    Mapping the morbidity and mortality of Chagas disease in an endemic area in Brazil

    Chagas disease is among the 21 neglected diseases according to the World Health Organization. This study aimed to investigate the morbidity and mortality distribution of Chagas disease for identifying areas with greater prevalences and deaths of the disease in Northeast Brazil. A population-based ecological study was performed from 2016 to 2018 using data on acute Chagas disease patients from the Disease Notification Information System, chronic cases from the Chagas Disease and the referral Heart Failure Outpatient Clinic in Pernambuco, and Chagas disease-related mortality from the Mortality Information System. The unit of analysis were Pernambuco State mesoregions. The indicators were spatialized into thematic maps on the occurrence and mortality of the disease per 100,000 inhabitants. No cases of acute disease were reported in the period analyzed. Data on 801 chronic Chagas disease patients were analyzed. The population showed an average age of 62 years, with female predominance. The most prevalent comorbidity was systemic arterial hypertension and cardiologic involvement without ventricular dysfunction. The average chronic disease occurrence rate was 3.2/ 100,000 people/ year. As for deaths in the mortality system; in total, 350 deaths were recorded, showing male predominance, age ≥ 60 years, and chronic disease with cardiac involvement as the main mortality cause. The annual average mortality proportion was 1.6/100,000 people. The chronic case distribution showed spatial heterogeneity, with the highest rates of chronic disease and deaths observed in two mesoregions, with the main cause of death being heart-related. This highlights the need for more specialized services in areas with higher burden of the disease to avoid delay in the patients’ care

    Comparación del equilibrio postural entre los atletas profesionales en tae kwon do y adultos jóvenes

    This study aimed to compare the postural balance of professional tae kwon do athletes with a non-tae kwon do adult group. Nineteen participants (nine tae kwon do practitioners and ten non-tae kwon do practitioners) were tested. To measure the postural sway, a force platform was used and the equipment recorded the main parameters: area of center of pressure; mean frequency, and velocity of center of pressure for both anteroposterior and mediolateral directions were measured for all participants. Before starting the assessment, the subjects received instructions and performed familiarization with the equipment and protocol. Participants were instructed to carry out three balance tests on a single-leg stance position with eyes opened. Values obtained in the postural assessment of professional athletes with the force platform were lower for all parameters compared to non-practitioners, except the frequency of center of pressure in the mediolateral direction. However, a significant difference (p=0.021) between the groups was found only in the center of pressure velocity parameter in the anteroposterior direction. These results have any implications on sport rehabilitation programs for balance assessments in athletes.Este estudio tuvo como objetivo comparar el equilibrio postural de atletas de tae kwon do profesionales con un grupo de adultos que no practican esto deporte. Diecinueve participantes (nueve practicantes profesionales de tae kwon do y diez no practicantes) se pusieron a prueba. Para evaluar la oscilación postural, se utilizó una plataforma de fuerza y el equipo registró los parámetros principales: el centro de la zona de presión; la frecuencia y la velocidad del centro de la presión tanto para la dirección anteroposterior, y para la medio-lateral. Estas mediciones se registraron para todos los participantes. Antes de iniciar la evaluación, los sujetos recibieron instrucciones y se familiarizaron con el equipo y el protocolo. También fueron instruidos para realizar tres pruebas de equilibrio en un solo pie con los ojos abiertos. Los valores obtenidos en la evaluación postural de los atletas profesionales utilizando la plataforma de fuerza eran más bajos para todos los parámetros en comparación con aquellos de los no profesionales, con la excepción de la frecuencia del centro de presión de la dirección medial-lateral. Sin embargo, se encontró una diferencia significativa (p=0,021) entre los grupos sólo con respecto a la velocidad anteroposterior del centro de presión. Esos resultados tienen implicaciones para los deportes de los programas de rehabilitación para la evaluación del equilibrio postural en los atletas

    Definição e diagnóstico de neuropatia de fibras finas: consenso do Departamento Científico de Neuropatias Periféricas da Academia Brasileira de Neurologia

    The aim of this study was to describe the results of a Brazilian Consensus on Small Fiber Neuropathy (SFN). Fifteen neurologists (members of the Brazilian Academy of Neurology) reviewed a preliminary draft. Eleven panelists got together in the city of Fortaleza to discuss and finish the text for the manuscript submission. Small fiber neuropathy can be defined as a subtype of neuropathy characterized by selective involvement of unmyelinated or thinly myelinated sensory fibers. Its clinical picture includes both negative and positive manifestations: sensory (pain/ dysesthesias/pruritus) or combined sensory and autonomic complaints, associated with an almost entirely normal neurological examination. Standard electromyography is normal. A growing list of medical conditions is associated with SFN. The classification of SFN may also serve as a useful terminology to uncover minor discrepancies in the normal values from different neurophysiology laboratories. Several techniques may disclose sensory and/or autonomic impairment. Further studies are necessary to refine these techniques and develop specific therapies.O objetivo deste estudo é descrever os resultados de um Consenso Brasileiro sobre Neuropatia de Fibras Finas (NFF). Quinze neurologistas (membros da Academia Brasileira de Neurologia) revisaram uma versão preliminar do artigo. Onze panelistas se reuniram na cidade de Fortaleza para discutir e terminar o texto para a submissão do manuscrito. NFF pode ser definida como um subtipo de neuropatia caracterizada pelo envolvimento seletivo de fibras sensitivas amielínicas ou pouco mielinizadas. Seu quadro clínico inclui manifestações negativas e positivas: sensitivas (dor/disestesias/prurido) ou queixas sensitivas e autonômicas combinadas, associadas a exame neurológico quase totalmente normal. A eletromiografia convencional é normal. Uma lista crescente de condições médicas causa NFF. NFF também pode servir como uma terminologia útil para referenciar pequenas discrepâncias nos valores normais de diferentes laboratórios de neurofisiologia. Diferentes técnicas podem evidenciar anormalidades sensitivas e/ou autonômicas.     Novel Zinc-Related Differentially Methylated Regions in Leukocytes of Women With and Without Obesity

    INTRODUCTION: Nutriepigenetic markers are predictive responses associated with changes in “surrounding” environmental conditions of humans, which may influence metabolic diseases. Although rich in calories, Western diets could be linked with the deficiency of micronutrients, resulting in the downstream of epigenetic and metabolic effects and consequently in obesity. Zinc (Zn) is an essential nutrient associated with distinct biological roles in human health. Despite the importance of Zn in metabolic processes, little is known about the relationship between Zn and epigenetic. Thus, the present study aimed to identify the epigenetic variables associated with Zn daily ingestion (ZnDI) and serum Zinc (ZnS) levels in women with and without obesity. MATERIALS AND METHODS: This is a case-control, non-randomized, single-center study conducted with 21 women allocated into two groups: control group (CG), composed of 11 women without obesity, and study group (SG), composed of 10 women with obesity. Anthropometric measurements, ZnDI, and ZnS levels were evaluated. Also, leukocyte DNA was extracted for DNA methylation analysis using 450 k Illumina BeadChips. The epigenetic clock was calculated by Horvath method. The chip analysis methylation pipeline (ChAMP) package selected the differentially methylated regions (DMRs). RESULTS: The SG had lower ZnS levels than the CG. Moreover, in SG, the ZnS levels were negatively associated with the epigenetic age acceleration. The DMR analysis revealed 37 DMRs associated with ZnDI and ZnS levels. The DMR of PM20D1 gene was commonly associated with ZnDI and ZnS levels and was hypomethylated in the SG. CONCLUSION: Our findings provide new information on Zn's modulation of DNA methylation patterns and bring new perspectives for understanding the nutriepigenetic mechanisms in obesity

    Avaliação eletroforética, cromatográfica e molecular da Hb D Los Angeles no Brasil

    A variante de hemoglobina (Hb) D mais comum, Hb D Los Angeles ou D Punjab, é originada de uma transversão GAA->CAA no códon 121 da globina beta; essa mutação resulta na substituição do ácido glutâmico por glutamina na proteína. É a terceira variante de hemoglobina mais freqüente da população brasileira. Como as hemoglobinas D apresentam migração similar à hemoglobina S em pH alcalino, e com a hemoglobina A em pH ácido, são necessários vários testes para o correto diagnóstico. No presente estudo objetivou-se relacionar os diferentes procedimentos laboratoriais de rotina diagnóstica, além da análise molecular, para estabelecer o perfil de Hb D Los Angeles no Brasil. Foram analisados 47 indivíduos da população brasileira com provável Hb D Los Angeles, por vários procedimentos eletroforéticos em diferentes condições de pH, além da cromatografia líquida de alta pressão, e testes moleculares para confirmação da mutação. Foram encontrados quatro tipos de combinações de hemoglobinas: 42 indivíduos portadores de hemoglobina AD Los Angeles, dois indivíduos com doença de Hb S/D Los Angeles, dois indivíduos com Hb D Los Angeles e talassemia beta e um indivíduo com Hb D Los Angeles e Hb Lepore. Os indivíduos heterozigotos para D Los Angeles são assintomáticos, entretanto, em associação com outras variantes e talassemias podem apresentar graus variáveis de manifestações clínicas. Os resultados apresentados enfatizaram a necessidade da associação de várias metodologias para a identificação da Hb D Los Angeles, além de auxiliar na elucidação de combinações raras.<br>The most common Hb D variant, the Hb D-Los Angeles, also know as Hb D-Punjab, originates through a GAA->CAA change at the 121 codon of the beta globin gene; this mutation results in the replacement of glutamic acid for glutamine in the protein. It is the third most common hemoglobin variant in the Brazilian population. This variant has electrophoretic migration in alkaline pHs similar to Hb S and identical migration to hemoglobin A in acidic pHs. Thus, several techniques are necessary for its correct diagnosis. The purpose of this work was to relate the different laboratorial techniques and molecular analyses to determine the profile of Hb D Los Angeles in Brazil. Forty-seven individuals from the Brazilian population with Hb D Los Angeles were studied. Multiple electrophoresis in several experimental conditions were carried out, in addition to high performance liquid chromatography (HPLC) and molecular analysis to confirm this mutation. Four compound heterozygotes were observed: 42 individuals heterozygous Hb AD Los Angeles, two with Hb S/D Los Angeles disease, two individuals with Hb D Los Angeles and beta-thalassemia and one with Hb D Los Angeles and Hb Lepore. The heterozygous hemoglobin D Los Angeles is asymptomatic, even though its association with other variants and thalassemias may present varying degrees of clinical manifestations. The results presented emphasize the significance of the association of different laboratorial techniques for D Los Angeles diagnosis, and help to elucidate rare combinations

    SentiBench - a benchmark comparison of state-of-the-practice sentiment analysis methods

    In the last few years thousands of scientific papers have investigated sentiment analysis, several startups that measure opinions on real data have emerged and a number of innovative products related to this theme have been developed. There are multiple methods for measuring sentiments, including lexical-based and supervised machine learning methods. Despite the vast interest on the theme and wide popularity of some methods, it is unclear which one is better for identifying the polarity (i.e., positive or negative) of a message. Accordingly, there is a strong need to conduct a thorough apple-to-apple comparison of sentiment analysis methods, \textit{as they are used in practice}, across multiple datasets originated from different data sources. Such a comparison is key for understanding the potential limitations, advantages, and disadvantages of popular methods. This article aims at filling this gap by presenting a benchmark comparison of twenty-four popular sentiment analysis methods (which we call the state-of-the-practice methods). Our evaluation is based on a benchmark of eighteen labeled datasets, covering messages posted on social networks, movie and product reviews, as well as opinions and comments in news articles. Our results highlight the extent to which the prediction performance of these methods varies considerably across datasets. Aiming at boosting the development of this research area, we open the methods' codes and datasets used in this article, deploying them in a benchmark system, which provides an open API for accessing and comparing sentence-level sentiment analysis methods
