Rapid Identification of Enterococcus Faecalis Isolates by Polymerase Chain Reaction
BACKGROUND AND OBJECTIVE: Rapid detection of Enterococcus faecalis as a frequent cause of nosocomial infections in immunocompromised patients is an important issue. Herein, the current study developed a PCR assay based on the ef0737 gene to detect E. faecalis isolates.
METHODS: In this cross-sectional study, 150 clinical isolates including E. faecalis, E. faecium and Staphylococcus aureus were collected. A set of pair primers was designed using the sequence of ef0737. All isolates were examined for the presence of ef0737 gene by PCR assay. The sensitivity of PCR assay was evaluated according to 50 clinical isolates of E. faecalis. The specificity of PCR primers was also determined using non-E. faecalis species including 50 E. faecium and 50 S. aureus isolates.
FINDINGS: In this study, from 150 clinical isolates that were collected; all the 50 E. faecalis isolates showed positive results for the ef0737 gene which showed 100% sensitivity. No amplification were observed in other isolates include E. faecium and S. aureus.
CONCLUSION: PCR assay is a more efficient and sensitive tool for detection and characterization of E. faecalis especially in patients with the critical condition. Identification of the preserved ef0737 gene in clinical samples may be able to determine infections caused by E. faecalis
The experience of thalassemic adults with their treatment
This study, based on medical anthropology and oral reports, analyzes the meanings attributed by thalassemic adults to their experiences with the treatment. Interviews were used to collect data, which were analyzed through inductive thematic analysis. Eleven young adults, six of whom were men, at different ages, with different educational levels and occupations participated in the study. The meanings are discussed through the theme "the lives of patients with thalassemia in relation to their treatment". This core meaning highlights the difference made in their identity by having the disease trait, the recognition of the importance of adhering to their treatment, the difficulties in maintaining their social functions, the patients' irregular treatment adherence and their justifications for non-adherence to their treatment. Thalassemic patients conform to their condition and employ a normalization strategy to control the disease and justify irregular treatment adherence.El objetivo de este estudio fue analizar los sentidos dados por los talasémicos adultos a su experiencia con el tratamiento, con base en la antropología médica y en el método del relato oral. Para la recolección de datos usamos entrevistas y seguimos el análisis temático inductivo. Participaron once adultos jóvenes, seis del sexo masculino; con edades, niveles educacionales y profesiones, distintas. Los sentidos da la experiencia son presentados por el tema "La vida del portador de talasemia con el tratamiento". El tema destaca la identidad de ser diferente por: el trazo de la enfermedad, el reconocimiento de la importancia de la adhesión al tratamiento, las dificultades en mantener sus funciones sociales y los episodios de irregularidad en la terapia y sus justificaciones. Aprendimos que los portadores están resignados con su condición y emplean la estrategia de normalización para el control de la enfermedad y para justificar la irregularidad en el tratamiento.O objetivo neste estudo foi analisar os sentidos dados pelos talassêmicos adultos à sua experiência em relação ao tratamento, com base na antropologia médica e no método do relato oral. Para a coleta de dados, usaram-se entrevistas e seguiu-se a análise temática indutiva. Participaram onze adultos jovens, seis do sexo masculino; com idades, níveis educacionais e profissões distintas. Os sentidos são apresentados pelo tema "a vida do portador de talassemia com o tratamento". O tema destacou a identidade de diferença pelo traço da doença, o reconhecimento da importância da adesão ao tratamento, as dificuldades em manterem suas funções sociais, os episódios de irregularidade na terapia e suas justificativas. Apreendeu-se que os portadores estão resignados com sua condição, empregam a estratégia de normalização para o controle da doença e justificaram a irregularidade no tratamento
Health related quality of life in Malaysian children with thalassaemia
BACKGROUND: Health Related Quality of Life (HRQoL) studies on children with chronic illness such as thalassaemia are limited. We conducted the first study to investigate if children with thalassaemia have a lower quality of life in the four dimensions as measured using the PedsQL 4.0 generic Scale Score: physical, emotional, social and role (school) functioning compared to the healthy controls allowing for age, gender, ethnicity and household income. METHODS: The PedsQL 4.0 was administered to children receiving blood transfusions and treatments at Hospital Kuala Lumpur, Malaysia using PedsQL 4.0 generic Scale Score. Accordingly, the questionnaire was also administered to a control group of healthy school children. Socio-demographic data were also collected from patients and controls using an interview schedule designed for the study. RESULTS: Of the 96 thalassaemia patients approached, 78 gave consent to be interviewed giving a response rate of 81.3%. Out of 235 healthy controls approached, all agreed to participate giving a response rate of 100%. The mean age for the patients and schoolchildren is 11.9 and 13.2 years respectively. The age range for the patients and the schoolchildren is between 5 to 18 years and 7 to 18 years respectively. After controlling for age and demographic background, the thalassaemia patients reported having significantly lower quality of life than the healthy controls. CONCLUSION: Thalassaemia has a negative impact on perceived physical, emotional, social and school functioning in thalassaemia patients which was also found to be worse than the children's healthy counterparts. Continuing support of free desferal from the Ministry of Health should be given to these patients. More understanding and support especially from health authorities, school authorities and the society is essential to enhance their quality of life
Bone mineral density in Iranian adolescents and young adults with β-thalassemia major
The authors investigated the prevalence of low bone mass in patients from Tehran, Iran, with β-thalassemia major (n = 203), aged 10-20 years, and the potential risk factors for osteoporosis in this patient population. Prevalence of osteoporosis was 50.7 in lumbar spine, 10.8 in femur, and 7.9 in both regions with no significant difference between the two genders. The following factors were associated with low BMD: height for age and weight for age below 3rd percentile, delayed puberty or hypogonadism, age when Desferal (for iron chelation) was started, duration of Desferal therapy, and serum zinc. Low serum copper and 25(OH)D were not associated with low BMD. Copyright © Informa Healthcare USA, Inc
The impact of iron overload and its treatment on quality of life: results from a literature review
BACKGROUND: To assess the literature for the impact of iron overload and infusion Iron Chelation Therapy (ICT) on patients' quality of life (QoL), and the availability of QoL instruments for patients undergoing infusion ICT. Also, to obtain patients' experiences of having iron overload and receiving infusion ICT, and experts' clinical opinions about the impact of treatment on patients' lives. METHODS: A search of studies published between 1966 and 2004 was conducted using Medline and the Health Economic Evaluation Database (HEED). Qualitative results from patient and expert interviews were analysed. Hand searching of relevant conference abstracts completed the search. RESULTS: Few studies measuring the impact of ICT with deferoxamine (DFO) on patients QoL were located (n = 15). QoL domains affected included: depression; fatigue; dyspnoea; physical functioning; psychological distress; decrease in QoL during hospitalization. One theme in all articles was that oral ICT should improve QoL. No iron overload or ICT-specific QoL instruments were located in the articles. Interviews revealed that the impact of ICT on patients with thalassemia, sickle cell disease, and myelodysplastic syndromes is high. CONCLUSION: A limited number of studies assessed the impact of ICT or iron overload on QoL. All literature suggested a need for easily administered, efficacious and well tolerated oral iron overload treatments, given the impact of current ICT on adherence. Poor adherence to ICT was documented to negatively impact survival. Further research is warranted to continue the qualitative and quantitative study of QoL using validated instruments in patients receiving ICT to further understanding the issues and improve patients QoL
Factors affecting health-related quality of life in Thai children with thalassemia
<p>Abstract</p> <p>Background</p> <p>Knowledge of the factors associated with health-related quality of life (HRQOL) among patients with thalassemia is essential in developing more suitable clinical, counseling, and social support programs to improve treatment outcomes of these patients. In light of the limited research in this area, this study aims to examine factors associated with HRQOL among children and adolescents with thalassemia in Thailand.</p> <p>Methods</p> <p>A cross-sectional survey was conducted in three selected hospitals in Thailand during June to November 2006. PedsQL™ 4.0 Generic Core Scale (Thai version) was used to assess HRQOL in 315 thalassemia patients between 5 and 18 years of age. Other related clinical characteristics of the patients were collected via medical record review.</p> <p>Results</p> <p>The mean (SD) of the total summary score was 76.67 (11.40), while the means (SD) for the Physical Health Summary score and Psychosocial Health Summary score were 78.24 (14.77) and 75.54 (12.76), respectively. The school functioning subscale scored the lowest, with a mean of 67.89 (SD = 15.92). The following factors significantly affected the HRQOL of the patients: age; age at onset of anemia and age at first transfusion; pre-transfusion hemoglobin (Hb) level; receiving a blood transfusion during the previous three months; and disease severity. In addition, iron chelation therapy had a significant negative effect on HRQOL in the school functioning subscale. In contrast, serum ferritin level, frequency of blood transfusions per year, and gender were not significantly related to HRQOL among these patients. The results from multivariate analysis also confirmed these findings.</p> <p>Conclusions</p> <p>To improve HRQOL of thalassemia patients, suitable programs aimed at providing psychosocial support and a link between the patient, school officials, the family and the physician are important, especially in terms of improving the school functioning score. The findings also confirmed the importance of maintaining a pre-transfusion Hb level of at least 9-10.5 g/dL. In addition, special care and attention should be given to patients with a severe condition, and those who are receiving subcutaneous iron chelation therapy.</p
