Starting Antihypertensive Drug Treatment With Combination Therapy
The 2018 European Society of Cardiology/European Society of Hypertension1 and the 2020 International Society of Hypertension2 guidelines for the management of hypertension proposed that initial combination therapy with 2 antihypertensive agents in a single-pill combination (SPC) is preferred in most patients in need of blood pressure (BP) lowering treatment and should replace the long-standing concept of starting treatment with a single agent, rotating through antihypertensive drug classes, and next moving towards combining drug classes. By moving SPCs forward as the initial BP-lowering strategy, the European1 and International2 Societies of Hypertension Guideline Committees overlooked several principles in hypertension management: (1) understanding the pathophysiology of hypertension; (2) prioritizing evidence from randomized clinical trials above observational studies and expert opinion; and (3) giving consideration to the cost-effectiveness of antihypertensive drug treatment and the sustainability of health care. This article addresses these points. Sources of information included (1) guidelines issued by European,1,3,4 American,5â7 International,2,8,9 and British10â12 Expert Committees, published between 19998 and 2020,2 summarized in Table S1 in the Data Supplement; (2) a PubMed search ran on May 5, 2020, without limitations with as search terms in the abstract or title âhypertensionâ combined with âfixed combinationâ OR âhypertensionâ combined with âsingleâ and âcostsâ; (3) the placebo-controlled trials of antihypertensive drug treatment, as identified from the reference lists of 5 systematic literature reviews,13â17 of which 2 were published by the Blood Pressure Lowering Trialistsâ Collaboration14,16; (4) 3 randomized controlled trials of usual versus intensive BP control18â20; and (5) the retail costs of antihypertensive drugs on the Belgian market (https://www.bcfi.be)
Effective Chemical Constituentâs Identification Of Extracting Solution From Herbs â A Review
Background and Objective: The aim of this article, was the definition, and targets of possible mechanisms of Yin lai Decoction by retrieving the herbals in the decoction and integrating information from multi-databases, which can be guidance for the followed experimental study.Matherials and Methods: The information from multi-databases was integrated. Results: In the study, we discovered 92 possible gene targets of Yin lai Decoction, 87% of which appeared in the targets of Flos Lonicerae, Fructus Forsythiae and Scutellaria baicalensis Georgi. The function of these targets focuses on the anti-infection and regulating immune system, and metabolism of the body.Conclusion: Yin lai Decoction may affect the process of metabolism, immune response and infection by impact on the 92 possible gene targets, then it can mitigate the damage caused by infection and promote the body to health. Theory of control of optomechanical transducers for quantum networks
We present a scheme of control for the arbitrary optical interface mediated by a nanoscale mechanical oscillator between flying qubits and optical nonactive solid-state qubits. This quantum interface lays the foundation for many key functions of a quantum network, such as transferring, swapping, and entangling qubits between distant nodes of a quantum network. Numerical simulations of the quantum interface operations show high fidelities and robust tolerance under realistic experimental conditions. Compared with a previous scheme, it may significantly increase the speed of state transfer operation of high fidelity

A metabolite specific 3D stack-of-spirals bSSFP sequence for improved bicarbonate imaging in hyperpolarized [1-13C]Pyruvate MRI
13C-bicarbonate is a crucial measure of pyruvate oxidation and TCA cycle flux, but is challenging to measure due to its relatively low concentration and thus will greatly benefit from improved signal-to-noise ratio (SNR). To address this, we developed and investigated the feasibility of a 3D stack-of-spirals metabolite-specific balanced steady-state free precession (MS-bSSFP) sequence for improving the SNR and spatial resolution of dynamic 13C-bicarbonate imaging in hyperpolarized [1-13C]pyruvate studies. The bicarbonate MS-bSSFP sequence was evaluated by simulations, phantoms studies, preclinical studies on five rats, brain studies on two healthy volunteers and renal study on one renal cell carcinoma patient. The simulations and phantom results showed that the bicarbonate-specific pulse had minimal perturbation of other metabolites (<1%). In the animal studies, the MS-bSSFP sequence provided an approximately 2.6-3 Ă improvement in 13C-bicarbonate SNR compared to a metabolite-specific gradient echo (MS-GRE) sequence without altering the bicarbonate or pyruvate kinetics, and the shorter spiral readout in the MS-bSSFP approach reduced blurring. Using the SNR ratio between MS-bSSFP and MS-GRE, the T2 values of bicarbonate and lactate in the rat kidneys were estimated as 0.5 s and 1.1 s, respectively. The in-vivo feasibility of bicarbonate MS-bSSFP sequence was demonstrated in two human brain studies and one renal study. These studies demonstrate the potential of the sequence for in-vivo applications, laying the foundation for future studies to observe this relatively low concentration metabolite with high-quality images and improve measurements of pyruvate oxidation
Propagation inhibition and wave localization in a 2D random liquid medium
Acoustic propagation and scattering in water containing many parallel
air-filled cylinders is studied. Two situations are considered and compared:
(1) wave propagating through the array of cylinders, imitating a traditional
experimental setup, and (2) wave transmitted from a source located inside the
ensemble. We show that waves can be blocked from propagation by disorders in
the first scenario, but the inhibition does not necessarily imply wave
localization. Furthermore, the results reveal the phenomenon of wave
localization in a range of frequencies.Comment: Typos in Fiures are correcte
Three-body recombination of ultra-cold atoms to a weakly bound level
We discuss three-body recombination of ultra-cold atoms to a weakly bound
level. In this case, characterized by large and positive scattering length
for pair interaction, we find a repulsive effective potential for three-body
collisions, which strongly reduces the recombination probability and makes
simple Jastrow-like approaches absolutely inadequate. In the zero temperature
limit we obtain a universal relation, independent of the detailed shape of the
interaction potential, for the (event) rate constant of three-body
