    Mid-adolescent predictors of adult drinking levels in early adulthood and gender differences: longitudinal analyses based on the South Australian school leavers study

    There is considerable public health interest in understanding what factors during adolescence predict longer-term drinking patterns in adulthood. The aim of this study was to examine gender differences in the age 15 social and psychological predictors of less healthy drinking patterns in early adulthood. The study investigates the relative importance of internalising problems, other risky health behaviours, and peer relationships after controlling for family background characteristics. A sample of 812 young people who provided complete alcohol consumption data from the age of 15 to 20 years (5 measurement points) were drawn from South Australian secondary schools and given a detailed survey concerning their psychological and social wellbeing. Respondents were classified into two groups based upon a percentile division: those who drank at levels consistently below NHMRC guidelines and those who consistently drank at higher levels. The results showed that poorer age 15 scores on measures of psychological wellbeing including scores on the GHQ-12, self-esteem, and life-satisfaction as well as engagement in health-related behaviours such as smoking or drug-taking were associated with higher drinking levels in early adulthood. The pattern of results was generally similar for both genders. Higher drinking levels were most strongly associated with smoking and marijuana use and poorer psychological wellbeing during adolescence.Paul H.     Reliability and Validity of Self-Reported Questionnaires Related to Adolescent Violence and Consequences, Thailand

Research in this area has been hampered by a lack of psychometrically sound tools that have been validated to assess problem behaviours in Asian youth. The purpose of this paper is to provide validity and reliability data on an instrument to measure violence in Thai youth. In this study, reliability and validity data for a sample of adolescent Thai youth are reported for the Communities That Care Youth Survey (CTC-YS), a measure of risk and protective factors for violent behaviour, and the STAXI-II, a measure of angry experience and expression. The findings showed overall high internal consistency for both questionnaires, and there was evidence of construct validity. It is concluded that these measures are appropriate for use in research that seeks to investigate youth violence among adolescents in Thailand

    Perceptions of unfairness in the management of bullying complaints: exploring the consequences

    This exploratory study aimed to examine why some bullied workers submitted compensable injury claims for psychological injury after they had made a workplace bullying grievance, and others did not. This study was carried out using a mixed methodology. Forty-four participants who had complained about bullying at work completed a survey about their experiences, and 31 were interviewed. A thematic analysis of the interview data was undertaken. Those participants who submitted workers’ compensation claims were found to be significantly more depressed than those who did not submit workers’ compensation claims, although no significant differences were found between the anxiety and stress scores of all participants. Results also indicated that participants who submitted a workers’ compensation claim perceived less organisational justice in the way their complaint of bullying was managed than those participants who did not submit a claim. These results were endorsed by the qualitative aspects of the study where themes of frustration and unfairness were closely linked with the decision to submit a workers’ compensation claim. This is one of the few studies that have examined the effect of an organisation’s response to workplace bullying allegations on an employee’s decision to claim workers’ compensation for psychological injury.Moira F Jenkins, Helen Winefield, Aspa Sarri

    Psychological well-being and psychological distress: is it necessary to measure both?

    Extent: 14p.Background: The objectives of the study were to explore a self-report measure for psychological well-being and to investigate the relationship between psychological well-being and psychological distress. Method: Telephone interviews of a representative sample of adults (N = 1933) collected information about sociodemographic variables, a standardised measure of psychological distress, and three brief existing scales to assess aspects of psychological well-being: Positive Relations with Others, Environmental Mastery, and Satisfaction with Life. The total of these three scales was also computed and explored as a measure of overall well-being. Results: Variables positively associated with psychological well-being were negatively associated with psychological distress and vice versa. For example low psychological well-being and high psychological distress were associated with being the only adult in the household, speaking a language other than English at home, being divorced or separated, having no educational qualifications beyond secondary school, being unable to work, having a low income, renting one’s accommodation, and receiving a pension. Conclusions: The measure of well-being shows psychometric promise for community surveys. Psychological well-being is not exactly the opposite end of the continuum to psychological distress, but more debate is needed about whether and when, research participants need to be asked questions about both.Helen R Winefield, Tiffany K Gill, Anne W Taylor and Rhiannon M Pilkingto

    Inequalities of quality of life in unemployed young adults: A population-based questionnaire study

    BACKGROUND: It is well known that unemployment is a great problem both to the exposed individual and to the whole society. Unemployment is reported as more common among young people compared to the general level of unemployment. Inequity in health status and life-satisfaction is related to unemployment. The purpose of this population-based study was to describe QOL among unemployed young people compared to those who are not unemployed, and to analyse variables related to QOL for the respective groups. METHODS: The sample consisted of 264 young unemployed individuals and 528 working or studying individuals as a reference group. They all received a questionnaire about civil status, educational level, immigration, employment status, self-reported health, self-esteem, social support, social network, spare time, dwelling, economy and personal characteristics. The response rate was 72%. The significance of differences between proportions was tested by Fisher's exact test or by χ(2 )test. Multivariate analysis was carried out by means of a logistic regression model. RESULTS: Our results balance the predominant picture of youth unemployment as a principally negative experience. Although the unemployed reported lower levels of QOL than the reference group, a majority of unemployed young adults reported good QOL, and 24% even experienced higher QOL after being unemployed. Positive QOL related not only to good health, but also to high self-esteem, satisfaction with spare time and broad latitude for decision-making. CONCLUSION: Even if QOL is good among a majority of unemployed young adults, inequalities in QOL were demonstrated. To create more equity in health, individuals who report reduced subjective health, especially anxiety need extra attention and support. Efforts should aim at empowering unemployed young adults by identifying their concerns and resources, and by creating individual programmes in relation not only to education and work, but also to personal development

    Does transition from an unstable labour market position to permanent employment protect mental health? Results from a 14-year follow-up of school-leavers

    <p>Abstract</p> <p>Background</p> <p>Having secure employment, in contrast to being unemployed, is regarded as an important determinant of health. Research and theories about the negative health consequences of unemployment indicated that transition from unemployment to a paid job could lead to improved health. The objective of this study was to test the hypothesis that obtaining permanent employment after being in an unstable labour market position protects mental health.</p> <p>Methods</p> <p>A 14-year follow-up of all graduates from compulsory school in an industrial town in northern Sweden was performed at ages 16, 18, 21 and 30 years. Complete data on the cohort were collected for 1044 individuals with the aid of a comprehensive questionnaire. The response rate was 96.4%. The health measurement used in this study was the psychological symptoms analysed by multivariate logistic regression. Those who obtained permanent employment were the focus of the analysis. This group consisted of people who were in an unstable labour market position for a year or more between the ages of 25 and 29, and who had acquired a permanent job one year before and at the time of the investigation.</p> <p>Results</p> <p>After controlling for gender as well as for an indicator of health-related selection, possible confounders and mediators, an association was found between the lower probability of psychological symptoms and obtaining permanent employment (OR = 0.35, 95% CI 0.19–0.63) as well as having permanent employment (OR = 0.22, 95% CI 0.10–0.51).</p> <p>Conclusion</p> <p>Our findings suggest that transition from an unstable labour market position to permanent employment could be health-promoting, even after controlling for possible confounders and mediators, as well as for an indicator of health-related selection. However, as there are few studies in the field, there is a need for more longitudinal studies in order to further analyse the relationship and to examine possible explanations. The policy implication of our study is that the transformation of unstable labour market positions into permanent employment could contribute to better public health.</p

    Do longer consultations improve the management of psychological problems in general practice? A systematic literature review

    <p>Abstract</p> <p>Background</p> <p>Psychological problems present a huge burden of illness in our community and GPs are the main providers of care. There is evidence that longer consultations in general practice are associated with improved quality of care; but this needs to be balanced against the fact that doctor time is a limited resource and longer consultations may lead to reduced access to health care.</p> <p>The aim of this research was to conduct a systematic literature review to determine whether management of psychological problems in general practice is associated with an increased consultation length and to explore whether longer consultations are associated with better health outcomes for patients with psychological problems.</p> <p>Methods</p> <p>A search was conducted on Medline (Ovid) databases up to7 June 2006. The following search terms, were used:</p> <p>general practice or primary health care (free text) or family practice (MeSH)</p> <p>AND consultation length or duration (free text) or time factors (MeSH)</p> <p>AND depression or psychological problems or depressed (free text).</p> <p>A similar search was done in Web of Science, Pubmed, Google Scholar, and Cochrane Library and no other papers were found.</p> <p>Studies were included if they contained data comparing consultation length and management or detection of psychological problems in a general practice or primary health care setting. The studies were read and categories developed to enable systematic data extraction and synthesis.</p> <p>Results</p> <p>29 papers met the inclusion criteria. Consultations with a recorded diagnosis of a psychological problem were reported to be longer than those with no recorded psychological diagnosis. It is not clear if this is related to the extra time or the consultation style. GPs reported that time pressure is a major barrier to treating depression. There was some evidence that increased consultation length is associated with more accurate diagnosis of psychological problems.</p> <p>Conclusion</p> <p>Further research is needed to elucidate the factors in longer consultations that are associated with greater detection of psychological problems, and to determine the association between the detection of psychological problems and the attitude, gender, age or training of the GP and the age, gender and socioeconomic status of the patient. These are important considerations if general practice is to deal more effectively with people with psychological problems.</p

    Duration of unemployment and depression: a cross-sectional survey in Lithuania

    BACKGROUND: In spite of a growing economy, unemployment is still a severe socio-economic problem in Lithuania. Nonetheless, no studies have been performed about the associations between unemployment and mental health in Lithuania. The aim of this study was to evaluate the associations between unemployment duration and depression in Lithuania. METHODS: The data was collected in a cross-sectional study in 2005. There were 429 filled-in questionnaires received (53.6% response rate) from unemployed persons registered with the Kaunas Labour Market Office. The severity of depression symptoms was evaluated using the Beck Depression Inventory (BDI). Logistic regression was used to estimate the risk factors for occurrence of depression. Sex, age, place of residence, marital status, education, income and practiced religion were the independent variables. Long-term unemployment was defined as lasting a duration of 12 months or more. RESULTS: The findings showed that long-term unemployed persons had more episodes of a depressive mood in the past 12 months in comparison with the group of the short-term unemployed. In addition, the BDI score mean was higher among the long-term unemployed compared with the short-term unemployed (10.1 ± 8.8 and 14.2 ± 9.5 respectively, p < 0.001). It was estimated that the duration of unemployment and BDI score had a positive correlation (r = 0.1968, p < 0.001). Among the short-term unemployed, the risk of depression increased significantly when the person was female, had an older age and had experienced more episodes of unemployment. Among the long-term unemployed, an older age was the risk factor for development of depression. However, higher education and income were the factors that significantly decreased the risk of developing depression for-short term as well as for long-term unemployed. CONCLUSION: The results indicated that depression is a severe problem in the unemployed population. Depression is more elevated among the long-term unemployed. This leads to arguing for common efforts in providing needed social support and health care to reduce the effects of unemployment on mental health
