    Origin of thyroid arteries in a Kenyan population

    Introduction: The thyroid gland receives blood supply predominantly from paired superior and inferior thyroid arteries. The superior thyroid artery originates from external carotid while the inferior thyroid artery is a branch of thyrocervical trunk. Unusual origins of superior thyroid artery include common carotid and cervical part of internal carotid arteries while those for the inferior thyroid artery are subclavian, common carotid or the vertebral arteries. These origins vary between populations. Knowledge of variant anatomy of these arteries is important in surgical procedures within the neck. This study describes variant origin of the thyroid arteries in a Kenyan population.Materials and Methods: Fifty formalin fixed cadavers from the Department of Human Anatomy, University of Nairobi were available for the study. Skin incisions were made from the chin to the supra-sternal notch, along the clavicle to the acromion and then to the mastoid process. Skin flaps and platysma muscle were refl ected for exposure of the anterior triangle of the neck. The sternocleidomastoid was cut close to its origins on the clavicle and reflected superiorly. The strap muscles were transected and reflected to expose the thyroid gland lying within the visceral fascia. The dissection field was cleaned by blunt and sharp dissection to expose the gland. The two thyroid arteries were identified from either pole of the gland and traced to their respective origins.Data Analysis: Data was coded, tabulated and analysed using SPSS 16.0 for windows® (SPSS Inc., Chicago, Illinois) for percentages and frequencies of the observed variations in origin and nerve relations. Results were presented in tables and macrographs.Results: Superior thyroid artery originated from external carotid and common carotid artery in 74.4% and 25.6 % of cases respectively. The right side displayed a higher frequency of origin from the common carotid artery. The inferior thyroid artery arose from thyrocervical trunk in 87.5% and from subclavian artery in 12.5% of cases. There was a higher incidence of the inferior thyroid artery originating from the subclavian on the left than the right side.Conclusion: Over 25% of superior thyroid arteries and 11 % of inferior thyroid arteries show variant origins. The common carotid and subclavian arteries were the most frequent variant sites of origin for superior and inferior thyroid arteries respectively. Bilateral asymmetry was observed. Neck surgeons should be aware of these variations during ligation and selective embolisation of thyroid arteries to avoid complications during surgery

    Survivin a radiogenetic promoter for glioblastoma viral gene therapy independently from CArG motifs

    BACKGROUND: Radiogenetic therapy is a novel approach in the treatment of cancer, which employs genetic modification to alter the sensitivity of tumor cells to the effect of applied radiation. AIM: To select a potent radiation inducible promoter in the context of brain tumors and to investigate if CArG radio responsive motifs or other elements in the promoter nucleotide sequences can correlate to its response to radiation. METHODS: To select initial candidates for promoter inducible elements, the levels of mRNA expression of six different promoters were assessed using Quantitative RTPCR in D54 MG cells before and after radiation exposure. Recombinant Ad/reporter genes driven by five different promoters; CMV, VEGF, FLT-1, DR5 and survivin were constructed. Glioma cell lines were infected with different multiplicity of infection of the (promoter) Ad or CMV Ad. Cells were then exposed to a range of radiation (0–12 Gy) at single fraction. Fluorescent microscopy, Luc assay and X-gal staining was used to detect the level of expression of related genes. Different glioma cell lines and normal astrocytes were infected with Ad survivin and exposed to radiation. The promoters were analyzed for presence of CArG radio-responsive motifs and CCAAT box consensus using NCBI blast bioinformatics software. RESULTS: Radiotherapy increases the expression of gene expression by 1.25–2.5 fold in different promoters other than survivin after 2 h of radiation. RNA analysis was done and has shown an increase in copy number of tenfold for survivin. Most importantly cells treated with RT and Ad Luc driven by survivin promoter showed a fivefold increase in expression after 2 Gy of radiation in comparison to non-irradiated cells. Presence or absence of CArG motifs did not correlate with promoter response to radiation. Survivin with the best response to radiation had the lowest number of CCAAT box. CONCLUSION: Survivin is a selective potent radiation inducible promoter for glioblastoma viral gene therapy and this response to radiation could be independent of CArG motifs

    A systematic review of knowledge, attitudes and beliefs about malaria among the South Asian population

    Background Malaria is one of the deadliest mosquito-borne diseases in the world. More than 80% of the total populations are at risk of malaria in the 22 countries in Asia and the Pacific. South Asia alone is home to an estimated 1.4 billion people at risk of contracting malaria. Despite the remarkable progress in reducing the burden of malaria, evidence of the disease based on knowledge of the social and cultural contexts from a South Asian perspective is limited. Our objective was to understand the knowledge, attitudes and beliefs about malaria in South Asian communities. Methodology We conducted a systematic literature review, searching six databases, between 1990 and 2015, focusing on knowledge, attitudes and beliefs about malaria in South Asia. Databases were searched using both ‘free terms’ and ‘index terms’ funnelled using Boolean operators and truncations. Inclusion and exclusion criteria were set, and included papers were scrutinised, employing a critical appraisal tool to find the best available evidences to support the study purpose. Results and discussion Evidence from 32 articles (26 quantitative, four qualitative and two mixed methods). General knowledge and awareness of the disease, its transmission, and control and preventative measures were generally found to be lacking amongst both the general public and healthcare professionals. In addition, the study shows that poor socio-economic factors – including limited access to services due to poor/limited availability – and issues of affordability are considered as major risk factors. Conclusion This review suggests the importance of increasing health awareness, mobilising the local or community healthcare professionals, for prevention as well as early detection and effective treatment of malaria among people who are at risk. Malaria is also a disease associated with poverty and socio-cultural factors; therefore, strong political will, wider partnerships between health and non-health sectors, and strengthening health systems’ technical and managerial capabilities at all level of primary healthcare systems, is inevitable

    A Period of Controlled Elevation of IOP (CEI) Produces the Specific Gene Expression Responses and Focal Injury Pattern of Experimental Rat Glaucoma

    PURPOSE: We determine if several hours of controlled elevation of IOP (CEI) will produce the optic nerve head (ONH) gene expression changes and optic nerve (ON) damage pattern associated with early experimental glaucoma in rats. METHODS: The anterior chambers of anesthetized rats were cannulated and connected to a reservoir to elevate IOP. Physiologic parameters were monitored. Following CEI at various recovery times, ON cross-sections were graded for axonal injury. Anterior ONHs were collected at 0 hours to 10 days following CEI and RNA extracted for quantitative PCR measurement of selected messages. The functional impact of CEI was assessed by electroretinography (ERG). RESULTS: During CEI, mean arterial pressure (99 ± 6 mm Hg) and other physiologic parameters remained stable. An 8-hour CEI at 60 mm Hg produced significant focal axonal degeneration 10 days after exposure, with superior lesions in 83% of ON. Message analysis in CEI ONH demonstrated expression responses previously identified in minimally injured ONH following chronic IOP elevation, as well as their sequential patterns. Anesthesia with cannulation at 20 mm Hg did not alter these message levels. Electroretinographic A- and B-waves, following a significant reduction at 2 days after CEI, were fully recovered at 2 weeks, while peak scotopic threshold response (pSTR) remained mildly but significantly depressed. CONCLUSIONS: A single CEI reproduces ONH message changes and patterns of ON injury previously observed with chronic IOP elevation. Controlled elevation of IOP can allow detailed determination of ONH cellular and functional responses to an injurious IOP insult and provide a platform for developing future therapeutic interventions

    A pair of degenerate polymerase chain reaction (PCR) primers (LEI-1, TCG GAT CC[C,T] [G,C]TG GGT AGG GGC GT; LEI-2, ACG GAT CC[G,C] [G,C][A,C]C TAT [A,T]TT ACA CC) defining a 0.15-kb segment ofLeishmania minicircle DNA was constructed. These primers amplified not only inter- but also intraspecifically polymorphic sequences. Individual sequences revealed a higher intraspecific than interspecific divergence. It is concluded that individual sequences are of limited relevance for species determination. In contrast, when a data base of 19 different sequences was analyzed in a dendrographic plot, an accurate species differentiation was feasible

    Solving the Salinity Control Problem in a Potable Water System

    Abstract. Salinity is the relative concentration of salts in water. In a city of southern China, the local water supply company pumps water from a nearby river for potable use. During the winter dry season, the intrusion of sea water raises the salinity of the river to a high level and affects approximately the daily life of 450,000 residents of the city. This paper reports the application of constraint programming (CP) to optimize the logistical operations of the raw water system so as to satisfy the daily water consumption requirement of the city and to keep the potable salinity below a desirable level for as many days as possible. CP is the key to the success of the project for its separation of concerns and powerful constraint language that allows for rapid construction of a functional prototype and production system. Flexibility and adaptiveness allow us to deal with our clients' many changes in the requirements. Deriving good variable and value ordering heuristics, and generating useful implied constraints, we demonstrate that branch-and-bound search with constraint propagation can cope with an optimization problem of large size and great difficulty

    Performance-Aware Speculative Resource Oversubscription for Large-Scale Clusters

    It is a long-standing challenge to achieve a high degree of resource utilization in cluster scheduling. Resource oversubscription has become a common practice in improving resource utilization and cost reduction. However, current centralized approaches to oversubscription suffer from the issue with resource mismatch and fail to take into account other performance requirements, e.g., tail latency. In this article we present ROSE, a new resource management platform capable of conducting performance-aware resource oversubscription. ROSE allows latency-sensitive long-running applications (LRAs) to co-exist with computation-intensive batch jobs. Instead of waiting for resource allocation to be confirmed by the centralized scheduler, job managers in ROSE can independently request to launch speculative tasks within specific machines according to their suitability for oversubscription. Node agents of those machines can however, avoid any excessive resource oversubscription by means of a mechanism for admission control using multi-resource threshold control and performance-aware resource throttle. Experiments show that in case of mixed co-location of batch jobs and latency-sensitive LRAs, the CPU utilization and the disk utilization can reach 56.34 and 43.49 percent, respectively, but the 95th percentile of read latency in YCSB workloads only increases by 5.4 percent against the case of executing the LRAs alone
