    A dynamic neural field model of temporal order judgments

    Temporal ordering of events is biased, or influenced, by perceptual organization—figure–ground organization—and by spatial attention. For example, within a region assigned figural status or at an attended location, onset events are processed earlier (Lester, Hecht, & Vecera, 2009; Shore, Spence, & Klein, 2001), and offset events are processed for longer durations (Hecht & Vecera, 2011; Rolke, Ulrich, & Bausenhart, 2006). Here, we present an extension of a dynamic field model of change detection (Johnson, Spencer, Luck, & Schöner, 2009; Johnson, Spencer, & Schöner, 2009) that accounts for both the onset and offset performance for figural and attended regions. The model posits that neural populations processing the figure are more active, resulting in a peak of activation that quickly builds toward a detection threshold when the onset of a target is presented. This same enhanced activation for some neural populations is maintained when a present target is removed, creating delays in the perception of the target’s offset. We discuss the broader implications of this model, including insights regarding how neural activation can be generated in response to the disappearance of information.     Hunting and sale of Pangolins across Sub-Saharan Africa: a preliminary analysis

    Pangolins (Pholidota: Manidae) are hunted and traded for their meat and scales. We conducted preliminary analyses on the hunting and sale at markets of four species of pangolin across Sub-Saharan Africa based on data from the OFFTAKE database. Our analyses show that all four species of African pangolin are hunted and sold at markets throughout much of Sub-Saharan Africa.     Physicians' perspectives on the treatment of patients with eating disorders in the acute setting

    Abstract Background Hospitalisation for an eating disorder is rare, however treatment in the acute medical setting can be a life-saving admission. While the multidisciplinary team delivers overall patient care, medical decisions are the responsibility of the treating physicians. Treatment decisions directly impact on patient care and outcomes. This study aimed to explore the considerations that influence the medical decisions of physicians when treating patients with eating disorders in the acute setting. Method Semi-structured interviews were conducted with ten medical physicians who had previously treated eating disorders on a general medical unit in two Australian tertiary hospitals. An interview schedule, based on the literature and four relevant domains from the Consolidated Framework for Implementation Research, was developed. Interviews were audio recorded, transcribed verbatim and analyzed thematically. Coding and interim themes and sub-themes were developed by two dietitian researchers; these were further refined through researcher discussion and triangulation with two additional dietitian researchers. Results Ten doctors were interviewed (3 consultants (1 adult general medical and 2 paediatricians: 13–16 years medical experience), 2 registrars (4–7 years experience), 1 resident (1 year experience), and 4 interns (< 1 year experience). Doctors described memorable patient cases, related to hospital stays over several weeks. Interviews ranged in length from 58 min to 91 min. Four themes (with five sub-themes) were developed: 1) navigating uncertainty (focusing on processes and goals and seeking information), 2) being “the good doctor” (doing the right thing), 3) seeing the big picture (depending on key players and considering short and long-term), and 4) involving family and patient. Conclusions Non-specialist physicians described challenges in the treatment of eating disorders in the inpatient setting. They take a holistic approach that considers both short and longer-term goals, relying on specialist colleagues, the wider multidisciplinary team and sometimes family members to guide treatment decisions during admissions on general medical wards. Additional support, education and training centered on the key themes may increase physicians’ confidence and ability to make effective treatment decisions for this patient group. The results are relevant to all health professionals working in this field to better understand the priorities of medical physicians and to support them to achieve positive outcomes in the inpatient treatment of patients with eating disorders

    Tracing the magnetic field of IRDC G028.23-00.19 using NIR polarimetry

    The importance of the magnetic (B) field in the formation of infrared dark clouds (IRDCs) and massive stars is an ongoing topic of investigation. We studied the plane-of-sky B field for one IRDC, G028.23-00.19, to understand the interaction between the field and the cloud. We used near-IR background starlight polarimetry to probe the B field and performed several observational tests to assess the field importance. The polarimetric data, taken with the Mimir instrument, consisted of H-band and K-band observations, totaling 17,160 stellar measurements. We traced the plane-of-sky B-field morphology with respect to the sky-projected cloud elongation. We also found the relationship between the estimated B-field strength and gas volume density, and we computed estimates of the normalized mass-to-magnetic flux ratio. The B-field orientation with respect to the cloud did not show a preferred alignment, but it did exhibit a large-scale pattern. The plane-of-sky B-field strengths ranged from 10 to 165 ÎĽG, and the B-field strength dependence on density followed a power law with an index consistent with 2/3. The mass-to-magnetic flux ratio also increased as a function of density. The relative orientations and relationship between the B field and density imply that the B field was not dynamically important in the formation of the IRDC. The increase in mass-to-flux ratio as a function of density, though, indicates a dynamically important B field. Therefore, it is unclear whether the B field influenced the formation of G28.23. However, it is likely that the presence of the IRDC changed the local B-field morphology.We thank J. Montgomery, T. Hogge, and I. Stephens for constructive discussions on the analysis.     Automated Image Analysis Comparing EDTA and Maleic Acid in Smear Layer Removal from Instrumented Human Posterior Root Canals

    EDTA has long been the gold standard for smear layer removal in endodontics. Recently Ballal has suggested maleic acid as a more effective alternative to EDTA, but more research is needed before it can be used in clinical practice. This study focused on comparing EDTA to maleic acid when removing the smear layer from curved root canals in human posterior teeth. Fifty-one posterior teeth were randomly selected and assigned to three groups: 17% EDTA, 7% maleic acid, and a 0.9% saline control. The teeth were prepared using K-files to an ISO size 40 MAF, step-backed to an ISO size 60 and irrigated with NaOCl between each file size. A final irrigation was performed with 5 mL of the respective group solution over 1 minute. The teeth were then dried and stored in a humidor until time for sectioning. The teeth were dehydrated, mounted and gold-palladium sputtered. Photomicrographs were taken using a SEM at 1,000 x, then 144 images were analyzed by digital image software to assess the surface area in pixels2 of open dentin tubules. The data was analyzed with a repeated measure ANOVA. It was found that the maleic acid produced greater surface area of open tubules than EDTA at the three defined canal regions (A. Coronal, B. Middle, C. Apical) but the difference was only significant in section B of the canal, where respectively, maleic acid was 120,433 pixels2 compared to EDTA with 66,399 pixels 2. Maleic acid needs to be seriously considered as an alternative to EDTA

    Telemedicine infectious diseases consultations and clinical outcomes: A systematic review

    Background: Telemedicine use is increasing in many specialties, but its impact on clinical outcomes in infectious diseases has not been systematically reviewed. We reviewed the current evidence for clinical effectiveness of telemedicine infectious diseases consultations, including outcomes of mortality, hospital readmission, antimicrobial use, cost, length of stay, adherence, and patient satisfaction. Methods: We queried Ovid MEDLINE 1946-, Embase.com 1947-, Scopus 1823-, Cochrane Database of Systematic Reviews (CDSR), Cochrane Central Register of Controlled Trials (CENTRAL), and ClinicalTrials.gov 1997- through August 5, 2019, for studies looking at clinical outcomes of infectious diseases in the setting of telemedicine use. We did not restrict by language or year of publication. Clinical outcomes searched included 30-day all-cause mortality, 30-day readmissions, patient compliance/adherence, patient satisfaction, cost or cost-effectiveness, length of hospital stay, antimicrobial use, and antimicrobial stewardship. Bias was assessed using standard methodologies. PROSPERO CRD42018105225. Results: From a search pool of 1154 studies, only 18 involved telemedicine infectious diseases consultation and our selected clinical outcomes. The outcomes tracked were heterogeneous, precluding meta-analysis, and the majority of studies were of poor quality. Overall, clinical outcomes with telemedicine infectious diseases consultation seem comparable to in-person infectious diseases consultation. Conclusions: Although in widespread use, the clinical effectiveness of telemedicine infectious diseases consultations has yet to be sufficiently studied. Further studies, or publication of previously collected and available data, are warranted to verify the cost-effectiveness of this widespread practice.
