The respiratory research agenda in primary care in Portugal: a Delphi study
Background: A research agenda can help to stimulate and guide research. The International Primary Care Respiratory Group (IPCRG) published a Research Needs Statement (RNS) in 2010 in which 145 research questions were identified. In 2012, priorities for respiratory research were established, based on these questions. To date, there has been no statement on primary care respiratory research needs in Portugal. The aim of the study was to develop a national consensus on research priorities in respiratory diseases in primary care in Portugal and to assess the applicability of the priorities for respiratory research set by the IPCRG.
Method: We conducted a Delphi study by electronic mail with a panel of experts on respiratory disease from primary and secondary care in Portugal. In the first round, the research needs in respiratory disease in Portugal were identified. In the second round, 196 research questions in six disease areas, derived from the first round and from the IPCRG Respiratory needs statement, were prioritised on a five-point Likert-type scale. In the third round, the questions were prioritized again with feed-back provided on the median scores for each item in the second round. Consensus was considered to have been reached when 80 % of the participants gave a score of 4 or 5 out of five on a given item.
Results: The 40 experts identified 121 respiratory research questions in Round 1 and expressed their views on 196 questions in Rounds 2 and 3. Twelve research questions (6 %) reached consensus. There were five questions in the asthma domain on early diagnosis, pulmonary function tests, the use of inhalers, and adherence to treatment. There were four questions in the chronic obstructive pulmonary disease domain on vaccinations, on routine monitoring and evaluation of treatment, on diagnosis, and on adherence to treatments. There was one question in the smoking domain on the effects of brief counselling. There were two questions on respiratory tract infections on the treatment of children and on the prescription of antibiotics. An additional 23 research questions (12 %) achieved consensus between 75 and 79 %.
Conclusion: The results reflect the Portuguese reality in response the international agenda for research on respiratory diseases published by the IPCRG. Foram pesquisados artigos nas principais
bases de dados internacionais de literatura médica.
IncluĂmos artigos publicados entre Janeiro de
2006 e Janeiro de 2012 que correspondiam aos
objetivos da revisĂŁo. Foi utilizada a taxonomia
SORT para a classificação dos resultados. Dos 253
artigos encontrados foram selecionados cinco que
cumpriam os critérios de inclusão. Estes incluem
trĂȘs revisĂ”es sistemĂĄticas (RS), uma meta-anĂĄlise
(MA) e uma norma de orientação clĂnica (NOC)
baseada numa RS. Os artigos selecionados avaliaram
a redução da mortalidade por cùncer da
mama através do rastreio com mamografia. A realização
do rastreio mamogrĂĄfico entre os 50 e os
69 anos Ă© recomendado em todos os artigos que
avaliam esta faixa etĂĄria. A NOC recomenda o
rastreio bienal. Em suma, a mamografia deverĂĄ
ser realizada entre os 50 e os 69 anos com uma
periodicidade bienal. Estes resultados vĂŁo ao encontro
do atual indicador de desempenho do rastreio
do cĂąncer da mama em Portugal.The scope of this review was to assess the strength of evidence for the current Portuguese performance indicator on breast cancer screening with mammography in order to determine the recommended age group and periodicity for screening. A search for articles was conducted in the main international databases of medical literature. Articles published between January 2006 and January 2012 addressing the objectives of this review were included. The SORT taxonomy was used to classify the results. Of the 253 articles, five articles met the inclusion criteria and were selected for review. These included three systematic reviews, one meta-analysis and one clinical guideline based on a systematic review. A reduction in breast cancer mortality with mamography screening was the outcome in all articles selected. Mammography screening between 50 and 69 years was recommended in all articles that assess this age group. The clinical guidelines recommended screening every two years. In conclusion, the current literature recommends mammography for women every two years between the ages of 50 and 69 years. This is consistent with the current performance indicator for breast cancer screening in Portugal
PRONUTRISENIOR: a holistic approach to the older adults living in the community; a rationale and methodology
O PRONUTRISENIOR Ă© uma abordagem holĂstica que considera os idosos integrados no seu meio ambiente com o objetivo de melhor capacitar os profissionais de saĂșde, cuidadores e outros profissionais para a monitorização do estado nutricional e assim diminuir a desnutrição neste grupo populacional. A formação e capacitação dos profissionais foram precedidas por uma avaliação da comunidade e seu ambiente. Esta informação foi subsequentemente integrada nos materiais educativos e informativos desenvolvidos para apoio aos programas de formação e capacitação. Este artigo apresenta os aspetos teĂłricos e metodolĂłgicos usados para atingir os objetivos definidos com foco nos idosos a viver no seu ambiente/ comunidade, e apresenta dados gerais sobre a população e amostra estudadas.PRONUTRISENIOR is a holistic approach that considers older adults as part of their environment, in order to better assist health professionals, caregivers and other professionals to monitor their nutritional status and thus reduce malnutrition in this population group. The education and empowerment of professionals were preceded by an assessment of the community and its environment. Such information was subsequently incorporated into the educational and informational materials to support training and empowerment programs. Cervical cancer screening: target age bracket, screening frequency and screening method: review of recent evidence and comparison with the Portuguese performance indicator
Esta revisĂŁo teve por objetivo avaliar a força de evidĂȘncia do indicador de desempenho portuguĂȘs relativo ao rastreio do CĂąncer do Colo do Ătero (CCU): (1) limites etĂĄrios das mulheres da população geral que o devem realizar, a (2) periodicidade com que deve ser realizado e (3) qual o melhor exame de rastreio. Foram pesquisados os seguintes termos MeSH: vaginal smears, age groups, periodicity, methods, uterine cervical cancer. Foram excluĂdos os artigos que nĂŁo abordavam o objetivo da investigação ou que nĂŁo fossem redigidos em InglĂȘs, PortuguĂȘs ou Espanhol. Para interpretar os artigos selecionados foi utilizada a classificação SORT. Foram encontrados 197 artigos, dos quais seleccionados 9: 1 revisĂŁo sistemĂĄtica (RS), 1 estudo clĂnico controlado aleatorizado, 2 estudos observacionais retrospectivos e 5 normas de orientação clĂnica (NOC). Os autores optaram por incluir nesta revisĂŁo mais 4 NOCs e 2 RSs por considerarem ser relevantes para a população Portuguesa, apesar de nĂŁo resultarem da pesquisa efectuada. Os estudos sugerem realização do rastreio entre os 21 e 25 atĂ© aos 65 anos, com uma periodicidade trienal usando a citologia convencional. Existe ainda controvĂ©rsia no que toca aos 3 objetivos deste artigo (limites etĂĄrios, frequĂȘncia e mĂ©todo).The scope of this review was to assess the strength of evidence of Portuguese performance indicators on Cervical Cancer screening: (1) age group of the women that should be screened for cervical cancer; (2) frequency of screening; and (3) the best method for screening. The following MeSH terms were searched: vaginal smears, age groups, periodicity, methods, uterine cervical cancer. Articles not reflecting the study objectives or not available in English, Portuguese or Spanish were excluded. The SORT classification was used to rate the articles selected.Of the 197 articles found, 9 that met all study criteria were selected for inclusion in this review. These included 1 systematic review, 1 randomized controlled clinical trial, 2 retrospective studies and 5 clinical guidelines. The authors also chose to include 4 clinical guidelines and two systematic reviews relevant to the Portuguese population even though they did not appear in the initial search of the literature. The studies suggest screening women between the ages of 21 to 25 years and 65 years of age, once every three years using conventional cytology. There is still controversy regarding the three objectives of this study (target age bracket, screening frequency and screening method)
Is the proteome of bronchoalveolar lavage extracellular vesicles a marker of advanced lung cancer?

Furthermore, the variance in the proteomics data sets is correlated mainly with lung cancer status and, to a lesser extent, smoking status and gender. Despite these advances BAL small and large extracellular vehicles (EVs) proteomes reveal aberrant protein expression in paracrine signaling mechanisms in cancer initiation and progression. We consequently present a case-control study of 24 bronchoalveolar lavage extracellular vesicle samples which were analyzed by state-of-the-art liquid chromatography-mass spectrometry (LC-MS). We obtained evidence that BAL EVs proteome complexity correlated with lung cancer stage 4 and mortality within two yearsÂŽ follow-up (p value = 0.006). The potential therapeutic target DNMT3B complex is significantly up-regulated in tumor tissue and BAL EVs. The computational analysis of the immune and fibroblast cell markers in EVs suggests that patients who deceased within the follow-up period display higher marker expression indicative of innate immune and fibroblast cells (four out of five cases). This study provides insights into the proteome content of BAL EVs and their correlation to clinical outcomes.publishersversionpublishe
Alterations in Vitamin D signalling and metabolic pathways in breast cancer progression: a study of VDR, CYP27B1 and CYP24A1 expression in benign and malignant breast lesions Vitamin D pathways unbalanced in breast lesions
<p>Abstract</p> <p>Background</p> <p>Breast cancer is a heterogeneous disease associated with different patient prognosis and responses to therapy. Vitamin D has been emerging as a potential treatment for cancer, as it has been demonstrated that it modulates proliferation, apoptosis, invasion and metastasis, among others. It acts mostly through the Vitamin D receptor (VDR) and the synthesis and degradation of this hormone are regulated by the enzymes CYP27B1 and CYP24A1, respectively. We aimed to study the expression of these three proteins by immunohistochemistry in a series of breast lesions.</p> <p>Methods</p> <p>We have used a cohort comprising normal breast, benign mammary lesions, carcinomas <it>in situ </it>and invasive carcinomas and assessed the expression of the VDR, CYP27B1 and CYP24A1 by immunohistochemistry.</p> <p>Results</p> <p>The results that we have obtained show that all proteins are expressed in the various breast tissues, although at different amounts. The VDR was frequently expressed in benign lesions (93.5%) and its levels of expression were diminished in invasive tumours (56.2%). Additionally, the VDR was strongly associated with the oestrogen receptor positivity in breast carcinomas. CYP27B1 expression is slightly lower in invasive carcinomas (44.6%) than in benign lesions (55.8%). In contrast, CYP24A1 expression was augmented in carcinomas (56.0% in <it>in situ </it>and 53.7% in invasive carcinomas) when compared with that in benign lesions (19.0%).</p> <p>Conclusions</p> <p>From this study, we conclude that there is a deregulation of the Vitamin D signalling and metabolic pathways in breast cancer, favouring tumour progression. Thus, during mammary malignant transformation, tumour cells lose their ability to synthesize the active form of Vitamin D and respond to VDR-mediated Vitamin D effects, while increasing their ability to degrade this hormone.</p
Expression of FOXA1 and GATA-3 in breast cancer: the prognostic significance in hormone receptor-negative tumours
The expression of additional genes, other than oestrogen receptor (ER), may be important to the hormone-responsive phenotype of breast cancer. Microarray analyses have revealed that forkhead box A1 (FOXA1) and GATA binding protein 3 (GATA-3) are expressed in close association with ERalpha, both encoding for transcription factors with a potential involvement in the ERalpha-mediated action in breast cancer. The purpose of this study was to explore if the expression of FOXA1 and GATA-3 may provide an opportunity to stratify subsets of patients that could have better outcome, among the ERalpha-negative/poor prognosis breast cancer group.The present study was supported by a research grant (SFRH/BD/15316/ 2005 to AA) financed by the Portuguese Science and Technology Foundation (FCT). Role of age and comorbidities in mortality of patients with infective endocarditis
[Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality.
[Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65âŻyears,65 to 80âŻyears,andâŻâ„âŻ80âŻyears.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk.
[Results]: A total of 3120 patients with IE (1327âŻ<âŻ65âŻyears;1291 65-80âŻyears;502âŻâ„âŻ80âŻyears) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients â„80âŻyears who underwent surgery were significantly lower compared with other age groups (14.3%,65âŻyears; 20.5%,65-79âŻyears; 31.3%,â„80âŻyears). In-hospital mortality was lower in the <65-year group (20.3%,<65âŻyears;30.1%,65-79âŻyears;34.7%,â„80âŻyears;pâŻ<âŻ0.001) as well as 1-year mortality (3.2%, <65âŻyears; 5.5%, 65-80âŻyears;7.6%,â„80âŻyears; pâŻ=âŻ0.003).Independent predictors of mortality were ageâŻâ„âŻ80âŻyears (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32â3.34), CCIâŻâ„âŻ3 (HR:1.62; 95% CI:1.39â1.88),and non-performed surgery (HR:1.64;95% CI:11.16â1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65âŻyears(pâŻ<âŻ0.001) for both in-hospital and 1-year mortality.
[Conclusion]: There were no differences in the clinical presentation of IE between the groups. AgeâŻâ„âŻ80âŻyears, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group
