    Awareness of Rhythm in SLI

    Children with specific language impairments (SLIs) show impaired perception and production of language, and also show impairments in perceiving auditory cues to rhythm [amplitude rise time (ART) and sound duration] and in tapping to a rhythmic beat. Here we explore potential links between language development and rhythm perception in 45 children with SLI and 50 age-matched controls. We administered three rhythmic tasks, a musical beat detection task, a tapping-to-music task, and a novel music/speech task, which varied rhythm and pitch cues independently or together in both speech and music. Via low-pass filtering, the music sounded as though it was played from a low-quality radio and the speech sounded as though it was muffled (heard "behind the door"). We report data for all of the SLI children (N = 45, IQ varying), as well as for two independent subgroupings with intact IQ. One subgroup, "Pure SLI," had intact phonology and reading (N = 16), the other, "SLI PPR" (N = 15), had impaired phonology and reading. When IQ varied (all SLI children), we found significant group differences in all the rhythmic tasks. For the Pure SLI group, there were rhythmic impairments in the tapping task only. For children with SLI and poor phonology (SLI PPR), group differences were found in all of the filtered speech/music AXB tasks. We conclude that difficulties with rhythmic cues in both speech and music are present in children with SLIs, but that some rhythmic measures are more sensitive than others. The data are interpreted within a "prosodic phrasing" hypothesis, and we discuss the potential utility of rhythmic and musical interventions in remediating speech and language difficulties in children.This project has been funded by the Nuffield Foundation, but the views expressed are those of the authors and not necessarily those of the Foundation.This is the final version of the article.     Enhancement of TbIII-CuII single-molecule magnet performance through structural modification

    We report a series of 3d–4f complexes {Ln2Cu3(H3L)2Xn} (X=OAc−, Ln=Gd, Tb or X=NO3−, Ln=Gd, Tb, Dy, Ho, Er) using the 2,2′-(propane-1,3-diyldiimino)bis[2-(hydroxylmethyl)propane-1,3-diol] (H6L) pro-ligand. All complexes, except that in which Ln=Gd, show slow magnetic relaxation in zero applied dc field. A remarkable improvement of the energy barrier to reorientation of the magnetisation in the {Tb2Cu3(H3L)2Xn} complexes is seen by changing the auxiliary ligands (X=OAc− for NO3−). This leads to the largest reported relaxation barrier in zero applied dc field for a Tb/Cu-based single-molecule magnet. Ab initio CASSCF calculations performed on mononuclear TbIII models are employed to understand the increase in energy barrier and the calculations suggest that the difference stems from a change in the TbIII coordination environment (C4v versus Cs)

    The person-based development and realist evaluation of a summary report for GP consultations

    Background: Use of telephone, video and e-consultations is increasing. These can make consultations more transactional. This study aimed to develop a complex intervention to address patients’ concerns more comprehensively in general practice and test the feasibility of this in a cluster-randomised framework. The complex intervention used two technologies: a patient-completed pre-consultation form used at consultation opening and a doctor-provided summary report provided at consultation closure. This paper reports on the development and realist evaluation of the summary report. Methods: A person-based approach was used to develop the summary report. An electronic protocol was designed to automatically generate the report after GPs complete a clinical template in the patient record. This was tested with 45 patients in 3 rounds each, with iterative adjustments made based on feedback after each round. Subsequently, an intervention incorporating the pre-consultation form with the summary report was then tested in a cluster-randomised framework with 30 patients per practice in six practices: four randomised to intervention, and two to control. An embedded realist evaluation was carried out. The main feasibility study results are reported elsewhere. Results: Intervention Development: 15 patients were recruited per practice. Eight patients and six GPs were interviewed and 18 changes made. The summary report improved substantially; GPs and patients in the final practice were more satisfied with the report than the first practice. Realist evaluation: The summary was most useful for consultations when safety-netting advice was important or with multiple complex follow-up steps in patients who have difficulty remembering or communicating. It generated greater clarity on the follow-up and greater patient empowerment and reassurance. Conclusions: The person-based approach was successful. The summary report creates clarity, empowerment and reassurance in certain consultations and patients. As it takes a few minutes per patient, GPs prefer to select patients who will benefit most

    Temperature-dependent phenotypic variation of Campylobacter jejuni lipooligosaccharides

    <p>Abstract</p> <p>Background</p> <p><it>Campylobacter jejuni </it>is a major bacterial cause of food-borne enteritis, and its lipooligosaccharide (LOS) plays an initiating role in the development of the autoimmune neuropathy, Guillain-Barré syndrome, by induction of anti-neural cross-reactive antibodies through ganglioside molecular mimicry.</p> <p>Results</p> <p>Herein we describe the existence and heterogeneity of multiple LOS forms in <it>C. jejuni </it>strains of human and chicken origin grown at 37°C and 42°C, respectively, as determined on sodium dodecyl sulphate-polyacrylamide electrophoresis gels with carbohydrate-specific silver staining and blotting with anti-ganglioside ligands, and confirmed by nuclear magnetic resonance (NMR) spectroscopy. The <it>C. jejuni </it>NCTC 11168 original isolate (11168-O) was compared to its genome-sequenced variant (11168-GS), and both were found to have a lower-M<sub>r </sub>LOS form, which was different in size and structure to the previously characterized higher-M<sub>r </sub>form bearing GM<sub>1 </sub>mimicry. The lower-M<sub>r </sub>form production was found to be dependent on the growth temperature as the production of this form increased from ~5%, observed at 37°C to ~35% at 42°C. The structure of the lower-M<sub>r </sub>form contained a β-D-Gal-(1→3)-β-D-GalNAc disaccharide moiety which is consistent with the termini of the GM<sub>1</sub>, asialo-GM<sub>1</sub>, GD<sub>1</sub>, GT<sub>1 </sub>and GQ<sub>1 </sub>gangliosides, however, it did not display GM<sub>1 </sub>mimicry as assessed in blotting studies but was shown in NMR to resemble asialo-GM<sub>1</sub>. The production of multiple LOS forms and lack of GM<sub>1 </sub>mimicry was not a result of phase variation in the genes tested of NCTC 11168 and was also observed in most of the human and chicken isolates of <it>C. jejuni </it>tested.</p> <p>Conclusion</p> <p>The presence of differing amounts of LOS forms at 37 and 42°C, and the variety of forms observed in different strains, indicate that LOS form variation may play a role in an adaptive mechanism or a stress response of the bacterium during the colonization of different hosts.</p

    Options for modulating intra-specific competition in colonial pinnipeds: the case of harbour seals (Phoca vitulina) in the Wadden Sea

    Colonial pinnipeds may be subject to substantial consumptive competition because they are large, slow-moving central place foragers. We examined possible mechanisms for reducing this competition by examining the diving behaviour of harbour seals (Phoca vitulina) after equipping 34 seals (11 females, 23 males) foraging from three locations; Romo, Denmark and Lorenzenplate and Helgoland, Germany, in the Wadden Sea area with time-depth recorders. Analysis of 319,021 dives revealed little between-colony variation but appreciable inter-sex differences, with males diving deeper than females, but for shorter periods. Males also had higher vertical descent rates. This result suggests that males may have higher overall swim speeds, which would increase higher oxygen consumption, and may explain the shorter dive durations compared to females. Intersex variation in swim speed alone is predicted to lead to fundamental differences in the time use of three-dimensional space, which may help reduce consumptive competition in harbour seals and other colonial pinnipeds

    Staff perceptions following a training programme about reducing psychotropic medication use in adults with intellectual disability : the need for a realistic professional practice framework

    Background: Adults with intellectual disability are at higher risk of being administered psychotropic medications. The UK-developed SPECTROM (Short-term PsychoEducation for Carers To Reduce Over Medication of people with intellectual disabilities) training programme educates disability support workers on psychotropic medications and alternatives to these medications. Method: Interviews were conducted with 10 participants who took part in the pilot SPECTROM training programme to elicit their views on the programme and its appropriateness in an Australian context. Results: The key theme was ‘Need for a psychotropic medication practice framework’. Four sub-themes were Broad satisfaction with the SPECTROM training programme; Disability support workers acknowledging the limitations of their scope of practice; Empowering training through prescriptive and reflective methods and; Need for future mentoring from Multi-Disciplinary Team members in the application of new knowledge. Conclusions: Participants felt that whilst they could improve their knowledge and attitudes surrounding psychotropic medication administration for behaviours of concern through SPECTROM training, a national practice framework is needed to execute its goals at scale
