    Increasing the Capacity of Primary Care Through Enabling Technology.

    Primary care is the foundation of effective and high-quality health care. The role of primary care clinicians has expanded to encompass coordination of care across multiple providers and management of more patients with complex conditions. Enabling technology has the potential to expand the capacity for primary care clinicians to provide integrated, accessible care that channels expertise to the patient and brings specialty consultations into the primary care clinic. Furthermore, technology offers opportunities to engage patients in advancing their health through improved communication and enhanced self-management of chronic conditions. This paper describes enabling technologies in four domains (the body, the home, the community, and the primary care clinic) that can support the critical role primary care clinicians play in the health care system. It also identifies challenges to incorporating these technologies into primary care clinics, care processes, and workflow

Background: Several studies have provided evidence to support the benefits of exercise in offering protection from depression and as an intervention in the treatment of mental illness. Exercise in mental illness is not only beneficial to the mental health of the patients, studies have also shown that patients with a mental illness are at a significantly higher risk of physical health concerns and complications and exercise can help decrease many of those complications. Objective: The purpose of this literature review is to synthesize the research on exercise in severe mental illness by answering the following research questions: What are the health outcomes of integrating an exercise program with patients diagnosed with severe mental illness? What are the essential components or barriers of an effective exercise program for patients diagnosed with a severe mental illness? Methods: The databases utilized were PubMed, the Cumulative Index of Nursing and Allied Health (CINAHL) Complete, PsycINFO, and ProQuest Nursing & Allied Health Source. Many terms were used in a combination of searches, including: Exercise, Benefits, Severe Mental Illness, and Physical Activity. An extensive search of several combinations of these key terms was performed, and sufficient literature was found. Results: After examining the literature, the following themes were found to be relevant: positive benefits mentally and physically, a therapeutic relationship between the patient and staff plays a major role in the positive outcomes, and different types of exercise used to improve mental and physical health in SMI. Conclusion: The findings suggest that exercise used along with pharmacotherapy has many mental, emotional, and physical benefits. Further research is needed to explore the relationship between exercise in mental illness to clarify its effects on mental outcomes and the correlation between exercise and physical improvements

    Evaluating Small Airways Disease in Asthma and COPD using the Forced Oscillation Technique and Magnetic Resonance Imaging

    Obstructive lung disease, including asthma and chronic obstructive pulmonary disease (COPD), is characterized by heterogeneous ventilation. Unfortunately, the underlying structure-function relationships and the relationships between measurements of heterogeneity and patient quality-of-life in obstructive lung disease are not well understood. Hyperpolarized noble gas MRI is used to visualize and quantify ventilation distribution and the forced oscillation technique (FOT) applies a multi-frequency pressure oscillation at the mouth to measure respiratory impedance to airflow (including resistance and reactance). My objective was to use FOT, ventilation MRI and computational airway tree modeling to better understand ventilation heterogeneity in asthma and COPD. FOT-measured respiratory system impedance was correlated with MRI ventilation heterogeneity and both were related to quality-of-life in asthma and COPD. FOT-measurements and model-predictions of reactance and small-airways resistance were correlated in asthma and COPD respectively. This study is the first to demonstrate the relationships between FOT-measured impedance, MRI ventilation heterogeneity, and patient quality-of-life

    Divalent Metal Ion Coordination by Residue T118 of Anthrax Toxin Receptor 2 Is Not Essential for Protective Antigen Binding

    The protective antigen (PA) subunit of anthrax toxin interacts with the integrin-like I domains of either of two cellular receptors, ANTXR1 or ANTXR2. These I domains contain a metal ion-dependent adhesion site (MIDAS) made up of five non-consecutive amino acid residues that coordinate a divalent metal ion that is important for PA-binding. The MIDAS residues of integrin I domains shift depending upon whether the domain exists in a closed (ligand-unbound) or open (ligand-bound) conformation. Of relevance to this study, the MIDAS threonine residue coordinates the metal ion only in the open I domain conformation. Previously it was shown that the MIDAS threonine is essential for PA interaction with ANTXR1, a result consistent with the requirement that the I domain of that receptor adopts an open conformation for PA-binding [1]. Here we have tested the requirement for the MIDAS threonine of ANTXR2 for PA-binding. We show that the toxin can bind to a mutant receptor lacking the MIDAS threonine and that it can use that mutant receptor to intoxicate cultured cells. These findings suggest that an open-like configuration of the ANTXR2 MIDAS is not essential for the interaction with PA

    Administrators' perspectives on ethical issues in long-term care research

    ETHICAL ISSUES ARE A SIGNIFICANT potential barrier to much-needed research in long-term care settings. LTC stakeholder perspectives are largely absent from the development of regulation and guidelines. Fifteen long-term care administrators were interviewed as part of a study of ethical issues in community-based, long-term care research. Established qualitative procedures for conducting content analysis were used to organize the data. Findings suggest that existing mechanisms to protect human subjects do not take into account important differences between academic and long-term care settings. The full potential of LTC research will not be realized until supportive processes to enhance human subjects protections are developed in a way that is reflective of the LTC environment

    Metal-Poor Stars Observed with the Magellan Telescope. III. New Extremely and Ultra Metal-Poor Stars from SDSS/SEGUE and Insights on the Formation of Ultra Metal-Poor Stars

    We report the discovery of one extremely metal-poor (EMP; [Fe/H]<-3) and one ultra metal-poor (UMP; [Fe/H]<-4) star selected from the SDSS/SEGUE survey. These stars were identified as EMP candidates based on their medium-resolution (R~2,000) spectra, and were followed-up with high-resolution (R~35,000) spectroscopy with the Magellan-Clay Telescope. Their derived chemical abundances exhibit good agreement with those of stars with similar metallicities. We also provide new insights on the formation of the UMP stars, based on comparison with a new set of theoretical models of supernovae nucleosynthesis. The models were matched with 20 UMP stars found in the literature, together with one of the program stars (SDSS J1204+1201), with [Fe/H]=-4.34. From fitting their abundances, we find that the supernovae progenitors, for stars where carbon and nitrogen are measured, had masses ranging from 20.5 M_sun to 28 M_sun and explosion energies from 0.3 to 0.9x10^51 erg. These results are highly sensitive to the carbon and nitrogen abundance determinations, which is one of the main drivers for future high-resolution follow-up of UMP candidates. In addition, we are able to reproduce the different CNO abundance patterns found in UMP stars with a single progenitor type, by varying its mass and explosion energy.Comment: 15 pages, 12 figures; accepted for publication in Ap

    Stimulus Threat and Exposure Context Modulate the Effect of Mere Exposure on Approach Behaviors

    Mere-exposure (ME) research has found that initially neutral objects made familiar are preferred relative to novel objects. Recent work extends these preference judgments into the behavioral domain by illustrating that mere exposure prompts approachoriented behavior toward familiar stimuli. However, no investigations have examined the effect of mere exposure on approach-oriented behavior toward threatening stimuli. The current work examines this issue and also explores how exposure context interacts with stimulus threat to influence behavioral tendencies. In two experiments participants were presented with both mere-exposed and novel stimuli and approach speed was assessed. In the first experiment, when stimulus threat was presented in a homogeneous format (i.e., participants viewed exclusively neutral or threatening stimuli), ME potentiated approach behaviors for both neutral and threatening stimuli. However, in the second experiment, in which stimulus threat was presented in a heterogeneous fashion (i.e., participants viewed both neutral and threatening stimuli), mere exposure facilitated approach only for initially neutral stimuli. These results suggest that ME effects on approach behaviors are highly context sensitive and depend on both stimulus valence and exposure context. Further implications of these findings for the ME literature are discussed

    High-Density Lipoprotein Metabolism in Human Apolipoprotein B\u3csub\u3e100\u3c/sub\u3e Transgenic/Brown Adipose Tissue Deficient Mice: A Model of Obesity-Induced Hyperinsulinemia

    Obese and diabetic humans display decreased plasma high-density lipoprotein cholesterol (HDL-C) concentrations and an increased risk for coronary heart disease. However, investigation on HDL metabolism in obesity with a particular emphasis on hepatic ATP-binding cassette transporter A1 (ABCA1), the primary factor for HDL formation, has not been well studied. Human apolipoprotein B100 transgenic (hApoBtg) and brown adipose tissue deficient (BATless) mice were crossed to generate hApoBtg/BATless mice. Male and female hApoBtg and hApoBtg/BATless mice were maintained on either a regular rodent chow diet or a diet high in fat and cholesterol until 24 weeks of age. The hApoBtg/BATless mice that were fed a HF/HC diet became obese, developed hepatic steatosis, and had significantly elevated plasma insulin levels compared with their hApoBtg counterparts, but plasma concentrations of total cholesterol, HDL-C, triglycerides, and free fatty acids and lipoprotein distribution between genotypes were not significantly different. Hepatic expression of genes encoding HDL-modifying factors (e.g., scavenger receptor, class B, type I, hepatic lipase, lecithin:cholesterol acyltransferase, and phospholipid transfer protein) was either altered significantly or showed a trend of difference between 2 genotypes of mice. Importantly, hepatic protein levels of ABCA1 were significantly lowered by ∼35% in male obese hApoBtg/BATless mice with no difference in mRNA levels compared with hApoBtg counterparts. Despite reduced hepatic ABCA1 protein levels, plasma HDL-C concentrations were not altered in male obese hApoBtg/BATless mice. The result suggests that hepatic ABCA1 may not be a primary contributing factor for perturbations in HDL metabolism in obesity-induced hyperinsulinemia
