    术中自体血回输在骨盆骨折中的应用研究

    目的通过分析自体血回输技术在骨盆骨折手术中的应用情况,为自体血回输技术在临床骨科应用的有效性及安全性提供一定参考。方法通过收集2015年2月至2018年2月期间骨盆骨折术中单纯使用自体血回输或异体血输血的患者,对符合纳入标准患者的基本信息、术中出血量、输血量、血常规、ATP酶活性、TNF-α及C3b,并使用SPSS 20.0软件进行统计学分析。结果自体血回输组与异体血输血组均为20例,其中自体血回输组术中出血量为1 127.50 ml,回输自体血量为464.00 ml;异体血输血组术中出血量为745.00 ml,异体血输血量为300.00 ml。术前自体血回输组的血红蛋白及红细胞比容(Hb:112.80 g/L,Hct:32.9%)与异体血输血组(Hb:112.00 g/L,Hct:33.4%)比较差异无统计学意义(Hb:P=0.908,Hct:P=0.510);输血后2 h及术后第3天两组的血红蛋白及红细胞比容有所下降,但组间比较差异无统计学意义。通过对比自体血回输与异体血输血后发现两组患者在输血后ATP酶活性都有所降低,但是采用自体血回输方法能够抑制ATP酶活性的降低,组间比较发现自体血回输组ATP酶活性明显高于异体血输血组(P=0.002)。输血后两组TNF-α与C3b值都有所升高,输血前后组间比较差异均无统计学意义。结论术中自体血回输可以提高患者红细胞水平,且与异体血回输相比有更高的ATP酶活性,但并不会明显增加机体的炎症反应,是一种值得推广应用的血液管理方法。福建省自然科学基金资助项目(2016J01597);;\n福州市卫生计生科技计划项目(2016-S-wq2);;\n福州市科技计划项目(2017-S-130-5

    The primary investigation of the value of whole body diffusion weighted imaging in diagnosis of metastatic tumor

    背景与目的:随着临床上晚期肿瘤病例的增多,为了解治疗前、后全身转移瘤的大小及数量变化的实际情况,以往多采用分部位进行CT和(或)Mr扫描,少部分有条件的患者采用PET检查,但这些方法可能存在过多地暴露于射线、检查时间过长及经济负担过重等问题。因此,我们探讨磁共振的全身弥散加权成像(WHOlE bOdy dIffuSIOn WEIgHTEd IMAgIng,WbdWI)方法对肿瘤及转移灶的检出准确性及其临床应用的可行性。方法:对68例临床已发现原发肿瘤及多个转移灶或发现多个肿瘤病灶,但原发灶不明确的患者进行WbdWI检查,其中男性49例;女性19例。平均年龄61岁。68例均与CT和(或)MrI的检查结果进行比较,其中17例与PET的检查结果相比较。对病灶的检出率,以病灶长径大小分为:<1CM,≥1~<2CM,≥2~<3CM及≥3CM以上4组进行计数分析,同时,对不同部位间的病灶检出情况进行分析。结果:WbdWI对4组病灶的检出率分别为30%、78%、96%及100%。在各个部位的敏感性和准确性中以骨骼最高。对位于肺部小于1CM的病灶和位于颈部、盆腔小于2CM的淋巴结的诊断存在一定困难。结论:WbdWI对大范围的肿瘤病灶筛查是安全、简便、有效和经济的检查新方法。随着技术参数的进一步完善,WbdWI在发现和诊断全身多发肿瘤方面,具有良好的发展潜力。因此,本技术有望作为一种新的、效果好于PET的全身检查技术。Background and purpose:With the increased number of advanced tumors,it is important to evaluate the actual pre-and post-treatment changes in metastatic diseases,such as tumor size and tumor quantity.Previously,CT and/or MRI scan were usually applied,only a few patients could pay for PET(positron emition tomography).But there were several problems we should face,including redundantly exposure to radiation,too long verification time and heavy financial burden.Therefore we evaluated the application of whole body diffusion weighted imaging in diagnosing of tumor and metastatic tumor.Methods:sixty-eight patients(49 male,19 female,age from 29 to 84 years with mean age of 61 years) with a variety of tumors were investigated by combined CT and/or MRI scan,seventeen patients were compared with positron emition tomograph(PET).All tumors were classified into four groups,according to longest diameter of <1 cm,≥1-<2 cm,≥2-<3 cm and ≥3 cm.At the same time,the positive detection rates between different parts of the lesions were analyzed.Results:The detection rate of the four groups were 30%,78%,96% and 100%,respectively.The skeletal system had the highest sensitivity and accuracy in every part of body.There was difficulty in diagnosing lung cancer with longest diameter less than 1 cm as well as tumors located in neck and pelvic with longest diameter less than 2 cm.Conclusion:Whole body diffusion weighted imaging is secure,convenient,effective and economic for screening wide-ranging tumor focus.After improvement of parameters,WBDWI could be as a new effective whole body examination technique.厦门市科技计划项目(No:3502Z20074017

    Y型聚乙二醇干扰素琢-2b注射液治疗HCV基因2/3型慢性丙型肝炎患者疗效和安全性的多中心随机对照试验研究

    目的以标准剂量的聚乙二醇干扰素(Peg IFN)α-2a联合利巴韦林作为阳性对照,评价新型试验药物Y型Peg IFNα-2b注射液联合利巴韦林治疗2型/3型慢性丙型肝炎(CHC)患者的疗效和安全性。方法采用多中心、随机开放、阳性药对照的Ⅲ期临床试验,筛选符合要求的2型/3型CHC患者,按照2:1的比例随机分配到Y型Peg IFNα-2b组和Peg IFNα-2a组,同时口服利巴韦林,疗程24 w,停药随访24 w。采用Abbott Real Time HCV Genotype II检测HCV基因型,采用Cobas Taq Man实时定量PCR法检测血清HCV RNA水平。详细记录不良事件。主要疗效指标为持续病毒学应答(SVR),并进行非劣效检验。结果本试验实际入组2型/3型CHC患者255例,实际治疗241例。全分析集(FAS)数据显示,158例试验组和83例对照组患者SVR分别为85.4%(95%CI 79.94%~90.94%)和79.5%(95%CI 70.84%~88.20%,P=0.2402);对符合方案分析集(PPS)人群分析显示,试验组和对照组患者SVR分别为87.9%(95%CI 82.45%~93.27%)和85.9%(95%CI 77.82%~94.01%,P=0.7060),率差的95%可置信区间均符合非劣效标准;对PPS人群分析显示,85.8%受试者获得了早期病毒学应答(RVR),RVR的阳性预测值为90.1%;试验组和对照组不良事件发生率相似,分别为95.6%和95.2%,严重不良事件发生率分别为3.8%和3.6%。结论应用Peg IFNα联合利巴韦林治疗2型/3型CHC患者,新型试验药物Y型Peg IFNα-2b具有与对照药物Peg IFNα-2a相似的疗效和安全性。国家科技部“十二五”重大专项(编号:2012ZX10002-003);“重大新药创制”十二五科技重大专项(编号:2012ZX09303019)

    Orientadores: Celio Pasquini, Joaquim de Araújo NóbregaDissertação (mestrado) - Universidade Estadual de Campinas, Instituto de QuímicaResumo: A espectrometria de absorção atômica com atomização eletrotérmica (ET AAS) é adequada para a determinação de metais e semimetais, apresentando alta sensibilidade. Dentre os atomizadores eletrotérmicos, os fornos de grafite destacam-se por sua boa seletividade, exigência de pequenos volumes de amostra e baixos limites de detecção (i.e. 'mu'g L-1). Porém, esses atomizadores requerem fontes de alta potência (i.e. superiores a 2 kW), sistema de resfriamento e, ainda, apresentam alto custo. Como alternativa, o uso de superfícies metálicas têm sido objeto de pesquisa desde 1970. Dentre todos os metais empregados na fabricação de superfícies metálicas e utilizados para os estudos em ET AAS, o tungstênio é o que vem apresentando maior aplicação. Porém, a espectrometria de absorção atômica baseada em atomizadores de filamento de tungstênio (WC AAS) apresenta vários desafios relacionados ao entendimento da geração da nuvem atômica e dos efeitos de interferência observados. Dessa maneira, neste trabalho construiu-se um espectrômetro de absorção atômica com duplo filamento de tungstênio (DWC AAS), com duas configurações, uma empregando como sistema de detecção um sistema echelle/detector de carga acoplada intensificado (ICCD) e outra um sistema filtro óptico-acústico sintonizável (AOTF)/fotomultiplicadora. Essas duas configurações do equipamento foram empregadas em estudos de formação e evolução da nuvem atômica de elementos com características voláteis e refratárias (i.e. Pb e Cr). Além disso, estudou-se o efeito de Na, K, Ca e Mg, em três diferentes concentrações (i.e. 1, 10 e 100 mg L-1) sobre os perfis das nuvens atômicas de Pb e Cr. Esse estudo foi conduzido de tal forma que interferências em fase condensada e gasosa puderam ser avaliadas. Além disso, o equipamento foi empregado em medidas de temperatura na superfície do filamento e na fase gasosaAbstract: Electrothermal atomic absorption spectrometry (ET AAS) is suitable for determination of metals and semimetals with high sensitivity. Among the existing electrothermal atomizers, graphite furnace stands out due to its selectivity, requirement of low volume of sample and low limits of detection (i.e. 'mu'g L-1). Notwithstanding, these atomizers present high cost and requires high power power supply (i.e. above 2 kW) and a cooling system. Alternatively, metallic surfaces have been studied since 1970. Among all metals employed as atomizers in ET AAS studies, tungsten has presented the best results and has found many analytical applications. However, tungsten coil atomic absorption spectrometry (WC AAS) presents several challenges related to understanding the atomic cloud generation and interferences effects. Thus, in this work, a double tungsten coil atomic absorption spectrometer (DWC AAS), with two different configurations was constructed. In one configuration, a detection system based on echelle/intensified charge-coupled device (ICCD) is employed and, in the other, an acousto-optic tunable filter (AOTF)/photomultiplier detection system is used. Both configurations were employed in the atomic cloud generation and evolution studies of Pb and Cr. Furthermore, interferences on the atomic cloud profile of Pb and Cr caused by Na, K, Ca and Mg, at three different concentrations (i.e. 1, 10 and 100 mg L-1), were studied.
