    Reporting of ethical approval and informed consent in clinical research published in leading nursing journals : a retrospective observational study

    Background: Ethical considerations play a prominent role in the protection of human subjects in clinical research. To date the disclosure of ethical protection in clinical research published in the international nursing journals has not been explored. Our research objective was to investigate the reporting of ethical approval and informed consent in clinical research published in leading international nursing journals. Methods: This is a retrospective observational study. All clinical research published in the five leading international nursing journals from the SCI Journal Citation Reports between 2015 and 2017 were retrieved to evaluate for evidence of ethical review. Results: A total of 2041 citations have been identified from the contents of all the five leading nursing journals that were published between 2015 and 2017. Out of these, 1284 clinical studies have been included and text relating to ethical review has been extracted. From these, most of prospective clinical studies (87.5%) discussed informed consent. Only half of those (52.9%) reported that written informed consent had been obtained; few (3.6%) reported oral consent, and few (6.8%) used other methods such as online consent or completion and return of data collection (such as surveys) to denote assent. Notably, 36.2% of those did not describe the method used to obtain informed consent and merely described that “consent was obtained from participants or participants agreed to join in the research”. Furthermore, whilst most of clinical studies (93.7%) mentioned ethical approval; 92.5% of those stated the name of ethical committee and interestingly, only 37.1% of those mentioned the ethical approval reference. The rates of reporting ethical approval were different between different study type, country, and whether financial support was received (all P<0.05). Conclusion: The reporting of ethics in leading international nursing journals demonstrates progress, but improvement of the transparency and the standard of ethical reporting in nursing clinical research is required

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Refined Analysis of Fatigue Crack Initiation Life of Beam-to-Column Welded Connections of Steel Frame under Strong Earthquake

    This paper presents a refined analysis for evaluating low-cycle fatigue crack initiation life of welded beam-to-column connections of steel frame structures under strong earthquake excitation. To consider different length scales between typical beam and column components as well as a few crucial beam-to-column welded connections, a multiscale finite element (FE) model having three different length scales is formulated. The model can accurately analyze the inelastic seismic response of a steel frame and then obtain in detail elastoplastic stress and strain field near the welded zone of the connections. It is found that the welded zone is subjected to multiaxial nonproportional loading during strong ground motion and the elastoplastic stress-strain field of the welded zone is three-dimensional. Then, using the correlation of the Fatemi-Socie (FS) parameter versus fatigue life obtained by the experimental crack initiation fatigue data of the structural steel weldment subjected to multiaxial loading, the refined evaluation approach of fatigue crack initiation life is developed based on the equivalent plastic strain at fatigue critical position of beam end seams of crucial welded connections when the steel frame is subjected to the strong earthquake excitation

    Effect of Welding Residual Stress on Along-Wind Fatigue Life of Welded Joints in Guyed Mast

    On the premise of only considering along-wind effect on guyed mast, the influence of welding residual stress on the fatigue life of welded joints is evaluated in this paper. Since the sum of residual stress and along-wind-induced dynamic stress exceeds the yield strength of structural steel, the effect of residual stress relaxation is included in the numerical analysis. The multiscale finite element analysis of guyed mast is developed in order to capture accurately stress field distribution of welded joint for the “welding-wind-induced” case in which both residual stress and along-wind-induced stress are taken into consideration, and the stress response characteristics at fatigue critical point of welded joints are pointed out. It is found that the “welding-wind-induced” stress field of welded joint can be approximately considered as multiaxial proportional loading state and hence the stress-based von Mises criterion can be adopted to evaluate the fatigue life of welded joints. Based on the S-N curve of stress fatigue life for welded specimens with structural steel commonly used in guyed masts, the fatigue damage of key welded joints is predicted, and as a consequence, the influence of welding residual stress on the fatigue life of welded joints is discussed
