Relapses In multiple sclerosis.

Exact causes of contradictions in the understanding of relapsing course as characteristic phenomenon of multiple sclerosis (MS) and primarily relapsing-remitting type are still considered to be “sub rosa”. We tried to determine connection and correlation between seasonal dynamic factors, chronometric parameters of disease progression (age, disease duration) and frequency of relapses and the role of gender differences as well. A four-year prospective study included complete registration of all cases of MS exacerbations in resident patients with a relapsing-remmiting course of the disease according to McDonald criteria in Volyn region of Ukraine in 2010. Each patient had neurological score based on EDSS scale to ensure compliance with inclusion criteria. 128 patients (44 males and 84 females) with a remitting-recurring course of the disease were involved in this study. Statistical analysis was performed using SPSS 18, MedCalc and Microsoft Excel software using standard descriptive statistics, correlation and regression analysis. In total, 197 cases of MS relapses were registered. Episodes of relapses prevailed in winter-spring period. Multiple regression model including suggested climate factors showed multiple correlation coefficient – 0.55 (p<0.01). The prevalence of MS was higher in females, while general incidence of recurrences was similar to males. However, relapse rate in different age groups differs: the rate of recurrence increases in women aged after 50 years (mean annualized relapse rate (ARR)=0.66), while it slowly decreases in men with aging. The duration of the disease did not have significant effect on the course of MS in women, whereas weak negative relationship (0.24, p<0.001) between the duration of the disease and the frequency of MS in men was showed. Correspondingly, the average negative correlation (-0.37, p<0.001) was detected between the age of disease onset and the incidence of MS recurrence in males, while this connection was not observed in females. We established that the recurrence of MS is a result of multifactorial influence of external and internal factors. Individual seasonal climate variability did not have sufficient force of influence. Therefore, combined effect of corresponding climate changes played a significant role. Furthermore, it is quite probable that interactions of MS with external factors influencing the human body can be mutually integrated

Globalization processes accelerate the pace of scientific and technological progress and innovation and require investment in innovation to enhance the efficiency of the national economy of our state and some of its branches, inter–industry complexes and sub–complexes at the level of developed countries.The article examines the current state of investment in agriculture of Ukraine. The main criteria to encourage capital investment taking into account regional peculiarities capital investment in agriculture. The necessity of attracting external sources of investment in the future to enhance investment processes in agriculture.Thus, the study of the current situation of investment activity of agricultural enterprises of Ukraine and structural analysis of sources of investment in agriculture, to determine that the trend of improving investment for 2010–2014., Changed deterioration in 2015 and complication of financing conditions from their own field investment. So one of the essential ways to increase the competitiveness of the industry is an effective investment policy aimed at attracting large–scale foreign investment sources, especially through financial investment. Addressing these and other issues will significantly increase investment flows and rational use of funds in the industry.В статье рассмотрено современное состояние инвестиционной деятельности в сельском хозяйстве Украины. Inverse spectral problems for Sturm--Liouville operators with matrix-valued potentials

ОСОБЛИВОСТІ ПОРУШЕННЯ СИСТЕМИ ГЕМОСТАЗУ У ХВОРИХ НА КОРОНАВІРУСНУ ІНФЕКЦІЮ

The aim of the work – to analyze the features of hemostasis disorders in patients with coronavirus infection. Materials and methods. 78 medical records of inpatients hospitalized in the Regional Clinical Infectious Diseases Hospital of Kharkiv for the period from May to September 2020 were analyzed. To assess the state of the hemostasis system and the risk of thrombohemorrhagic complications, the level of platelets, fibrinogen, D-dimer, active partial thromboplastic time (APTT), international normalized ratio (INR) was monitored. Results. During the study, disorders of cellular and plasma indicators of hemostasis were analyzed. Patients were divided into groups, depending on the severity of respiratory failure. Coagulation tests were monitored every 48 hours. Conclusion. In patients with moderate severity, disorders of the hemostasis system in the form of hypercoagulation prevailed. Prolonged course of the disease for more than 21 days or in patients with severe, extremely severe course, hypercoagulation changed to hypocoagulation and DIC syndrome occurred. There is a direct relationship with the state of hemostasis and the severity of coronavirus infection.Мета роботи – проаналізувати особливості порушення системи гемостазу у хворих на коронавірусну інфекцію. Матеріали та методи. Проаналізовано 78 медичних карт стаціонарних хворих, госпіталізованих в обласну клінічну інфекційну лікарню м. Харкова за період з травня по вересень 2020 р. Для оцінки стану системи гемостазу та ризику виникнення тромбогеморагічних ускладнень проводили моніторинг рівня тромбоцитів, фібриногену, D-димеру, активного часткового тромбопластинового часу (АЧТЧ), міжнародного нормалізованого відношення (МНВ). Результати дослідження. Під час дослідження були проаналізовані порушення клітинних і плазматичних показників гемостазу. Хворі були поділені на групи, залежно від ступеня тяжкості дихальної недостатності. Моніторинг коагуляційних тестів проводили кожні 48 год. Висновок. У пацієнтів із середнім ступенем тяжкості переважали порушення системи гемостазу у вигляді гіперкоагуляції. Тривалий перебіг хвороби понад 21 добу або у хворих із тяжким чи вкрай тяжким ступенем гіперкоагуляція змінювалася на гіпокоагуляцію та виникав ДВЗ-синдром. Показники гемостазу прямо корелювали зі ступенем тяжкості коронавірусної інфекції

Inverse spectral problems for Sturm-Liouville operators with singular potentials

The inverse spectral problem is solved for the class of Sturm-Liouville operators with singular real-valued potentials from the space $W^{-1}_2(0,1)$. The potential is recovered via the eigenvalues and the corresponding norming constants. The reconstruction algorithm is presented and its stability proved. Also, the set of all possible spectral data is explicitly described and the isospectral sets are characterized.Comment: Submitted to Inverse Problem

Structure of the icosahedral Ti-Zr-Ni quasicrystal

The atomic structure of the icosahedral Ti-Zr-Ni quasicrystal is determined by invoking similarities to periodic crystalline phases, diffraction data and the results from ab initio calculations. The structure is modeled by decorations of the canonical cell tiling geometry. The initial decoration model is based on the structure of the Frank-Kasper phase W-TiZrNi, the 1/1 approximant structure of the quasicrystal. The decoration model is optimized using a new method of structural analysis combining a least-squares refinement of diffraction data with results from ab initio calculations. The resulting structural model of icosahedral Ti-Zr-Ni is interpreted as a simple decoration rule and structural details are discussed.Comment: 12 pages, 8 figure

При ТЛ/ТМ з ВІЛ внутрішньовенне лікування І та Е було більш ефективним, ніж пероральне лікування І та Е, через 2 місяці інтенсивного лікування в конверсії мокротиння, а також у клінічному поліпшенні, що супроводжується значно вищими середніми концентраціями у сироватці. Крім того, рівень смертності був нижчим при внутрішньовенному лікуванні І та Е порівняно з пероральним лікуванням.The aim of this study was to investigate the effectiveness of intravenous isoniazid (H) and ethambutol (E) administered in patients with new sputum positive drug-susceptible pulmonary tuberculosis (TB) with tuberculous meningoencephalitis (TM) and human immunodeficiency virus (HIV) co-infection in the intensive phase of treatment. In TB/TM with HIV, the intravenous H and E treatment was more effective than oral H and E treatment at 2 months of intensive treatment in sputum conversion as well as in clinical improvement, accompanied by significantly higher mean serum concentrations. In addition, the mortality rate was lower in intravenous H and E treatment compared to oral treatment.Целью данного исследования было изучение эффективности внутривенного введения изониазида (И) и этамбутола (Э) у пациентов с лекарственно-чувствительным туберкулезом легких (ТЛ) с положительным результатом мокроты, туберкулезным менингоэнцефалитом (ТМ) и вирусом иммунодефицита человека (ВИЧ) в интенсивной фазе лечения. При ТЛ/ТМ с ВИЧ внутривенное лечение И и Э было более эффективным, чем пероральное лечение И и Э, через 2 месяца интенсивного лечения в конверсии мокроты, а также в клиническом улучшении, сопровождаемом значительно более высокими средними концентрациями в сыворотке. Кроме того, уровень смертности был ниже при внутривенном лечении И и Э по сравнению с пероральным лечением

Radiative Decay Modes of the D0D^{0} Meson

Using data recorded by the CLEO-II detector at CESR we have searched for four radiative decay modes of the $D^0$ meson: $D^0\to\phi\gamma$, $D^0\to\omega\gamma$, $D^0\to\bar{K}^{*}\gamma$, and $D^0\to\rho^0\gamma$. We obtain 90% CL upper limits on the branching ratios of these modes of $1.9\times 10^{-4}$, $2.4\times 10^{-4}$, $7.6\times 10^{-4}$ and $2.4\times 10^{-4}$ respectively.Comment: 15 page postscript file, postscript file also available through http://w4.lns.cornell.edu/public/CLN
