    Get PDF
Objectives: Individuals with chronic obstructive pulmonary disease (COPD) are usually > 50 years of age and have a low body mass index (BMI). An imbalance between matrix metalloproteinases (MMPs), including MMP-3, and tissue inhibitor of metalloproteinase 1 (TIMP-1), play a role in tissue degradation of lung extracellular matrix among COPD individuals. The purpose of the present study was to correlate age and/or BMI with salivary levels of MMP-3 and TIMP-1 among Indonesian subjects with COPD. Results: Thirty COPD patients were recruited to undergo thorough physical assessment and saliva collection for evaluating TIMP-1 and MMP-3 levels using commercially available kits enzyme-linked immunosorbent assay method. The mean (standard deviation) participant age and BMI were 60.5 (8.13) years, and 23.1 (4.75) kg/m , respectively. Furthermore, the mean (standard deviation) of TIMP-1 and MMP3 levels were 23.99 (6.85) ng/mL and 1.81 (1.167) Ī¼M, respectively. Age was negatively correlated with MMP-3 (P < 0.05), but not with TIMP-1 levels. Age and BMI were not correlated with TIMP-1 level (P > 0.05). Collectively, this study demonstrated that age has a negative correlation with the protease marker (i.e. MMP-3), but not the anti-protease marker (TIMP-1). BMI was not correlated with either protease/anti-protease marker among Indonesian subjects with COPD

    Get PDF
This research aims to analyze the relationship between cross-organizational support and the effectiveness of empowerment programs in the Bulukumba, Bone, and Makassar regions. Using a qualitative method, the research was conducted from August to December 2022, with a focus on three pilot villages in South Sulawesi. With 15 key informants selected through purposive sampling, data were collected through in-depth interviews and observations. Data analysis involved stages of data reduction, data display, and conclusion drawing or verification. The results of this research reveal the importance of integrating physical, human, and social capital in community empowerment. It was found that the implementation of the program faced challenges in integrating these three capitals, often due to limitations in human resources and coordination among institutions. The Indonesia Juara program, which aims to develop an integrated empowerment model, encountered difficulties in applying these principles in the field. Furthermore, this study highlights the importance of the Pentahelix concept, which involves government, academia, business, communities, and media, in community empowerment. However, it was found that Pentahelix collaboration is still not optimal in practice, especially in the context of the Indonesia Juara program. Collaborative leadership is needed to enhance the effectiveness of this program by effectively integrating various stakeholders in empowerment efforts. It is recommended to promote stronger cooperation between Ministries/Institutions and Regional Device Organizations (OPD) to ensure program effectiveness and efficiency. Collaborative leadership is required to effectively integrate various stakeholders

Atrazine herbicide is known to disrupt the endocrine system and is potentially carcinogenic. Its continual application leads to high residue levels in soil, causing water pollution, which when consumed is associated with devastating health effects. This research reported the isolation and characterization of a new bacterial strains from active agricultural soil with the potential to biodegrade atrazine as a sole carbon source. An enrichment method was utilized to isolate the bacteria (A1, A2, B1, B2, C1 and C2) on mineral salt media (MSM) following serial dilution. Six isolates were screened for their tolerance to various concentrations of atrazine (500 to 1500 mgāˆ™Lā€1), and only isolate B1 tolerated up to 1500 mgāˆ™Lā€1 atrazine. The isolate was identified molecularly as Bacillus safensis strain BUK_BCH_BTE6 based on 16S rRNA gene sequencing and molecular phylogenetic analysis. Characterization of the isolate based on the effects of temperature, pH, substrate concentration, incubation time, inoculum size, and heavy metals revealed optimum growth and atrazine degradation at 35 Ā°C, a pH of 7.5, 400 mgāˆ™Lā€1, at 48 h, and inoculum size of 600 ĀµL, respectively. The growth of the isolate was inhibited by 2 ppm Hg, Cd, Cr, Pb, Ar, and Ni, while Fe, Cu, and Zn stimulated it. GCā€MS analysis revealed a degradation efficiency of 88.85% within 120 h, while metabolites such as desethyldeisopropylatrazine, deisopropylatrazine, Nā€ethylammelide, and cyanuric acid were also detected. This isolate is a highly atrazineā€tolerant and efficient atrazine degrader that could be employed for bioremediation of atrazineā€polluted sites

Ruptured ectopic pregnancy presenting as an irreducible inguinal hernia

    Get PDF
An inguinal hernia that suddenly becomes irreducible may be secondary to a variety of other underlying conditions which can occasionally mislead the attending surgeon. Benign, inflammatory or neoplastic processes, as well as surgical emergencies such as intraperitoneal or retroperitoneal haemorrhage, have all been previously reported to mimic an inguinal hernia that suddenly becomes irreducible with or without clinical features of strangulation. We add an additional interesting presentation to this list in the form of a ruptured ectopic pregnancy, which is the first such case reported in the literature. A swelling in the groin may be much more complicated than it seems on superficial consideration and good clinical acumen is constantly required in managing such cases if a satisfactory outcome without any morbidity is to be expected

Aim: to analyze the occurrence of chronic obstructive pulmonary disease (COPD) and its association with the polymorphisms of -308G/A, -238G/A of TNFĪ± gene and +252A/G LTĪ± gene polymorphism in smokers. Methods: cross-sectional study, comparing the genetic group of people who have COPD and who do not have COPD with the same smoking history. The study was conducted from January 2011 to March 2012 at several health centers; such as Adam Malik Hospital, Pirngadi Hospital, Tembakau Deli Hospital, Siti Hajar Hospital in Medan and several health centers in the city of Medan. Examination of lung function was done using spirometry and the existence of genetic polymorhisms in the TNFĪ± and LTĪ± gene was performed using polymerase chain reaction-restriction fragment length polymorphisms (PCR-RFLP). Results: of the total of 227 samples that met inclusion criteria and after equalizing the age and history of smoking, 186 subjects were enrolled, of which, 93people as COPD group and 93 people as non-COPD group. Analysis of the association between the -308G/A polymorphism with the COPD revealed the odds ratio (OR) 0.436 (95% CI 0.224-0.850, p=0.014). Polymorphism at -238G/A of the TNFĪ± gene showed the OR 2.094 (95% CI 0.608 - 7.211, p=0.241). Polymorphism at +252A/G LTĪ± gene showed OR 1.256 (95% CI 0.694 - 2.272; p=0.450). The findings indicated that polymorphism at -308G showed a protective factor whereas the -238G/A of the TNFĪ± and +252A/G of the LTĪ± genes did not show any significant association with the COPD. Conclusion: polymorphism -308 TNFĪ± gene shown to be a protective factor for the occurrence of COPD. Polymorphism -238 TNFĪ± gene and +252 LTĪ± gene did not show any significant association with COPD.Key words: chronic obstructive pulmonary disease, TNFĪ± gene, LTĪ± gene, polymorphism

Asthma is a chronic inflammation of the respiratory tract characterized by coughing, shortness of breath, wheezing and limited expiratory airflow. Interleukin 10 (IL-10) is a cytokine produced by T regulators which functions to suppress pro-inflammatory production and development. Interleukin 5 (IL-5) is glycoprotein produced by Th2 which acts as a proinflammatory agent. This study aimed to analyze how to balance between IL-10 and IL-5 levels on persistent atopic asthma. The study design was an experimental research method with invitro study used isolation and cell culture of Peripheral Blood Mononuclear Cell (PBMC) with negative control Roswell Park Memorial Institute (RPMI) medium, positive control Phytohaemaglutinin (PHA) and house dust mite allergen stimulation 10 Āµl. This study involved 39 samples asthmatic. There was a significant increase in IL-5 levels after stimulation of house dust mite allergens with p: 0.000; there was a significant decrease in IL-10 levels after house dust mite allergen stimulation with p: 0.000. Our study suggests that IL-5 and IL-10 balance is related to persistent atopic asthma after house dust mite allergen Der p stimulation.
