    FDG-PET positive pilomatrixoma — reconsidering multicentricity in Langerhans cell histiocytosis

    We report a case of a woman who underwent a curative resection of an upper jaw tumor histologically verified as eosinophilic granuloma. To exclude possible multiorgan involvement, PET/CT imaging was performed and revealed a metabolically active, partially calcified lesion located on the chest wall surface and clinically corresponding to a gradually developing, round, subcutaneous infiltrate with erythematous overlying skin. After complete extirpation, the pathological finding was consistent with pilomatrixoma surprisingly, thus dismissing the suspected diagnosis of multi-system Langerhans cell histiocytosis

    Dynamic impact modeling as a road transport crisis management support tool

    Crisis management must provide data to allow for real-time decision-making. Accurate data is especially needed to minimize the risk of critical infrastructure failure. Research into the possible impacts of critical infrastructure failure is a part of developing a functional and secure infrastructure for each nation state. Road transport is one such sector that has a significant impact on its functions. When this fails, there may be a cascading spread of impacts on the energy, health, and other sectors. In this regard, this paper focuses on the dynamic modeling of the impacts of critical road infrastructure failures. It proposes a dynamic modeling system based on a stochastic approach. Its essence is the macroscopic model-based comparative analysis of a road with a critical element and detour roads. The outputs of this system are planning documents that determine the impacts of functional parameter degradation on detour roads-not only applicable in decision-making concerning the selection of the optimal detour road, but also as a support mechanism in minimising possible risks.     Measuring diffuse metabolic activity on FDG-PET/CT: new method for evaluating Langerhans cell histiocytosis activity in pulmonary parenchyma

    Pulmonary Langerhans cell histiocytosis (PLCH) is a rare cause of interstitial lung disease characterized by formation of nodules in the active phase of the disease that evolve into nonactive cystic lesions later on. To evaluate PLCH activity in patients, we developed a new method for measuring diffuse metabolic activity on fluorine-18-fluorodeoxyglucose positron emission tomography/ computed tomography (FDG-PET/CT) using a lung-to-liver activity ratio. Material and Methods: We retrospectively studied a series of 4 FDG-PET and 23 FDG-PET/CT scans from 7 patients with PLCH and analyzed a sample of 100 randomly chosen FDG-PET/CT studies free from any known lung or hepatic diseases. Maximum standardized uptake value (SUVmax) in a spherical volume (6–8 cm in diameter) in the right lung was put into relation with SUVmax in a spherical volume (9–10 cm in diameter) in the reference liver parenchyma to set up the SUVmaxPULMO/SUVmaxHEPAR index. The index values were compared to the disease course in each patient. Results: In patients with PLCH, a close correlation between the index value and the disease course was found in all seven subjects, where the increasing index values indicated disease activity, while decreasing index values were observed after therapy administration. In the group of 100 healthy control subjects, we found index values lower than 0.3 in 80% and lower than 0.4 in 96%. Conclusion: Measuring SUVmaxPULMO/SUVmaxHEPAR values and their time-trend monitoring represent simple, noninvasive screening tools allowing an early diagnosis and treatment response follow-up assessment in patients with PLCH

    Increasing Incidence of Geomyces destructans Fungus in Bats from the Czech Republic and Slovakia

    BACKGROUND: White-nose syndrome is a disease of hibernating insectivorous bats associated with the fungus Geomyces destructans. It first appeared in North America in 2006, where over a million bats died since then. In Europe, G. destructans was first identified in France in 2009. Its distribution, infection dynamics, and effects on hibernating bats in Europe are largely unknown. METHODOLOGY/PRINCIPAL FINDINGS: We screened hibernacula in the Czech Republic and Slovakia for the presence of the fungus during the winter seasons of 2008/2009 and 2009/2010. In winter 2009/2010, we found infected bats in 76 out of 98 surveyed sites, in which the majority had been previously negative. A photographic record of over 6000 hibernating bats, taken since 1994, revealed bats with fungal growths since 1995; however, the incidence of such bats increased in Myotis myotis from 2% in 2007 to 14% by 2010. Microscopic, cultivation and molecular genetic evaluations confirmed the identity of the recently sampled fungus as G. destructans, and demonstrated its continuous distribution in the studied area. At the end of the hibernation season we recorded pathologic changes in the skin of the affected bats, from which the fungus was isolated. We registered no mass mortality caused by the fungus, and the recorded population decline in the last two years of the most affected species, M. myotis, is within the population trend prediction interval. CONCLUSIONS/SIGNIFICANCE: G. destructans was found to be widespread in the Czech Republic and Slovakia, with an epizootic incidence in bats during the most recent years. Further development of the situation urgently requires a detailed pan-European monitoring scheme

    Integral approach to assessing the criticality of railway infrastructure elements

    In the last ten years, considerable attention has been paid to analysing and assessing the criticality of railway infrastructure elements. Publications on the subject mostly assess elements only from a certain point of view, such as purpose, reliability or risk. This leads to only a partial assessment of criticality without continuous correlation, which may result some critical elements of the system being omitted. The article introduces the railway infrastructure criticality assessment tool (RICA tool), which was created to evaluate the criticality of railway infrastructure elements in all aspects. The integral approach of the tool lies in comprehensively assessing the technical and process factors of rail transport. The criticality of railway infrastructure elements is therefore assessed not only in terms of the relevance and resilience of elements but also their interdependence, substitutability, risk and impact.Web of Science16212910

    Qualitative approach to environmental risk assessment in transport

    The purpose of this paper is to present the development of a qualitative approach to environmental risk assessment (QAERA) in transport. The approach is described as a model developed for the future software tool which will be utilizable as a risk decision support system. The basic part is aimed on developing a quantitative environmental risk assessment. Thus, this paper describes a set of 6 pillars of safety and security. Accordingly, the paper contains both chosen safety and security indicators and selected criteria for assessing the risk of launching the environmental change of global model thinking in the transport sector. The environmental risk assessment as a global model of thinking was originally based on historical experience but, nowadays, it is changing. Based on new expert knowledge, more precisely, on input of new global data, paper displays an environmental risk assessment with actual interpretation. The discussion of the paper is oriented to support research results, a new knowledge-oriented approach to global climate changes, using suitable risk assessment methods and technics. The result of the paper is a new approach for the modeling of environmental risk assessment in the transport sector.Web of Science1715art. no. 549

    Identifying critical elements of road infrastructure using cascading impact assessment

    Road transport is a key means of transporting people and cargo on land. Its particular advantages are speed and operability, which are balanced, however, by dependence on road infrastructure. Road infrastructure reliability is an important factor in its functioning. If some elements of road infrastructure are disrupted or fail, the function of dependent infrastructures, such as the integrated rescue system or industry, are also impaired and may fail. These important elements of road infrastructure should be identified as critical and be given greater attention when identifying weaknesses and implementing subsequent security measures. This article introduces the Identifying Critical Elements of Road Infrastructure (ICERI) method, which was designed to make use of Cascading Impact Assessments (CIA). The use of CIA allows critical elements to be identified through impact escalation analysis. These impacts can therefore be monitored not only in road transport infrastructure but also across the entire critical infrastructure system.Web of Science35331430
