    psychometric validation of the Portuguese Staff Attitudes to Coercion Scale (SACS)

    Publisher Copyright: © The Author(s) 2024.Background: There is a growing recognition that staff attitudes toward coercion in mental health care may influence its application. This study presents the psychometric properties of the Portuguese version of the Staff Attitudes towards Coercion Scale (SACS) and describes mental health professionals’ attitudes towards coercion in Portugal. Methods: Mental health professionals working in five Portuguese mental health services in urban and rural regions of Portugal were invited to complete a questionnaire comprising the SACS and a socio-demographic form. Psychometric analyses including construct validity and internal consistency were carried out using R software. Results: A total of 91 out of 119 questionnaires completed were valid for analysis. Fifty-seven (62.64%) respondents were female, with an age range of 24 to 69 years (M = 39.33; SD = 11.09). More than half of them were nurses (52.75%, n = 48), and a third were psychiatrists (36.26%, n = 33). A three-factor structure was confirmed and showed the best fit compared to previously proposed models with a cumulative explained variance of 59%. The Portuguese SACS exhibited adequate internal consistency for both the full-scale and subscales. The highest mean score was in the pragmatic attitude domain (20.60; SD = 3.37). A negative correlation was observed between the critical attitude domain and both age and years of experience (p < 0.05). Conclusion: A three-factor structure was confirmed and showed the best fit compared to previously proposed models. The Portuguese SACS showed excellent psychometric properties and is acceptable for assessing staff attitudes towards coercion.publishersversionpublishe

The present study aimed to assess the psychometric properties of the Portuguese Admission Experience Survey(P-AES). The study employed a cross-sectional approach in five psychiatric departments in three regions of Portugal. A total of 208 patients participated in the survey. Reliability was assessed through internal consistency and test-retest procedures. Internal validity was analyzed using a two-parameter logistic item response model, exploring three models, including a bifactor model. Convergent validity was determined by correlating AES scores with the Coercion Ladder (CL), Client Assessment of Satisfaction (CAT), and Global Assessment of Functioning (GAF) scale. Discriminatory power was assessed by comparing scores between patients with voluntary and involuntary admission status. The P-AES demonstrated satisfactory internal consistency and test-retest reliability. The bifactor model exhibited superior fit compared to the one-factor and three-factor models. Correlations between P-AES and CL, as well as CAT scores, indicated good convergent validity. Additionally, P-AES scores were notably higher in patients with compulsory psychiatric hospital admission compared to those admitted voluntarily, confirming its discriminatory power.     Boas práticas de enfermagem na utilização de dreno de tórax: revisão integrativa

    The aim was to identify in the scientific literature the best nursing practices related to the use of a chest tube in an adult Intensive Care Unit. This is an integrative review with consultation of the Latin American Caribbean Literature on Health Sciences, Nursing Database, Online Medical Literature Search and Analysis System, Scientific Electronic Library Online, PubMed, CINAHL and EBSCO. Studies published between January 2014 and December 2018 in English, Portuguese and Spanish and available in full of full texts are included. Those with pediatric patients and duplicities were excluded. 133 studies were identified that gave rise to five articles analyzed. Of the selected articles, 80% corresponded to international studies; 40% published in 2016 and 60% with quantitative analysis. The categories of good practices with chest tube, complications related to chest tube, and use of a care protocol related to chest tube emerged. Therefore, the use of care protocols in the use of chest tubes and evidence-based practices contribute to the improvement and quality of nursing care for these patients.Objetivou-se identificar na literatura científica as melhores práticas de enfermagem relacionadas a utilização de dreno torácico em Unidade de Terapia Intensiva adulta. Trata-se de uma revisão integrativa com consulta à Literatura Latino-Americana do Caribe em Ciências da Saúde, Base de Dados de Enfermagem, Sistema Online de Busca e Análise de Literatura Médica, Scientific Electronic Library Online, PubMed, CINAHL e EBSCO. Incluídos estudos publicados entre janeiro de 2014 a dezembro de 2018 nos idiomas inglês, português e espanhol e disponíveis na íntegra com textos completos. Excluídos aqueles com pacientes pediátricos e duplicidades. Foram identificados 133 estudos que deram origem a cinco artigos analisados. Dos artigos selecionados 80% corresponderam a estudos internacionais; 40% publicados em 2016 e 60% com análise quantitativa. Emergiram as categorias de boas práticas com dreno torácico, complicações relacionadas ao dreno torácico e uso de protocolo de cuidados relacionados ao dreno torácico.     The aim was to characterize effective communication actions in the teaching of Patient Safety aimed at primary care in an Undergraduate Nursing Course, from the student's perspective. Descriptive-exploratory and qualitative research, whose data were collected between December 2020 and March 2021, through an online questionnaire and addressed by the Thematic-Categorial Content Analysis. Participated 31 students who pointed out the teaching of Patient Safety still concentrated in theoretical classes, with a focus on hospital care and little addressed in primary care. Students' understanding of effective communication in primary care was related to user-accessible and interprofessional communication through electronic medical records. Students understand effective communication as a Patient Safety action. Its approach goes beyond hospital care and in practical scenarios to promote the quality of care and Patient Safety also in primary care.Objetivou-se caracterizar as ações de comunicação efetiva no ensino da Segurança do Paciente voltadas à atenção primária em um Curso de Graduação em Enfermagem, sob a ótica discente. Pesquisa descritiva-exploratória e qualitativa, cujos dados foram coletados entre dezembro de 2020 e março de 2021, por meio de questionário online e abordados pela Análise de Conteúdo Temático-Categorial. Participaram 31 discentes que apontaram o ensino da Segurança do Paciente ainda concentrado em aulas teóricas, com foco na atenção hospitalar e pouco abordado na atenção primária. A compreensão discente sobre comunicação efetiva na atenção primária relacionou-se à comunicação acessível ao usuário e interprofissional pelo prontuário eletrônico.     Decrease of perforin positive CD3+γδ-T cells in patients with obstructive sleep disordered breathing

We investigated whether SRBD patients with or without intermittent hypoxia show substantial differences in perforin and granzyme-B positive peripheral blood lymphocytes. Methods: A total of 87 subjects were included and distributed as follows: 24 controls (C), 19 patients with respiratory effort related arousals due to increased upper airway resistance (UAR) without hypoxic events, 24 obese patients with obstructive sleep apnea (OSA) (oOSA), and 20 without obesity (noOSA). After olysomnographic recording, we analyzed in fasting blood samples routine hematologic and biochemical parameters and the percentage of lymphocytes containing the proteins perforin and granzyme-B (GrB). Kruskal-Wallis tests and a posteriori multiple comparisons were applied for statistical analysis of results. Results: Perforin-positive γδ-cells revealed significant differences between groups (p = 0.017), especially between the Control group and the oOSA (p-value = 0.04); the remaining SRBD groups also showed differences from the control (C vs UAR: p = 0.08; C vs noOSA = 0.09), but they did not raise to statistical significance. There were no differences among the SRBD groups. Granzyme-B cells were decreased in SRBD patients, but the differences were not statistically significant. No additional statistical significant result was found in the other investigated lymphocyte subsets. Conclusions: Obstructive sleep-disordered breathing is associated with a decrease in perforin-positive CD3+γδ-T cells. Although this finding was detected in lean patients without intermittent hypoxia, the reduction was only statistically significant in obese patients with severe OSA.     No supportive evidence for TIA1 gene mutations in a European cohort of ALS-FTD spectrum patients

    We evaluated the genetic contribution of the T cell-erestricted intracellular antigen-1 gene (TIA1) in a European cohort of frontotemporal dementia (FTD) and amyotrophic lateral sclerosis (ALS) patients. Exonic resequencing of TIA1 in 1120 patients (693 FTD, 341 ALS, 86 FTD-ALS) and 1039 controls identified in total 5 rare heterozygous missense variants, affecting the TIA1 low-complexity domain (LCD). Only 1 missense variant, p.Met290Thr, identified in a familial FTD patient with disease onset at 64 years, was absent from controls yet received a combined annotation-dependent depletion score of 11.42. By contrast, 3 of the 4 variants also detected in unaffected controls, p.Val294Glu, p.Gln318Arg, and p.Ala381Thr, had combined annotation-dependent depletion scores greater than 20. Our findings in a large European patient-control series indicate that variants in TIA1 are not a common cause of ALS and FTD. The observation of recurring TIA1 missense variants in unaffected individuals lead us to conclude that the exact genetic contribution of TIA1 to ALS and FTD pathogenesis remains to be further elucidated

    MANEJO DA RESSUSCITAÇÃO CARDIOPULMONAR NO NEONATO

    INTRODUÇÃO: A ressucitação cardiopulmonar (RCP) neonatal é a aplicação de manobras que dever ser executada de forma eficaz quando diagnósticado a PCR, visando promover o aporte sanguineno apropriado para o cerebro e orgão vitais, sendo realizado o a frequencia e profunidade das compressões toracicas de forma que melhore a perfusão cardiaca e cerebral, que deve ser iniciada após a detecção precoce. OBJETIVOS: evidenciar as práticas de manejo da ressuscitação cardiopulmonar neonatal. MÉTODOS: Revisão integrativa, realizada com base na pergunta norteadora: " Qual o manejo para a ressucitação cardiopulmonar em neonatos?”, através de artigos em língua portuguesa e inglesa, publicados no período de 2018 a 2023, originados das bases de dados MEDLINE, SciELO, BDENF-Enfermagem, LILACS. Realizou-se a coleta de dados em agosto de 2023. RESULTADOS: A amostra foi constituída por 8 artigos, que denotaram um núcleo: Atuação da equipe na sala de parto no manejo da ressuscitação cardiopulmonar em neonatos. CONCLUSÃO: As manobras de RCP, trouxe a percepção a importância dos profissionais em se atualizar, aumentando sua bagagem de conhecimento técnico-científico, a fim de proporcionar assistência adequada à RNs e suas famílias, para que, assim, alcance resultado eficaz em tempo reduzido, e promovendo a redução de complicações decorrentes a PCR

    First report of severe acute respiratory syndrome coronavirus 2 detection in two asymptomatic cats in the state of Pernambuco, Northeastern Brazil

    Background and Aim: Despite worldwide case reports, including Brazilian cases, no frequency study on infection of pets by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been conducted to date in Brazil. Accordingly, the present study was aimed to assess dogs and cats belonging to positive owners in Recife, Northeastern Brazil. Materials and Methods: This was a longitudinal prospective study on dogs and cats in the city of Recife whose owners were in isolation at home due to a confirmed laboratory diagnosis of SARS-CoV-2 through reverse-transcriptase polymerase chain reaction (RT-qPCR). Oral and rectal swabs from the pets were tested for the presence of SARS-CoV-2-specific RNA by means of RT-qPCR. Results: Among the pets tested, 0/16 dogs and 2/15 cats were positive for SARS-CoV-2. Interestingly, the two positive cats were owned by two unrelated asymptomatic veterinary students, which, therefore, post a warning to veterinarians worldwide. Conclusion: The findings herein indicate that cats may act as sentinels for human cases, particularly sharing households with asymptomatic human cases. Although with small sampling and convenient recruiting, the presence of infected cats by SARS-CoV-2 was most likely due to close cat-human contact with positive owners, posting a human-animal health threat when pets share the same bed and interact with owners without protection, particularly during owner self-isolation. Thus, infected owners should follow the same human preventive guidelines with their pets to avoid spreading infection
