    Determinación de distintos parámetros relacionados con el estado nutricional en músicos. Importancia para el desarrollo de su actividad profesional

    Los músicos constituyen un colectivo profesional en los que la demanda constante del perfeccionamiento de la técnica instrumental, la elevada competitividad y los altos niveles de angustia, son condicionantes, que pueden originar una situación de riesgo para su salud. En los últimos años la comunidad científica se ha interesado por los efectos de la música derivados tanto de su uso como de su praxis. Es muy limitada la evidencia disponible sobre la adaptación fisiológica durante los conciertos, momento culminante de la práctica profesional, resultando interesante ahondar en este sentido, para llegar a una excelencia en los cuidados. OBJETIVOS Conocer determinados parámetros relacionados con el estado nutricional y la actividad física, con el fin de establecer recomendaciones específicas que puedan contribuir a mejorar el estado de salud de los profesionales de la música. POBLACIÓN Y METODOLOGÍA Estudio descriptivo transversal, realizado entre 2010-2014 y cuya la población diana son músicos profesionales miembros de orquestas y grupos de cámara. Las muestras son de conveniencia. Consta de dos partes: una donde se analizan ciertos parámetros antropométricos, (talla, peso, así como otros relacionados con el riesgo de sufrir alteraciones cardiometabólicas) y de la actividad física; la otra parte relativa a los hábitos alimentarios. Para valorar la actividad física y la carga de trabajo, se utilizan métodos combinados así como la frecuencia cardiaca, medida durante los ensayos generales/conciertos. Por su parte, la tasa de sudoración se obtuvo por la diferencia de peso antes y después de la actuación. Para obtener información sobre los hábitos alimentarios, se utiliza el llamado “recuerdo de 24-horas” aplicado a cada participante..

    Differential expression of circulating miRNAs as a novel tool to assess BAG3-associated familial dilated cardiomyopathy.

    A new familial dilated cardiomyopathy (FDCM) was found related to mutations in BAG3 gene. MicroRNAs (miRNAs) represent new targets of FDCM, although no studies have assessed clinical association between Bcl2-associated athanogene 3 (BAG3)-related DCM and miRNAs. Here, we studied whether a clinical association between BAG3-related FDCM and circulating miRNAs may have diagnostic and prognostic value in a small cohort of familial related individuals carrying a BAG3 mutation (BAG3+) and/or diagnosed of dilated cardiomyopathy (DCM) (DCM+). The analysis of 1759 circulating miRNAs showed significant differences between BAG3+ and BAG3- individuals for miRNAs mir-3191-3p, 6769b-3p, 1249-ep, 154-5p, 6855-5p, and 182-5p, while comparisons between BAG3+/DCM+ versus BAG3+/DCM- were restricted to miRNAs mir-154-5p, 6885-5p, and 182-5p, showing significant correlation with systolic and diastolic blood pressure, A wave, left atrium length, and left atrium area. Additionally, when stratified by gender and age, miRNAs were statistically correlated with critical parameters, including left ventricle ejection fraction (LVEF) and ventricular diameter, in women and young men. Likewise, 56% of BAG3+/DCM+, significantly co-expressed mir-154-5p and mir-182-5p, and a slight 4% did not express such combination, suggesting that co-expression of mir-154-5p and mir-182-5p may potentially show diagnostic value. Evaluar el efecto de una formación online en la mejora de conocimientos y habilidades de alumnos de enfermería de cuarto curso, en comparación con la realizada de forma convencional. 2. Estimar la variación de conocimientos, pre-post intervención, de los alumnos de cuarto de enfermería sobre las diferentes técnicas de vendaje.3.Comprobar si la formación online permite la adquisición de habilidades (skills), en comparación con la formación convencional.Metodología: Estudio cuasiexperimental con 3 grupos paralelos (experimental, control habitual, control seminario habitual). La población diana son los alumnos de 4º grado en Enfermería, que cursan sus estudios en la Facultad de Enfermería, Fisioterapia y Podología (FEFP) de la UCM. Se utiliza una plataforma on-line como medio formativo (Elsevier Clinical Skills. Resultados: La formación llevada a cabo se posiciona como un método eficaz para la adquisición de conocimientos. Este tipo de metodologías no presenciales, suponen un importante ahorro económico. Además, esta metodología virtual consigue mejorar los conocimientos en la materia estudiada y adquirir habilidades de la misma de una forma más cómoda, barata, accesible y flexible

    Abdominal adiposity increases lordosis and doubles the risk of low back pain

    Vertebral disorders have significant health and economic impacts, and due to aging and current lifestyle habits, there is a trend toward their increase. Obesity and the alignment of vertebral curvatures can be associated with back pain. Objective: This study aims to analyze whether general and abdominal obesity are associated with cervical, dorsal, and lumbar vertebral pain as well as increased or decreased values of cervical, dorsal, and lumbar vertebral curvatures. Methodology: Body composition, degree of vertebral curvature, and the perception of cervical, dorsal, and lumbar pain were evaluated in a study population of 301 people (>18 years old). Linear and logistic regression analyses were performed to evaluate the influence of several variables of body composition on vertebral angles and cervical, dorsal, and lumbar pain. Results: Lumbar pain was the most prevalent (66.1%), mainly affecting women (70.9%). They were also shown to have greater lumbar angles (p < 0.001). The degrees of lumbar curvature increased, as did the BMI, waist circumference, and waist-to-height ratio. Cervical and dorsal curvatures were increased by all the variables of adiposity and abdominal adiposity. It was found that people with abdominal obesity carried twice the risk of lower back pain than those without abdominal obesity (OR = 2.172, p < 0.05). In addition, an increased lumbar angle was related to an increased risk of low back pain (OR = 1.031, p < 0.05). Cervical pain, on the other hand, was associated with the waist-height index (OR = 0.948, p <0.01). Conclusions: This study shows that increased lumbar curvature and abdominal obesity may be risk factors for lower back pain. In addition, it shows an association between the amount of body and abdominal fat in relation to the degree of curvature of the spine in the sagittal plane.     Assessment of a New Lateral Cushioned Casting Orthosis: Effects on Peroneus Longus Muscle Electromyographic Activity During Running

    Background: Classical medial wedge (CMW) orthoses have been prescribed to treat overpronation foot pathologies in runners. The effects of a novel supination orthosis (NSO) on the surface electromyography (EMG) activity of the peroneus longus (PL) muscle during a complete cycle of running have yet to be tested. Purpose/Hypothesis: The purpose of this study was to compare the EMG activity of the PL in participants wearing CMW orthoses and NSOs versus neutral running shoes (NRS) during a full cycle of running gait. It was hypothesized that the PL muscle activity would be lower for the NSO compared with CMW or NRS. Study Design: Controlled laboratory study. Methods: Included were 31 healthy recreational runners of both sexes (14 male and 17 female; mean age, 38.58 ± 4.02 years) with a neutral Foot Posture Index and standard rearfoot-strike pattern. Participants ran on a treadmill at 9 km/h while wearing NSO (3-, 6-, and 9-mm thicknesses), CMW (3-, 6-, and 9-mm thicknesses), and NRS, for a total of 7 different conditions randomly selected, while the EMG signal activity of the PL was recorded for 30 seconds. Each trial was recorded 3 times, and the intraclass correlation coefficient (ICC) to test reliability of the measurements was calculated. The Wilcoxon pair to pair nonparametric test with Bonferroni correction was performed to analyze differences among the conditions. Results: The reliability of all assessments was almost perfect (ICC, >0.81). For both the CMW and NSO, regardless of thickness, the PL activity was statistically significantly lower compared with the NRS (P < .05 for all). For all CMW thicknesses, the PL activity was lower compared with the respective NSO thicknesses, with the 3-mm thickness having the largest difference (CMW3mm, 18.63 ± 4.64 vs NSO3mm, 20.78 ± 4.99 mV; P < .001). Conclusion: Both CMW and NSO produced reduced EMG activity of the PL muscle; therefore, they can be prescribed to treat overpronation pathologies without associated PL strain concerns. In addition, the NSO saved the enhancement material placed on the medial-rear side of CMW, making it easier to wear sports shoes. Clinical Relevance: Knowing the safety of CMW and NSO will aid in understanding treatments for overpronation pathologies

    The impact of ABCDE bundle implementation on patient outcomes: a nationwide cohort study

    Background: The ABCDE bundle is a set of evidence-based practices to systematically reduce the risks of sedation, delirium, and immobility in intensive care patients. Implementing the bundle improves clinical outcome. Aims and Objectives: To investigate the association between patient outcomes and compliance with bundle components ABC (analgosedation algorithms), D (delirium protocol), and E (early mobilization protocol). Design: A Spanish multicentre cohort study of adult patients receiving invasive mechanical ventilation (IMV) for ≥48 h until extubation. Methods: The primary outcome was pain level, cooperation to permit Medical Research Council Scale administration, patient days of delirium, and mobility. The secondary outcome was cumulative drug dosing by IMV days. Tertiary outcomes (ICU days, IMV days, bed rest days, ICU mortality, ICUAW) and independent variables (analgosedation, delirium, early mobilization protocols) were also studied. Results: Data were collected from 605 patients in 80 ICUs and 5214 patient days with IMV. Two-thirds of the ICUs studied applied no protocols. Pain was not assessed on 83.6% of patient days. Patient cooperation made scale administration feasible on 20.7% of days. Delirium and immobility were found on 4.2% and 69.9% of days, respectively. Patients had shorter stays in ICUs with bundle protocols and fewer days of IMV in ICUs with delirium and mobilization bundle components (P = 0.006 and P = 0.03, respectively). Analgosedation protocols were associated with more opioid dosing (P = 0.02), and delirium and early mobilization protocols with more propofol (P = 0.001), dexmedetomidine (P = 0.001), and lower benzodiazepine dosing (P = 0.008). Conclusions: The implementation rate of ABCDE bundle components was very low in our Spanish setting, but when implemented, patients had a shorter ICU stay, more analgesia dosing, and lighter sedation. Relevance to clinical practice: Applying some but not all the bundle components, there is increased analgesia and light sedation drug use, decreased benzodiazepines, and increased patient cooperation and mobility, resulting in a shorter ICU stay and fewer days of IMV

    Análisis de la influencia de la pandemia por el virus SARS-CoV-2 sobre la tasa de bacteriemia en catéteres venosos tunelizados en una unidad de hemodiálisis

    Antecedentes: La infección relacionada con catéter venoso central (CVC) está asociada a una elevada morbimortalidad y se ha relacionado con la bacteriemia asociada a CVC, disfunciones de catéter, la manipulación y las medidas higiénicas tomadas durante la misma. Objetivo: El objetivo del presente estudio fue comparar la tasa de bacteriemia entre los años 2019 y 2020, primer año de pandemia por el virus SARS-CoV-2, en una unidad de hemodiálisis de un hospital de la Comunidad Autónoma de Madrid. Material y Método: Se ha llevado a cabo un estudio observacional retrospectivo transversal en el que se compararon datos relativos a la manipulación y al funcionamiento del catéter, y tasa de bacteriemias relacionadas con el catéter entre los años 2019 y 2020 a través de la base de datos de la unidad. Resultados: En 2019 se incluyeron 35 pacientes, que presentaron una tasa de bacteriemia de 1,42/1000 días de catéter y 29 pacientes en 2020, con una tasa de bacteriemia de 1,82/1000 días de catéter, no encontrándose diferencias significativas (p=0,54), no encontrando un riesgo de bacteriemia incrementado o disminuido durante el primer año de pandemia producida por el virus SARS-CoV-2. Conclusiones: Como principal conclusión de este estudio, podemos afirmar que no se ha evidenciado que la llegada de la pandemia producida por la enfermedad del coronavirus y las medidas adoptadas frente al virus SARS-CoV-2, tanto organizacionales como de prevención, sean un factor de riesgo o protección respecto a la tasa de bacteriemia, probablemente debido al bajo número de eventos encontrados
