    Prospective memory functioning among ecstasy/polydrug users: evidence from the Cambridge Prospective Memory Test (CAMPROMPT)

    Rationale: Prospective memory (PM) deficits in recreational drug users have been documented in recent years. However, the assessment of PM has largely been restricted to self-reported measures that fail to capture the distinction between event-based and time-based PM. The aim of the present study is to address this limitation. Objectives: Extending our previous research, we augmented the range laboratory measures of PM by employing the CAMPROMPT test battery to investigate the impact of illicit drug use on prospective remembering in a sample of cannabis only, ecstasy/polydrug and non-users of illicit drugs, separating event and time-based PM performance. We also administered measures of executive function and retrospective memory in order to establish whether ecstasy/polydrug deficits in PM were mediated by group differences in these processes. Results: Ecstasy/polydrug users performed significantly worse on both event and time-based prospective memory tasks in comparison to both cannabis only and non-user groups. Furthermore, it was found that across the whole sample, better retrospective memory and executive functioning was associated with superior PM performance. Nevertheless, this association did not mediate the drug-related effects that were observed. Consistent with our previous study, recreational use of cocaine was linked to PM deficits. Conclusions: PM deficits have again been found among ecstasy/polydrug users, which appear to be unrelated to group differences in executive function and retrospective memory. However, the possibility that these are attributable to cocaine use cannot be excluded

    The Leadership Role of Psychiatrists in the NHS

    This article explores leadership in the context of psychiatry in the NHS. It identifies considerable policy challenges facing psychiatry such as recent attempts to upgrade mental health services and bring them up to par with other services. It suggests it will become increasingly necessary for the profession to respond to such challengers by engaging with leadership at various levels. The article suggests there may be merit in exploring one particular approach to leadership, namely transformational leadership, given its emphasis on dealing with change. Psychiatrists may find the behaviours associated with this approach of particular relevance. However, it suggests that there may need to adopt a particular variant of transformational leadership that acknowledges both the achievement of objectives and showing genuine concern for followers. The latter is important given the on going problem of clinical engagement in the NHS. The article concludes with the developmental implications in adopting this leadership approach

    A Web-Based Flexible Communication System in Radiology

    A web-based system for rapid multidirectional communication has been created in the Radiology department at San Francisco General Hospital. The system allows messaging among radiology attendings, residents, and technologists, as well as other members of the hospital community, such as Emergency Department physicians and nurses. Instead of being tied to a particular workflow, this system provides a flexible communication infrastructure which can be easily adapted for different functions and user roles. The system has so far been configured to successfully support the standard “wet reading” workflow, to support marking and tracking of critical results, as well as multiple educational and quality improvement workflows. In the 19 months of operation, the system has gained over 1,800 users (virtually all providers at our institution), it has been accessed by radiologists over 39,000 times and by non-radiologists over 34,000 times. It has become an integral part of the radiology department operations and non-radiology clinical workflows. Unlike most existing softwares, our system is not a task-specific application, but a multipurpose communication system. It is able to effectively accommodate multiple workflows and user roles through configuration (without additional programming). This flexibility has helped this system to be rapidly and widely adopted within our enterprise. The extended reach of the system enables improved monitoring and documentation of workflows, helping with management decision making, and quality assurance. We report a successful radiology communication system based on the principles of flexibility and inclusiveness of users inside and outside the radiology department

    Two fossil species of Metrosideros (Myrtaceae) from the Oligo-Miocene Golden Fleece locality in Tasmania, Australia

    Premise of the Study: The capsular-fruited genus Metrosideros (Myrtaceae) is one of the most widely distributed flowering plant genera in the Pacific but is extinct in Australia today. The center of geographic origin for the genus and the reason for and timing of its extinction in Australia remain uncertain. We identify fossil Metrosideros fruits from the newly discovered Golden Fleece fossil flora in the Oligo-Miocene of Tasmania, Australia, shedding further light on these problems.Methods: Standard paleopalynological techniques were used to date the fossil-bearing sediments. Scanning electron microscopy and an auto-montage camera system were used to take high-resolution images of fossil and extant fruits taken from herbarium specimens. Fossils are identified using a nearestliving-relative approach.Key Results: The fossil-bearing sediments are palynostratigraphically dated as being Proteacidites tuberculatus Zone Equivalent (ca. 33-16 Ma) in age and provide a confident Oligo Miocene age for the macrofossils. Two new fossil species of Metrosideros are described and are here named Metrosideros dawsonii sp. nov. and Metrosideros wrightii sp. nov.Conclusions: These newly described fossil species of Metrosideros provide a second record of the genus in the Cenozoic of Australia, placing them in the late Early Oligocene to late Early Miocene. It is now apparent not only that Metrosideros was present in Australia, where the genus is now extinct, but that at least several Metrosideros species were present during the Cenozoic. These fossils further strengthen the case for an Australian origin of the genus

    Transfer of learning between unimanual and bimanual rhythmic movement coordination: transfer is a function of the task dynamic.

    Under certain conditions, learning can transfer from a trained task to an untrained version of that same task. However, it is as yet unclear what those certain conditions are or why learning transfers when it does. Coordinated rhythmic movement is a valuable model system for investigating transfer because we have a model of the underlying task dynamic that includes perceptual coupling between the limbs being coordinated. The model predicts that (1) coordinated rhythmic movements, both bimanual and unimanual, are organised with respect to relative motion information for relative phase in the coupling function, (2) unimanual is less stable than bimanual coordination because the coupling is unidirectional rather than bidirectional, and (3) learning a new coordination is primarily about learning to perceive and use the relevant information which, with equal perceptual improvement due to training, yields equal transfer of learning from bimanual to unimanual coordination and vice versa [but, given prediction (2), the resulting performance is also conditioned by the intrinsic stability of each task]. In the present study, two groups were trained to produce 90° either unimanually or bimanually, respectively, and tested in respect to learning (namely improved performance in the trained 90° coordination task and improved visual discrimination of 90°) and transfer of learning (to the other, untrained 90° coordination task). Both groups improved in the task condition in which they were trained and in their ability to visually discriminate 90°, and this learning transferred to the untrained condition. When scaled by the relative intrinsic stability of each task, transfer levels were found to be equal. The results are discussed in the context of the perception–action approach to learning and performance

    Asthma symptoms associated with depression and lower quality of life: a population survey

Design and setting: A face-to-face Health Omnibus Survey of a random and representative sample of the South Australian population in August 1998. Participants: 3010 randomly selected participants aged 15 years and over. Main outcome measures: Prevalence of doctor-diagnosed asthma, and scores for depression (measured by PRIME-MD instrument) and quality of life (measured by SF-36) in affected participants. Results: The prevalence of asthma was 9.9%. The prevalence of major depression was significantly higher for those who experienced dyspnoea, wakening at night with asthma, and morning symptoms of asthma. Quality-of-life scores were also lower for the same groups. Conclusions: Depression is a serious but potentially remediable comorbidity with asthma that may affect appropriate diagnosis and outcome.Robert D Goldney, Richard Ruffin, Laura J Fisher and David H Wilso

    A New Lipid Formulation Of Low Dose Ibuprofen Shows Non-Inferiority To High Dose Standard Ibuprofen: The Flare Study (Flaring Arthralgia Relief Evaluation In Episodic Flaring Knee Pain) - A Randomised Double-Blind Study

    OBJECTIVE: To investigate short-term efficacy and safety of a novel lipid ibuprofen formulation 1200mg/day compared with standard ibuprofen 1200mg/day and 2400mg/day in episodic knee arthralgia/flaring pain. DESIGN: Multicentre, randomised, double-blind, 3-arm, non-inferiority trial conducted at 27 primary care centres. Adults with ≥1 knee flare episode within 12 months were recruited within 24 hours of new flare with pain severity ≥5 on a 0-10 numerical rating scale (NRS). Primary outcome was change from baseline in WOMAC pain subscale over 5 days. Main secondary outcome was Gastrointestinal Symptom Rating Scale (GSRS) change from baseline. Other endpoints included assessment of WOMAC total subscale scores and self-reported NRS for pain, subject nominated activity, stiffness and swelling. RESULTS: 462 patients were enrolled (58.9% males; mean age 52.2 years). Treatment allocation comprised 148 lipid 1200mg, 155 soft-gel 1200mg, 159 soft-gel 2400mg. WOMAC pain subscale scores decreased in all groups, with lipid 1200mg being non-inferior to soft-gel 1200mg (adjusted mean difference -0.26 [95% CI-0.69, 0.17]) and to soft-gel 2400mg (difference 0.19 [95% CI -0.24, 0.62]). No differences were seen in mean GSRS total scores. NRS secondary endpoints suggested greater improvements in the lipid 1200mg group compared to soft-gel 1200mg, with similar results to soft-gel 2400mg. The most frequent drug-related AEs were GI disorders, with statistically fewer events for lipid 1200mg vs soft-gel 2400mg (p=0.01, post-hoc analysis). CONCLUSIONS: Ibuprofen 1200mg/day lipid formulation was non-inferior to standard ibuprofen soft-gel capsules 1200mg and 2400mg/day in relieving flaring knee pain. NRS endpoints showed lipid 1200mg was numerically similar to soft-gel 2400mg

    Grassmannian flows and applications to nonlinear partial differential equations

    We show how solutions to a large class of partial differential equations with nonlocal Riccati-type nonlinearities can be generated from the corresponding linearized equations, from arbitrary initial data. It is well known that evolutionary matrix Riccati equations can be generated by projecting linear evolutionary flows on a Stiefel manifold onto a coordinate chart of the underlying Grassmann manifold. Our method relies on extending this idea to the infinite dimensional case. The key is an integral equation analogous to the Marchenko equation in integrable systems, that represents the coodinate chart map. We show explicitly how to generate such solutions to scalar partial differential equations of arbitrary order with nonlocal quadratic nonlinearities using our approach. We provide numerical simulations that demonstrate the generation of solutions to Fisher--Kolmogorov--Petrovskii--Piskunov equations with nonlocal nonlinearities. We also indicate how the method might extend to more general classes of nonlinear partial differential systems.Comment: 26 pages, 2 figure

    Anastral spindle assembly and γ-tubulin in Drosophila oocytes

    <p>Abstract</p> <p>Background</p> <p>Anastral spindles assemble by a mechanism that involves microtubule nucleation and growth from chromatin. It is still uncertain whether γ-tubulin, a microtubule nucleator essential for mitotic spindle assembly and maintenance, plays a role. Not only is the requirement for γ-tubulin to form anastral <it>Drosophila </it>oocyte meiosis I spindles controversial, but its presence in oocyte meiosis I spindles has not been demonstrated and is uncertain.</p> <p>Results</p> <p>We show, for the first time, using a bright GFP fusion protein and live imaging, that the <it>Drosophila </it>maternally-expressed γTub37C is present at low levels in oocyte meiosis I spindles. Despite this, we find that formation of bipolar meiosis I spindles does not require functional γTub37C, extending previous findings by others. Fluorescence photobleaching assays show rapid recovery of γTub37C in the meiosis I spindle, similar to the cytoplasm, indicating weak binding by γTub37C to spindles, and fits of a new, potentially more accurate model for fluorescence recovery yield kinetic parameters consistent with transient, diffusional binding.</p> <p>Conclusions</p> <p>The FRAP results, together with its mutant effects late in meiosis I, indicate that γTub37C may perform a role subsequent to metaphase I, rather than nucleating microtubules for meiosis I spindle formation. Weak binding to the meiosis I spindle could stabilize pre-existing microtubules or position γ-tubulin for function during meiosis II spindle assembly, which follows rapidly upon oocyte activation and completion of the meiosis I division.</p
