irrigation management of maize and soybean in Nebraska

    Accurate knowledge of plant and field characteristics is crucial for irrigation management. Irrigation can potentially be better managed by utilizing data collected from various sensors installed on different platforms. The accuracy and repeatability of each data source are important considerations when selecting a sensing system suitable for irrigation management. The objective of this study was to compare data from multispectral (red and near-infrared bands) and thermal (long wave thermal infrared band) sensors mounted on different platforms to investigate their comparative usability and accuracy. The different sensor platforms included stationary posts fixed on the ground, the lateral of a center pivot irrigation system, unmanned aircraft systems (UAS), and Planet (PlanetScope multispectral imager, Planet Labs, Inc., San Francisco, Calif.) satellites. The surface reflectance data from multispectral (MS) sensors were used to compute the Normalized Difference Vegetation Index (NDVI) and Soil Adjusted Vegetation Index (SAVI). The experimental plots were managed with rainfed and irrigated treatments. Irrigation was applied according to a spatial evapotranspiration model informed with Planet satellite imagery. The NDVI and SAVI curves computed from the different sensing systems exhibited similar patterns and were able to capture differences between the rainfed and irrigated treatments when the crops were approaching senescence. Strong correlations were observed for canopy temperature measurements between the stationary and pivot-mounted infrared thermometer (IRT) sensors (p-value of less than 0.01 for the correlations) when canopy were scanned with no irrigation application (dry scans). The best correlation was obtained for the irrigated maize, which yielded r2 of 0.99, RMSE of 0.4°C, and MAE of 0.3°C. The correlation for the canopy temperature data collected during dry scan between UAS and pivot-mounted thermal sensors was weak with r2 = 0.26 to 0.28, larger RMSE values of 3.7°C and MAE values of 3.4°C. Secondary analysis between thermal data from stationary and pivot-mounted IRTs collected during wet scans (during an irrigation event) demonstrated reduced canopy temperature from pivot-mounted IRTs by approximately 2°C for irrigated soybean due to wetting of the canopy by the irrigation. Understanding the performance of these sensor systems is valuable in configuring practical design and operational considerations when using sensor feedback for irrigation management

    Conserved Quantities in f(R)f(R) Gravity via Noether Symmetry

    This paper is devoted to investigate f(R)f(R) gravity using Noether symmetry approach. For this purpose, we consider Friedmann Robertson-Walker (FRW) universe and spherically symmetric spacetimes. The Noether symmetry generators are evaluated for some specific choice of f(R)f(R) models in the presence of gauge term. Further, we calculate the corresponding conserved quantities in each case.
