Comparative prevalence of atopic sensitization in the border areas between Russia and Finland
Current century has been highlighted with rise in allergic pathology, and environment markedly affects it via epigenetic mechanisms. Allergy research in Russia and Finland has been conducted for many years, of larger scale recorded in the latter, still being maintained over time. Atopy prevalence was studied in the border areas in both countries. The level of total, dietary, inhaled IgE level, as well as dietary and inhalation range were examined. The Phadiatop® radio allergen sorbent test was used (UniCAP 1000 v.2; Pharmacia Upjohn, Uppsala, Sweden), with cut-off point: IgE > 110 kUA/l and sIgE > 2.5 kUa/l and 0.35 kUA/l. The study was subdivided into 2 stages conducted during 2003 and 2010/2012 timeframe. Stage 1 was performed with children from Finland (n = 344), Russia (n = 427), aged 7—15 years, whereas stage 2 — 180 subjects (Finland, n = 98, Russia, n = 82). Results. It was found that total IgE level was higher in children from Finland. The inhaled sIgE in 2003 and 2010/2012 was higher among children from Finland, p = 0.019 and p = 0.027, showing temporal elevation. In 2003, dietary sIgE was higher among those from Finland (27.6% and 18.3%), p = 0.142, whereas in 2010/2012 it was decreased by 2- and 3-fold in subjects from Finland and Russia, respectively. In 2003, high atopy rate against dust mites, cats, timothy, birch, and dogs was found in Russia, which was increased at stage 2. Such parameter was most evidently elevated for timothy — by 2-fold, p = 0.176, wormwood — by 2-fold, p = 0.312, equine allergens — by 2-fold, p = 0.563, canine allergens — by 1.5-fold, mite — by 1.5-fold, p = 0.220. In 2003, atopy rate in Finland to canine allergens reached 26.5%, timothy — 24.5%, birch — 20.4%, feline allergens — 20.4%. In 2010, it was noted rise in atopy rate mainly to dust mites — by 2-fold, equine allergens — by 2-fold. In 2003, atopy rate in Russia to egg, milk, and wheat prevailed, whereas in Finland it was mostly found against milk, nuts, wheat, eggs, in both cases declined in dynamics. Interestingly, no sensitization to fish in subjects from Russia and Finland was observed at both stages. Finally, at the stage 2 dietary atopy rate was declined in both countries. Differences in atopic sensitization as well as during 7—9 year follow-up observation were preserved between both subject cohorts from Finland and Russia
V*-algebras, independence algebras and logic
Independence algebras were introduced in the early 1990s by specialists in semigroup theory, as a tool to explain similarities between the transformation monoid on a set and the endomorphism monoid of a vector space. It turned out that these algebras had already been defined and studied in the 1960s, under the name of v*-algebras, by specialists in universal algebra (and statistics). Fiber Acoustic Waveguide : A Sensor Candidate
Sensor development plays a key role in the field of nondestructive evaluation and process control. The annual fiber optic sensor market alone is a multimillion dollar business (1). Acoustic waves are about five orders of magnitude slower than optical waves and can also be guided in cladded glass fibers, similar to optical fibers, with low loss and low dispersion (2–7). Fiber acoustic waveguides are believed to be a very attractive and basic component for further sensor development (8). In this paper a brief theoretical description of a weakly guiding acoustic fiber (7) is given. The material selection criteria for the core and the cladding of the fiber guide, the properties of single-mode operation, and some sensing mechanisms for temperature and pressure variations are discussed. Estimation of the autoimmune thrombophylia in pregnant women with hypothyroidism of different origin
Aim of the research: estimation of the role ot autoimmune thrombophylia in the development of gestation complications on the background of hypothyroidism. Material and methods: prospective research of 60 pregnant women with hypothyroidism of different origin aged 19-39 years (29±0,51), divided into two equal groups. The first group — 30 pregnant women with hypothyroidism connected with autoimmune thyroiditis (AIT). The second group included 30 pregnant women with postoperative hypothyroidism (PT). All patients have compensated hypothyroidism from the beginning of pregnancy. Study results: complex of clinical signs, thrombocytopenia and increased lupus anticoagulant (LA) circulation has been revealed in the 1 group — 23,3% (7/30) and in the 2 group — 10,0% (3/30). Low placentation as the marker of primary fetoplacental insufficiency (FPI) was diagnosed reliably more often in the 1 group (46,7%(14/30) when compared with 10%(3/30) in the 2 group (χ²=8,2, (df)=1, P=0,004). Conclusion: hypothyroidism resulted from AIT is the marker of systemic «autoimmune damage» in the development of miscarriage and FPI. The presence of autoimmune thrombophylia in the pregnant women with hypothyroidism connected with AIT requires the indication of low molecular weight heparins twice daily in combination with systemic enzymotherapy and this allows improving perinatal prognosis.Цель работы: изучить роль аутоиммунной тромбофилии в формировании гестационных осложнений при гипотиреозе различного генеза в регионе с легким йодным дефицитом. Материалы и методы: проведено проспективное исследование 60 беременных с гипотиреозом различного генеза в возрасте от 19 до 39 лет (29±0,51) распределенных на две равные группы. Первая группа — 30 беременных с гипотиреозом вследствие аутоиммунного тиреоидита (АИТ). Вторая — 30 беременных с послеоперационным гипотиреозом (ПГ). Все пациентки с момента взятия на учет были компенсированы по гипотиреозу. Результаты исследований: выявлено сочетание клинических проявлений, тромбоцитопении и увеличения циркуляции волчаночного антикоагулянта (ВА) в 1 группе — 23,3%(7/30) и во 2 группе — 10,0%(3/30). Низкая плацентация, как маркер первичной фетоплацентарной недостаточности (ФПН) достоверно чаще встречалась в 1 группе (46,7%(14/30) по сравнению с 10,0%(3/30) во 2 группе (χ²=8,2, (df)=1, Р=0,004). Выводы: в формировании невынашивания и ФПН гипотиреоз вследствие АИТ является маркером системной «аутоиммунной агрессии». Наличие аутоиммунной тромбофилии у беременных с гипотиреозом вследствие АИТ требует назначения низкомолекулярных гепаринов в двукратном суточном режиме в сочетании с системной энзимотерапией и позволяет улучшить перинатальный прогноз
Information-analytical assessment systems for perinatal outcomes and children's health status born by assisted reproductive technologies
Purpose: To integrate clinical databases based on the information and analytical systems of medical organizations (MO) and to evaluate outcomes and health of children born by assisted reproductive technologies (ART). Research methods: To form database by integrating information and analytical systems of the medical organizations. To search and unify the data by means of freeware relational database management system (DBMS) - MySQL. Results. A prototype application for the management and support of a unified clinical database to analyze and evaluate the outcomes of ART. To compare information from individual databases of MO one need to match fields in the form of unique personal records, field validation, processing of missing data. Significant (p <0.001) differences in gestational age and anthropometric data in the databases of medical organizations (76% of full-term children from the Automated System “Regional obstetric monitoring” (AS ROM) database compared with 18% according to the IS “Register of Children requiring Early Care”) were determined by the introduction of the conception of ART, along with the presence of perinatal risk factors in children. The created software can be used to create registers of medical organizations, as well as to support clinical decision-making in forecasting, modeling outcomes and children's health after ART, developing personalized treatment and rehabilitation programs. Conclusion. It is necessary to develop and implement information systems, to create ART registers on the basis of regional and national registers, to analyze the outcomes of the use of reproduction methods. © 2020 National Academy of Pediatric Science and Innovation. The measurement of airway occlusion pressure over the first 100 ms of inspiration (P100 index) is a simple non-invasive test for the evaluation of the patients’ central inspiratory activity. This test does not require any complicated diagnostic equipment, is easy for the patient, does not require any respiratory manoeuvres on his part. P100 index measurements enrich our concepts of the condition of external respiration system, and substantiate pharmacological and non-pharmacological treatment. P100 index helps to evaluate physiological mechanisms of respiratory insufficiency and their extent.
Spontaneous regression in alveolar soft part sarcoma: case report and literature review

They are most of the time found in extremities</p> <p>Case presentation</p> <p>We are presenting a case of adult male patient, who was found to have huge abdominal mass and multiple gastric and duodenal polyps. Pathological diagnosis for all lesions was Alveolar soft part sarcoma. Although he complained from metastasis to both lungs and right atrium, all these deposits regressed spontaneously. Patient stated that he used some herbs (Teucrium polium, Cat Thyme) prescribed for him. No chemotherapy or radiotherapy was given. The duration of regression was about 5 months before other lesions appeared. Later on, he died secondary to brain metastasis.</p> <p>Conclusion</p> <p>ASPS is a rare type of sarcomas that affect primarily the lower limbs. This tumor does rarely metastasize to the gastrointestinal tract.</p
Syndrome dimensions and the Child Behavior Checklist and the Teacher Report Form: A critical empirical evaluation
The construct representation of the cross-informant model of the Child Behavior Checklist (CBCL) and the Teacher Report Form (TRF) was evaluated using confirmatory factor analysis. Samples were collected in seven different countries. The results are based on 13,226 parent ratings and 8893 teacher ratings. The adequacy of fit for the cross-informant model was established on the basis of three approaches: conventional rules of fit, simulation, and comparison with other models. The results indicated that the cross-informant model fits these data poorly. These results were consistent across countries, informants, and both clinical and population samples. Since inadequate empirical support for the cross-informant syndromes and their differentiation was found, the construct validity of these syndrome dimensions is questioned

A multilevel neo-institutional analysis of infection prevention and control in English hospitals: coerced safety culture change?
Despite committed policy, regulative and professional efforts on healthcare safety, little is known about how such macro-interventions permeate organisations and shape culture over time. Informed by neo-institutional theory, we examined how inter-organisational influences shaped safety practices and inter-subjective meanings following efforts for coerced culture change. We traced macro-influences from 2000 to 2015 in infection prevention and control (IPC). Safety perceptions and meanings were inductively analysed from 130 in-depth qualitative interviews with senior- and middle-level managers from 30 English hospitals. A total of 869 institutional interventions were identified; 69% had a regulative component. In this context of forced implementation of safety practices, staff experienced inherent tensions concerning the scope of safety, their ability to be open and prioritisation of external mandates over local need. These tensions stemmed from conflicts among three co-existing institutional logics prevalent in the NHS. In response to requests for change, staff flexibly drew from a repertoire of cognitive, material and symbolic resources within and outside their organisations. They crafted 'strategies of action', guided by a situated assessment of first-hand practice experiences complementing collective evaluations of interventions such as 'pragmatic', 'sensible' and also 'legitimate'.
