    Horse shoe retinal tear, vitreous hemorrage, macular pucker 15 months after laser in Situ keratomileusis: case report

    A 44 year old woman presented with decreased vision and floaters, started since 4 days before, in her right eye referred to our ophthalmology clinic. On examination, it was found that the best corrected visual acuity was limited to R-E: 20/80 and that of the left eye: 20/20. Fundus examination of the right eye revealed a rhegmatogenous subclinical retinal detachment and vitreous hemorrhage 15 months after surgery for myopia, but the left fundus was normal. Barrier diod laser photocoagulation around the tear was done. Subclinical retinal detachment and vitreous hemorrhage resolved one month after barrier laser photocoagulation, but visual acuity of patient reduced to 3/200 due to development of macular pucker

    Intravitreal Bevacizumab Injection Effect in Exudative Age Related Macular Degeneration

    Exudative age-related macular degeneration is the most common cause of vision loss of people over the age 60 in the developed countries. The cause is not clear yet, but since Avastin is largely used for the treatment, therefore, this study was conducted to assess the influence of avastin. Methods: All the 76 patients with exudative age-related macular degeneration came under intravitreal bevacizumab injection by an ophthalmologist with 1/25 mg in 0/05 ml. The second and third injections were repeated one month after the first injection. Patients were followed up one week, one month,and then in the third and sixth months after treatment. All patients had eye examination including visual acuity, slit lamp assessment (Slit-Lamp), intra ocular pressure and fundus examination.Results: Visual acuity was 0/906±0/080 in patients before the study, a month after the injection 0/818 ± 0/097, and 3 months and 6 months after injection were 0/677± 0/126 and 0.488±0.164, respectively. The average difference in measured visual acuity was significant in all cases before injections, one month after injection, 3 months and 6 months after injection. (p&lt;0.001). The only complication observed during this study was mild temporary anterior uveitis in two patients.(2/631%)Conclusion: Intravitreal injection of bevacizumab showed improvement in vision during this study within 6 months. The cost effectiveness of bevacizumab and no systemic side effect except for only two patients with mild temporary uveitis were the main finding.</p

    The classical clinical triad of retinitis pigmentosa is arteriolar attenuation , retinal bone-spicule pigmentation and waxy disc pallor.   A 33 year old female patient is introduced here. She had unilateral posterior subcapsular cataract, heterochromic iris, and uveitis.The patient also suffered night blindness , had a family history of low vision ,and reduced visual acuity in her right eye. After the best correction, visual acuity was: OD=20.200 and OS=20.20 (with-1.5 spher-0.50cyl x 170). Anterior segment examination revealed trace cell with diffused moderate keratic precipitate and heterochoromia with posterior subcapsular cataract .But the examination revealed a normal condition in the left eye. Both eyes had characteristic retinal changes of retinitis pigmentosa. Retinitis pigmentosa can be associated with Fuch’s- like uveitis sporadicallybut Fuch’s heterochromic uveitis of affected pedigree was not found

    Background and Aim: Myopia is one of the most prevalent complaints of patients referring to ophthalmologists. Regarding the progressive nature of this complication, it was decided to assess the patients referring to the eye clinic of Valieasr hospital in Birjand (in 2012). Materials and Methods: In this cross- sectional survey,with simple non-probable sampling, a total of 403 myopic patients who had referred to the eye clinic of Valieasr hospital in Birjand in 2012 were celected. Data collection tool included a researcher-designed checklist has been compeleted by the optometrist. Increase of 0.5 diopter was accounted as the progression of myopia. After the questionaires were filled out, the obtained data was fed into SPSS software (V:16) and finally analyzed by Chi-square. Results: The present study revealed progression of myopia in 47. 4% of cases in right eye and 47.6% in left &nbsp;eye. progression of myopia had also a significant relationship with variables of age and hours of study in a day in both of the eyes and with variables of sex and number of hours wearing medical glasses in one eye. Conclusion: It was found that progression of myopia had a significant relationship with variables sex, age, hours of study and hours of wearing medical glasses. Thus, controlling these variables can have both a preventive and curing role for the patients and those prone to the disease

    The comparison of total capacity antioxidant in the serum of people with pterygium and control subjects

    AIM: To investigate the comparison of total antioxidant capacity in the serum of patients with pterygium and control subjects.METHODS: This case-control study was conducted on all persons referred to Ophthalmology Clinic of teaching Hospital of Vali-Asr(peace upon to him)with clinical symptoms of pterygium during the year 2016. The control group was selected among patients referred to the Ophthalmology Clinic of Vali-Asr(peace without pterygium)that the two groups were matched in terms of age, gender and place of residence. Sixty-six persons \〖31 people(47%)in patient group and 35 people(53%)in the control group\〗 were enrolled by convenience sampling. Venous blood sample was taken from all patients after the sampling using ferric reducing antioxidant power(FRAP); FRAP- as a quick 10min measurement, the antioxidant power measurement of samples according to the conversion of ferric iron(Fe3+)to ferrous iron(Fe2+)was checked. The collected data ware entered to software SPSS 21 and were analyzed by chi-square and Mann-Whitney tests at the level of α =0.05.RESULTS: The mean of antioxidant capacity in patients was 842.55±161.46 μmol/L and antioxidant capacity in healthy controls was 856.77±209.41 μmol/L(P=0.8). In the comparison of mean serum antioxidant capacity in healthy individuals and in the serum of people with pterygium based on gender the results showed that the antioxidant capacity mean in male control subjects has been 894.05±176.82 μmol/L and in females control 780.01±118.33 μmol/L that the observed difference have been reported statistically significant(P=0.008)but the other comparison according the gender between cases and control does not show any significant difference.CONCLUSION: The results of this study showed that the full level of serum antioxidant capacity in patients has been less than the mean of antioxidant capacity in control subjects, however the observed difference has not been significant. The results of this study were consistent with basic results carried out on the damaging effects of oxidative stress in the pterygium pathogenesis. Recommending diet with minerals and vitamins containing antioxidants may be preventing the onset and progression of pterygium

    Correlation between cystatin-C, acute phase reactants, and retinopathy severity in diabetic patients

    AIM: To evaluate correlation of cystatin-C(Cys-C)with severity of diabetic retinopathy(DR)and acute phase reactants, including erythrocyte sedimentation rate(ESR)and high-sensitivity C-reactive protein(hs-CRP).METHODS: All diabetic patients who were referred for diabetic retinopathy(DR)screening during 1mo were enrolled. Demographic data were recorded. All patients have undergone full ophthalmic exam. At the same day, all patients were tested for hemoglobin A1c(HbA1c), ESR, hs-CRP, and Cys-C serum levels.RESULTS: Sixty seven diabetics were enrolled, including 19(28.3%)without retinopathy, 22(32.8%)non-proliferative retinopathy, and 26(38.8%)proliferative retinopathy patients. The mean age, sex distribution, mean duration of diabetes, prevalence of hypertension and dyslipidemia, smoking status and HbA1c levels were not significantly different among the three groups. The mean levels of Cys-C increase significantly as retinopathy progress \〖1.1±0.48; 1.22±0.38; 1.71±0.92(P=0.007), respectively\〗. In multiple regression analysis, just Cys-C was significantly associated with severity of DR(P=0.025).CONCLUSIONS: This study revealed that serum levels of Cys-C increase while DR progress independently of acute phase reactants. Therefore, it could be used as an associated marker by primary care physicians to distinguish patients at higher risk of severe DR. Larger randomized studies are warranted to confirm findings. Reviewing physiological role of the Cys-C, we proposed that the Cys-C may be a protective response to catalytic stress rather than being a pathogenic factor in microangiopathies
